Book: Shao Lansun's Medical Cases Author: Shao Lansun Dynasty: Qing Chapter: Vomiting and Dysphagia Category: Medical Discussion
Patient Yang from Anchang (Female) Blood deficiency with Qi stagnation, damp-heat entrenched in the interior, Liver Qi rebelling and invading the Stomach. Every time menstruation arrives, there is a feeling of nausea in the heart; pulse is soggy (Ru), abdomen is full accompanied by leukorrhea. Therefore, it is appropriate to harmonize the Liver and regulate menstruation.
Da Fu Rong (3 qian) An Lv Zi (1.5 qian) Pei Lan (2 qian) Hou Po (1.5 qian) Xian Ban Xia (1.5 qian) Fu Ling (4 qian)
Dan Shen (2 qian) Luo Mei Hua (5 flowers) Xiang Fu (2 qian) Ji Xue Teng (3 qian) Chuan Lian Zi (0.8 qian) Wu Zhu Yu (0.4 qian, stir-fried with Pinellia)
Five doses.
Commentary: This is what Qin Yueren referred to as disease of the Dai Mai (Girdle Vessel), presenting with a sensation of the waist being loose and watery, as if sitting in water. Currently, due to the insecurity of the Dai Mai, compounded by entrenched damp-heat, Jue Yin (Liver) Qi rebels and invades, causing nausea in the heart. Treatment prioritizes harmonizing the Liver and removing damp-heat.
Patient Huang from Daxizhuang Liver Qi rebelling and invading the Stomach, epigastric obstruction with vomiting and nausea; pulse is fine and stagnant on the right, wiry on the left, tongue color is relatively normal. It is appropriate to use bitter and acrid methods to unblock and descend (Ku Xin Tong Jiang). (August 14th.)
Gan Jiang (0.4 qian) Pi Chen (1.5 qian) Kou Qiao (1.5 qian) Wu Yao (2 qian) Hou Po (1 qian) Tong Cao (1.5 qian) Chi Fu Ling (4 qian) Luo Mei Hua (5 flowers) Xian Ban Xia (1.5 qian) Gu Ya (4 qian) (Wu Zhu Yu 0.5 qian stir-fried) Chuan Lian Zi (0.8 qian)
Decoct in clear water, three doses.
Follow-up: Vomiting and nausea have been eliminated; pulse is wiry and fine, tongue coating is slightly white with a pale yellow root, damp-heat is not yet completely cleared, Qi mechanism is obstructed. It is appropriate to harmonize the Middle Jiao and promote diuresis to remove dampness. (August 28th)
Huo Geng (2 qian) Kou Qiao (1.5 qian) Gu Ya (4 qian) Gan Song (0.4 qian) Sheng Tou Cao (1.5 qian) Chi Fu Ling (4 qian) Zhi Ke (1.5 qian) Lv E Mei (1.5 qian) Hou Po (1 qian) Xin Hui Pi (1.5 qian) Tong Cao (1.5 qian)
Decoct in clear water, three doses.
Commentary: Qi mechanism obstruction prevents the Clear Yang from expanding. The initial prescription focused on calming the Liver and harmonizing the Stomach, after which Stomach Qi gradually harmonized and vomiting stopped. In the second visit, since dampness was not yet cleared, a formula promoting Qi movement and渗湿 (seeping out dampness) was administered.
Patient Shen from Yuzhuang (Female) Daughter's diarrhea persists; pulse is weak and fine, vomiting and nausea have improved, Stomach Qi is deficient, epigastrium feels stifled with insomnia. It is appropriate to nourish the Stomach, harmonize the Liver, and calm the Spirit.
Dan Shen (3 qian) Pei Lan Ye (1.5 qian) Zao Ren (3 qian) Gu Ya (4 qian) Bian Dou Yi (3 qian) Pi Chen (1 qian)
Xiang Fu (1.5 qian) Luo Mei Hua (5 flowers) Fu Shen (4 qian) Huo Hu (3 qian) Tong Cao (1.5 qian)
Decoct in clear water, three doses.
Commentary: Liver Qi rebelling and invading the Stomach causes vomiting, nausea, and epigastric stifling; Liver Yang carrying dampness insults the Spleen, causing diarrhea; furthermore, disharmony of the Stomach leads to restlessness in sleep. Thus, treatment involves restraining the Liver, harmonizing the Stomach, supporting the Spleen, and calming the Spirit.
Patient Xu from Anchang (Female) Leukorrhea persists; pulse is deep and wiry, vomiting and nausea present, tongue coating is slightly yellow. This indicates Liver-Stomach disharmony and irregular menstruation. Prevention of dysphagia (Ye Ge) is advised. (April 20th, Lunar Calendar.)
Ci Ji Li Pi Chen (1.5 qian) (Wu Zhu Yu 0.4 qian stir-fried) Chuan Lian Zi (0.7 qian) Dan Shen (3 qian) Fu Ling (4 qian) Sheng Bai Shao (1.5 qian)
Guang Pi Chen (1.5 qian) Lv E Mei (1.5 qian) Xian Ban Xia (1.5 qian) Sheng Tou Cao (3 qian) Kou Qiao (1.5 qian) Ji Xue Teng (3 qian)
(Leading herb) Lu Lu Tong (7 pieces)
Four doses.
Commentary: Liver Qi rebelling and invading the Stomach causes vomiting and nausea, further complicated by blood depletion leading to irregular menstruation. The prescription calms the Liver and harmonizes the Stomach, incorporating blood-harmonizing herbs; this indicates that Stomach fluids are being depleted and scorched by Liver Yang. However, since dysphagia is being prevented, Kou Qiao may inadvertently deplete fluids and should be used with caution.
Patient Xu from Anchang (Female) Obstructive Qi causing vomiting; pulse is deep and wiry on the left, wiry and slippery on the right, menstruation has ceased for five months, tongue coating is thin and white with a slightly thick root. This is Liver Qi rebelling and invading the Stomach; treatment should address both Jue Yin and Yang Ming meridians simultaneously. (May 17th.)
Xian Ban Xia (1.5 qian) Chao Gu Ya (4 qian) Xin Hui Pi (1.5 qian) Huo Geng (2 qian) (Wu Zhu Yu 0.5 qian stir-fried) Chuan Lian Zi (0.7 qian)
Su Geng (1.5 qian) Lv E Mei (1.5 qian) Pi Chen (1 qian) Chuan Pu (1 qian) Mu Hu Die (0.4 qian) Kou Qiao (1.5 qian)
Decoct in clear water.
Follow-up: Obstructive Qi symptoms have improved, epigastrium feels slightly comfortable; pulse is wiry and slippery upon palpation of both hands, menstruation has ceased for five months, tongue root is slightly yellow. It is appropriate to follow the previous method with modifications. (June 20th.)
Pi Chen (1 qian) Guang Huo Xiang (1.5 qian) Wu Yao (1.5 qian) Chuan Lian Zi (1.5 qian) Su Geng (1.5 qian) Kou Qiao (1.5 qian)
Mu Hu Die (0.4 qian) Lv E Mei (1.5 qian) Chai Hu (3 qian) Chao Gu Ya (4 qian) Xin Hui Pi (1.5 qian)
Decoct in clear water, three doses.
Commentary: Liver depression with Qi rebellion and depletion of Stomach fluids lead towards dysphagia; thus, simultaneous treatment of Jue Yin and Yang Ming is employed. However, a slippery pulse and amenorrhea accompanied by vomiting, although indicating Liver-Stomach disharmony, suggest pregnancy. Furthermore, the treatment does not involve unblocking menstruation or activating blood. Only methods to harmonize the Stomach and regulate Qi are used. Since pregnancy is not mentioned in the case, speculation is difficult. Nevertheless, Kou Qiao depletes fluids and should ultimately be used with caution.
Qian from Meiling Tongue coating is slippery and white, pulse is wiry and tight; food enters the epigastrium but meets obstruction and rejection. This is Liver Qi rebelling and attacking the Middle Jiao, with Spleen Qi losing its transport function. Therefore, it is appropriate to harmonize the Middle Jiao and soothe the Liver. (June 21st, Bingwu Day 8th.)
Ji Nei Jin (3 qian) Chuan Lian Zi (3 qian) Xiao Qing Pi (1.5 qian) Kou Qiao (1.5 qian) Chen Xiang Qu (1.5 qian) Hou Po (1 qian) Gan Song (0.4 qian) Luo Mei Hua (5 flowers) Sheng Mu Li (4 qian) An Lv Zi (3 qian) Zuo Jin Wan (0.8 qian) (Leading herb) Lu Lu Tong (7 pieces)
Four doses.
Commentary: This is Liver Yang attacking and insulting the Spleen and Stomach, making food difficult to digest, with damp-heat accumulating in the diaphragm. Therefore, treatment focuses on draining, descending, and harmonizing the Stomach; otherwise, there is a fear of bloating and fullness.
Li from Baimashan Tongue coating is slippery, grayish-yellow; epigastric pain with obstruction and rejection, vomiting and nausea, difficulty swallowing liquids, pulse is wiry and soggy, tidal fever after eating, constipation. The condition is severe and critical; bitter and acrid methods to unblock and descend are appropriate. Wait for Zheng Qi (Righteous Qi) to recover.
Dan Gan Jiang (0.2 qian) Chao Zhi Shi (1.5 qian) Gua Lou Pi (5 qian) Sheng Bai Shao (1.5 qian) Chao Chuan Lian Zi (0.8 qian) Hua Shi (4 qian) Chao Mai Ya (3 qian) Fo Shou Hua (0.8 qian) Xian Ban Xia (1.5 qian) Jiang Xiang (0.8 qian) Chao Zao Ren (3 qian)
Decoct in clear water, two doses.
Commentary: Upper inability to accept food, lower inability to pass stool; this is Clear Yang increasingly knotting, epigastric narrowing, Yin fluid depletion, and Fu organs lacking fluids and Ying blood. Stomach Qi has lost its principle of descending smoothly. Bowel movements must become smooth and unobstructed before food can enter the epigastrium without hindrance. However, this condition has become Guan Ge (Obstruction and Rejection), which is a difficult-to-treat disease.
