防风通圣散（出保命集）治一切风热郁结。气血蕴滞。筋脉拘挛。身倦。手
足麻痹。肢体焦痿。头痛目眩。腰脊强痛。耳鸣鼻塞。口苦舌干。咽噎不利。胸膈痞

解毒。三药合而饮之。日二服。若小便淋闭。去麻黄。加滑石、连翘。煎药汤调木香末二钱
匕。若腰胁痛。走疰疼痛。加硝石、当归、甘草。一服内各二钱。调车前子末。海金砂末各
一钱。 若破伤风。每一两加荆芥穗、大黄各二钱。调全蝎末一钱。羌活末一钱。 若诸风

砂。无芒硝。名贾同知通圣散。内经曰。胆移热于脑。则辛额鼻渊。鼻渊者。浊涕下不
已也。王注曰。胆液下澄。则为浊涕。下不已如水泉。故曰鼻渊也。此为足太阳脉与阳明脉

上磨瓦一小片作盖。钻一窍。可度得绿豆。用六一泥固济瓶子。火 令干。却入砂子末在瓶
内。按令平实。然后下盖子泥。合缝留窍子。候干用火半秤。四面约四寸已来。烧至一食顷。
更加火渐近瓶子。待黑气出尽。后取湿纸搭瓶窍上。如纸才干便易之。至三十易为止。待冷
取出细研。以酒浸润一宿。再焙为末。每石药末一两。入前药末二两。同研取匀。炼蜜丸如
鸡头实大。中风者。以豆淋酒研下一丸。至二丸。以浓衣服覆出汗。未汗更服。用热生姜稀
粥投。汗出慎外风。
大秦艽汤（出保命集）治中风。外无六经之形证。内无便溺之阻隔。知为血
弱。不能养于筋
秦艽 石膏（各二两） 甘草 川芎 当归 羌活 独活 防风 黄芩 白芍药 白术
吴白芷 生地黄 熟地黄 白茯苓（各一两） 细辛（半两） 上锉。每服一两。水二盏。
煎至一盏。去滓。温服无时。如遇天阴。加生姜七八片。如心下痞。每服一两内。加枳实一

备急黑神丸（出圣济总录）治中风。

即急抽火。令人按盖子。急泥合缝周遍。用净筛土。窨定药罐子。不得令露。透出药气。上
以大盆合之。来日取出药捣罗。用湿重帛包裹。以净湿土内窨盆。令出火毒。三日三夜。逐
日取盆微洒

同研匀。用青州大枣蒸熟。去皮核。研如膏。和药捣一千杵。丸如梧桐子大。中风急风牙关
急。口噤不开。用豆淋酒。生姜酒。去滓。研二丸至三丸。斡开口灌下。如人行五里更一服。
连三服。汗出解。宜食生姜酒。粟米粥。此药宜三月三日。五月五日。或腊月。于净室中焚

五积散（出如宜方）
陈皮（制六两） 麻黄（三两） 枳壳（六两） 白芍药 川芎 当归 甘草（炙） 茯

梗（十二两） 上 咀。慢火炒令色转。续入肉桂、枳壳、姜葱散。去麻黄名和气饮。入麝

（以上各二两） 薰陆香 龙脑（各研） 苏合油（各一两） 安息香（二两八钱为末用好
酒一升熬成膏将苏合油入之） 上末。入研药令匀。用安息香膏。同白蜜和成。临用时旋丸。
又擦牙关。
太乙赤丸（出圣济总录）治中风。积涎在膈下。四肢瘫痪。不知人事。
丹砂 硼砂 砂 铅白霜 粉霜 硫黄（细研各半两） 金箔 银箔（各十片与前六
味同研） 金牙（研） 紫石英（研） 天麻 羌活（去芦头） 独活（去芦头） 巴豆（去
心膜出油尽） 干牛膝（炒烟出各半两） 上为末。用黄蜡三两。熔作汁。拌诸药。乘热丸
如鸡头实大。用丹砂为衣。入瓷盒盛。每服一丸。用糯米饮半盏。龙脑腻粉各少许。薄荷自
然汁同化下。取下风

龟甲汤（出自《圣济总录》）治疗中风，手足不能随意运动，全身疼痛，或者筋脉挛急。
龟甲（醋炙） 虎骨（酥炙各六两） 海桐皮 羌活（去芦头） 丹参 独活（去芦头） 萆薢 牛膝（去苗，酒浸切焙） 五加皮 酸枣仁（炒制各三两） 附子（炮裂去皮脐） 天雄（炮裂去皮脐） 天麻（去蒂） 防风（去叉） 威灵仙（去土） 川芎（各二两半） 当归（切焙） 桂枝（去粗皮） 紫参（各三两） 薄荷（焙干六两） 槟榔（煨六两） 菖蒲（九节者去须，米泔水浸后切焙一两半）。将药材锉成麻豆大小。每次用八钱匕，加水一盏，酒一盏，生姜十片，同煎去渣，取一大盏，分三次温服。空腹、日午、夜卧时服用。若要出汗，连服两次。服药后人行走五里路，用熟生姜稀粥送服，穿厚衣服覆盖以出汗。注意避外风。
通神散（出自《圣济总录》）治疗中风昏愦，肢体不收摄，无论病情缓急。

Zhisheng Taiyi San (Sourced from *Shengji Zonglu*) Treats stroke resulting in body paralysis, hemiplegia, deviation of the mouth and eyes, speech impediment (dysarthria), mental confusion resembling drunkenness, palpitations with manic speech, restless sleep at night, or general numbness, skin insensitivity to pain and itch, inability to move limbs, heaviness of the body like stone, stiffness of waist and knees, spasms and convulsions preventing walking, blockage of all joints, stagnation of phlegm and saliva, sudden attack by evil qi, external pathogenic influences, ghostly qi and nightmares, corpse syncope, or sudden death.

If labor sweat encounters wind, causing acne-like lesions due to stagnation of sebum from pores: Remove Mangxiao, add Shaoyao and Danggui. If hives appear (red or white): Double Mahuang, Yanchi (Salted Soybeans), and Congbai to induce sweating. Remove Mahuang nodes and Mangxiao. If adding Siwu Tang and Huanglian Jiedu Tang to the base formula, drink the three medicines together, twice daily. For painful/difficult urination: Remove Mahuang, add Huashi and Lianqiao. Decoct and mix with 2 qianbi of Muxiang powder. For flank pain and wandering pain: Add Xiaoshi, Danggui, and Gancao (2 qian each per dose), mix with Cheqianzi powder and Haijinsha powder (1 qian each). Decoct for 5-7 boils. Take half warm, induce vomiting with a chopstick, then take the remaining half slightly warm. Sweating resolves the condition immediately. For stroke after drinking alcohol with severe headache: Add Huanglian Xu (2 qian) and Congbai (10 stems); follows the method for immediate cure. For dizziness, brain heat, nasal congestion with discharge: Add Bohe and Huanglian (2.5 qian each per liang). For wind injuring Lung with cough and wheezing: Add Banxia, Jiegeng, and Ziwan (2 qian each per liang). The first twelve ingredients are pulverized and kept separate. The last four are finely ground. Using a crucible, melt Liuhuang first, then add Zhusha, Shuiyin, and Xionghuang. Use strong fire to form cinnabar grains. Cool, scrape off, and pulverize. Take a porcelain bottle, grind a tile piece as a lid, drill a hole the size of a mung bean, seal with Liuyi mud, and dry over fire. Put the cinnabar powder inside, press flat, seal with mud leaving a small vent, dry, and fire with half a sheng of fuel around the bottle for about one meal time. Gradually bring fire closer until black gas exhausts. Cover the vent with wet paper, replacing when dry, for thirty changes. Cool, extract, and grind finely. Soak in wine overnight, dry, and grind again. For every 1 liang of this powder, mix with 2 liangs of the previous twelve-ingredient powder. Mix with refined honey to form pills the size of cockle seeds. For stroke, dissolve one to two pills in Doulin Wine, cover with thick clothes to sweat. If no sweat, take another dose, followed by hot ginger thin porridge. Avoid external wind after sweating.
Da Qin Jiao Tang (Sourced from *Baoming Ji*) Treats stroke without exterior Six Meridian symptoms or interior urinary/fecal obstruction, indicating Blood deficiency failing to nourish sinews.
秦艽 (Qinjiao) 石膏 (Shigao) [Each 2 liangs] 甘草 (Gancao) 川芎 (Chuanxiong) 当归 (Danggui) 羌活 (Qianghuo) 独活 (Duhuo) 防风 (Fangfeng) 黄芩 (Huangqin) 白芍药 (Baishaoyao) 白术 (Baizhu) 吴白芷 (Wu Baizhi) 生地黄 (Sheng Dihuang) 熟地黄 (Shu Dihuang) 白茯苓 (Baifuling) [Each 1 liang] 细辛 (Xixin, half liang). Chop herbs. Take 1 liang per dose, add two zhan of water, decoct to one zhan, remove dregs, and take warm at any time. On cloudy days, add 7-8 slices of fresh ginger. If epigastric fullness occurs, add 1 qian of Zhishi to each 1 liang dose and decoct together.
Beiji Heishen Wan (Sourced from *Shengji Zonglu*) Treats stroke.
雄黄 (Xionghuang, ground) 雌黄 (Cihuang, ground) 朱砂 (Zhusha, ground) 硫黄 (Liuhuang, ground) 水银 (Shuiyin - First melt Liuhuang in iron crucible over slow fire, pour in Shuiyin, stir quickly with fire chopsticks to prevent flames, extinguish with wet cloth, cool, scrape, and grind with the other four). Each of these five ingredients is 1 liang. Take a Hunan medicine firing jar, seal with Liuyi mud, dry, add the ground powders, press flat, seal four seams leaving only a 1-cun gap unsealed. Rhinoceros Horn (sliced, 1 liang) [Note: Substitute with Buffalo Horn] 牛黄 (Niuhuang, ground, half liang) 天竺黄 (Tianzhu Huang, ground) 鹿茸 (Lurong,酥-processed, de-haired, each 1 liang) 升麻 (Shengma) 天麻 (Tianma) [Each 1 liang] 木香 (Muxiang, half liang) 干蝎 (Ganxie, wine-stir-fried, 1 liang) 阿胶 (Ejiao, slow-fire roasted dry, 2 liangs) 天南星 (Tiannanxing, boiled in cow bile for one chu, sun-dried, 1 liang). The latter ten ingredients (excluding those already ground) are pulverized. Then add Niuhuang, Tianzhu Huang, and the previously ground five ingredients, mixing thoroughly. Use Qingzhou dates steamed ripe, peeled, pitted, and mashed into paste. Mix with medicine and pound 1000 times. Form pills the size of Sophora seeds. For acute stroke with jaw stiffness and lockjaw, use Doulin Wine and Shengjiang Wine, remove dregs, grind 2-3 pills, pry open mouth, and administer. After walking five li, take another dose. Three consecutive doses resolve symptoms with sweating. Consume ginger wine and millet porridge. This medicine should be prepared on the 3rd day of the 3rd lunar month, 5th day of the 5th lunar month, or in the 12th lunar month, in a clean room with incense, fasting, and purification. 陈皮 (Chenpi, processed, 6 liangs) 麻黄 (Mahuang, 3 liangs) 枳壳 (Zhiqiao, 6 liangs) 白芍药 (Baishaoyao) 川芎 (Chuanxiong) 当归 (Danggui) 甘草 (Gancao, honey-fried) 茯苓 (Fuling) 半夏 (Banxia, processed) 肉桂 (Rougui) 白芷 (Baizhi) [Each 3 liangs] 浓朴 (N Hou) 干姜 (Ganjiang) [Each 4 liangs] 苍术 (Cangzhu, processed, 24 liangs) 桔梗 (Jiegeng, 12 liangs). Chop herbs, stir-fry slowly until color changes, then add Rougui, Zhiqiao, Jiang, and Cong. Removing Mahuang names it Heqi Yin. Adding Shexiang constitutes another variation; one formula adds only Ruxiang.

白术 (Baizhu) 青木香 (Qingmuxiang) 朱砂 (Zhusha) 乌犀屑 (Wuxi Xie) [Note: Substitute with Buffalo Horn] 沉香 (Chenxiang) 麝香 (Shexiang) 诃黎勒 (Helile) 丁香 (Dingxiang) 荜茇 (Bibo) 白檀香 (Baitanxiang) 香附子 (Xiangfuzi) [Each 2 liangs] 薰陆香 (Xunluxiang) 龙脑 (Longnao) [Grind separately] 苏合油 (Suheyóu) [Each 1 liang] 安息香 (Anshixiang, 2.8 qian, powdered, boiled in 1 sheng good wine to make paste, mix with Suheyóu). Process into powder, mix with ground ingredients evenly. Use Anshixiang paste and white honey to form pills. Prepare fresh for use. Also rub on jaw joints.
Taiyi Chi Wan (Sourced from *Shengji Zonglu*) Treats stroke with phlegm accumulation below the diaphragm, limb paralysis, and unconsciousness.
