(两液同冲)
Luo (Left), aged just over fifty, had early deficiency of Yang Qi. Pathogenic wind invaded the meridians, causing Bi obstruction of Ying and Wei Qi, leading to sudden collapse and stroke. The tongue is stiff with aphasia, consciousness is fluctuating between clear and obscure, there is lethargy and unrousability, and the right hand and foot are paralyzed. Wind nature tends upward, carrying phlegm-dampness with it, obstructing the Lianquan point (Ren-23) and blocking the pathway of Shen (Spirit). The pulse is deep and fine at the Chi position, wiry, tight, and slippery at the Cun and Guan positions; the coating is white and greasy, resembling a gloomy haze where Yang Qi fails to function. Fortunately, urinary incontinence has not occurred, indicating that Kidney Qi remains secure, and there is no immediate sign of Collapse, which is fortunate. Urgently prescribe a modified Xiao Xuming Tang (Minor Life-Sustaining Decoction) from Zhang Zhongjing to assist Yang, dispel wind, open Bi obstruction, transform the Middle Jiao, eliminate phlegm, and unblock the collaterals, hoping for an immediate response and a turning point.
Net Ephedrae Herba (Mahuang, 4 fen) Cooked Aconiti Lateralis Radix Preparata (Fupian, 1 qian) Cinnamon Twig (Guizhi, 8 fen) Raw Glycyrrhizae Radix et Rhizoma (Gancao, 6 fen) Whole Angelicae Sinensis Radix (Danggui, 3 qian) Chuanxiong Rhizome (Chuanxiong, 8 fen)
Ginger-processed Pinelliae Tuber (Banxia, 3 qian) Bitter Apricot Seed (Xingren, 3 qian) Ginger Juice (taken with decoction, 1 qian) Bamboo Sap (Zhuli, taken with decoction, 1 liang)
Additionally, Zaizao Wan (Rejuvenation Pill) (shell removed, ground to fine powder, dissolved and taken, 1 pill.)
Second visit: After taking two doses of Xiao Xuming Tang, consciousness has slightly cleared, and lethargy has gradually decreased, which is a good omen. However, the tongue remains stiff with inability to speak, and the right hand and foot remain paralyzed. The pulse is deep and fine at the Chi position, while the Cun and Guan positions are wiry and tight but slightly harmonized; the coating is thin and greasy. Yang Qi was originally deficient, and the defensive barrier was insecure. Pathogenic wind invaded the meridians, causing meridian Bi obstruction and retention of phlegm-dampness. Zong Qi (Pectoral Qi) cannot distribute, hence the paralysis of the right hand and foot. The Kidney meridian connects to the root of the tongue, and the Spleen meridian connects to the side of the tongue. Phlegm obstructs the collaterals of the Heart and Spleen, causing tongue stiffness and aphasia, blocking the spiritual mechanism. Although slight efficacy is seen, one must not be complacent. Prescribe again to warm and tonify Yang Qi to dispel pathogenic wind, eliminate phlegm-turbidity to unblock the collaterals, strive forward, and observe subsequent effects.
Cooked Aconiti Lateralis Radix Preparata (Fupian, 1 qian) Poria (Fuling, 3 qian) Cinnamon Twig (Guizhi, 8 fen) Ginger-processed Pinelliae Tuber (Banxia, 2 qian) Raw Glycyrrhizae Radix et Rhizoma (Gancao, 6 fen) Charred Immature Bitter Orange (Zhishi Tan, 1 qian)
Whole Angelicae Sinensis Radix (Danggui, 2 qian) Bitter Apricot Seed (Xingren, 3 qian) Large Chuanxiong Rhizome (Chuanxiong, 8 fen) Honey-fried Silkworm (Jiangcan, 2 qian) Ginger Juice (taken with decoction, 1 qian) Bamboo Sap (Zhuli, taken with decoction, 1 liang)
Third visit: After taking three more doses, consciousness is clearer, lethargy is greatly reduced, and speech is slightly possible. Yang Qi shows signs of circulation, and turbid phlegm shows signs of resolution; this indicates effective treatment. However, the right hand and foot remain paralyzed, and bowel movements have not occurred for six or seven days. The coating is greasy, the pulse is wiry and tight but gradually harmonizing, and the Chi position is deep and fine. Kidney Yang was early deficient, and Zong Qi cannot distribute. Turbid filth in the Fu organs requires the unblocking of Yang Qi to descend and be discharged. Pathogenic wind in the meridians and Fu organs can only be expelled outward when Zheng Qi (Upright Qi) is strong. Continue to assist Yang and tonify Qi to drive out pathogenic wind, unblock the Stomach, eliminate phlegm, and discharge turbid filth. Since descending movement is favorable for Fu organ Qi, unblocking the bowels should not be delayed.
Raw Astragali Membranacei Radix (Huangqi, 3 qian) Cinnamon Twig (Guizhi, 8 fen) Aconite Root (Fuzi, 1 qian) Raw Glycyrrhizae Radix et Rhizoma (Gancao, 5 fen)
Angelicae Sinensis Radix (Danggui, 3 qian) Chuanxiong Rhizome (Chuanxiong, 8 fen) Poria (Fuling, 3 qian) Weathered Natrii Sulfas (Fenghua Xiaofeng, 5 fen) Whole Trichosanthis Fructus (Gualou, 3 qian) Charred Immature Bitter Orange (Zhishi Tan, 1 qian)
Cistanche Stem (Rongshu, 3 qian) Banliu Wan (Half-Sulfur Pill) (swallowed, 1.5 qian)
Fourth visit: Bowel movement has been unblocked, turbid filth has descended, and consciousness is clear. However, the tongue is stiff, speech is not yet fluent, and the right hand and foot remain paralyzed. The pulse is wiry and tight but turning harmonious, and the Chi position is deep and fine. This is a constitution with weak Yang Qi. Wind is the chief of a hundred diseases; Yin-deficiency-generated pathogenic wind is essentially moving Qi from Cold invasion. For every unit increase in Yang Qi, there is a corresponding decrease in pathogenic wind. Damp-phlegm stagnation also requires sufficient Yang Qi to be resolved. As stated in the Classics: "Yang Qi is like the sky and the sun; if it loses its proper place, lifespan is shortened and brilliance is obscured." This reasoning is indeed true. Continue to assist Yang Qi to dispel pathogenic wind, resolve damp-phlegm, and unblock the collaterals. Proceed step by step, and results will naturally be achieved.
Raw Astragali Membranacei Radix (Huangqi, 5 qian) Raw Atractylodis Macrocephalae Rhizoma (Baizhu, 2 qian) Raw Glycyrrhizae Radix et Rhizoma (Gancao, 5 fen) Cooked Aconiti Lateralis Radix Preparata (Fuzi, 1 qian) Cinnamon Twig (Guizhi, 8 fen) Whole Angelicae Sinensis Radix (Danggui, 3 qian)
Chuanxiong Rhizome (Chuanxiong, 8 fen) Ginger-processed Pinelliae Tuber (Banxia, 3 qian) Western Gentian Root (Qinjiao, 2 qian) Achyranthes Bidentatae Radix (Niuxi, 2 qian) Tender Mulberry Twig (Sangzhi, 3 qian) Zhimi Fuling Wan (Phytolaccae Radix and Poria Pill) (wrapped, 5 qian)
After taking the previous prescription, all ailments lightened. Continue with the original method and expand. Increase Raw Astragali Membranacei Radix (Huangqi) to 8 qian. Take Deer Antler Lu Rong (2 fen) every other day, ground to fine powder, made into pills with rice, swallowed with aged wine. Take Daluoluo Dan (Great Unblocking Collateral Pill), one pill every five days, shell removed, ground to powder, dissolved in aged wine. After taking more than sixty doses in total, the tongue can speak, the hand can grasp, and the foot can walk. Follow up with a paste formula, with ingredients similar to the decoction, to consolidate recovery.
Shen (Left), aged over seventy, had Qi and Yin decline before illness onset. Previously suffering from headache and eye disease, suddenly collapsed today due to stroke. Tongue is stiff with aphasia, loss of consciousness, and left hand and foot paralysis. Tongue body is gray-red. Pulse is deep and weak at the Chi position, wiry, slippery, and rapid at the Cun and Guan positions, forceful upon pressure. This is due to Water deficiency failing to nourish Wood, causing internal wind to swirl upward, carrying previously accumulated phlegm-heat to cloud the clear orifices and block the pathways of Shen, resulting in unconsciousness. Phlegm-heat obstructs Lianquan, causing tongue stiffness and aphasia. Wind evil crisscrosses the meridians, causing left-sided paralysis. The *Jin Gui Yao Lue* states: "Wind invading the meridians causes weakness in lifting heavy objects; wind invading the Fu organs causes loss of recognition." This is a severe case of simultaneous meridian and Fu organ involvement. Urgently prescribe nourishing Yin to extinguish wind, open orifices, and eliminate phlegm, hoping for a turning point.
Large Ophiopogonis Tuber (Maidong, 3 qian) Scrophulariae Radix (Xuanshen, 2 qian) Antelope Horn (Lingyang Pian, decocted first, juice taken, 8 fen) Fairy Pinelliae Tuber (Xianbanxia, 2 qian) Fritillariae Bulbus (Beimu, 2 qian) Tianshu Huang (Tianzhuhuang, 1.5 qian) Tianma (Tomorrow's Hemp Seed/Tianma, 8 fen) Chen Danning (Aged Calculus Bovis/Star Phlegm, 8 fen) Bamboo Sheath (Zhuru, 1.5 qian) Immature Bitter Orange (Zhishi, 1 qian) Whole Trichosanthis Fructus (Gualou, cut, 4 qian) Hooked Uncaria (Gouteng, added later, 3 qian) Bamboo Sap (Zhuli, taken with decoction, 1 liang) Ginger Juice (taken with decoction, 2 drops) Zhibao Dan (Treasure of Life Pill) (1 pill, shell removed, ground to powder, dissolved and taken)
Second visit: After two doses of nourishing Yin, extinguishing wind, opening orifices, and eliminating phlegm, consciousness has gradually returned. However, the tongue is stiff with inability to speak, and the left hand and foot remain paralyzed. The pulse is fine and weak at the Chi position, wiry, slippery, and rapid at the Cun and Guan positions; the tongue is gray-red. In advanced age, Ying Yin is depleted, and wind arises internally. Wind disturbs the Stomach; as the Sea of Water and Grain, the Stomach transforms fluids into phlegm-saliva, which clouds the clear orifices above and crisscrosses the meridians horizontally, explaining the origin of this ailment. This syndrome is Yin deficiency, precarious as a candle in the wind! Now imitating Liu Hejian's Dihuang Yinzi (Rehmannia Decoction) with modifications, nourish Yin and Blood to extinguish internal wind, transform phlegm-heat, and clarify Shen. Only when wind calms and waves subside can danger turn to safety.
Large Rehmanniae Radix (Dishengdi, 4 qian) Large Ophiopogonis Tuber (Maidong, 2 qian) Dendrobii Caulis (Shihu, 3 qian) Antelope Horn (Lingyang Pian, decocted first, juice taken, 4 fen) Fairy Pinelliae Tuber (Xianbanxia, 2 qian)
Tianma (1 qian) Left Oyster Shell (Muli, 4 qian) Chuanbei Mu (Chuanbeimu, 3 qian) Chen Danning (8 fen) Honey-fried Polygala Root (Yuanzhi, 1 qian) Nine-Joint Acorus (Shichangpu, 8 fen) Whole Trichosanthis Fructus (Gualou, cut, 4 qian)
Hooked Uncaria (Gouteng, added later, 3 qian) Bamboo Sap (Zhuli, taken with decoction, 1 liang)
Third visit: After multiple doses of nourishing Yin, extinguishing wind, clearing heat, and resolving phlegm, consciousness is clear, but the tongue is stiff with hesitant speech, and the left hand and foot remain paralyzed. Tongue coating is gray-red. Pulse is wiry and rapid but quieter, with a fine and weak Chi position. Internal wind is gradually calming, but Yin Blood is difficult to restore. Fluids refined by fire become phlegm; phlegm is the manifestation of fire, and fire is the root of phlegm. If fire is not pacified, phlegm will not resolve; if Yin is not filled, fire will not be pacified. Meridians are dry and rough, like channels without water to flow through. The previous Dihuang Yinzi showed efficacy; continue with the original intent. However, the function of herbs and plants cannot easily and rapidly generate vital essence and blood.
American Ginseng (Xiyangshen, 1.5 qian) Large Ophiopogonis Tuber (Maidong, 3 qian) Large Rehmanniae Radix (Dishengdi, 3 qian) Dendrobii Caulis (Shihu, 3 qian) Raw Left Oyster Shell (Muli, 4 qian) Roasted Tianma (8 fen)
Bamboo Sap-processed Pinelliae Tuber (Zhulibanxia, 2 qian) Fritillariae Bulbus (Beimu, 3 qian) Honey-fried Polygala Root (Yuanzhi, 1 qian) Whole Trichosanthis Fructus (Gualou, cut, 4 qian) Fresh Bamboo Sheath (Zhuru, 2 qian) Hooked Uncaria (Gouteng, added later, 3 qian)
Black Sesame Seed (Heihema, ground and wrapped, 3 qian)
Fourth visit: Consciousness is clear, tongue stiffness has eased, speech is not yet fluent, bowel movements are smooth, and phlegm-heat shows signs of descending. The left hand and foot remain paralyzed. Pulse is wiry, small, and rapid. Fluid depletion means sinews lack blood nourishment, like a withered tree lacking irrigation. Still propose nourishing Yin of the Lower Jiao, clearing heat of the Upper Jiao, transforming phlegm of the Middle Jiao, and activating blood in the meridians. Treating with compound formulas may still prolong life.

Elderly Woman Qi, one year since stroke onset. Left hand cannot lift, left foot cannot walk, tongue root seems stiff, speech is hesitant. Pulse is deep and fine at the Chi position, soft and slippery at the Cun and Guan positions. Tongue edges are peeled with thin greasy coating. Over seventy years old, Qi and Blood are both deficient. Pathogenic wind invaded the meridians, causing Bi obstruction of Ying and Wei Qi. Phlegm obstructs the tongue root, causing hesitant speech. The books state: "Qi warms, Blood moistens." Now appropriate to tonify Qi and nourish Blood, assist Yang to transform phlegm, and connect the collaterals. Hope for Yang generating Yin, Qi prospering and Blood flowing, so pathogenic wind can be removed and damp-phlegm self-resolved.

After taking thirty doses of this prescription, all ailments decreased. Subsequently taking paste formula led to recovery.
Elderly Woman Li, previously suffering from headache and vertigo. Suddenly tongue became stiff with aphasia, consciousness fluctuates between clear and obscure, limbs are flaccid, urinary incontinence. Pulse is fine and small at the Chi position, wiry, small, and rapid at the left Cun and Guan, deficient and slippery at the right Cun and Guan. Tongue is peeled and red. This is severe deficiency of Yin Blood, internal wind disturbing upward, phlegm-heat obstructing collaterals, blocking spiritual orifices. Severe stroke. Urgently prescribe moisturizing fluids to extinguish wind, clear Shen and eliminate phlegm, sweet-cool and moistening methods, hoping for rescue.

After taking ten doses of this prescription, all ailments lightened. Remove Bamboo Sap, Pearl Powder, and Tianshu Huang from the original formula. Add American Ginseng (Xiyangshen, 1.5 qian) and Prepared Donkey-hide Gelatin (Ejiao Zhu, 1.5 qian).
Li (Left), two years ago right thumb was numb. Suddenly tongue became stiff with hesitant speech, right hand and foot are numb and weak. Pulse is deficient, wiry, and slippery. Tongue coating is thin and greasy. This is a robust body with Qi deficiency, pathogenic wind entering collaterals, phlegm obstructing the tongue root, Shen Qi not agile. Initial severe stage of stroke. Urgently prescribe tonifying Qi to dispel wind, eliminate phlegm, and unblock collaterals.

Suddenly collapsed due to stroke, unconscious, urinary incontinence, rattling phlegm sound in throat, profuse sweating, hidden pulse, fever in body and cold limbs. This is prior extraction of the root, flying away of True Yang. Qi and Blood scattered, pivot not connecting. Although termed Zang organ involvement, it is actually sudden Collapse. Tentatively prescribe one formula to do what human effort allows.
