Mr. Fu, right side. He has a chronic history of chest and epigastric pain. Now, half a month after childbirth, the chest and epigastric pain has flared up severely, with severe pain causing vomiting, abdominal tenderness (refusal to press), inability to take food, aversion to cold, thin greasy gray tongue coating, wiry-tight pulse on the left, and slow-choppy pulse on the right. New external cold pathogen has triggered the rebellious Qi of the Jueyin Liver channel, causing food stagnation to obstruct the Middle Jiao, preventing Stomach Qi from descending. There is concern that severe pain may lead to complications. Urgently prescribed to dispel cold, regulate Qi, harmonize the Stomach, and eliminate stagnation, prioritizing pain relief first; the deficiency of constitution is temporarily neglected.
Guizhi Xin (Cinnamon Twig Heart) 3 fen each, Xian Banxia (Pinellia Rhizoma Prepared) 3 qian, Zuojin Wan (Left Metal Pill) wrapped, 6 fen, Gualou Pi (Trichosanthes Peel) stir-fried, 3 qian, Chenpi (Citrus Reticulatae Pericarpium) 1 qian, Xie Baitou (Allii Macrostemonis Bulbus) wine-fried, 1.5 qian, Yun Fuling (Poria) 3 qian, Da Sharen (Amomi Fructus) crushed, 1 qian, Jinlingzi (Meliae Semen) 2 qian, Yanhusuo (Corydalis Rhizoma) 1 qian, Zhishi Tan (Immature Bitter Orange Charcoal) 1 qian, Chao Guya Maiya (Stir-fried Hordeum Sprout and Setaria Germ) 3 qian each, Chen Foshou (Citrus Sarcodactylis Fructus) 8 fen, Shenren Dan (Shenren Pill) taken separately with boiled water, 4 fen.
Second visit: After two doses, chest and epigastric pain gradually decreased, vomiting stopped, but food tastes bland, dizziness occurs, heart palpitations and anxiety are present, thin greasy tongue coating, wiry pulse on the left, slow-rapid pulse on the right. This indicates inherent deficiency of Ying-Blood, rising Liver Qi and Liver Yang, unresolved dampness stagnation, and loss of Spleen-Stomach transportation function. Now prescribed to soften the Liver, drain the Liver, harmonize the Stomach, and unblock the Middle Jiao.
Chao Baishao (Stir-fried White Peony Root) 1.5 qian, Jinlingzi 2 qian, Yanhusuo 1 qian, Yun Fuling 3 qian mixed with Cinnabar, Xian Banxia 2 qian, Chen Guangpi (Aged Tangerine Peel) 1 qian, Gualou Pi 2 qian, Xie Baitou wine-fried 1.5 qian, Zidan Shen (Salvia Miltiorrhiza Radix et Rhizoma) 2 qian, Da Sharen crushed 1 qian, Zi Shiyin (Purple Quartz) 3 qian, Chen Foshou 8 fen, Chao Guya Maiya 3 qian each.
Third visit: Pain and vomiting have ceased, appetite is reduced, dizziness and palpitations persist. Original formula remove Yanhusuo, Jinlingzi; add Zhi Xiangfu (Prepared Cyperi Rhizoma) 3 qian, Qinglongchi (Dragon Bone) 3 qian.
Ms. Zhang, right side. Chest and epigastric pain for years, recurring frequently. Now pain radiates to the hypochondrium, Qi rebels with nausea, insomnia at night, thin yellow tongue coating, wiry pulse on the left, choppy pulse on the right. Caused by Blood deficiency failing to nourish the Liver, Liver Qi rebelling horizontally, invading the Stomach and suppressing the Spleen, loss of descending function. "When the Stomach is not harmonious, sleep is disturbed." The Liver is a rigid organ, requiring softening agents to control it; the Stomach benefits from unblocking. Now prescribed to soften the Liver, unblock the Stomach, and regulate Qi mechanism.
Sheng Baishao (Raw White Peony Root) 3 qian, Jinlingzi 2 qian, Zuojin Wan wrapped 8 fen, Zhu Fushen (Poria with Cinnabar) 3 qian, Xian Banxia 1.5 qian, Bei Shumi (Millet) wrapped 3 qian, Xuanfuhua (Inula Flower) wrapped 1.5 qian, Zhen Xinjiang (Safflower Silk) 8 fen, Zhi Wumei (Smoked Plum Fruit) 5 fen, Waleng (Arca Shell) 4 qian, Chuan Beimu (Fritillaria Cirrhosa Bulb) 2 qian, Jiangshui Chao Zhuru (Bamboo Shavings Stir-fried in Ginger Water) 1.5 qian.
Second visit: Chest and hypochondriac pain slightly reduced, but palpitations and insomnia persist, dizziness and nausea occur, internal heat and dry mouth, no desire to eat, dry constipation, wiry-thin-rapid pulse, red tongue edges with yellow coating. Excess Qi becomes Fire; intense internal Fire damages Yin, unrestrained Jueyang rises, Stomach Qi rebels and fails to descend. The Liver is rigid, aided by softening; the Stomach is dry earth, harmonized by Yin. Now prescribed to nourish Yin, soften the Liver, clear dryness, and unblock the Stomach.
Chuan Shihu (Dendrobium Stem) 3 qian, Sheng Baishao 2 qian, Jinlingzi 2 qian, Zuojin Wan wrapped 7 fen, Chuan Beimu 2 qian, Zhu Fushen 3 qian, Hei Shanzhi (Black Gardenia Fruit) 2 qian, Wumei Rou (Plum Meat) 5 fen, Zhenzhumu (Pearl Mother-of-Pearl) 6 qian, Qinglongchi 3 qian, Waleng 4 qian, Quan Gualou (Whole Trichosanthes Fruit) cut 3 qian, Qibi (Water Chestnut) washed and crushed 2 liang.
Mr. Zhang, right side. Chest and epigastric pain has lasted a month, radiating to the hypochondrium, poor appetite with nausea, red tongue body with yellow coating, wiry-rapid pulse. Caused by Qi stagnation transforming into Fire, consuming Stomach Yin, Stomach Qi failing to descend, excessive Liver rising. As stated in medical texts: "Acute pain belongs to Cold, chronic pain belongs to Heat; acute pain is in the Channels, chronic pain is in the Collaterals." Should use draining Liver, regulating Qi, harmonizing Stomach, and unblocking Collaterals.
Sheng Baishao 3 qian, Jinlingzi 2 qian, Zuojin Wan wrapped 7 fen, Hei Shanzhi 2 qian, Chuan Shihu 3 qian, Chuan Beimu 2 qian, Gualou Pi 3 qian, Dai Ge San (Malaquita and Clam Shell Powder) wrapped 4 qian, Xuanfuhua wrapped 1.5 qian, Zhen Xinjiang 8 fen, Waleng 4 qian, Dazi Sigualuo (Loofah Tendril with Seeds) 2 qian.
Follow-up visit: After two doses, pain reduced, vomiting stopped. Remove Zuojin Wan from original formula; add Nanshashen (Adenophora Root) 3 qian, Hehuanpi (Albizia Bark) 1.5 qian.
Child Zhu. Epigastric pain prefers pressure, relieved by eating, wiry-slow pulse, thin white tongue coating. Middle Jiao deficiency with Cold invasion, Liver-Spleen Qi stagnation. Prescribed Xiao Jianzhong Tang (Minor Construct the Middle Decoction) modified.
Da Baishao (Large White Peony Root) 3 qian, Zhi Gancao (Honey-fried Licorice Root) 1 qian, Rouguixin (Cinnamon Bark Heart) 4 fen, Yun Fuling 3 qian, Chen Guangpi 1 qian, Chun Shaqiao (Amomi Fructus Shell) 8 fen, Wumei Rou 4 fen, Quan Danggui (Whole Angelica Sinensis Root) 2 qian, Wei Jiang (Ginger Roasted) 2 slices, Hongzao (Jujube) 4 pieces, Yitang (Maltose) melted and taken 4 qian.
Mr. Wei, left side. Epigastric and abdominal pain for two years. Pain eases and bloating occurs after full meals; pain intensifies with palpitations when slightly hungry, pale white tongue, wiry-thin pulse on the left, deficient-slow pulse on the right. A robust constitution implies Middle Qi deficiency. Deficient-Cold and Qi stagnation cause pain; dispersed rebellious Qi causes bloating; Liver Wood insults Spleen Earth, Middle deficiency craves food. Previous treatments with Da/Xiao Jianzhong Tangs were ineffective, not due to wrong prescription, but because the disease is deep and medicine is shallow. Originally prescribed Xiao Jianzhong Tang plus Xiaochaihu Tang (Minor Bupleurum Decoction), combined with Jinggong Miaoxiang San (Prime Minister's Miraculous Incense Powder), using compound formulas, following the principle "if singular method fails, try paired method." First soothe Liver Wood so Middle Qi can be balanced.
Da Baishao 3 qian, Zhi Gancao 1 qian, Rouguixin 4 fen, Lushen Shen (Codonopsis Root) 3 qian, Yinzhou Chaihu (Bupleurum Root from Yinzhou) 1.5 qian, Xian Banxia 2 qian, Yun Fuling 3 qian, Chen Guangpi 1 qian, Wumei Rou 4 fen, Quan Danggui 2 qian, Wei Jiang 3 slices, Hongzao 5 pieces, Yitang melted 6 qian.
Miaoxiang San Formula: Ren Shen (Ginseng Root) 1.5 qian, Zhi Huangqi (Astragalus Root Honey-fried) 1 liang, Huai Shanyao (Dioscorea Rhizome) 1 liang, Fuling Shen (Poria) 5 qian each, Longgu (Fossilized Bones) 5 qian, Yuanzhi (Polygalae Radix) 3 qian, Jiegeng (Platycodon Root) 1.5 qian, Muxiang (Aucklandiae Radix) 1.5 qian, Gancao 1.5 qian.
Grind above herbs into powder, take 2 qian daily, delivered with aged wine; rice soup can be used if alcohol is not tolerated.
Original Note: Mr. Wei is from Anqing, suffering for two years, having taken hundreds of prescriptions without effect. He came specifically for treatment. After five consecutive visits, maintaining the same prescription, he recovered after taking fifteen doses.
Mr. Guan, right side. History of epigastric pain, now extreme pain leading to syncope. During syncope, jaw clamps shut, limbs are cold, unconsciousness, recovering after some time, thin greasy tongue coating, deep-slow-pulse resembling hidden pulse. Caused by depression and anger injuring the Liver, Liver Qi rebelling horizontally, Phlegm stagnation obstructing, Stomach losing harmony and descent, Liver distension causes pain, Qi closure causes syncope. Wood likes free flow, Stomach likes descending. Now prescribed to unblock Qi mechanism, drain Jueyin, resolve Phlegm stagnation, and smooth the Middle Jiao.
Yinzhou Chaihu 1.5 qian, Da Baishao 1.5 qian, Qing Zhi Cao (Clear Honey-fried Licorice) 5 fen, Zhishi Tan 1 qian, Jinlingzi 3 qian, Yanhusuo 1 qian, Chuan Yujin (Curcuma Root) 1.5 qian, Chenxiang Pian (Agarwood Slice) 4 fen, Chun Shaqiao 8 fen, Yun Fuling 3 qian, Chen Guangpi 1 qian, Chao Guya Maiya 3 qian each, Suhexiang Wan (Styrax Pill) shell removed, ground and dissolved, one pill.
Second visit: After two doses, syncope resolved, pain stopped, only chest/epigastric fullness, belching, no desire to eat, dry constipation, wiry-choppy pulse on left/right. Jue Qi is calming, Spleen-Stomach disharmony, transportation lost its normal rhythm. Now prescribed to soften/drain Liver, harmonize Stomach, smooth Middle Jiao, and maintain emotional balance to assist medicinal efficacy.
Quan Danggui 2 qian, Da Baishao 2 qian, Yinzhou Chaihu 1 qian, Yun Fuling 3 qian, Chen Guangpi 1 qian, Chao Zhiqiao (Stir-fried Immature Bitter Orange) 1 qian, Chuan Yujin 1.5 qian, Jinlingzi 2 qian, Chenxiang Pian 4 fen, Chun Shaqiao 8 fen, Quan Gualou cut 4 qian, Foshou 8 fen, Chao Guya Maiya 3 qian each.
Elderly Ms. Huang. After great anger, immediate chest/epigastric pain. Extreme pain leads to uncontrollable laughter; extreme laughter leads to syncope with unconsciousness, jaw clamping, limb coldness, recovery after time, occurring over ten times daily. Deep-slow-pulse resembling hidden, thin greasy coating. Caused by depression/anger injuring Liver, rebellious Qi of Foot Jueyin rising from below, affecting Hand Jueyin channel, Qi closure causes syncope, blockage causes pain, Qi return brings recovery. Classic states: "Great anger causes form and Qi to sever, blood accumulates above, causing sudden syncope." Urgently prescribed to unblock Qi mechanism, drain Jueyin, stop pain and syncope. Not presumptuous, awaiting wise judgment.
Chuan Yujin 2 qian, Hehuanpi 1.5 qian, Jinlingzi 2 qian, Yanhusuo 1 qian, Zhu Fushen 3 qian, Zhi Yuanzhi (Prepared Polygalae Radix) 1 qian, Qinglongchi 3 qian, Chenxiang Pian 5 fen, Chun Sharen (Amomi Fructus) crushed 8 fen, Chen Guangpi 1 qian, Waleng 4 qian, Gold Utensil (for decoction) one piece, Suhexiang Wan shell removed, ground, dissolved in boiled water first, two pills.
Second visit: Since medication, pain, syncope, and laughter have stopped. Only chest/epigastric stuffiness, belching, inability to eat, wiry-choppy pulse on left/right. Jue Qi is calm, Spleen-Stomach not harmonized, Middle Jiao transportation powerless. Now prescribed to drain Liver, unblock Stomach, expand Qi mechanism, maintain emotional balance, light diet, to prevent recurrence.
Da Baishao 1.5 qian, Jinlingzi 2 qian, Daizhe Shi (Haematite) 2 qian, Xuanfuhua wrapped 1.5 qian, Zhu Fushen 3 qian, Zhi Yuanzhi 1 qian, Xian Banxia 2 qian, Chen Guangpi 1 qian, Chun Sharen crushed 8 fen, Zhi Xiangfu 1.5 qian, Chuan Yujin 1.5 qian, Foshou 8 fen, Chao Guya Maiya 3 qian each.
Ms. Shen, right side. Worry and overthinking injure the Liver, Liver lacks Blood nourishment, deficient Qi does not return, epigastric pain prefers pressure, palpitations, scant sleep. Previous Liver-draining Qi-regulating formulas taken for many doses were ineffective. Now imitating *Jinkui* Liver deficiency disease: supplement with Sour, assist with Bitter, benefit with Sweet medicines to regulate.
Da Baishao 3 qian, Zhi Gancao 1 qian, Jinlingzi 2 qian, Chao Zaoren (Stir-fried Ziziphus Seed) 3 qian, Wuweizi (Schisandra Fruit) 4 fen, Ejiao Zhu (Donkey-hide Gelatin Pearl) 2 qian, Zuo Muli (Left Oyster Shell) 3 qian, Qinglongchi 3 qian, Zhi Yuanzhi 1 qian, Zhu Fushen 3 qian, Lushen Shen 1.5 qian, Chenpi 1 qian, Yitang melted 4 qian.
Mr. Li, right side. Hypochondrium is the domain of the Liver, Liver Qi enters Collaterals, hypochondriac pain偏向 left, difficulty turning, chest stuffiness, poor appetite, even nausea. From winter to spring, pain intensity increases. Ancients said: "Acute pain in Channels, chronic pain in Collaterals." Imitate treatment for Ganzhe (Liver Fixation) case.
Xuanfuhua wrapped 1.5 qian, Zhen Xinjiang 8 fen, Da Baishao 2 qian, Jinlingzi 2 qian, Zuojin Wan wrapped 7 fen, Ju Bai Luo (Tangerine Peel and Tendril) 1 qian each, Chao Zhuru (Stir-fried Bamboo Shavings) 1 qian, Chun Shaqiao 8 fen, Danggui Xu (Angelica Sinensis Root Tip) 1.5 qian, Sigualuo (Loofah Tendril) 2 qian, Chuan Yujin 1.5 qian, Zijiangxiang (Red Sandalwood) 4 fen.
