Ren Shen (Ginseng Radix), Yi Zhi Ren (Alpinia Oxyphylla Fructus), Fu Ling (Poria), Xin Hui Pi (Tangerine Peel), Sheng Nong Po (Magnolia Bark), Yi Ren (Coix Seed), Mu Gua (Chaenomeles Fruit), Sha Ren (Amomum Fruit).
Again: The pulse is wiry on the right, irregular in onset and termination; small and weak on the left. The tongue edges are red, the center is white-yellow with slight cracking. The nose and four limbs are cold. During fever, there is slight thirst. There is no appetite and no desire to eat.
Previous discussion indicated that the Taiyin Spleen organ was diseased, and the Malaria pathogen attacked the middle jiao from the extremities, inevitably affecting the Spleen and Stomach. Coldness of the nose tip and four limbs indicates weak Yang Qi, further injured by the disease. The body remains unwarmed throughout the day, while the flesh wastes away. One dare not use harsh or attacking drugs to expel the pathogen; instead, one should promote the circulation of Spleen and Stomach Yang. For pathogens latent in the Yin aspect, there is no rationale for diaphoresis or clearing heat.
Prescription: Ren Shen (Ginseng Radix), Cao Guo (Tsaoko Cardamom Fruit), Chao Ban Xia (Pinellia Rhizome, stir-fried), Sheng Jiang (Fresh Ginger), Fu Ling (Poria), Xin Hui Pi (Tangerine Peel), Zheng Wu Mei Rou (Steamed Smoked Plum Flesh).
After two doses, add Fu Zi (Aconite Lateral Root). Later, add Mu Li (Oyster Shell).
Wu (17 years old): Malaria has damaged the Spleen and Stomach, causing disharmony in the abdomen. The pulse is choppy on the right; distension worsens after eating. The previous prescription regulated Qi and Blood, assisting in draining Liver Wood. After administration, accumulated stagnation was expelled, and pain decreased with the passage of stool. It is still appropriate to subdue Liver Wood and stabilize Earth (Spleen); it should not be treated as Yang deficiency requiring warming tonification.
Prescription: Sheng Bai Zhu (Atractylodes Macrocephala Rhizome, raw), Chuan Lian (Coptis Rhizome), Jiao Mu (Zanthoxylum Seed), Mai Ya (Barley Sprout), Ji Zhun Pi (Gallus Crocuta Skin), Guang Pi (Tangerine Peel), Nong Po (Magnolia Bark), Chao Shan Zha (Crataegus Fruit, stir-fried).
Chen (46 years old): Malaria pathogen disturbs the middle jiao from the extremities, involving the domains of Yangming and Jueyin. When Yangming declines, Jueyin attacks. With insufficient fluids, internal Wind must arise. Ancient sages considered Mahuang (Ephedra) related to Qi deficiency, and Wood related to damp-phlegm and ruined blood. Currently, frequent heat in Wuhai days, sweating during walking, clearly indicates fluid deficiency. Yang movement generates Wind, and lipid fluids fail to irrigate the extremities. This is not a disorder of damp-phlegm in the Qi aspect. (Liver Wind)
Prescription: Dong Sang Ye (Mulberry Leaf, nine-steamed), Shou Wu Di (Polygonum Multiflorum Prepared Root), Hei Zhi Ma (Black Sesame Seed), Bai Zi Ren (Biota Seed), Fu Shen (Poria with Hostwood), Dang Gui (Angelica Sinensis Root), Qi Zi (Goji Berry), Ju Hua Tan (Chrysanthemum Flower, charred), made into honey pills.
Li: Pulse is wiry on the left. Vomiting. After fever, there is fullness and discomfort in the epigastrium. Diet should be cautious. Clear and purify the Upper and Middle Jiao to prevent progression into malaria. Administer Bitter-Spicy method. (Pi Syndrome - Fullness)
Prescription: Xing Ren (Apricot Seed), Yu Jin (Curcuma Aromatica Rhizome), Shan Zhi (Gardenia Fruit), Dou Chi (Prepared Soybean), Bai Kou (White Cardamom Fruit), Zhi Ke (Bitter Orange Fruit).
Sun: Elderly patient with malaria, chills and fever occurring at night. Chest stuffiness, no desire to eat, irritability, thirst, and frequent heat. Most concerning is the pathogen sinking leading to Jue syndrome. Administer Yang Dan method.
Prescription: Gui Zhi (Cinnamon Twig), Huang Qin (Scutellaria Root), Hua Fen (Trichosanthes Root), Sheng Bai Shao (White Peony Root, raw), Sheng Zuo Mu Li (Left Oyster Shell, raw), Wei Jiang (Roasted Ginger), Nan Zao (Chinese Date).
Ji: Malaria pulse is deep and choppy, with epigastric knotting. This belongs to interior syndromes. Use Xi Xin method (Draining Heart method).
Prescription: Ban Xia (Pinellia Rhizome), Chuan Lian (Coptis Rhizome), Ju Hong (Tangerine Pith), Zhi Shi (Immature Bitter Orange), Huang Qin (Scutellaria Root), Sheng Jiang Zhi (Fresh Ginger Juice).
Again: Pulse is deep, Right Guan position is large. Malaria has not stopped; chills and fever occur after midnight. Irritability, thirst, epigastric stuffiness, no desire to eat.
Prescription: Cu Chao Ban Xia (Pinellia Rhizome, vinegar-stir-fried), Xing Ren (Apricot Seed), Huang Qin (Scutellaria Root), Hua Fen (Trichosanthes Root), Cao Guo (Tsaoko Cardamom Fruit), Sheng Jiang (Fresh Ginger).
Lu: Malaria was forcibly interrupted, leading to palpitations due to deficient Qi knotting, resulting in body pain.
Prescription: Gui Zhi (Cinnamon Twig), Chao Jiao Ban Xia (Pinellia Rhizome, stir-fried until charred), Jiang Zhi (Ginger Juice), Guang Pi Bai (Tangerine Peel White), Dang Gui (Angelica Sinensis Root), Fu Ling (Poria).
Qian (Family): Summer-heat damaging Qi leads to malaria, chest fullness and knotting, vomiting of phlegm foam, all due to knotting of hot Qi. The previous doctor's use of Xi Xin method was very correct.
Prescription: Ren Shen Zhi (Ginseng Radix Juice), Zhi Shi Zhi (Immature Bitter Orange Juice), Huang Lian (Coptis Rhizome), Huang Qin (Scutellaria Root), Chao Ban Xia (Pinellia Rhizome, stir-fried), Xing Ren (Apricot Seed), Nong Po (Magnolia Bark), Jiang Zhi (Ginger Juice).
Wang: This is damp-heat steaming together with food and drink, entirely in the Qi aspect. Previously obtained relief through Sanjiao differentiation and dispersion, clearing and resolving. Having become an intermittent malaria, pathogen and healthy Qi contend, originally no major harm. Initially mistaken for Taiyang Shaoyang Yangming Taiyin Jueyin Shaoyin Six Meridian diseases, then erroneously treated with Ginseng and Atractylodes for tonification, causing pathogen to spread. Confusion, panting, delirium, and spasms occurred because the pathogen had no exit; interior closure leads to exterior collapse. Seeking協热下利 (diarrhea with fever) is now impossible. Pulse is fine and choppy; abdomen is distended upon palpation. Fullness belongs to Qi, dryness/stubbornness belongs to Blood. Now, after profuse diarrhea, abdominal fullness increases; all Qi is knotted, is this not due to blockage? Ruin and rupture have reached this point; rescue is difficult to expect.
Prescription: Xi Ye Chang Pu Gen Zhi (Acorus Tatarinowii Rhizome Juice, 2 qian), Cao Guo Ren (Tsaoko Cardamom Seed, 0.5 fen), Fu Ling Pi (Poria Cortex, 3 qian), Zi Nong Po (Magnolia Bark, 1 qian), Mian Yin Chen (Artemisia Capillaris Herba, 3 qian), Chen Sha Yi Yuan San (Cinnabar Yiyuan Powder, 5 qian), Lian Qiao Xin (Lonicera Bud Heart, 1.5 qian), Jin Yin Hua (Honeysuckle Flower, 3 qian). Additionally, take one pill of Niu Huang Wan (Calculus Bovis Pill), slowly dissolve with a tea spoon in cool boiled water.
Pan (Family): Latent pathogen causes fever, becoming malaria after Jue syndrome, occurring every other day. Cough with much phlegm, nausea, epigastric fullness. The pathogen is in the collaterals of Lung and Stomach. Plan to administer light Bitter-Spicy agents.
Prescription: Xing Ren (Apricot Seed), Huang Qin (Scutellaria Root), Ban Xia (Pinellia Rhizome), Ju Hong (Tangerine Pith), Bai Kou (White Cardamom Fruit), Hua Fen (Trichosanthes Root).
Cheng (Family): Pulse is large on the right, chills and fever with slight vomiting, epigastric fullness and inability to accept food. Malaria pathogen from extremities interacts with the Center Palace. Should use Bitter-Spicy to drain and descend, Sour-Bitter to clear heat. If pathogen force reduces by two or three parts, recovery will follow from clearing and tonifying.
Prescription: Chuan Lian (Coptis Rhizome), Chao Ban Xia (Pinellia Rhizome, stir-fried), Jiang Zhi (Ginger Juice), Huang Qin (Scutellaria Root), Zhi Mu (Anemarrhena Rhizome), Cao Guo (Tsaoko Cardamom Fruit), Chao Nong Po (Magnolia Bark, stir-fried), Wu Mei Rou (Smoked Plum Flesh).
Mao (Family): Used Yu Nu Jian (Jade Woman Decoction). Chills and fever persist, thirst and drinking continue, nausea/vomiting reduced, whole body painful. Pulse is rapid in both hands, tongue color grey-white with red edges, sweating does not resolve. Plan to use Sour-Bitter to drain interior heat pathogens. Ensure malaria stops, then regulate constitution.
Prescription: Huang Qin (Scutellaria Root), Huang Lian (Coptis Rhizome), Cao Guo (Tsaoko Cardamom Fruit), Bai Shao (White Peony Root), Wu Mei (Smoked Plum), Zhi Mu (Anemarrhena Rhizome). Decoct with Autumn Dew Water.
Again: Chills and fever disturb the Center Palace from the extremities; the stomach mouth suffers most damage. Heat and stuffiness, no appetite, Stomach damaged, pathogen remains. Clearing heat and benefiting phlegm is indeed a key method. However, the patient is elderly and weak, and the illness has lasted a month. While clearing pathogens, one must assist healthy Qi. Discuss using Ban Xia Xie Xin Tang (Pinellia Decoction to Drain the Epigastrium).
Prescription: Ren Shen (Ginseng Radix), Ban Xia (Pinellia Rhizome), Huang Lian (Coptis Rhizome), Huang Qin (Scutellaria Root), Zhi Shi (Immature Bitter Orange), Jiang Zhi (Ginger Juice).
Fang: Lao Nue (Consumptive Malaria) recurs. (Lao Nue)
Prescription: Ren Shen (Ginseng Radix), Cao Guo (Tsaoko Cardamom Fruit), Sheng Jiang (Fresh Ginger), Wu Mei (Smoked Plum), decoct with Autumn Dew Water.
Again: Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) plus Cao Guo (Tsaoko Cardamom Fruit), Zhi Mu (Anemarrhena Rhizome), Sheng Jiang (Fresh Ginger), Da Zao (Chinese Date).
Chen: Collateral deficiency causes pain, Yang weakness causes distension. Left hypochondriac area has Nue Mu (Malaria Mass). Pathogen remains, healthy Qi injured. This is Lao Nue (Consumptive Malaria).
Prescription: Ren Shen (Ginseng Radix), Dang Gui (Angelica Sinensis Root), Rou Gui (Cinnamon Bark), Jiao Bai Zhu (Atractylodes Macrocephala Rhizome, stir-fried until charred), Zhi Gan Cao (Licorice Root, honey-fried), Fu Ling (Poria), Guang Pi (Tangerine Peel), Sheng Jiang (Fresh Ginger), Nan Zao (Chinese Date).
After four doses, take one dose of Wu Ling San (Five-Ingredient Powder with Poria).
Certain case: Lao Nue (Consumptive Malaria) with aversion to cold, lower deficiency failing to receive. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) plus Rou Gui (Cinnamon Bark) and Wu Wei Zi (Schisandra Fruit).
Certain case: Malaria has not stopped, heat sinks causing dysentery, epigastric fullness, no desire to eat. (Malaria complicated by Hot Dysentery)
Prescription: Ren Shen (Ginseng Radix), Chuan Lian (Coptis Rhizome), Huang Qin (Scutellaria Root), Sheng Bai Shao (White Peony Root, raw), Guang Pi (Tangerine Peel), Chao Dang Gui (Angelica Sinensis Root, stir-fried), Chao Shan Zha (Crataegus Fruit, stir-fried), Gan Jiang (Dried Ginger), Zhi Shi (Immature Bitter Orange), Yin Hua (Honeysuckle Flower).
Again: Post-malaria relapse due to overexertion.
Prescription: Ren Shen (Ginseng Radix), Dang Gui (Angelica Sinensis Root), Bai Shao (White Peony Root), Zao Ren (Ziziphus Seed), Fu Shen (Poria with Hostwood), Guang Pi (Tangerine Peel), Sheng Jiang (Fresh Ginger), Nan Zao (Chinese Date).
Fan (33 years old): Pulse is small and choppy. Illness began after malaria, food digestion was poor, continued reading and mental labor, leading to pain in the left rib extending to the lower abdomen. Frequent Jue sensations arise, attacking either the epigastrium or gathering in the abdomen, causing凝着胀 (stagnant distension). Ancient saying: "Malaria never leaves the Liver and Gallbladder," just as "Cough never leaves the Lung." When Liver receives pathogen assistance, Wood energy flares up, inevitably insulting Central Earth. Original Qi becomes timid, transport and reception powers decrease. Clear Yang cannot expand, Turbid Yin occupies daily, gradually forming fullness. Last year, long-term intake of warming, acrid-sweet herbs did not remove the disease. The method of treating the constitution likely failed. Patient states condition worsens when resting quietly, slightly improves with activity. This is long-standing obstruction of Qi and Blood leading to stagnation. Discuss using Tong Luo method (Unblocking Collaterals). The root cause is prolonged malaria, pathogen remaining in collaterals. (Qi and Blood Congealing in Collaterals)
Prescription: Zi Jiang Xiang (Dalbergia Odorifera Wood), Tao Ren (Peach Kernel), Xiao Xiang Fu (Cyperi Rhizoma, small), Dan Jiang Zha (Ginger Dregs, light), Shen Qu (Medicated Leaven), Ji Zhun Pi (Gallus Crocuta Skin), Nan Shan Zha (Crataegus Fruit, southern), Jiu Gen Zhi (Allium Tuberosum Root Juice), made into pills.
Gao: Malaria attacks frequently, pathogen enters collaterals. Collaterals belong to Blood aspect. Sweating and purging failed to expel pathogen. Zhang Zhongjing formulated Bie Jia Jian Wan (Turtle Shell Decoction Pill) to search out and eliminate latent pathogens in collaterals. Efficacy expected in five to six days.
Take seven pills each morning, noon, and evening.
Certain case: Malaria pathogen unresolved for a month, pathogen has entered collaterals. Collaterals gather blood, pathogen attack causes blood discharge. Ultimately, chills, fever, irritability, thirst, yellow eyes, greasy tongue, dark scanty urine are all due to uncleared interior pathogens. Avoid fishy and meat foods; vegetables do not aid pathogen congestion. Previous medicines used Chai Ge (Bupleurum and Kudzu) to attack the exterior, digestive aids to unblock bowels, which have nothing to do with malaria. Use Zhang Zhongjing's Bie Jia Jian Wan (Turtle Shell Decoction Pill). Ten pills early morning, ten at noon, ten at dusk, taken with boiled water.
Fei: Malaria pathogen forces injury to fluids, Stomach function decreases, no appetite, intestinal dryness with red stool, left hypochondriac area slightly firm, resembling Nue Mu (Malaria Mass) aggregation. Should promote collateral heat clearance to肃余邪 (clean up residual pathogens).
Prescription: Sheng Di Huang (Rehmannia Root, raw), Zhi Mu (Anemarrhena Rhizome), Dan Pi (Moutan Cortex), Ma Ren (Hemp Seed), Sheng Bie Jia (Turtle Shell, raw).
Certain case (Family): Malaria pathogen sinks internally, transforming into Yin Malaria, prolonging into consumption. Must ensure menstruation flows smoothly to prevent pathogen seizing Dry Blood.
Prescription: Sheng Bie Jia (Turtle Shell, raw, 1 liang), Tao Ren (Peach Kernel, 3 qian), Chao Dan Pi (Moutan Cortex, stir-fried, 1 qian), Chuan Shan Jia (Pangolin Scale, 3 qian), Zha Rou (Crataegus Flesh, 1.5 qian), Sheng Xiang Fu (Cyperi Rhizoma, raw, 1.5 qian).
Gu: Left hypochondriac area has Nue Mu (Malaria Mass), due to Qi and Blood interknitting. Treatment should prioritize Tong Luo (Unblocking Collaterals). (Nue Mu)
Prescription: Bie Jia (Turtle Shell), Tao Ren (Peach Kernel), Jin Ling Zi (Melia Toosendan Fruit), Mu Li (Oyster Shell), Dan Pi (Moutan Cortex), Xia Ku Cao (Prunella Spike).
Jiang: Distant guest, different water and soil. Stomach received food without harmony, further angered by rage moving Liver. Wood invades Stomach Earth, malaria injury.有形瘕聚 (tangible mass) in hypochondriac region, three-year chronic ailment. Qi and Blood secretly consumed, but long-term involvement must enter Blood. Can herbal soup achieve efficacy? Should use slow method to疏通其络 (dredge its collaterals). If not pursued and extracted, leading to Yang knotting and Yin drying, brewing into Ye Ge (Dysphagia) which is difficult to treat.
Prescription: Sheng Bie Jia (Turtle Shell, raw), Tao Ren (Peach Kernel), She Xiang (Musk), Quan Chong (Whipworm), Jiu Bai Gen Fen (Allium Tuberosum Root Powder), Gui Xu (Angelica Sinensis Root Tip), Yu Li Ren (Prunus Armeniaca Kernel), Dong Kui Zi (Abutilon Seed), boiled into paste.
Wu (24 years old): Malaria recurs, left hypochondriac Nue Mu (Malaria Mass).
Prescription: Sheng Bie Jia (Turtle Shell, raw), Sheng Mu Li (Oyster Shell, raw), Chao Tao Ren (Peach Kernel, stir-fried), Dang Gui Xu (Angelica Sinensis Root Tip), Chao Yan Hu Suo (Corydalis Rhizome, stir-fried), Chai Hu Shao (Bupleurum Root Tip), Gui Zhi Mu (Cinnamon Twig Wood), Chao Shan Zha (Crataegus Fruit, stir-fried), Qing Pi (Green Tangerine Peel).
Certain case: Summer-Autumn damp-heat malaria-dysentery, healthy Qi deficient, pathogen remains, mixed into blood collaterals, forming Jia Nue Mu (Mass Malaria). Damp and Hot Qi are originally formless. Illegal medical treatment causes blood vessels to harbor pathogen, hence chills and fever alternate. Zhang Zhongjing's legislation emphasizes slow attack; urgency leads to Zhong Man (Middle Fullness). Be careful. Concurrently take Bie Jia Jian Wan (Turtle Shell Decoction Pill).
Prescription: Zhi Mu (Anemarrhena Rhizome), Cao Guo (Tsaoko Cardamom Fruit), Ban Xia (Pinellia Rhizome), Huang Qin (Scutellaria Root), Wu Mei (Smoked Plum), Sheng Jiang (Fresh Ginger). Decoct with Autumn Dew Water.
Jin (11 years old): Long-standing malaria, left hypochondriac Nue Mu (Malaria Mass) already formed. Pathogen has entered collaterals, congealed with Qi and Blood into shape. Mere exterior-interior dispersing drugs, how can they enter collaterals to unblock blood vessels and attack strongholds? Assist with Acrid-Aromatic, this is the main principle of Collateral diseases.
Prescription: Sheng Mu Li (Oyster Shell, raw, 3 qian), Gui Xu (Angelica Sinensis Root Tip, 2 qian), Tao Ren (Peach Kernel, 2 qian), Gui Zhi (Cinnamon Twig, 0.5 fen), Chao Shu Qi (Polygala Root, stir-fried, 1 qian), Gong Ding Xiang (Clove, 3 grains).
Certain case (38 years old): Shaoyin Three-Day Malaria for a long time, should raise Yang and warm meridians. (Three-Day Malaria Yang Deficiency)
Prescription: Lu Rong (Deer Antler Velvet), Shu Fu Zi (Aconite Lateral Root, prepared), Ren Shen (Ginseng Radix), Cu Gui Zhi (Cinnamon Twig, coarse), Dang Gui (Angelica Sinensis Root), Chao Hei Shu Qi (Polygala Root, stir-fried until black).
Lv (24 years old): Yin Malaria stopped only after one year, emaciated, hindered eating, food not transported after ingestion, no desire for soup/water, four limbs weak. Pulse is weak and faint, not drumming. Repeatedly administered Liu Jun Zi Tang (Six-Gentlemen Decoction) for Qi tonification without effect. Should warm interior and unblock Yang, following the idea of Fire generating Earth.
Prescription: Ren Shen (Ginseng Radix), Shu Fu Zi (Aconite Lateral Root, prepared), Sheng Yi Zhi Ren (Alpinia Oxyphylla Fructus, raw), Fu Shen (Poria with Hostwood), Bai Shao (White Peony Root), Sheng Jiang (Fresh Ginger).
Wu (41 years old): Recurrence after cure of Three-Day Malaria, much chills with sweating, exertion causes Yang leakage leading to malaria. Plan to protect Yang and repel pathogen.
Prescription: Chuan Gui Zhi (Cinnamon Twig), Shu Fu Zi (Aconite Lateral Root, prepared), Sheng Bai Zhu (Atractylodes Macrocephala Rhizome, raw), Zhi Gan Cao (Licorice Root, honey-fried), Sheng Jiang (Fresh Ginger), Nan Zao Rou (Chinese Date Flesh).
Certain case: Yin Malaria with much sweating, lower Jiao cold. Use Sheng Yang method (Raising Yang).
Prescription: Ren Shen (Ginseng Radix), Lu Rong (Deer Antler Velvet), Gui Zhi Mu (Cinnamon Twig Wood), Dang Gui (Angelica Sinensis Root), Zhi Gan Cao (Licorice Root, honey-fried), Sheng Jiang (Fresh Ginger), Da Zao (Chinese Date).
Lu (47 years old): Pathogen deeply retained in Yin, malaria manifests only after three days. But upon fever onset, confusion and delirium occur, caused by disturbance of Dan Zhong (Pericardium). Plan to administer Qing Xin Niu Huang Wan (Clear Heart Calculus Bovis Pill) once daily for two days before the expected onset, to test later stages. How will malaria present? (Pathogen Disturbing Heart Ying/Nutrient Level)
Cai (Family): Three-Day Malaria for over a year. Exertion triggers internal heat. Chronic knotting/fullness, now expanding and moving unpredictably, Qi rebellion with desire to vomit acid. Menstruation closed for four years. Should treat Jueyin and Yangming simultaneously. (Liver-Stomach)
Prescription: Ban Xia (Pinellia Rhizome), Chuan Lian (Coptis Rhizome), Gan Jiang (Dried Ginger), Wu Zhu Yu (Evodia Fruit), Fu Ling (Poria), Gui Zhi (Cinnamon Twig), Bai Shao (White Peony Root), Chuan Jiao (Sichuan Pepper), Wu Mei (Smoked Plum).
Cai (53 years old): Three-Day Malaria, no appetite, unable to receive food, nausea, thirst preferring hot drinks. Pulse deep and fine, Spleen Yang exhausted, unable to send pathogen outward. Treat with Si Shou Yin (Four Beast Decoction). (Spleen Yang Deficiency)
Shen (52 years old): Three-Day Malaria, abdominal distension, no thirst, vomiting water. Pathogen in collaterals of Spleen and Stomach. Warm and disperse interior pathogen, do not use exterior-dispersing methods.
Prescription: Cao Guo (Tsaoko Cardamom Fruit), Cu Gui Zhi (Cinnamon Twig, coarse), Sheng Jiang (Fresh Ginger), Nong Po (Magnolia Bark), Chao Shu Qi (Polygala Root, stir-fried), Fu Ling (Poria).
Again: Warming Spleen and unblocking Stomach achieved efficacy.
Prescription: Sheng Bai Zhu (Atractylodes Macrocephala Rhizome, raw), Dan Fu Zi (Aconite Lateral Root, mild), Chuan Gui Zhi (Cinnamon Twig), Chao Hei Shu Qi (Polygala Root, stir-fried until black), Nong Po (Magnolia Bark), Sheng Jiang (Fresh Ginger).
Certain case (45 years old): Three-Day Malaria for a year, recently re-exposed to damp pathogen. Upon onset these days, white tongue, epigastric stuffiness, thirst preferring hot drinks. Should warm Taiyin.
Prescription: Xing Ren (Apricot Seed), Cao Guo (Tsaoko Cardamom Fruit), Zhi Mu (Anemarrhena Rhizome), Gui Zhi (Cinnamon Twig), Ban Xia (Pinellia Rhizome), Sheng Jiang (Fresh Ginger), Nong Po (Magnolia Bark), Wu Mei (Smoked Plum).
Zhan (29 years old): Three-Day Malaria, Spleen manifestation. Use Lu Jiang Fa (Dew Ginger Method). Chills stop, heat prevails, add Wu Mei (Smoked Plum) 0.5 fen, taking its sour flavor to harmonize Yin. Because malaria is long-standing, Yin is also injured.
Hu: Yin Malaria, stagnation damages Spleen and Stomach. Bitter-Spicy-Warm achieved efficacy. Malaria not yet stopped, abdominal distension, loose stools. Discuss Li Zhong Yang (Regulating Middle Yang).
Prescription: Ren Shen (Ginseng Radix), Yi Zhi Ren (Alpinia Oxyphylla Fructus), Mu Xiang (Aucklandia Root), Fu Ling (Poria), Nong Po (Magnolia Bark), Guang Pi (Tangerine Peel), Sheng Jiang (Fresh Ginger).
Qian (35 years old): Upon exertion, malaria occurs for several years. Initially involved the Three Yin. This injury is already in the collaterals of Zang Yin, hardest to achieve quick effect. Sweet-Warm tonifies Qi, long-term administration of Yi Qi Tang (Augment Qi Decoction).
Certain case (43 years old): Early interruption of Three-Day Malaria, Middle Yang blocked, epigastric distension and failure to transport, back cold, four limbs cold.
Prescription: Cao Guo Ren (Tsaoko Cardamom Seed), Xing Ren (Apricot Seed), Ban Xia (Pinellia Rhizome), Fu Ling (Poria), Gui Zhi (Cinnamon Twig), Nong Po (Magnolia Bark), Guang Pi (Tangerine Peel), Sheng Jiang (Fresh Ginger).
Certain case: Pulse is soft, face yellow, tongue white, epigastric rejection, all soups/waters vomited. Three-Day Malaria occurs once. Based on color and pulse diagnosis, it is Foot Taiyin Yang micro-deficiency with fluid knotting. Should harmonize with warm medicines.
Prescription: Ban Xia (Pinellia Rhizome), Bi Bo (Long Pepper Fruit), Ding Xiang Bing (Clove Peduncle), Cao Kou (Lesser Cardamom Fruit), Nong Po (Magnolia Bark), Jiang Zhi (Ginger Juice).
Li: Initial illness fatigue afternoon fever, administered Dong Yuan's Yi Qi Tang (Augment Qi Decoction), not contrary to principle. But after taking soup, symptoms worsened, smell of grain qi offensive, alternating-day malaria comes, thirst desires cold drinks. This must be latent Summer-Heat pathogen, causing Ying and Wei circulation to touch the pathogen, resulting in chills and fever contention. Thus, Sweet-Warm tonifies Qi, raises Spleen Qi and Blood, differing from Summer-Heat. Stomach heat burns, Yang Earth becomes drier, Upper Epigastrium fails to receive, intestines bind, stool closes. Initially in meridians and Qi, later enters collaterals and Blood, from Yang entering Yin, alternating-day extends to Three-Day Malaria. Extraordinary vessels Qiao and Wei are all injured by pathogen. Nei Jing states Yang Wei disease suffers chills and fever. Wei is the framework of the body, thus starting from cold extremities. But still, it is a meridian-collateral disease. Thus, Shen, Zhu, Fu cannot solidify Yang to benefit deficiency; Gui, Gui, Di, Shao cannot nourish Ying to repel pathogen.
Ancient Xuanqi had the principle of puncturing malaria, deeply fearing pathogen mixing with Qi and Blood into one place, where sweating, vomiting, and purging cannot separate the pathogen. Later Han Zhang Zhongjing expanded the profound meaning of the Sacred Canon, stating if Malaria pathogen is unresolved for a month, pathogen will surely knot in blood, causing Nue Mu (Malaria Mass) accumulation. Formulated Bie Jia Jian Wan (Turtle Shell Decoction Pill). The main idea of the formula uses four types of insects/ants, implying those that fly ascend, those that walk descend, agile and rapid pursuing and extracting stagnant, mixed Qi and Blood pathogens. Scattering does not resolve, pathogen is not in exterior; attacking does not drive, pathogen is not fixed in interior. Tonifying healthy and repelling pathogen, establishing both healthy and pathogenic is useless. Thus, the Sage opens new perspectives to search out and eliminate pathogens mixed in collaterals. Treating hundreds of classics, historically verified. Taking twelve days completes one cycle of Heavenly Stems and Earthly Branches. If not fully successful, should raise Qi of Eight Extraordinary Vessels, returning from Extreme Yin to Yang position, without fail bringing peace. (Qi and Blood Congealing in Collaterals)
Certain case: Yin Malaria for two months, varying severity, left hypochondriac area sore and painful upon pressing. Pathogen latent in Jueyin Blood Collaterals, fearing formation of Nue Mu (Malaria Mass). Discuss Tong Luo (Unblocking Collaterals) to expel pathogen. Use Zhang Zhongjing's Bie Jia Jian Wan (Turtle Shell Decoction Pill). Take thirty pills every morning. Do not use on days of chills and fever.
Chang (24 years old): Three-Day Malaria all involve pathogen entering Yin Collaterals, thus sweating and purging are contraindicated. After a year, malaria stops, Jia Nue Mu (Mass Malaria) still gathers in lateral costal region. Pathogen attacks knotting of Qi and Blood, attacking and driving stasis accumulation, ascending and descending to unblock Yin and Yang, this is Zhang Zhongjing's established method. But pulse is fine and minute, food intake decreased, spirit faded. Applying attack methods again may disturb Middle Fullness. Previous warm tonification and Yang unblocking were quite safe. However, central tonification should be delayed appropriately. (Nue Mu)
Prescription: Ren Shen (Ginseng Radix), Dang Gui (Angelica Sinensis Root), Dan Fu Zi (Aconite Lateral Root, mild), Gan Jiang (Dried Ginger, mild), Fu Ling (Poria), Rou Gui (Cinnamon Bark), Bie Jia Jiao Wan (Turtle Shell Gelatin Pills).
All malarias are formed by latent pathogens, not caused by sudden factors. Among the Six Evils, only Dryness cannot cause harm, but New Cool contraction is truly relevant to the sage's teachings. Hand Three Yang and Hand Jueyin lack specific syndrome names. Physicians must distinguish which of the Six Qi is injured, and which of the Six Meridians is involved. If Xiao Chai Hu Tang (Minor Bupleurum Decoction) exclusively governs Shaoyang, how can it encompass all? Wen Nue (Warm Malaria), Dan Nue (Intense Malaria), Tan Shi (Phlegm-Food), Zhang Li (Miasma) various malarias all have established formulas, I will not elaborate. But this syndrome occurs occasionally in Spring and Winter, mostly in Summer-Autumn due to Summer-Heat and Dampness. Summer-Heat must carry Dampness, specifically injuring Qi aspect. First, distinguish causes in Upper and Middle Jiao, which of Summer-Heat or Dampness is heavier. If Summer-Heat is heavy, focus on clearing Upper Jiao Lung Qi. Upon malaria onset, heat is heavy and chills slight, lips and tongue must be crimson-red, irritability and thirst preferring cold drinks, drinking much without fullness. Pulse and color naturally show Yang superiority. Follow Gui Zhi Bai Hu Tang (Cinnamon Twig and White Tiger Decoction) and Tian Shui San (Heavenly Water Powder), adding Acrid-Cool agents for treatment. If Dampness is heavy, discuss Middle Jiao Spleen-Stomach Yang. Upon malaria onset, although heat steams intensely, tongue must have sticky coating, thirst prefers warm soup, chest and epigastrium feel full, distended, nauseous. Pulse and color naturally show Yang Qi discomfort. Follow Zheng Qi San (Upright Qi Powder) and Er Chen Tang (Two-Chen Decoction) removing Gan Cao (Licorice), adding Xing (Apricot Seed), Kou (Cardamom), Sheng Jiang (Fresh Ginger) etc. as main treatment. Must achieve Yang superiority over Yin, then formulate Yang-harmonizing agents, so later no lingering burden. If symptoms combine both, apply both methods. Generally, if pathogen is light and healthy Qi not very deficient, chills and fever are equal, with regular onset and cessation. If pathogen is heavy and healthy Qi timid, chills and fever are blurred, onset mixes indistinguishably. Also says: Pathogen shallow means daily发作; pathogen slightly deeper means alternating-day发作; pathogen deepest means three-day发作, anciently called Three-Yin Great Malaria. Key symptoms involve Liver, Spleen, Kidney organs. Tonification, purging, cold, warmth also do not depart from Zhang Zhongjing's treatment of Three Yin as foundation.可知阳经轻浅之方，治之无益也 (可知阳经轻浅之方，治之无益也 -可知阳经轻浅之方，治之无益也). It is said shifting early brings pathogen to Yang, shifting late sinks pathogen into Yin, Yin-Yang victory and repetition can be referenced here. If prolonged and unceasing, fear other syndromes. Taiyin deficiency floating distension has Tong Bu Zhi Li Zhong Fa (Unblocking and Tonifying Middle Regulator Method), Kai Fu Zhi Wu Ling Tang (Opening Bowels Five-Ling Decoction). Shaoyin atrophy becoming consumption has Zi Yin Zhi Fu Mai Tang (Nourishing Yin Restoring Pulse Decoction), Wen Yang Zhi Sheng Qi Fa (Warming Nourishing Rising Extraordinary Qi Method). Jueyin reversal vomiting roundworms, and pathogen knotting into Nue Mu (Malaria Mass), has Wu Mei Wan (Mume Pill) and Bie Jia Jian Fa (Turtle Shell Decoction Method). Also, if Heart Meridian malaria lingers, inevitably moves to affect Ying, causing irritability, thirst, seeing red. Lung Meridian malaria lingers, necessarily injures Jin, causing Stomach closure and Intestinal obstruction. One focuses on Cooling Yin, one focuses on Clearing Descending. However, malaria names are diverse, and accompanying syndromes numerous. Without adapting to changes, how can one be perfect in all? For example, Summer-Damp blocking Sanjiao with vomiting and inability to receive, follow Ban Xia Xie Xin Fa (Pinellia Draining Epigastrium Method). Filthy turbidity obscuring Dan Zhong with dull spirit, use Niu Huang Qing Xin Wan (Calculus Bovis Clear Heart Pill). Heart Yang sudden collapse, use Long Mu (Dragon Bone and Oyster Shell) rescue reversal. Stomach deficiency vomiting, use Xuan Fu Dai Zhe Tang (Inula and Hematite Decoction) established formula. Such as exterior dispersing and harmonizing, unblocking...
