The Yellow Emperor asked: What should be prohibited in the treatment of heat diseases? Qibo replied: When a heat disease is slightly improving, eating meat will cause a relapse, and overeating will leave residual pathogens. This is what needs to be prohibited. Because although the heat disease has slightly improved, it has not been completely cleared, if one does not observe dietary restrictions, labor moderation, and emotional desires, due to the deficiency of Spleen and Stomach Qi, which cannot digest solid food, the heat pathogen will arise again. All five Zang organs can have heat syndromes. The so-called heat disease belongs to the category of Cold Damage (Shanghan). When humans are injured by cold pathogens, they develop heat diseases. Cold toxins invade the skin and muscles, Yang Qi fails to disperse, leading to internal stagnation and knotting; thus, Cold Damage manifests as heat. Even if the heat is severe, it will not cause death. The Great Yang Meridian (Foot-Taiyang Bladder Meridian) governs all Yang meridians. All Yang meridians govern Qi and belong to Heat, while all Yin meridians govern Blood and belong to Cold. After a heat disease heals, if it recurs, it means that although the pathogenic factor has declined, residual heat remains latent within the body. Due to the interaction between postprandial grain Qi and residual heat, the two heats combine, resulting in lingering heat. This is because heat pathogens still exist, so the disease may recur. One must observe conditions where all five Zang organs have heat: Liver heat causes redness on the left cheek first; Heart heat causes redness on the face first; Spleen heat causes redness on the nose tip first; Lung heat causes redness on the right cheek first; Kidney heat causes redness on the chin first. In Liver heat, urine is yellow, there is abdominal pain, lethargy, and fever; when heat pathogens contend fiercely, mania,惊悸 (palpitations/fright), and hypochondriac fullness occur; hands and feet are dry, and one cannot lie down peacefully. In Heart heat, mood is low, fever appears after several days; when heat pathogens contend fiercely, sudden heart pain, vexation, vomiting, headache, and facial redness occur. In Lung heat, there is chills with shivering, goosebumps, aversion to wind and cold, yellow tongue coating, and fever; when heat pathogens contend fiercely, wheezing and coughing, chest and back pain, difficulty breathing, severe headache, and chills after sweating occur. In Kidney heat, there is lower back pain and soreness, bitter taste, thirst with frequent drinking, and fever; when heat pathogens contend fiercely, neck pain and stiffness, alternating chills and soreness, hot soles of feet, and reluctance to speak occur. Medical classics state: Sweating with rapid and restless pulse is a fatal sign (Death 1); pulse inconsistent with sweating is a fatal sign (Death 2); delirium and loss of spirit are a fatal sign (Death 3). These are all due to heat pathogens being stagnant and unable to dissipate, leading to critical conditions. *Suwen* states: Various symptoms in heat diseases such as visual confusion, convulsions, sudden aphonia, coma, restlessness, mania, cursing, fright, swelling, pain, soreness, Qi rebellion, all belong to the Sanjiao Minister Fire of the Hand-Shaoyang meridian and the Qi mechanism of the Pericardium. The True Yin of the Kidney is originally deficient, and the Minister Fire of the Heart has accumulated excessive heat, leading to stagnation of Wind-Heat, causing dizziness of the head and face, numbness of limbs, itching of the skin, spasms and fatigue of sinews and vessels, stifling sensation in the chest and diaphragm, occasional pain and oppression, nasal congestion, rhinorrhea or epistaxis, mouth and tongue ulcers, throat discomfort, dental caries, generalized skin sores, tinea, scabies, bruxism during sleep, startle responses, and spontaneous sweating, forgetfulness and palpitations, irritability and hypersomnia, obstruction of urination and defecation, vexatious heat and abdominal fullness, accumulation of toxins from excessive alcohol consumption, exhaustion from overwork, various internal and external consumptive injuries, manic spirit, shortness of Qi, restless mind, bitter taste and dry throat, reduced appetite, transformation into various Wind-Heat diseases, emaciation and fatigue, wasting diseases from alcohol, frailty in the elderly, deficiency of Spleen and Kidney meridians, stagnation of Wind-Heat and Dryness, dark complexion, dry teeth, emaciation and atrophy, stomach heat with vexation and fullness, hunger without desire to eat, Damp-Heat transforming into Xiao Zhong (Middle Consumptive Disease) characterized by polyphagia but emaciation, or Xiao Ke (Wasting-Thirst) with dizziness and frequent urination, or excessive use of sweet, acrid, and hot medicines transforming into Upper, Middle, and Lower San Xiao (Three Consumptions). Upper Xiao presents as Thirst Consumptive Disease, Middle Xiao as Middle Consumptive Disease, Lower Xiao as...
Anyone suffering from severe heat must examine the pulse. For severe heat, one cannot strictly adhere to fixed formulas but must treat according to the specific condition. If the heat is unbearable, the dosage should be doubled, tripled, or even tenfold to counteract it. Some suffer from persistent heat caused by taking mineral drugs (Stone Drugs); various herbal preparations fail to control it, only Decoction of Natrii Sulfas (Puxiao Jian) can settle it. During the Wude era, a noble master was known in the market as "Jinshi Ling," which is incorrect. This formula uses only the Two Nitrates (Natrii Sulfas and Natrii Nitras), which are cold in nature.
If there is a Exterior syndrome accompanied by heat, it is called Exterior Heat; if there is no Exterior syndrome but heat exists, it is called Interior Heat. Sudden heat diseases involve the Heart and Lungs; accumulated heat diseases involve the Liver and Kidneys. To treat sudden heat, use Jufang Xionghuang Jiedu Wan (Realgar Detoxification Pill). To treat accumulated heat, use Jufang Miaoxiang Wan (Wonderful Incense Pill). Sudden heat with upper asthma involving Heart and Lungs is called High Asthma, treated with Muxiang Jinling San (Costus and Goldbell Powder). Heat in the Upper Jiao with vexation is treated with Dahuang San (Rhubarb Powder). Constipation of the Zang-Fu organs is treated with Niuhuang San (Calculus Bovis Powder). Heat in the Upper Jiao without other symptoms is treated with Jiegeng Tang (Platycodon Decoction). Chronic illness with emaciation, fever, and night sweats is called Damage of All Five Zang Organs, a disease of Heat Labor and Bone Steaming. Emaciation, weakness, false vexation, dysentery with blood in stool are all signs of Steaming Labor. The treatment method should nourish Blood and benefit Yin, allowing the heat to recede naturally; this is called treating without direct treatment, using Qian's Dihuang Wan (Rehmannia Pill). Fried foods made from wine and flour...
The key lies in balancing cold and heat. Overcorrection is a concern for the wise. So, how to remove accumulated heat? The answer is Sanhuang Tang Wan (Three Yellows Decoction Pill) is the primary medicine. Only extreme heat injures Blood; since heat originates from the Heart, Xixin San (Heart-Washing Powder) is indispensable. Since heat can injure Blood, Sishun Qingliang Yin (Four-Obedience Cooling Drink) is also necessary. This is a theory rooted in the fundamental principles. Foods like doughnuts after drinking and alcohol after meals easily accumulate heat; prevention is better than cure. The classics state: All cases of twisting and contracture with turbid urine belong to Heat; all cases of dryness, mania, and excess belong to Fire. Internal deficiency with tidal dryness, delirium, abdominal knotting with difficult defecation, external attack leading to swelling, toxic sores, red eyes, blockage of sores in the throat, and flaming upward heat are treated with cold-cooling formulas.
