天师说：如今人们认为痘疹（天花）难以治疗，是因为不知道正确的方法；如果掌握了法则，就没有什么困难了。在发病初期，无论患儿体质虚弱还是强壮，首先使用补气补血并辅助解毒的第一日方剂。
若小儿身体发热，眼睛看起来像喝醉了一样浑浊无神，这是出痘的征兆。如果不是这种“醉眼”状态，则不是出痘，不可使用此方，否则会被误用于治疗外感病。
第一日方剂组成：黄芪（三钱）、白术（一钱）、甘草（一钱）、当归（二钱）、川芎（二钱）、茯苓（三钱）、柴胡（一钱）、升麻（五分）、麦冬（二钱）、玄参（三钱）、陈皮（五分）、荆芥（一钱）、金银花（三钱）。
此方适用于五岁以上儿童，均按此剂量服用。五岁以下儿童减半，周岁以内的婴儿再大幅减少剂量。服用此药后，自然精神清爽。
第二日方剂组成：白术（二钱）、麦冬（三钱）、甘草（一钱）、桔梗（二钱）、当归（五钱）、生地（五钱）、玄参（二钱）、柴胡（一钱）、升麻（三分）、荆芥（一钱）、茯苓（二钱）、白芷（三钱）、白芥子（二钱）、金银花（三钱）。
服用此药后，全身都会出现痘点。痘色必定红润，而没有颜色发白或发黑的担忧了。
第三日方剂组成：人参（五分）、白芍（三钱）、白术（二钱）、茯苓（二钱）、玄参（二钱）、神曲（三钱）等。
服用此方后，痘疹全部灌浆，没有气血不足的情况，永远不会有退症（痘毒内陷）的担忧了。
第四日方剂组成：人参（一钱）、当归（二钱）、熟地（五钱）、茯苓（二钱）、金银花（三钱）、陈皮（五钱）等。煎服。
服用此方后，小儿脾胃功能必然强健，会不停地想吃东西。可以少量多次喂食，但也不可吃得过多。第五方不必再列出。
第五日方剂组成：人参（一钱）、茯苓（三钱）、白术（二钱）、甘草（一钱），水煎服。
如果有积食，加麦芽五分、山楂五粒；如果不伤食，不必添加，只加金银花三钱。
能够服用这五个方剂，期望在七日前就能康复。以上是根据患儿年龄制定的，年幼者都参照第一方减量。如小儿已经身热……

The Heavenly Master said: Nowadays, people consider smallpox (Doudou) difficult to treat because they do not know the correct method; if one masters the principles, there is no difficulty. At the initial stage of onset, regardless of whether the child's constitution is weak or strong, one should first administer the First Day Formula, which tonifies Qi and Blood and assists in detoxification.
If a child has a fever and their eyes appear cloudy and spiritless, resembling drunkenness, this is a sign that the rash is erupting. If it is not "drunken eyes," then it is not smallpox, and this formula must not be used; otherwise, it would be mistakenly used to treat external contraction diseases.
First Day Formula Composition: Huangqi (Astragali Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Danggui (Angelicae Sinensis Radix), Chuanxiong (Chuanxiong Rhizoma), Fuling (Poria), Chaihu (Bupleuri Radix), Shengma (Cimicifugae Rhizoma), Maidong (Ophiopogonis Radix), Xuanshen (Scrophulariae Radix), Chenpi (Citri Reticulatae Pericarpium), Jingjie (Schizonepetae Herba), Jinyinhua (Lonicerae Japonicae Flos).
This formula applies to children over five years old, using the stated dosages. For children under five, the dosage should be halved; for infants within their first year, the dosage should be further reduced significantly. After taking this medicine, the mind will naturally become clear and refreshed.
Second Day Formula Composition: Baizhu (Atractylodis Macrocephalae Rhizoma), Maidong (Ophiopogonis Radix), Gancao (Glycyrrhizae Radix et Rhizoma), Jiegeng (Platycodonis Radix), Danggui (Angelicae Sinensis Radix), Shengdi (Rehmanniae Radix), Xuanshen (Scrophulariae Radix), Chaihu (Bupleuri Radix), Shengma (Cimicifugae Rhizoma), Jingjie (Schizonepetae Herba), Fuling (Poria), Baizhi (Angelicae Dahuricae Radix), Baijiezhi (Brassicae Semen), Jinyinhua (Lonicerae Japonicae Flos).
After taking this medicine, spots will appear all over the body. The color will necessarily be red, eliminating any concern about white or black discoloration.
Third Day Formula Composition: Renshen (Ginseng Radix), Baishao (Paeoniae Radix Alba), Baizhu (Atractylodis Macrocephalae Rhizoma), Fuling (Poria), Xuanshen (Scrophulariae Radix), Shenqu (Massa Medicata Fermentata), etc.
After taking this formula, the pustules will fill completely with fluid. There will be no deficiency of Qi and Blood, and thus no fear of regression symptoms (internal collapse of the toxin).
Fourth Day Formula Composition: Renshen (Ginseng Radix), Danggui (Angelicae Sinensis Radix), Shudi (Rehmanniae Radix Preparata), Fuling (Poria), Jinyinhua (Lonicerae Japonicae Flos), Chenpi (Citri Reticulatae Pericarpium), etc. Decoct and take orally.
After taking this formula, the child's Spleen and Stomach function will inevitably strengthen, leading to an insatiable appetite. Food can be given in small amounts frequently, but one must not allow excessive eating. The Fifth Day Formula need not be listed separately.
Fifth Day Formula Composition: Renshen (Ginseng Radix), Fuling (Poria), Baizhu (Atractylodis Macrocephalae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), decocted in water and taken orally.
If there is food stagnation, add Maiya (Hordei Fructus Germinatus) 0.5 qian and Shanzha (Crataegi Fructus) 5 pieces; if there is no food stagnation, do not add these, but only add Jinyinhua (Lonicerae Japonicae Flos) 3 qian.
By being able to take these five formulas, recovery is expected before the seventh day. The above dosages are based on the child's age; younger children should refer to the First Day Formula for reduction. If the child already has a fever...
Additionally, the "Pediatrics" section contains two formulas for treating "Fire Erythema" (Huodan), which are absent in the "Shishi Milu." The "Formula Treatise" also includes formulas for "washing fetal toxins," "mouth ulcers," "night crying," "hematuria," and "cold accumulation and food stagnation."
