This syndrome and conditions such as Xiao Ke (Wasting and Thirsting Syndrome), Chi Bai Zhuo (Red and White Turbid Urine), and frequent urination have similar treatments, and their formulas should be used with mutual consideration.
Pulse Diagnosis: For Yi Jing (Spermatorrhea) and Bai Zhuo (White Turbid Urine), the pulse is often Se (Choppy), which indicates damage to Jing (Essence). (*Ji Sheng Fang*)
For Yi Jing (Spermatorrhea) and Bai Zhuo (White Turbid Urine), verification should be done at the Chi (Cun Guan Chi - distal position on both wrists) pulse. The presence of Jie (Knotted), Kou (Hollow), Dong (Fluttering), or Jin (Tight) pulses are definitive signs of these two conditions. (*Cui Shi Mai Jue*)
For Yi Jing (Spermatorrhea) and Bai Zhuo (White Turbid Urine), a Wei Se (Slight Choppy) and Ruo (Weak) pulse indicates Fire Excess and Yin Deficiency; Kou (Hollow), Ru (Floating and Soft), Hong (Surging), or Shu (Rapid) pulses may also be seen. (*Si Yan Ju Yao*)
The disease belongs to the Heart and Kidney. *Huang Di Nei Jing* states: "Endless thinking, unfulfilled desires, lustful thoughts directed outward, and excessive sexual intercourse lead to flaccidity and relaxation of the Zong Jin (Sinews), resulting in Jin Wei (Sinew Flaccidity) and Bai Yin (White Sperm/Loss)."
The Kidney stores Tian Yi (Primordial Essence) and acts with parsimony. When will and intention are regulated internally, Jing (Essence) remains complete and is retained sparingly. When thoughts wander externally into lust and sexual activity is excessive, what was firm becomes unstable. Thus, Jing cannot be guarded and flows down with urine. However, the root lies in Jin Wei (Sinew Flaccidity) due to the flaccidity and relaxation of the Zong Jin (Sinews). (*Sheng Ji Zong Lu*) [Note: Bai Yin refers to white discharge resembling semen, flowing out with urine in men, and continuously from the vagina in women. Xian Jiaoyu stated: *Ling Shu · Ben Shen Pian* says, "When Jing is damaged, bones ache, there is flaccidity and coldness in the limbs, and Jing leaks spontaneously." *Su Wen · Yu Ji Zhen Zang Lun* says, "Discharge of white fluid is called Gu." This is the meaning. Thus, Bai Yin is essentially sperm in urine. The explanation in *Ben Shi Fang* seems correct, so it is listed first.]
Medical classics state: "At age sixteen, a man's Kidney Qi flourishes, and Tian Gui arrives. Tian Gui is Jing (Essence); Jing is the foundation of the body. The Kidneys store Jing, and the function of storing Jing must not be injured. All this arises from improper health preservation: irregular emotions, inappropriate labor and rest, worry and overthinking, excessive desires, and irregular daily routines. This causes Heart Fire to blaze upward without ceasing, and Kidney Water to scatter without returning, preventing mutual nourishment between upper and lower. Disease in the Heart causes Yi Jing (Spermatorrhea) and Bai Zhuo (White Turbid Urine); disease in the Kidney also causes Yi Jing (Spermatorrhea) and Bai Zhuo (White Turbid Urine). These are all caused by the failure of Heart-Kidney interaction and an insecure key mechanism. Kidney disease requires securing and consolidating; Heart disease requires calming and tranquilizing." (*Ji Sheng Fang*)
Meng Yi (Dream Emission) and Jing Hua (Slippery Emission) both belong to Fire, but differ in involving the Heart or Kidney. When moved by the Heart, spirit trembles above, causing essence to leak below. There is no need to treat the Kidney; one only needs to clear the Heart, and dreams will cease naturally. Although the storage of Jing is in the Kidney, its master is the Heart. Therefore, clever young people often suffer from this, while rustic country folk do not; it all depends on the movement or stillness of the Heart. (*Jin Kui Yi*)
Etiology due to Kidney Deficiency: Xu Lao Niao Jing (Deficiency Consumption with Sperm in Urine) syndrome is due to weak Kidney Qi. The Kidneys store Jing, and their Qi connects to the external genitalia. Damage from overwork leads to Kidney deficiency, inability to store Jing, thus causing seminal fluid to exit with urine. (*Bing Yuan Lun*)
Xu Fang Yi Jing Jian Wen Jing Chu (Deficiency Formula for Overflowing Essence upon Seeing/Hearing): Due to weak Kidney Qi, essence overflows. Seeing, hearing, or being touched agitates the Kidney Qi. The Kidneys store Jing; now, due to deficiency, they cannot control the essence, so it overflows upon sensory stimulation.
Xu Lao Shi Jing (Deficiency Consumption with Loss of Essence): Kidney Qi is deficient and damaged, unable to store Jing, causing leakage. Symptoms include tightness and urgency in the lower abdomen, coldness at the glans penis, pain in the eye sockets, hair loss, and a pulse that is Rapid and Scattered, which is the pulse of lost essence.
Xu Lao Meng Xie Jing (Deficiency Consumption with Dream Emission): Kidney deficiency allows pathogenic factors to invade; pathogens lodge in the Yin, causing dreams of sexual intercourse. The Kidneys store Jing; now, due to Kidney deficiency, they cannot control the essence, leading to emission stirred by dreams.
Xu Lao Jing Xue Chu (Deficiency Consumption with Essence and Blood Discharge): This is due to injury of Kidney Qi by overwork. The Kidneys store Jing, and Jing is formed from Blood. Deficiency consumption generates Seven Injuries and Six Extremes, damaging both Qi and Blood. The Kidney family is particularly deficient and cannot store Jing, causing both essence and blood to discharge. (All above five items: although symptoms differ, the cause is the same, so they are listed together.) (Same source)
Dai Yun said: Emission of essence due to dreaming of sexual intercourse is called Meng Yi (Dream Emission). Emission of essence without dreaming is called Jing Hua (Slippery Emission). Both are moved by Ministerial Fire; long-term cases involve only deficiency without cold. (*Gou Xuan*)
There are several aspects to the disease. Medical texts state that dream emission is due to Kidney deficiency, suggesting only tonifying the Kidney and astringing essence, yet this has rarely been effective. I argue this disease has three patterns: First, extreme deficiency where the Kidney cannot grasp essence and the Heart cannot grasp thoughts, resulting in emission with or without dreams; these cases are severe and require large doses of tonic medicine, but few people suffer from this. The rest are merely Heart deficiency or Heart Heat, where emotional triggers cause dream emission; these cases are milder, and most patients fall into this category, healing simply with Fu Ling San (Poria Powder). I have used this repeatedly to save people with excellent results. Secondly, young people with vigorous Qi, or widowers/monks who forcibly suppress desires, may emit due to mental arousal; this is considered non-pathological. Doctors and health preservers often claim dream emission is worse than sexual fatigue, which is likely incorrect. I have verified that people who engage in sexual activity before recovering from epidemic fevers often die, whereas dream emission never causes such distress; this is certain, though dream emission varies in severity. (*Su Shen Liang Fang*)
There are several types of Meng Yi (Dream Emission): For lower origin deficiency and exhaustion with inability to control essence, Hui Xiang Wan (Fennel Pill) is suitable. For those in their prime with vigorous Qi who have long abstained from sex, causing meridian stagnation, Qing Xin Wan (Clear Heart Pill) is suitable. For those with desire stirring within, described in the classics as "unfulfilled desires" named Bai Yin (White Sperm), Liang Fang Fu Ling San (Good Formula Poria Powder) is suitable. It is like boiling soup in a bottle: if Qi is full and overflowing, it spills like boiling soup; if desire stirs the heart, it spills like a tilted bottle; if deficient and exhausted, it leaks like a cracked bottle. Medication cannot be applied uniformly. Another theory states: The classics say, "If Kidney Qi is closed, essence leaks." *Su Wen* says, "The Kidneys are the officials of strength and vigor, producing skill and ingenuity." It also says, "Kidney Qi stores essence." Essentially, the Kidneys control essence and Qi for human reproduction, governing both retention and dispersion. Now, if Kidney Qi is blocked, the essence and Qi of the whole body lack control, wandering and exiting irregularly. Zhu Ling Wan (Polyporus Pill) is specifically designed for this. (*Ben Shi Fang*)
Yi Jing (Spermatorrhea) has four causes: 1. Overuse of the mind, where the Heart fails to control the Kidney, leading to loss of essence; 2. Unfulfilled lustful thoughts, causing essence to lose its position and discharge; 3. Excessive sexual desire, leading to slippery and uncontrollable emission; 4. Youthful vigor and abundance, with long abstinence, causing fullness and overflow of essence. However, the manifestations vary. (*Yao Jue*) [Note: There is also a meridian heat pattern, see under Qing Xin Wan. Ming dynasty physicians added more causes, which are slightly redundant, so they are not cited extensively.]
More than half of Meng Yi (Dream Emission) cases belong to Stagnation. Quack doctors, unaware of the stagnation, use astringents like Long Gu (Dragon Bone) and Mu Li (Oyster Shell) to secure collapse, not realizing that excessive astringency worsens the stagnation and aggravates the disease. I once treated a middle-aged man with Meng Yi (Dream Emission); after using astringent medicines, his condition worsened, with emissions occurring for several nights consecutively. I first administered Shen Xiong Wan (Divine Celery Pill - containing Da Huang, Huang Qin, Huang Lian, Qian Niu Zi, Hua Shi, Bo He, Xiong Diao) to purge heavily, then prescribed Zhu Ling Wan (Polyporus Pill), resulting in recovery. This demonstrates that many Meng Yi (Dream Emission) cases belong to stagnation. (*Ben Cao Gang Mu*) (Excerpt)
Various theories on syndrome differentiation and treatment. As mentioned, Yi Jing (Spermatorrhea) has four causes, but manifestations vary. Some experience uncontrollable discharge after urination; some discharge spontaneously without urinating; some have itching and pain at the urethral meatus with a constant urge to urinate. All these should initially use Chen Sha Miao Xiang San (Cinnabar Wonderful Fragrance Powder), swallowing Yu Hua Bai Dan (Jade Flower White Pill - containing Bai Shi Zhi, Mu Li, Yang Qi Shi, Zhong Ru), assisted by Wei Xi Wan (Wei Xi Pill) or Li Qing Yin (Clarify Clear Decoction), and separately decocting Long Gu (Dragon Bone) wrapped in cotton. Alternatively, take half a dose of Li Qing Yin (Clarify Clear Decoction), adding Wu Bei Zi, Mu Li powder, Bai Fu Ling, and Wu Wei Zi, each half qian. If caused by overuse of the mind, use Yuan Zhi Wan (Polygala Pill - containing Er Fu, Ren Shen, Long Chi, Yuan Zhi, Chang Pu), taking Jiao Gan Tang (Interacting Decoction - likely Jiao Gan Wan made into decoction, containing Sheng Di, Dang Gui, Yan Hu Suo, Sheng Jiang, Pu Huang, Hu Po) with Lian Rou and Wu Wei Zi swallowed, assisted by Ling Sha Dan (Spirit Cinnabar Pill). If confirmed as caused by unfulfilled lustful thoughts, use Si Qi Tang (Four Seven Decoction) to swallow Bai Wan Zi (White Pills). In severe cases where hearing or seeing triggers emission, named Bai Yin (White Sperm), use Miao Xiang San (Wonderful Fragrance Powder) swallowing Yu Hua Bai Dan (Jade Flower White Pill). If confirmed as excessive sexual desire, lower origin deficiency and exhaustion, with slippery and uncontrollable emission, use Zheng Yuan Yin (Correct Origin Decoction - see *He Ji*) adding Mu Li powder and Rou Cong Rong, each half qian, swallowing Yang Qi Dan (Nourish Qi Pill - see *He Ji*) or Ling Sha Dan (Spirit Cinnabar Pill), assisted by Lu Rong Wan (Deer Antler Pill - see *Ji Sheng*), Shan Yao Wan (Dioscorea Pill), Da Tu Si Zi Wan (Large Dodder Pill), Gu Yang Wan (Secure Yang Pill - all see *He Ji*) etc. [Note: *Zhun Sheng* states: These medicines are too aggressive and drying; if misused in excess, Yin fluids will be depleted, ministerial fire will blaze alone, flesh will waste away, bones will stand out, sinews will flaccid, becoming incurable. Users must be cautious.] If confirmed as youthful fullness and overflow, use *Ben Shi Fang* Qing Xin Wan (Clear Heart Pill), still taking Huang Qi Liu Yi Tang (Astragalus Six-to-One Decoction - attached to *Han Zheng*), although the formula is not perfectly matched, it addresses the overflow of essence and marrow by using Atractylodes to dry dampness. Yi Li (Residual Dribbling) is slightly less than Yi Jing (Spermatorrhea); a few drops of thick sticky fluid appear during urination, slight pain at the glans, or one or two drops appear shortly after urination stops. The etiology does not exceed the previously mentioned causes; carefully apply the previous medicines. Meng Yi (Dream Emission), commonly known as night dreams of ghost intercourse, should use Wen Dan Tang (Warm Gallbladder Decoction) removing Zhu Ru, adding Ren Shen, Yuan Zhi, Lian Rou, Chao Suan Zao Ren, and Fu Shen, each half qian, swallowing Yu Hua Bai Dan (Jade Flower White Pill) and Gu Yang Wan (Secure Yang Pill). Meng Yi (Dream Emission) also covers the previous four syndromes; determine the trigger and use the previous medicines. (*Yao Jue*)
Meng Yi (Dream Emission) belongs to Liver Heat and Gallbladder Cold. Liver Heat causes fire to overflow externally, Hun (Ethereal Soul) fails to remain internal, leading to many erotic dreams and loss of essence. Palpitations may occur; this is common in obese people. Treat by clearing the Liver, no need to tonify the Kidney. Use Wen Dan Tang (Warm Gallbladder Decoction) adding Ren Shen, Fu Shen, Zao Ren, and Lian Rou. (*Yi Tong*)
Zhang Zhongjing treated restless heat in hands and feet, dry throat and mouth, or palpitations and epistaxis. This is Yang rising without descending, Yin dwelling exclusively above, causing Meng Yi (Dream Emission). Use Xiao Jian Zhong Tang (Minor Construct the Middle Decoction) to harmonize. This is misunderstood by the世俗 (common world). (*Sha Zhao*) [Note: This theory originates from *Zhun Sheng*.]
For Yi Jing (Spermatorrhea) without dreams, due to insecure Kidney gate and slipping essence aperture, use Sang Piao Xiao San (Mantis Egg Case Powder - a modification of Kou Zongshi's formula, see *Xiao Bian Duo*). Fill essence, secure and grasp, employing methods of combining slipperiness and astringency. For Yi Jing (Spermatorrhea) with dreams, due to excessive fatigue, spleen-stomach injury, Heart-Kidney non-interaction, and mutual damage of upper and lower, use Gui Pi Tang (Restore the Spleen Decoction), Miao Xiang San (Wonderful Fragrance Powder), Shen Zhu Gao (Ginseng and Atractylodes Paste), Bu Xin Dan (Tonify Heart Pill), etc., representing a method of treating Heart, Spleen, and Kidney simultaneously. For Yin deficiency failing to grasp, with Damp-Heat pouring downward causing slippery emission, use Huang Bo, Bi Xie, Huang Lian, Fu Ling, Ze Xie, etc., focusing on bitter-draining of Jue Yin Heat and unblocking fu organ qi. (*Lin Zheng Zhi Nan* Zou Zijiu)
A person suffered from continuous seminal discharge with urine after contracting Shang Han (Cold Damage). Many doctors failed to treat it, and the syndrome was unknown. I cured him with Xiao Chai Hu Tang (Minor Bupleurum Decoction) adding Zhi Mu, Huang Bo, Mu Li, and Qing Dai. Initially, residual heat from the Shao Yang Meridian lingered in the Kidney; over time, it agitated Kidney Fire causing emission. Although the external cold syndrome resolved, this internal fire remained. (*Xing Hai*)
Treating Brain Cold. Someone had Meng Yi (Dream Emission). Initially, visual triggers were present; later, even without visual triggers in dreams, leakage occurred day and night indiscriminately. Treated as Heart Qi deficiency, Heart Qi medicines failed. Treated as Kidney Qi deficiency, Kidney tonics failed. The doctor asked if the patient felt cold in the brain. The patient replied, "Only because my brain is cold." Taking Qu Han San (Expel Cold Powder - formula unknown) brought relief. The brain is the meeting place of all Yang meridians and the sea of Marrow. Brain cold means Marrow is insecure, causing leakage. For this disease, first expel wind-cold from the brain; when Brain Qi harmonizes, assist with Heart-Kidney tonics. No case fails to heal. (*Yi Shuo*)
Bai Jing Shi Dao (Rotten Essence Losing its Path). Spleen and Kidney both deficient, rotten essence loses its path, causing slippery and insecure essence. Jiu Long Dan (Nine Dragon Pill - containing Gou Qi, Jin Ying, Lian Xu, Lian Rou, Qian Shi, Shan Zhu Yu, Dang Gui, Di Huang) remove Dang Gui, add Bi Xie and Wu Wei Zi. However, Cui Xian Wan (Extract Immortal Pill - containing Ji Li, Shan Zhu Yu, Qian Shi, Lian Rui, Gou Qi, Tu Si Zi, Xu Duan, Fu Pen, Jin Ying) is even better. "Rotten essence losing its path" does not mean rotten essence is retained due to lack of discharge, but rather that the essence aperture opens frequently, failing to close tightly, losing its normal degree, and flowing endlessly. Thus, astringent agents are appropriate. (*Yi Tong*)
Jing Sai Shui Qiao (Essence Blocking Water Aperture). Essence blocking water aperture, causing obstruction, belongs to incomplete sexual release, unfulfilled lustful thoughts, holding back ejaculation due to fear, or elderly people lacking Qi to send essence out. Avoid Qi-breaking, purgative, diuretic, and drying-hot drugs. Promote expulsion of rotten essence. Robust individuals should additionally drain fire. Elderly individuals should additionally tonify Qi. (*Ben Cao Jing Jiu Shu*)
Jing Zhong Hong Xian (Red Lines in Essence). With Fire Excess, red threads often appear in essence. Let urine fall into a bucket; clarify and view to see them. Use Bu Yin Wan (Tonify Yin Pill) and Shen Qi Wan (Kidney Qi Pill). (*Ru Men*)

Take thirty to forty pills on an empty stomach with warm wine and salt water. For regular use, take with cold wine between Shen and You hours (3-7 PM). This medicine treats Meng Yi (Dream Emission). [Note: The following discusses several types of Meng Yi (Dream Emission), cited earlier.] This is an ancient formula, currently popular, but few understand its rationale. Ban Xia has draining properties, and Zhu Ling guides water; the intent is to guide Kidney Qi to flow freely. I kept this medicine in my pouch, distributing it in emergencies. Three to five doses always worked promptly. Lin Jiancheng Bimin also recovered after a few doses. (*Ben Shi Fang*) [Note: *Ji Feng* names it Jian Zhong Dan, treating leukorrhea in unmarried women.]
