Fourth Visit: The chest oppression has subsided by seven or eight tenths, the fu-organ qi is now unobstructed, and turbid qi has descended. However, appetite remains poor, urine is scanty, red, dark, and turbid, with slight tidal fever in the evening. The pulse is soft, slippery, and rapid on the right, and wiry, fine, and slightly rapid on the left; the tongue coating is thin, greasy, and slightly yellow. Kidney yin was deficient prior to the onset of disease; damp-heat lingers in the Mu Yuan (semi-exterior semi-interior region), causing dysfunction of the San Jiao's diffusion and transformation, and weakness in the Spleen and Stomach's transportation and transformation. Mr. Ye Tianshi stated: "Damp-heat is a sticky, steaming pathogen, most difficult to resolve abruptly, hence the lingering nature of this condition." We again propose to diffuse qi and descend stomach qi, using bitter-cold methods to drain dampness.

Fifth Visit: Disciple Yu Jihong continued the consultation. Urine turbidity and redness have gradually faded, stomach qi is recovering, and hunger sensations are returning. Dizziness and mental fatigue occurred because he went hungry last night without eating, leading to floating yang disturbing upward. Epigastric fullness has been resolved, but slight discomfort persists in the afternoon, likely due to damp-heat pathogens flourishing in the afternoon and steaming upward. Although the Spleen and Stomach are gradually transporting, damp-heat still lingers in the San Jiao and cannot be cleared instantly. Excessive saliva indicates Spleen deficiency failing to control fluids. Now we adhere to the original method, adding herbs to harmonize the stomach and promote Spleen transport.

Sixth Visit: Diet has gradually increased, taste sensation has returned, indicating the recovery of Spleen and Stomach transport functions. Urine color has lightened, volume is slightly longer than yesterday, showing damp-heat descending, all favorable signs. Mental fatigue and weakness, eye distension when viewing objects, and photophobia indicate deficiency of righteous qi with floating yang disturbing upward. Saliva has decreased, showing the Spleen can now control fluids. Tongue coating is thin and greasy, yellow color has transformed. Pulse at Right Cun and Guan is harmonious, Left Guan is weak, both Chi positions are fine and soft; pathogenic factors are few, while righteous qi is deficient. We again prescribe Wendan Decoction (Phlegm-Resolving Decoction), adding herbs to support the Spleen, diffuse qi, and transform damp-heat, treating both root and branch simultaneously.

This formula originally used Immature Bitter Orange and Trichosanthes Peel, but they were removed because stool became loose.
Seventh Visit: Abdominal distension has eased, food tastes good, urine is gradually clear with only a pale yellow tint. Yesterday, one bowel movement was smooth and old-yellow colored, indicating long-standing damp-heat stagnation exiting via urination and defecation. This morning, an urge to defecate yielded no result, with slight blood observed, likely due to damp-heat accumulating in the Large Intestine blood level and reaching outward; this is not harmful. Spleen and Stomach transport function is restored, righteous qi recovers daily. One must be cautious with daily routine and diet to avoid complications. We again provide formulas to support the Spleen, diffuse transformation, and unblock Stomach qi.

Eighth Visit: The Spleen and Stomach are the root of nourishment; diet is the source of qi and blood. Illness causes deficiency; removing pathogens restores righteousness. Pathogens are gone, diet increases daily, urine clears with slight yellow tint, damp-heat in the San Jiao descends via excretions, Spleen and Stomach nourish effectively, and righteous qi strengthens. Tongue root coating remains greasy, not fully transformed. Pulse is harmonious except for fine/small Chi position. We again provide formulas to support the Spleen, harmonize the Stomach, and transform residual dampness.

Ninth Visit: Pulse gradually becomes harmonious; zang-fu organs and qi-blood increase daily. Post-illness convalescence prioritizes diet over medicine. Classics state: "The Stomach treasures grain intake." Also: "Non-toxic treatments remove ten of eight pathogenic factors; do not overdo it to injure righteousness." Nourishing the body is best achieved through mild diet. With pathogens gone, slow dietary regulation is appropriate. Residual minor pathogens will dissipate as righteousness strengthens; no need to worry. We again provide formulas to regulate Spleen and Stomach and transform residual pathogens.

Tenth Visit: Pathogens completely eliminated, appetite robust, pulse harmonious and spirited, righteous qi strengthening daily. Urine volume is high but color yellow, tongue coating thin and greasy, residual dampness not fully cleared. No bowel movement for four days due to liquid diet, not dryness constipation, so no concern. In summer, one should be cautious with routine and diet, carefully convalescing for a month to recover vitality. We again propose formulas to strengthen Spleen and Stomach transport and transform residual dampness.
