Pulse: The pulse in malaria is mostly wiry (Xian, 弦). If wiry and rapid (Shu, 数), it indicates more heat; sweating method (Han fa, 汗法) is suitable. If wiry and slow (Chi, 迟), it indicates more cold; warming method (Wen fa, 温法) is suitable. If wiry, tight, and solid, purgation method (Xia fa, 下法) is suitable. If wiry, deficient, and thin, tonification method (Bu fa, 补法) is suitable. If wiry, excess, and large, emesis method (Tu fa, 吐法) is suitable. A short pulse indicates food retention; a slippery (Hua, 滑) pulse indicates phlegm-dampness. If the malaria pulse is slow and relaxed, the disease will heal itself. In chronic malaria that does not heal, the pulse must be deficient; supporting healthy qi to expel pathogenic factors (Yang zheng qu xie, 养正祛邪) is suitable.
Malaria arises from external contraction of wind-cold, summer-heat, and dampness, internal injury from diet and overwork, or excessive hunger/fullness and sexual desire, leading to disharmony of the Spleen and Stomach and phlegm stagnating in the Middle Jiao (Zhong Wan, 中脘). However, without phlegm, there is no malaria. The Spleen and Stomach belong to Earth; their Qi movement has regularity and does not lose its timing. If the timing shifts, being either early or late, it means the pathogen has no place to reside, and the malaria is about to heal. When malaria attacks, symptoms include yawning, aversion to cold, cold hands and feet, chills, high fever with thirst, headache, soreness and pain in the waist, hips, and joints. Some have cold first then heat, some heat first then cold, some only cold or only heat, some more cold than heat, some more heat than cold. Attacks once a day indicate shallow disease location, which is easy to treat. Attacks every other day, or two days consecutively followed by one day off, are difficult to cure. Treatment: if on the exterior with no sweat, use San Xie Tang (Scatter Pathogen Decoction) as the main formula; if with sweat, use Zheng Qi Tang (Upright Qi Decoction) as the main formula; if at half-exterior half-interior, use Chai Ling Tang (Bupleurum and Polyporus Decoction) as the main formula; if Yin and Yang separation is not yet resolved, use Ren Shen Yang Wei Tang (Ginseng Stomach-Nourishing Decoction) with modifications, after which malaria-stopping drugs can be used. If using malaria-stopping drugs causes vomiting of yellow, sticky mucus, the malaria will heal itself. Do not stop malaria too early (within one or two days); doing so causes pathogenic factors to block and become complicated syndromes. Do not stop malaria too late; doing so exhausts Yuan Qi (Original Qi) and leads to deficiency and timidity syndromes. It is best to stop malaria around the third or fourth day. One must wait until fever subsides and body cools before eating. Do not eat while still having residual heat, lest indigestion form a mass called "Malaria Mother" (Nu Mu, 疟母); if this mass dissipates, it may become drum distension (Gu Zhang, 鼓胀).
Generally, for initial malaria, scattering pathogen and supporting upright Qi come first. For no sweat, induce sweat; focus on scattering pathogen.
San Xie Tang (Scatter Pathogen Decoction): Treats initial malaria with aversion to cold, severe fever, headache, body ache, and no sweat.
