嘈杂吞酸 口臭口淡，数欲饮食，属胃火。

急治用胆矾、朴硝、牛黄，为末和匀，吹

头面赤热 属上焦火升。

如胃弱加人参三四钱，莲子四十粒，橘红二钱，升麻二钱。如腹痛，以黄连四钱
，白芍三钱，炙草一钱五分，黄柏一钱，升麻七分煎服。如里急，同上药加当归
二钱。如后重甚，加槟榔一钱五分，枳壳一钱五分，木香汁七匙。如口渴去木香倍滑石。
如小便赤涩短少，或不利亦倍之。赤多倍乌梅、山楂、红曲，白多加吴茱萸七分。恶心欲呕，即噤
口痢，多用人参、莲肉、扁豆、白芍，以绿色升麻七分佐之。久痢不止，加肉豆蔻一钱，人参三钱，
砂仁一钱五分，白茯苓二钱。
泄泻
忌湿润，破气，下，苦寒，滑利。
宜安胃（补脾），升，利小便。
人参 茯苓 莲肉 白术 升麻 车前子 橘红 藿香 木瓜 干葛 炙草 白莱菔 扁豆
虚寒者，加肉豆、茯苓、补骨脂、吴茱萸。虚热者，去白术加川黄连，倍芍药、莲肉。
暑湿为病，则小水短赤或口渴，倍用姜黄连为君，佐以干葛、升麻。繇于感风寒者，二术、吴茱萸、
砂仁、陈皮、干姜、紫苏主之。若繇饮食停滞者，兼消导，山楂、麦芽、
神曲、陈皮，肉豆蔻。
诸疸
龙脑香、苏合香、藿香、檀香、乳香、芒硝、童便（儿童尿液）
煎煮药物时，也适宜冷服。
中恶（因感受秽浊之气而突然发病），脑部疼痛，属于胃气虚弱，被恶邪侵袭所致。
禁忌与绞肠痧相同。
适宜辟除恶气，通畅胃气，使用辛散的药物。
可用药物：龙脑香、檀香、麝香（孕妇忌用）、牛黄、乳香、苏合香、沉水香、丹砂、雄黄、鬼臼、藿香、白豆蔻、石菖蒲、橘皮、木香、远志、干姜、桂枝。
反胃，属于气虚；饮酒过度导致的反胃，属于胃弱。
禁忌破气、升提、苦寒、甘味、燥热等药物，具体记录在后面。
适宜补气、降气、和胃、清热，以及酸敛以制约肝气。
可用药物：枇杷叶、人参、苏子、橘皮、木瓜、麦门冬、芦根汁、竹茹、石斛、梅酱、蔗浆、白茯苓、白芍药。
如果是因虚寒引起的，加入生姜、白术、白豆蔻。
大肠虚（四种证候）
禁忌破气、泻下、燥热等药物，具体记录在后面。
适宜补气、润燥，使用甘温的药物。
可用药物：人参、黄芪、麦冬、五味子、白芍、炙甘草。
虚热导致的大便秘结不通，属于血虚，津液不足。
禁忌破气、泻下、燥热、苦温以及损伤津液的药物。
郁李仁（会损伤津液，故忌用）。
适宜生津液、润燥、凉血、益血。
可用药物：生地黄、五味子、麦冬、天冬、芝麻、麻仁、肉苁蓉、生蜜、当归、芦荟、炙甘草。
虚热导致的滑泄不禁，属于气虚。
禁忌破气、泻下、湿润、苦寒的药物。
适宜补气、升提、甘温、酸敛。
可用药物：人参、黄芪、白术、炙甘草、吴茱萸、肉豆蔻、莲肉、升麻、木瓜、补骨脂、五味子、赤石脂。
肠鸣、脱肛，属于气虚，兼有湿热。
禁忌同大肠虚。
适宜补气、升提、除湿热。
可用药物：人参、黄芪、炙甘草、白术、莲肉、白扁豆、升麻、干葛、柴胡、黄柏、防风、白芍药、黄连、黄芩、樗根白皮。
外用五倍子敷贴患处。
膀胱虚（三种证候）
禁忌破气、燥性、利小便的药物。
适宜补气、酸敛。
可用药物：人参、五味子、山茱萸、益智子、金樱子。
小便不禁，属于气血虚。
禁忌降下、湿润、燥热的药物。
适宜同膀胱虚的治法，加牡蛎、龙骨、鹿茸、桑螵蛸、鸡内金。
尿频且不能忍耐，加麦门冬、五味子、山茱萸、天门冬、黄柏、柏子仁、鳖甲、牛膝、甘枸杞子。
遗尿，属于本经气虚，详见小肠虚条目。因膀胱虚也能导致遗尿，所以再次列出。
禁忌和适宜均见小肠虚条目。
膀胱虚的禁忌和适宜均同疝气。
三焦虚（两种证候）
禁忌破气、降气，又忌升发，忌苦寒。
适宜补中益气，佐以辛温。
可用药物：人参、黄芪、白术、益智子、沉香、五味子。
心实，即实火实热。（五种证候）
禁忌补益收敛、升提、热性、温燥的药物。
适宜降火清热，用苦寒以折其势，辛寒以散其邪，甘寒以缓其急，咸寒以润其燥。
可用药物：黄连、犀角、石膏、丹砂、牡丹皮、生甘草、滑石、竹叶、麦冬、童便。
大便结燥者，加芒硝、大黄；发狂者亦如此处理。
谵语（胡言乱语），属于心家邪热。舌破，属于心火。烦躁，属于心家邪热及心火内炎。（烦属心，躁属肾。）自笑，属于心家有热邪。发狂，属于心家有邪热甚重。

禁忌补气、升提、酸敛、辛热、辛温、燥性药物，具体记录在后面。
适宜清热降气，使用苦寒、辛寒、甘寒、酸寒的药物。
可用药物：橘皮、青皮、苏子、黄连、龙胆草、生甘草、黄芩、柴胡、竹叶、黄黛、赤芍药。
善怒（容易发怒），怒则气上逆，严重则呕血及飧泄（完谷不化的腹泻）。

急症宜用三棱针刺破眼眶肿处，挤出热血即可立解，迟则血贯瞳人导致目损。

虫胀（腹胀如虫咬或寄生虫引起的肿胀），由于脾家湿热积滞或内伤、瘀血停积而成。

可用药物：车前子、木通、防己、猪苓、泽泻、茯苓、鸟喙鱼、葶苈、山楂、红曲、三棱、蓬术、桑白皮。
易饥（容易饥饿），属于脾家邪火。
禁忌升提、辛温、大热、香燥。
沉香、麝香、龙脑、豆蔻、藿香、缩砂密（以上均为香燥之品，故忌用）。
适宜清火除热、生津液、益脾阴，使用甘寒、苦寒、酸寒。
可用药物：黄连、青黛、连翘、山栀、麦门冬、酸枣仁、芍药、石膏、竹叶、石斛。
中消（多食易饥），口糜，口唇生疮，属于脾家热。
禁忌温燥、热性药物。
适宜甘寒、酸寒、苦寒、辛寒。
可用药物：麦门冬、甘草、乌梅、黄连、黄柏、生地黄、白芍药、玄参、连翘、干葛、石膏、龙胆草、栝蒌根、大青、竹叶。
湿热腹痛，按压后疼痛加剧。
禁忌闷气、酸敛、温热、燥性药物。
适宜利小便，兼升提，使用苦寒。
可用药物：滑石、木通、黄连、黄芩、升麻、柴胡、葛根、防风、车前子。
若不加重，即遵循“土郁则夺”的意义（通过泻下祛除脾胃郁滞）。
肺热（八种证候）
禁忌敛涩、补气、升提、燥热、酸、咸味药物。
适宜降气、润燥，使用甘寒、苦寒，佐以辛散。
可用药物：枇杷叶、苏子、桑白皮、贝母、杏仁、白前、天门冬、前胡、车前子、知母、桑黄、石膏、栝蒌根、黄芩。
喘急，属于肺有实热，及肺气上逆。
禁忌同肺实。
适宜也相同，加桔梗、甘草、栝蒌仁、玄参、青黛。
声重（声音沉重浑浊），气壅痰稠，属于肺热。
禁忌和适宜均同肺热。
喉癣、肺胀、肺痈，属于肺热。
禁忌同肺实。
适宜清热、消痰、降火、解毒散结，使用甘寒、苦寒、辛寒。
可用药物：桑白皮、桑黄、黄芩、栝蒌根、贝母、薏苡仁、虎耳草、蕺米（鱼腥草籽）、连翘、鼠粘子（牛蒡子）、甘草、败酱草。

强阳不倒，属于命门火实，孤阳无阴所致，此证大多难治。

小水不利及赤（小便不畅且色红），或涩痛尿血。

禁忌汗法、吐法、下法。
适宜和解，使用辛寒、甘寒、苦寒、辛温。

口苦耳聋胁痛，往来寒热。

适宜使用仲景小柴胡汤，根据所见兼证加减。
鼻渊（鼻流浊涕），属于胆移热于脑。

胃实（六种证候）
禁忌升提、补敛、辛温、燥热、湿润。
适宜泻下，如果邪气未结聚，适宜清热发散，使用苦寒、辛寒、甘寒。
可用药物：大黄、枳实、知母、石膏、葛根、竹叶、大青、小青、青黛、麦冬、甘草。
谵语发狂，发斑，弃衣而走，登高而歌，属于胃家邪热实。
禁忌同胃实。
适宜也相同。
如果大便结者，加芒硝紧急泻下。发斑者，加鼠粘子、玄参、栝蒌根，多用石膏为君药，大便结者加大黄泻下。
嘈杂吞酸，口臭口淡，数欲饮食，属于胃火。
禁忌同胃实。
适宜清热降火，使用苦寒、甘寒、辛寒。
可用药物：黄连、青黛、连翘、麦冬、石斛、芦根汁、竹叶、石膏。
呕吐，属于胃火者，必面赤，小便短赤，或涩，大便多燥，口苦或干渴。
禁忌同胃实。
适宜也相同，加枇杷叶、竹茹、木瓜、芦根、橘皮、通草、茯苓。
大肠实（四种证候）
禁忌补敛、燥热。
适宜润下，使用苦寒、辛寒。
可用药物：麻仁、桃仁、黄连、黄芩、槐花、生地黄、大黄、石膏、知母、枳壳。
便硬闭结
禁忌同大肠实。
适宜也相同，加芒硝、猪胆、槟榔、郁李仁、石蜜。
脏毒肠风下血，属于大肠湿热。
禁忌泻下、燥热。
适宜清热、凉血，兼升提，使用甘寒、苦寒。
可用药物：槐花、地榆、黄连、黄芩、生地黄、白芍药、荆芥、防风、甘草、红曲、侧柏叶、白头翁、蒲黄、鸡子、葛根。
肠痈，属于大肠实火。
禁忌同肠风下血。
适宜泻下，苦寒，解寒（此处原文可能有误，通常指清热解毒或活血排脓，依文意保留）。
可用药物：大黄、白芷、白芨、白蔹、白药子、忍冬藤、连翘、甘草、黄连、黄芪、生地黄、天明精、明矾、黄蜡、生蜜（以上三味作丸）。
膀胱实（一种证候）
禁忌燥热、收涩。
适宜润燥、淡渗。
可用药物：知母、黄柏、木通、瞿麦、车前子、滑石、茯苓、猪苓、泽泻。
癃闭（小便不通），属于膀胱实热。
禁忌破气、发散、燥热，如果属于水液不足，兼忌利小便。
适宜同膀胱实，佐以升提，用升麻、柴胡。
三焦实（三种证候）
禁忌补敛、升提、燥热。
适宜降气、清热、调气，使用甘寒、苦寒、咸寒。
可用药物：苏子、麦冬、知母、黄柏、玄参、山栀、黄芩、黄连、童便。
喉痹，即缠喉风，属于少阳相火，少阴君火并炽，经曰：“一阴一阳结为喉痹。”一阴者，少阴君火也。一阳者，少阳相火也。禁忌同三焦实。
适宜辛散，佐以苦寒、咸寒，急症则有针、吹、吐三法。
可用药物：鼠粘子、射干、黄连、黄柏、山豆根、麦门冬、生犀角、知母、玄参、童便、山茨菇、苦桔梗、续随子、苏子、贝母、甘草。
急治用胆矾、朴硝、牛黄，研末和匀，吹入喉中。又法用明矾三钱，巴豆七粒去壳，同矾煅烧，矾枯去巴豆，即取矾为细末，吹入喉中，流出热涎即宽。
头面赤热，属于上焦火升。
禁忌同三焦实。
适宜降气、清热、甘缓，佐以酸敛。
可用药物：苏子、天冬、麦冬、玄参、薄荷、枇杷叶、梨、柿、蔗、童便、五味子、栝蒌根、芍药。
赤白游风，属于血热，热则生风，故善游走，俗名火丹。小儿多患此，大人亦时有之。

疟母
禁忌纯补。
适宜补中行滞。
可用药物：鳖甲、射干、牡蛎、三棱、桂枝、缩砂密、橘皮、青皮、人参。
诸痢
禁忌破气、闭气、收涩、燥、温热、咸寒、滑腻。
适宜清热消积、开胃气、升提、利小便。
可用药物：黄连、黄芩、白芍、红曲、山楂、广橘红、升麻、葛根、滑石、莲肉、甘草、白扁豆、乌梅。
如果胃弱，加人参三四钱，莲子四十粒，橘红二钱，升麻二钱。如果腹痛，以黄连四钱、白芍三钱、炙草一钱五分、黄柏一钱、升麻七分煎服。如果里急后重，同上药加当归二钱。如果后重甚，加槟榔一钱五分、枳壳一钱五分、木香汁七匙。如果口渴，去木香倍滑石。如果小便赤涩短少，或不利，亦倍之。赤多倍乌梅、山楂、红曲，白多加吴茱萸七分。恶心欲呕，即噤口痢，多用人参、莲肉、扁豆、白芍，以绿色升麻七分佐之。久痢不止，加肉豆蔻一钱、人参三钱、砂仁一钱五分、白茯苓二钱。
泄泻
禁忌湿润、破气、泻下、苦寒、滑利。
适宜安胃（补脾）、升提、利小便。
可用药物：人参、茯苓、莲肉、白术、升麻、车前子、橘红、藿香、木瓜、干葛、炙甘草、白莱菔、扁豆。
虚寒者，加肉豆蔻、茯苓、补骨脂、吴茱萸。虚热者，去白术加川黄连，倍芍药、莲肉。
暑湿为病，则小水短赤或口渴，倍用姜黄连为君，佐以干葛、升麻。由于感风寒者，二术、吴茱萸、砂仁、陈皮、干姜、紫苏主之。若由于饮食停滞者，兼消导，山楂、麦芽、神曲、陈皮、肉豆蔻。

For hard stool, add Mirabilite, Rhubarb (Dahuang); for mania, treat similarly.
Delirium, belongs to pathogenic heat in the heart. Tongue ulceration, belongs to heart fire. Irritability/restlessness, belongs to pathogenic heat in the heart and internal flaming of heart fire. (Irritability belongs to heart, restlessness belongs to kidney.) Spontaneous laughter, belongs to heat pathogen in the heart. Mania, belongs to severe pathogenic heat in the heart.

Liver excess (five syndromes)

Sanjiao excess (three syndromes)
Contraindicate tonifying/astringing, ascending, dry-hot.
Suitable descending, clearing heat, regulating qi, sweet-cold, bitter-cold, salty-cold.
Herbs: Perilla seed, Ophiopogon, Anemarrhena, Phellodendron, Scrophularia, Gardenia, Scutellaria, Coptis, Child's urine.
Throat obstruction (Houbi), i.e., winding throat wind, belongs to flourishing Shaoyang ministerial fire and Shaoyin sovereign fire. The classic says: "One yin and one yang knotting causes throat obstruction." One yin is Shaoyin sovereign fire. One yang is Shaoyang ministerial fire. Contraindicated same as Sanjiao excess.
Suitable acrid-dispersing, assisted by bitter-cold, salty-cold; acute cases have three methods: acupuncture, blowing powder, vomiting.
Herbs: Burdock seed, Belamcanda (Shegan), Coptis, Phellodendron, Sophora root (Shandougen), Ophiopogon, Raw rhinoceros horn, Anemarrhena, Scrophularia, Child's urine, Smilax bulbil (Shancigu), Bitter platycodon (Kujiegeng), Euphorbia (Xuisui), Perilla seed, Fritillaria, Licorice.
Acute treatment: use Chalcanthite (Danfan), Glauber's salt (Puxiao), Calculus Bovis, grind to powder, mix well, blow into throat. Another method: use 3 qian Alum, 7去皮 Bay beans, calcine with alum until alum is dry, discard bay beans, take alum powder, blow into throat, hot saliva flows out and relief occurs.
Red and hot head and face, belongs to rising fire in upper jiao.
Contraindicated same as Sanjiao excess.
Suitable descending, clearing heat, sweet-moderating, assisted by sour-astringent.
Herbs: Perilla seed, Asparagus, Ophiopogon, Scrophularia, Mint, Loquat leaf, Pear, Persimmon, Sugarcane, Child's urine, Schisandra, Trichosanthes root, Peony.
Red and white wandering wind, belongs to blood heat, heat generates wind, hence prone to wandering, commonly known as Fire Dan. Children often suffer from this, adults occasionally do too.

Suitable to clear heat, cool blood, combined with moving blood, acrid-cold, sweet-cold, bitter-cold, salty-cold.

Herbs: Coptis, Scutellaria, White peony, Red yeast rice, Hawthorn, Broad-leaved tangerine peel (Guangjuhong), Cimicifuga, Kudzu root, Talc, Lotus seed, Licorice, White hyacinth bean, Dark plum.
If stomach is weak, add 3-2 qian Ginseng, 40 Lotus seeds, 2 qian Tangerine peel, 2 qian Cimicifuga. If abdominal pain, decoct 4 qian Coptis, 3 qian White peony, 1.5 qian Honey-fried licorice, 1 qian Phellodendron, 0.7 qian Cimicifuga. If tenesmus, add above herbs plus 2 qian Angelica sinensis. If severe heaviness in anus, add 1.5 qian Areca nut, 1.5 qian Aurantium, 7 spoons Cyperus juice. If thirsty, remove Cyperus and double Talc. If urine is red,涩, short, or difficult, also double it. If red discharge is heavy, double Dark plum, Hawthorn, Red yeast rice; if white discharge is heavy, add 0.7 qian Evodia. If nausea/vomiting, i.e., Throat-lock dysentery, mostly use Ginseng, Lotus seed, Hyacinth bean, White peony, assisted by 0.7 qian Green Cimicifuga. For chronic dysentery not stopping, add 1 qian Myristica, 3 qian Ginseng, 1.5 qian Amomum, 2 qian Poria.
Diarrhea
Contraindicate moistening, breaking qi, purging, bitter-cold, slippery.
Suitable to calm stomach (tonify spleen), lift, promote urination.
Herbs: Ginseng, Poria, Lotus seed, Atractylodes macrocephala, Cimicifuga, Plantago seed, Tangerine peel, Agastache, Quince, Kudzu root, Honey-fried licorice, White radish (Bailai fu), Hyacinth bean.
For deficiency-cold, add Myristica, Poria, Psoralea, Evodia. For deficiency-heat, remove Atractylodes macrocephala, add Sichuan Coptis, double Peony and Lotus seed.
For summer-dampness disease, resulting in short red urine or thirst, double Ginger-Coptis as chief, assisted by Kudzu root, Cimicifuga. Due to wind-cold invasion, use two Atractodises, Evodia, Amomum, Chenpi, Dried ginger, Perilla. If due to food stagnation, combine with digestion-promoting, Hawthorn, Malt, Medicated leaven (Shenqu), Chenpi, Myristica.
