Those whose typhoid fever transforms into malaria do so because, after the initial illness has subsided, external pathogens have not yet dissipated and the true qi of the internal organs remains deficient. Subsequent overexertion causes a struggle between yin and yang. When yin prevails, chills occur; when yang prevails, fever occurs. The predominance of either yin or yang results in alternating chills and fever with regular intervals of remission. If it occurs twice a day, sweating will lead to resolution. If, after eight or nine days of typhoid fever, there is more heat than cold, no vomiting, and normal bowel and bladder function, self-healing is anticipated. Alternatively, if symptoms of Yangming disease appear—such as irritability, fever, sweating, and afternoon (Ri Bu) fever—a floating pulse indicates the need for exterior-releasing methods, while a solid pulse indicates the need for interior-purging methods. In women, if heat enters the Blood Chamber (Xue Shi), causing fever with alternating chills, careful differentiation is required.
The verse states: For malaria-like conditions involving the Bladder meridian, Guizhi Tang (Cinnamon Twig Decoction) can be used; if bowel/bladder functions are normal and there is no vomiting, Banxia (Pinellia Rhizome), Mahuang (Ephedra Herb), and Guizhi (Cinnamon Twig) may be used; for Yangming excess requiring purgation, exterior release is needed for a floating pulse; for heat entering the Blood Chamber, Xiaochaihu Tang (Minor Bupleurum Decoction) yields miraculous results.
"Malaria-like" (Si Nüe), also known as "malaria pattern," refers to episodes that start and stop at fixed times, unlike typical alternating chills and fever which may vary in frequency or duration without regularity. In cases of common cold where one suddenly feels cold hair, trembling thighs, muscle and joint spasms, whole-body tremors, nausea, and loss of appetite, followed by intense chills that cannot be resisted and then turning into fever, this is termed Warm Malaria (Wen Nüe). One should not assume that all malaria presents with a string-taut (Xian) pulse; whether the pulse is surging-rapid (Hong Shu), tight-solid (Jin Shi), deficiency-slow (Xu Huan), or scraping-rough (Gua Se), it may indicate a malaria pattern, distinguished by external signs. While there are standard treatments, modifying Xiaochaihu Tang (Minor Bupleurum Decoction) according to these principles is also a flexible approach. Although blood deficiency can generate chills and fever, and stagnant blood can also cause them, and despite different formulas for each aspect of yin-yang conflict, Chuanxiong Rhizome (Chuanxiong) should generally serve as an assistant herb.
For Taiyang (Greater Yang) malaria-like patterns with a surging-floating pulse, administer Guizhi Tang (Cinnamon Twig Decoction); if bowel/bladder functions are normal, no vomiting, and attacks occur once or twice daily, Guizhi Mahuang Ge Ban Tang (Cinnamon Twig and Ephedra Half-Decoction) is indicated; a slightly slow and slightly floating pulse suggests impending recovery. For Yangming (Bright Yang) malaria-like patterns with irritability, fever, sweating, and afternoon fever, a floating-deficient pulse warrants Guizhi Tang (Cinnamon Twig Decoction), while a solid pulse warrants Chengqi Tang (Bowel-Purging Decoction). For women with heat entering the Blood Chamber, blood coagulation likely occurs, presenting like malaria; Xiaochaihu Tang (Minor Bupleurum Decoction) is the primary treatment. For post-malaria chills and fever, or tidal fever, accompanied by lethargy after recovery, Erchen Tang (Two-Aged Decoction) is universally applicable for regulating various malaria types. Add Chuanxiong Rhizome (Chuanxiong) and Qianhu (Peucedani Radix) for more heat; add Chuanxiong Rhizome (Chuanxiong) and Caoguo (Tsaoko Fruit) for more cold.
