《索问》云∶痛痒、疮疡、痈疽、疡疹、瘤气、结核，怫郁甚者，皆热。五脏不和，九
窍不通，六腑不和，留结为痈。近于火气，微热则痒，热甚则痛，附近则灼而为疮，皆火之用也。
人之疮肿，因内热外虚所生也。为风湿之所乘，则生疮肿。然肺主气，候于皮毛，脾
主肌肉，气虚则肤腠开，为风湿所乘，脾气温而内热，即生疮也。肿者，皆由寒热毒瓦斯客于经
络，使血涩而不通，壅结成肿。风邪内作，即无头无根。气血相搏作者，即有头有根。热壅
盛，则为脓赤。核肿，则风气流结也。疮以痛痒，痛则为实，痒则为虚，非谓虚寒也，正
谓热之微甚也。
痒者，美疾也。故火旺于夏，而万物蕃鲜荣美也，炙之以火，渍之以汤，而其痒转甚
者，微热之所使也。痒去者，谓火热令皮肤纵缓，腠理开通，阳气得泄，热散而去。或夏热
皮肤痒，而以冷水沃之，其痒不去，谓寒收敛，腠理闭密，阳气郁结，不能散越，怫热内作
故也。
疮疡皆为火热，而反腐出脓水者，犹谷肉果菜，热极则腐烂，而溃为污水也。渍而腐
烂者，水之化也。痈浅而大，
疽深而恶。热盛血，则为痈脓也。疡有头，小疮也。疹浮而小，瘾疹也。瘤气，赤瘤、丹 、
热盛气火之色也。
如意散 治疥癣无时痛痒，愈发有时，不问久新者。
吴茱萸 牛蒡子 荆芥（各一分） 牡蛎（半两） 轻粉（半钱） 信砒（二钱）
上为细末，研匀，每临卧，抄一钱，油调，遍身搓摩上一半。如后有痒不止，更少许涂
之股髀之间，闻香悉愈。
信效散 治风热上客阳明之经，牙齿疳蚀，断宣腐臭，血出色黄，气腐注闷，动摇疼痛，
发作有时，兼解中金石一切毒药。
信砒（一钱） 黄丹（二钱） 千古锻石（如无，但以陈者，炒，细研，四钱）
上为细末，入青盐一分、麝香少许，（如无此二味，亦得。）每上，抄三两大豆许，先
洗漱净，以手蘸药，捺上下牙齿龈沥涎，勿咽之，须臾漱净，或有蚀处，再上少许，日三
四次。常用如意一上，以频为妙。或服金石药致病，使一日三四上，更不伤牙齿，如神。或
已伤牙齿者，敷之即愈。或平人常用，颇能清利头目，宽膈美食，使髭须迟白。久用亦能固
牙齿，使迟老。
人气于面，而手足阳明气血灌注，忧思则气结而血液不行，燥热怫郁而血衰，不能荣
养脏腑，故早苍黄而斑白也。此药能使阳明血气宣通，故性能然也。
又方，使龙骨，不用锻石。
神圣饼子 治一切打扑伤损，金石刀刃出血不止者，立效。
乌鱼骨（一两，五月五日前先准备下） 莴苣菜（一握） 韭菜（一握） 青蓟草（一握，
约一虎口，人手取团丸是也） 锻石（四两）
上以五月五日日未出，本人不语，将取三味同杵烂，次后下余药味，杵得所，搏作饼子，
晒干，用时旋刮，敷之。此药上，无脓退痂便愈。
芙蓉膏 治遍满头面大小诸靥子，或身体者。
料炭灰 桑柴灰 荞麦点灰（各半升。上灰用热汤淋，取一升，熬至五分） 独角仙（一
个，不用角） 红娘子（半钱，不去翅足） 糯米（四十九粒） 锻石（一两，风化者）
上为末，将前项灰汁调如面糊相似，在瓷合子内，于土底埋五七日，取出使用。取瘢痕，
靥内刺破，用细竹签子点之，放药，用湿纸揩药，再点至三。上见瘢痕时，冷水淋洗。
忌姜、醋、鱼、马肉。
铅白霜散 治大小人口疮，牙齿腐蚀，气臭出血者。
铅白霜（二钱） 铜绿（二钱） 白矾（一块，指大许）
上为末，以翎羽扫疮上，以温浆水漱之。
麝香散 治大小人口齿腐蚀出血，断根宣烂者。
上好咸土（不以多少） 麝香（真好者少许）
上土热汤淋，取汁，去滓，用清汁，银石器中熬干，刮下，再与麝香同研匀，掺于疮上，
以纸贴。神效！
乳香散 治一切瘰 疮，新久远近不已者。
乳香（一钱） 砒霜（一钱） 砂（一钱半） 红娘子（十四个，去翅足） 黄丹（半钱）
上为末，糯米粥和作饼子，如折三钱浓，小铜钱裹卷，大破疮，上白面糊。如不破音，
灸柴炷大者，不过一日，其瘰 核自下。
五香汤 治一切恶疮瘰 结核无头尾，及诸疮肿。
沉香 木香 鸡舌香（各一两） 熏陆香 麝香（各三钱） 连翘（一两半）
上研为细末，每服二钱，水一盏，煎至六分，不拘时服。
紫参丸 治热毒瘰 肿痛，已肉消，疮已破，出脓水，服此。
紫参 苦参（锉。各一两） 连翘（二两） 丹参（一两半） 腻粉（二钱，别研） 滑石（二两）
上为末，别用玄参一斤，捣碎，以酒三碗，浸三日，揉取汁，去滓，用皂角子三百枚，
煨熟，捣为末，用玄参酒熬皂角子末，成膏，和前药，如桐子大，每服一丸，以黄 汤下。
一日加一丸，至患人岁数即止。如四十，则二十，每日却减一丸。疮自干，有结，肉消。
麝香雄黄散 治十七般恶虫咬伤人，及疮肿者。
麝香 雄黄 乳香 砂（各二钱） 土蜂窝 露蜂窝（烧灰。各一个）
上研为细末，以醋调少许，涂咬着处。或不辨认得，多疑是恶疮，三五日不疗，即毒入
心，难瘥。忌鸡、血、油腻物。
砂散 治一切疔疮。
砂 雄黄 天南星 砒霜（各等分） 麝香（少许）
上研为末，用竹针针开，用药。到黄水出，疮已。
圣力散 治诸疔疮肿。
草乌头 白芨 白蔹 木鳖子（去皮） 地龙 金毛狗脊（各二钱半） 麝香（三钱）

上为细末，用针针到生肉痛者，用药。黄水出为度。
穿山甲散 治一切通气破疮肿。行脓血如神。妙！
穿山甲 木鳖子 乌龙角（各等分，都烧存性）
上为末，每服一钱半，空心，热酒调下。至中午疮破，脓血便行。
守瘿丸 治瘿瘤结硬。
通草（二两） 杏仁（一大合，去皮尖，研） 牛蒂子（一合，出油） 吴射干 昆布（去咸）
诃黎勒 海藻（去咸。各四两）
上为末，炼蜜为丸，如弹子大，含化，咽津下，日进三服。
鬼代丹 治打着不痛。
无名异（研） 没药（研） 乳香（研） 自然铜（醋淬，研） 地龙（去土）
木鳖子（去壳。各等分）
上为末，炼蜜为丸，如弹子大，温酒下一丸。
龙脑润肌散 治杖疮，热毒疼痛。
黄丹（一两） 密佗僧（半两） 轻粉（一钱半） 麝香（半两） 龙脑（一字）
上为末，掺药在疮上，用青帛子覆之，内留一眼子。
香药丸 治瘰 疮。
砂 乳香 没药 半夏 轻粉 赤石脂（各等分）
上为末，糯米粉为丸，如桐子大，每服十丸，加至二十丸，皂角子汤下，临卧。
红玉梃子 治一切牙疳。
砒霜（一块，皂角子大） 黄丹（ 过） 卤土（各等分）
上为细末， 饼和作剂子，任牙。
桃花散 治一切疮。生肌药。
白芨 白蔹 黄柏 黄连 乳香（别研） 麝香（别研） 黄丹（各等分）
上为细末，掺于疮上。三二日生肌肉满。
追毒散 治生疮发闷，吐逆霍乱。
螺儿青 拣甘草（各一两） 白矾（二钱半）
上为细末，每服一钱，新汲水调下。立止。
胆矾丸 治男子年少而鬓发斑白。
土马综（烧存性） 胡桃（十个） 真胆矾（半两） 川五倍子（一两）
上为末，和作一块，绢袋子盛，如弹子大，热酒、水各少许，浸下药汁，淋洗头发一
月。神效！
茯神散 治胆热多睡，神思不安，昏闷。
茯神（去皮） 麦门冬 地骨皮 茯苓（各一两） 白藓皮 酸枣仁 沙参 甘草（炙。
各半两）
上为末，每服三钱，水一盏，煎至六分，去滓，食后服。
铁脚丸 治大小便不通。
皂角（不以多少，去皮，去壳子，炙）
上为末，酒、面糊为丸，如桐子大，每服三十丸，酒下。
金圣散 治小肠膀胱气痛不可忍者。
地胆（半两，去足翅，微炒） 滑石（一两） 朱砂（半钱）
上为末，每服二钱，用苦杖酒调下，食前服。
琥珀散 治五淋。
滑石（二两） 木通 当归 木香 郁金 扁竹（各一两） 琥珀（二两）
上为末，每服三五钱，用芦苇叶同煎，食后，日三服。
葵子散 治小便不通。
葵子 茯苓（去黑皮。各等分）
上为末，每服四钱，水一盏，煎二沸，食前。
倒换散 治无问久新癃闭不通，少腹急痛，肛门肿痛。
大黄（小便不通减半） 荆芥穗（大便不通减半。各等分）
上件药味，各别为末，每服一二钱，温水调下，临时加减服。
败毒散 治男子往来寒热，妇人产后骨蒸血运。
大黄 黄药子 紫河车 赤芍药 甘草（各等分）
上为末，每服一钱，如发热，冷水下，如发寒，煎生姜、栝蒌汤乘热调下。此药偏治
妇人。
补真丹 治男子元藏虚冷。兴阳固肾补虚。
黑附子（一两，煨） 阳起石（火烧，酒淬，三钱） 海马（二钱） 乳香 雄黄（为衣）
血竭（各三钱） 石莲子（去壳、皮、心） 黑锡（炒，去砂子，半两） 石燕子（烧，以醋淬）
麝香（一分）
上为细末，面糊为丸，每服二十丸，用五香汤空心下。
五香汤 调神益气。
沈香 笺香 乳香 麝香 檀香（各等分）
上为细末，每服半钱，煎汤下。
新添一醉乌法，治头须白再黑方∶
诃黎勒（十个，不去核） 酸石榴（三个，大者，取汁） 绿矾（五分） 生地黄汁（一升）
砂（研） 硫黄（研。各一钱）
上药同入瓷瓶内，用二味汁浸，密封，勿令透气，至四十九日后取出，其诃子状若
黑梅，至夜临卧含一枚，咽津，到晓烂嚼，以酒一盏下之，三二日后再服。忌葱、大蒜、萝卜。
According to the *Suwen* (Basic Questions): Pain, itching, sores, carbuncles, ulcers, rashes, tumors with qi stagnation, and nodules, when severely stagnant, all belong to heat syndromes. Disharmony of the Five Zang organs leads to blockage of the Nine Orifices; disharmony of the Six Fu organs causes pathogenic factors to remain and knot, forming carbuncles. Proximity to fire energy results in mild heat causing itching, severe heat causing pain, and scorching nearby tissues leading to sores; these are all functions of Fire. Human sores and swellings arise from internal heat and external deficiency. When attacked by Wind-Dampness, sores and swellings are generated. The Lung governs Qi and corresponds to the skin and hair; the Spleen governs muscles. When Lung Qi is deficient, the skin pores open, making one susceptible to Wind-Dampness; although the Spleen Qi is warm, internal heat generates sores. Swelling is caused by cold-heat toxic qi lodging in the meridians and collaterals, causing blood stasis and obstruction, leading to congestion and knotting into swelling. Internal action of Wind pathogen results in lumps without heads or roots; when Qi and Blood fight each other, lumps have heads and roots. Severe heat congestion leads to pus formation and redness. Nodular swelling is due to the knotting of Wind and Qi flow. Sores manifest as pain or itching; pain indicates Excess syndrome, while itching indicates Deficiency syndrome. Here, "Deficiency" does not refer to Cold-Deficiency, but rather to the slightness or severity of Heat.
Itching is a beautiful sensation (as Yang Qi circulates freely). Thus, Fire prospers in Summer, and all things flourish and appear vibrant. If roasted by fire or soaked in hot water, and itching intensifies, it is caused by mild heat. The disappearance of itching occurs because heat relaxes the skin, opens the interstices (Cou Li), allows Yang Qi to dissipate, and heat scatters away. Alternatively, if summer skin itching is washed with cold water and persists, it is because Cold constricts, closing the interstices, causing Yang Qi to stagnate internally and unable to disperse, resulting in trapped heat arising within.
Sores and ulcers are all caused by Fire and Heat. Those that rot and discharge pus and water are like grains, meats, fruits, and vegetables: when heat reaches its extreme, they rot and溃 (break down) into foul liquid. Rotting due to soaking is the transformation of Water. Carbuncles (Yong) are superficial and large; Gangrenous sores (Ju) are deep and malignant. Heat damaging Blood forms carbuncle pus. Ulcers with heads are small sores. Rashes (Zhen) float and are small, known as Urticaria (Yin Zhen). Tumor Qi refers to red tumors or Erysipelas (Dan), which are colors of excessive Heat and Qi-Fire.
Ruyi San (As-You Powder): Treats tinea and ringworm with intermittent pain and itching, regardless of duration or novelty.
Evodia Fruit (Wuzhuyu), Arctium Seed (Niubangzi), Schizonepeta (Jingjie) (each 1 fen), Oyster Shell (Muli) (0.5 liang), Calomel (Qingfen), Realgar mixed with Arsenic Trioxide (Xinpi) (2 qian).
Grind the above into fine powder and mix evenly. Each time before sleep, take 1 qian, mix with oil, and rub over half the body. If itching persists later, apply a small amount between the thighs and hips. Healing occurs upon smelling the fragrance.
Xinxiao San (Faithful Effect Powder): Treats Wind-Heat attacking the Yangming Meridian, causing dental caries and erosion, stopping foul odor, yellowish blood color, foul breath and stuffiness, pain upon movement, intermittent attacks, and detoxifying all metallic/mineral poisons.
Realgar mixed with Arsenic Trioxide (Xinpi) (1 qian), Lead Oxide (Huandan) (2 qian), Ancient Calcined Iron Ore (Qiangu Duanshi) (4 qian, if unavailable, use aged ones, stir-fried and finely ground).
Grind the above into fine powder, add Green Salt (Qingyan) 1 fen and Musk (Shexiang) a little (if unavailable, omit). Take an amount the size of a soybean per application. First rinse mouth clean, dip fingers in medicine, press on upper and lower teeth and gums to drain saliva, do not swallow. Rinse clean after a moment. Apply a little more to eroded areas. Use 3-4 times daily. Often used with Ruyi San, frequent application is best. If illness is caused by taking metallic/mineral drugs, apply multiple times a day without damaging teeth, as if by divine power. If teeth are already damaged, applying heals immediately. Common use by healthy people can clear the head and eyes, broaden the chest, improve appetite, and delay graying of beard and hair. Long-term use also strengthens teeth and delays aging.
Qi ascends to the face, where the Hand-Yangming and Foot-Yangming meridians pour their Qi and Blood. Worry and thought cause Qi stagnation and blood stasis, dryness-heat traps and depresses, blood becomes deficient, failing to nourish the Zang-Fu organs, hence premature grayness, sallow complexion, and white spots. This medicine promotes the circulation of Yangming Qi and Blood, hence its efficacy.
Another prescription uses Dragon Bone (Gulong) instead of calcined iron ore.
Shensheng Bingzi (Divine Holy Cake): Treats all types of blunt trauma injuries, bleeding from metal stones or blades that does not stop, with immediate effect.
Cuttlebone (Wuyu Gu) (1 liang, prepared before May 5th), Lettuce (Woju Cai) (a handful), Leek (Jiucai) (a handful), Green Thistle (Qingji Cao) (a handful, approx. tiger's mouth size, gathered by hand into a ball), Calcined Iron Ore (Duanshi) (4 liang).
On May 5th before sunrise, the patient should remain silent, pound the first three ingredients together until paste-like, then add remaining ingredients, pound appropriately, shape into cakes, and sun-dry. Scrape fresh for use and apply to the lesion. No pus forms, scabs fall off, and healing occurs.
Furong Gao (Hibiscus Ointment): Treats various pits/moles on the head and face, or on the body.
Charcoal Ash (Liaotan Hui), Mulberry Branch Ash (Sangchai Hui), Buckwheat Ash (Qiaomai Dian Hui) (each 0.5 sheng. Pour hot water over ashes, collect 1 sheng, boil down to 0.5 sheng), Rhinoceros Beetle (Dujiao Xian) (1 piece, remove horn), Red Ladybug (Hongniangzi) (0.5 qian, keep wings and legs), Glutinous Rice (Nuomi) (49 grains), Weathered Calcined Iron Ore (Duanshi) (1 liang).
Grind into powder. Mix with the aforementioned ash juice to resemble batter. Place in a porcelain jar, bury in soil for 5-7 days, then take out for use. To remove scars, puncture the pit, apply medicine with a thin bamboo stick, cover with wet paper, repeat application 3 times. When scar appears, wash with cold water. Avoid ginger, vinegar, fish, and horse meat.
Qianbaishuang San (Lead White Frost Powder): Treats oral ulcers in adults and children, tooth erosion, foul breath, and bleeding.
Lead White Frost (Qianbaishuang) (2 qian), Copper Verdigris (Tonglv) (2 qian), Alum (Baifan) (1 piece, finger-sized).
Grind into powder. Sweep onto ulcer with feather. Rinse mouth with warm rice water.
Shexiang San (Musk Powder): Treats oral and dental erosion with bleeding and root decay in adults and children.
Good Salty Soil (Shanghao Xiantu) (any amount), High-Quality Musk (Shexiang) (a little).
Pour hot water over soil, extract juice, filter residue. Boil clear juice in silver/copper vessel until dry. Scrape down, grind together with Musk, sprinkle on ulcer, cover with paper. Divine effect!
Ruxiang San (Frankincense Powder): Treats all types of scrofula sores, whether new, old, distant, or persistent.
Frankincense (Ruxiang) (1 qian), Arsenic Trioxide (Pishuang) (1 qian), Realgar mixed with Arsenic Trioxide (Niaoshasha) (1.5 qian), Red Ladybug (Hongniangzi) (14 pieces, remove wings/legs), Lead Oxide (Huandan) (0.5 qian).
Grind into powder. Mix with glutinous rice porridge to form cakes, thickness approx. 3 qian. Wrap in small copper coin. For large broken sores, apply white flour paste. If not broken, moxa with柴 (chai) stick size, within one day, the scrofula nodule will drop off.
Wuxiang Tang (Five Fragrances Decoction): Treats all malignant sores, scrofula nodules without heads/tails, and various swellings.
Agarwood (Chenxiang), Costus Root (Muxiang), Clove (Jitixiang) (each 1 liang), Myrrh (Xunluxiang), Musk (Shexiang) (each 3 qian), Forsythia (Lianqiao) (1.5 liang).
Grind into fine powder. Take 2 qian per dose, decoct in 1 cup of water until 0.6 cup remains. Take at any time.
Zishen Wan (Purple Ginseng Pill): Treats heat-toxic scrofula with swelling and pain, flesh wasting, broken sores discharging pus/water. Take this formula.
Radix Codonopsis (Zishen), Sophora Root (Kushen) (cut, each 1 liang), Forsythia (Lianqiao) (2 liang), Salvia Root (Danshen) (1.5 liang), Calomel (Nifen) (2 qian, separately ground), Talcum (Huashi) (2 liang).
Grind into powder. Separately take Radix Scrophulariae (Xuanshen) 1 jin, crush, soak in 3 bowls of wine for 3 days, knead to extract juice, filter residue. Roast 300 Soap Beans (Saojiaozizi) until cooked, grind to powder. Boil Soap Bean powder with Xuanshen wine to form a paste. Mix with previous powder. Shape into pills the size of Torreya seeds. Take 1 pill per dose, swallow with Astragalus (Huangqi) decoction.
Increase by 1 pill daily until reaching the patient's age in years. E.g., if 40 years old, start with 20 pills, decrease by 1 daily. Sores dry up, knots form, flesh wastes.
Shexiang Xionghuang San (Musk and Realgar Powder): Treats bites from seventeen types of venomous insects and associated swellings.
Musk (Shexiang), Realgar (Xionghuang), Frankincense (Ruxiang), Realgar mixed with Arsenic Trioxide (Niaoshasha) (each 2 qian), Earth Honeycomb (Tu Fengwo), Dew Honeycomb (Lu Fengwo) (burned to ash, each 1).
Grind into fine powder. Mix with vinegar to make a small amount, apply to bite site. If unrecognized, often suspected as malignant sore, if not treated for 3-5 days, toxin enters Heart, difficult to heal. Avoid chicken, blood, and greasy foods.
Naosha San (Sal Ammoniac Powder): Treats all types of furuncles (Ding Chuang).
Sal Ammoniac (Naosha), Realgar (Xionghuang), Pinellia Tuber (Tiannanxing), Arsenic Trioxide (Pishuang) (equal parts), Musk (Shexiang) (a little).
Grind into powder. Puncture with bamboo needle, apply medicine. When yellow fluid exits, sore heals.
Shengli San (Holy Power Powder): Treats various furuncle swellings.
Aconite Root (Caowutou), Bletilla Tuber (Baiji), Ampelopsis Rhizome (Bailian), Momordica Seed (Mubiezi) (remove peel), Earthworm (Dilong), Golden Dogtail Fern Rhizome (Jinmao Gouji) (each 2.5 qian), Musk (Shexiang) (3 qian), Lead Oxide (Huandan) (a little).
Grind into fine powder. Puncture painful living flesh with needle, apply medicine. Stop when yellow fluid exits.
Chanshanjia San (Pangolin Scale Powder): Treats all conditions involving Qi unblocking and breaking sores/swellings. Moves pus and blood divinely. Excellent!
Pangolin Scale (Chanshanjia), Momordica Seed (Mubiezi), Black Dragon Horn (Wulong Jiao) (equal parts, burnt to retain properties).
Grind into powder. Take 1.5 qian per dose on empty stomach, mix with hot wine. By noon, sore breaks, pus and blood flow.
Shouying Wan (Goiter-Suppressing Pill): Treats goiter tumors with hard knots.
Akebia Caulis (Tongcao), Apricot Seed (Xingren) (large measure, remove peel/point, grind), Arctium Seed (Niudizi) (1 measure, express oil), Belamcanda Rhizome (Wu Yegan), Kelp (Kunbu) (remove salt), Chebula Fruit (Helilei), Seaweed (Haizao) (remove salt) (each 4 liang).
Grind into powder. Make pills with refined honey, size of bullet. Dissolve in mouth, swallow saliva. Take 3 doses daily.
Guidai Dan (Ghost Substitute Pellet): Treats bruises that do not hurt.
Black Hematite (Wumingyi) (ground), Myrrh (Moyao) (ground), Frankincense (Ruxiang) (ground), Natural Pyrite (Ziran Tong) (quench in vinegar, grind), Earthworm (Dilong) (remove dirt), Momordica Seed (Mubiezi) (remove shell) (equal parts).
Grind into powder. Make pills with refined honey, size of bullet. Take 1 pill with warm wine.
Longnao Runji San (Borneol Moisturizing Muscle Powder): Treats beating wounds, heat-toxic pain.
Lead Oxide (Huandan) (1 liang), Colloidal Gold (Mituoseng) (0.5 liang), Calomel (Qingfen) (1.5 qian), Musk (Shexiang) (0.5 liang), Borneol (Longnao) (1 zi).
Grind into powder. Sprinkle on wound, cover with green silk cloth, leaving a small hole inside.
Xiangyao Wan (Fragrant Medicine Pill): Treats scrofula sores.
Sal Ammoniac (Naosha), Frankincense (Ruxiang), Myrrh (Moyao), Pinellia Tuber (Banxia), Calomel (Qingfen), Red Kaolin (Chishizhi) (equal parts).
Grind into powder. Make pills with glutinous rice flour, size of Torreya seeds. Take 10 pills per dose, increase to 20, swallow with Soap Bean decoction before bed.
Hongyu Tingzi (Red Jade Wand): Treats all types of gum disease (Ya Gan).
Arsenic Trioxide (Pishuang) (1 piece, soap bean size), Lead Oxide (Huandan) (refined), Halophilic Soil (Lutu) (equal parts).
Grind into fine powder. Mix with malt sugar cake to form agents. Chew as needed.
