Human constitution is generated by heaven and earth, with the Heart as the master and the Kidney as the root. Once the Heart and Kidney are diseased, spirit diminishes and will scatters, giving rise to myriad diseases. Why does *Neijing · Tangye Laoliao Pian* say: "If spirit does not advance and will is not regulated, the disease cannot be cured"? It means that if essence is ruined and spirit departs, Ying (Nutritive Qi) and Wei (Defensive Qi) cannot be restored; when Ying stagnates and Wei dissipates, the disease cannot be cured. The Kidney stores essence and will, serving as the root of life, the root of Yuan Qi (Original Qi), and the dwelling place of spirit. As stated in *Ling Shu Jing*: "Accumulate essence to preserve spirit." Moreover, having essence allows for spirit. Now that spirit and essence are ruined, Heart and Kidney fail to support each other, so how can symptoms like night sweats and turbid discharge be absent? Essence is the fluid of the Kidney; sweat is the fluid of the Heart. The Heart stores Spirit (Shen); the Kidney stores Essence (Jing). When the Heart is diseased, Shen does not reside in its house, leading to daytime palpitations (Zhengchong), mental confusion, loss of will, vivid dreams at night, hallucinations of climbing heights or being chased, or sexual encounters with spirits, accompanied by night sweats. When the Kidney is diseased, the Sea of Marrow is depleted, leading to daytime fatigue, weak knees and legs, spermatorrhea, and white turbidity. All deficiencies and hundred damages stem from the Heart and Kidney. Furthermore, those who suffer from such conditions yet persist in pursuing wealth and indulging in lust, ignoring moderation and taboos, further damage Kan and Li (Kidney and Heart), resulting in emaciation and consumptive diseases like Laozai (Tuberculosis-like consumption). These include Five Labors, Six Extremes, Six Damages, Seven Injuries, Five Corpse Diseases, Nine Parasites, Ten Tuo Diseases, and various Guzheng (Bone-steaming) syndromes.
Five Labors refer to Zhi Lao (Will Labor), Si Lao (Thinking Labor), You Lao (Worry Labor), Xin Lao (Heart Labor), and Shou Lao (Emaciation Labor), which are diseases of the Five Zang Organs.
Six Extremes refer to Qi Extreme, Blood Extreme, Sinew Extreme, Bone Extreme, Flesh Extreme, and Jing Extreme, which are ailments of the Six Fu Organs.
Seven Injuries refer to injuries to the Heart, Liver, Spleen, Lung, and Kidney, both internal and external, or injuries to the Five Zang and bones/vessels.
Five Corpse Diseases refer to Fei Shi (Flying Corpse), Dun Shi (Escaping Corpse), Han Shi (Cold Corpse), Sang Shi (Funeral Corpse), and Shi Zhu (Corpse Tuo).
Nine Parasites refer to Fu Chong (Hidden Worm), Hui Chong (Roundworm), Bai Chong (White Worm), Rou Chong (Flesh Worm), Fei Chong (Lung Worm), Wei Chong (Stomach Worm), Ruo Chong (Weak Worm), Chi Chong (Red Worm), and Tuo Chong (Pinworm).
Ten Tuo Diseases refer to Feng Zhu (Wind Tuo), Qi Zhu (Qi Tuo), Han Zhu (Cold Tuo), Sheng Ren Zhu (Living Person Tuo), Liang Zhu (Cool Tuo), Jiu Zhu (Alcohol Tuo), Shi Zhu (Food Tuo), Shui Zhu (Water Tuo), Shi Zhu (Corpse Tuo), and Tu Zhu (Earth Tuo).
Various Guzheng refer to steaming sensations in the Heart, Liver, Spleen, Lung, Kidney, Gallbladder, Stomach, Large Intestine, Small Intestine, Bladder, Sanjiao (Triple Burner), Diaphragm, Zongjin (Major Sinews), Huichang (Returning Intestine), Yufang (Jade Chamber), Brain, Marrow, Sinews, Vessels, Flesh, and Blood.
However, regarding the causes and symptoms of these labors, extremes, corpse diseases, and steaming syndromes:
Holding fast to moral principles harms the Kidney, corresponding to Bone Extreme; worrying about future events harms the Lung, corresponding to Qi Extreme; unexpected thoughts harm the Spleen, corresponding to Flesh Extreme; strenuous planning harms the Liver, corresponding to Sinew Extreme; excessive mental exertion harms the Heart, corresponding to Pulse Extreme. Exhausting essence and spirit through labor generates various diseases; this is the correspondence between Five Labors and Six Extremes. "Labor" refers to exhaustion of Shen and Qi; "Injury" refers to damage to physical appearance.
Qi Extreme causes internal emptiness, deficiency of Five Zang, excess of pathogenic qi, deficiency of healthy qi, and reluctance to speak.
Blood Extreme causes lackluster complexion, hair and eyebrow loss, forgetfulness, and mental fogginess.
Sinew Extreme causes muscle spasms, pain in fingernails and toes, and inability to stand for long periods due to fatigue.
Bone Extreme causes bone soreness and wasting, toothache, painful limbs, and inability to move or walk.
Flesh Extreme causes emaciation, sallow complexion, and failure of food intake to nourish the body.
Jing Extreme causes panting, internal emptiness of Five Zang, qi deficiency, hair loss, and emotional instability (sadness and forgetfulness).
Six Damages:
Cold injury damages Yang; Yang deficiency leads to Yin excess, so damage proceeds from top to bottom.
Heat injury damages Yin; Yin deficiency leads to Yang excess, so damage proceeds from bottom to top.
Damage from top to bottom:
First damage affects the Lung, causing skin contraction and hair loss.
Second damage affects the Heart, causing blood and vessels to become deficient, failing to nourish the Zang organs.
Third damage affects the Stomach, causing food intake to fail to nourish the flesh. If damage exceeds this, the disease is incurable.
Damage from bottom to top:
First damage affects the Kidney, causing bone flaccidity and inability to get out of bed.
Second damage affects the Liver, causing sinew laxity and inability to control movement.
Third damage affects the Spleen, causing muscle wasting. If damage exceeds this, the condition is difficult to treat.
Thus, Lung and Heart damage cause poor complexion; Liver and Kidney damage cause bone flaccidity; Spleen and Stomach damage cause indigestion. This is called Six Damages.
Seven Injuries:
Dietary irregularities injure the Spleen; excessive thinking injures the Heart; excessive lust injures the Kidney; irregular lifestyle injures the Liver; excessive emotions (anger, joy, sorrow, worry) injure the Lung; external Wind, Cold, Summer-heat, and Dampness injure the exterior; hunger, fullness, and overwork injure the interior. This is called Seven Injuries.
Daytime exposure affects Ying (Nutritive) Qi; nighttime exposure affects Wei (Defensive) Qi. Ying and Wei circulate internally and externally, each fulfilling its function.
Five Corpse Diseases details:
Fei Shi (Flying Corpse): Appears suddenly without obvious cause, moving swiftly like flying insects, causing stabbing pain in the heart and abdomen, rapid breathing, distension, and upward rushing of qi to the chest and heart.
Dun Shi (Escaping Corpse): Lingers in muscles and blood vessels. Upon sudden provocation, it causes abdominal distension, stabbing pain, rapid breathing, pain attacking the hypochondrium, and upward rushing to the chest. It recurs after recovery and lingers unresolved.
Han Shi (Cold Corpse): Internal corpse worms interact with external pathogens to trigger attacks, causing chest and abdominal distension and stabbing pain. Attacks occur specifically upon exposure to cold in winter.
Sang Shi (Funeral Corpse): Occurs when vitality is weak and one visits places of death. Fear and disgust awaken internal corpse worms, which attract external pathogens. Symptoms include stabbing pain, distension, and rapid breathing, triggered by encountering funerals or death.
Shi Zhu (Corpse Tuo): Carries external evil qi, flowing through the body, causing alternating chills and fever, lethargy, unknown suffering, abdominal pain, distension, rapid breathing, pain attacking the hypochondrium, masses rising, back and spine cramping, heavy body, mental confusion, frequent dizziness. Worsens with solar terms. Progresses over years to death, then transmits to others, potentially wiping out families. Hence called "Corpse Tuo."
Nine Parasites details:
1. Fu Chong (Hidden Worm): 4 fen long, leader of worms.
2. Hui Chong (Roundworm): 1 chi long, piercing the heart kills.
3. Bai Chong (White Worm): 1 cun long, reproduces and grows up to 4-5 chi, also lethal.
4. Rou Chong (Flesh Worm): Looks like rotten apricot, causes irritability and fullness.
5. Fei Chong (Lung Worm): Looks like silkworm, causes cough.
6. Wei Chong (Stomach Worm): Looks like toad, causes vomiting and retching.
7. Ruo Chong (Weak Worm/Gedi Chong): Looks like melon seed, causes excessive salivation.
8. Chi Chong (Red Worm): Looks like raw meat, causes borborygmus.
9. Tuo Chong (Pinworm): Very small, resembles vegetable worm, resides in Huichang. Excess causes hemorrhoids; extreme cases cause leprosy, carbuncles, sores, tinea, and dental caries.
These parasites reside in the intestines. When Zang-Fu are full, they harm organs; when deficient, they erode organs, intestines, blood, and marrow, causing untreatable conditions. Before death, they exit through nine orifices and pores, infecting the weak, becoming continuous transmission of Tuberculosis-like consumption.
Ten Tuo Diseases details:
"Tuo" means "to pour/transfer," implying staying/residing. Caused by Yin-Yang imbalance, empty meridians, and invasion of Wind, Cold, Summer-heat, Dampness, dietary errors, labor, or spiritual influences. There are 36 types.
1. Feng Zhu (Wind Tuo): Skin twitching, wandering symptoms. After one year, hair falls, neck pain, bone cracking sounds, eye pain, nasal soreness, dental decay. Transforms into Warm Wind, Sweat Wind, Soft Water Wind, Great Wind, etc. After death, soul becomes wind/dust, infecting others.
2. Qi Zhu (Qi Tuo): [Text fragment missing/obscured] Body swelling comes and goes. After one year, body loses luster. After two years, bloody stool, vomiting green foam, heart pain, abdominal distension, stiff back. After three years, body pain, shifting牵引, twisting heart/hypochondrium. After one year, red eyes, dark complexion. After two years, cough, diarrhea, transforms into worm disease, hard to treat.
3. Han Zhu (Cold Tuo): [Text fragment missing/obscured]
4. Sheng Ren Zhu (Living Person Tuo): [Text fragment missing/obscured]
5. Liang Zhu (Cool Tuo): Alternating heat/cold below heart, recurring yearly. Limbs prefer lying down, belching sourness. Constant edema, intermittent symptoms. Black skin, emaciation, masses, yellow eyes, blue nails/lips.
6. Jiu Zhu (Alcohol Tuo): Body feels hot, qi moves up/down from chest, pain everywhere. After one year, heavy limbs, frequent retching.
7. Shi Zhu (Food Tuo): Pain below heart, anguish penetrating back. [Text fragment missing/obscured]
8. Shui Zhu (Water Tuo): Swollen hands/feet. After 100 days, flesh changes, hair falls, blindness. Hard to treat after one year. After three years, severe swelling, body produces worms, death.
9. Shi Zhu (Corpse Tuo): Body pain,牵引. After seven days, flesh turns white patches. Sensation of lump in throat, hypochondriac pain.
10. Tu Zhu (Earth Tuo): Earth corresponds to Spleen Qi. Violating taboos and soil energy interacts with blood/qi, causing disease. Earth qi flows into skin/organs, heavy joints, generalized edema that bursts spontaneously. Hence "Earth Tuo."
Steaming Syndromes (Guzheng):
In Heart: Shortness of breath, oppression, internal borborygmus, constipation or diarrhea.
In Liver: Blurred vision, dizziness, restlessness, fear.
In Gallbladder: Deafness, bitter taste, hardness/pain under hypochondrium.
In Kidney: Dry/crispy earlobes, sore waist/legs.
In Right Kidney: Unstable emotions, seminal emission like white cotton.
In Lung: Cough, wheezing, hemoptysis, hoarse voice.
In Large Intestine: Dry/painful right nostril, dull intestinal pain.
In Spleen: Dry lips/mouth, abdominal/hypochondriac distension, aversion to cold, anorexia.
In Stomach: Dry nose/mouth, abdominal distension, spontaneous sweating, restless sleep.
In Bladder: Yellow/red urine, turbid like paste.
In Sanjiao: Alternating cold/heat, oppression in Zhongwan/Shanzhong.
In Diaphragm: Chest/throat blockage, discomfort/pain.
In Zongjin: Flaccid sinews, urgent lower abdominal pain.

In Vessel: Heart vexation, body heat, helplessness, limb tremors.
In Blood: Dry/brittle hair, occasional epistaxis, hematuria.
This covers all forms of Guzheng.
Generally, if patients have intact blood/qi and undispersed spirit, treatment is possible. If blood/qi/spirit are ruined, muscles wasted, with daily evening fever, cough, and diarrhea, it is incurable. Even divine medicines would be ineffective.
