A certain female patient has suffered from dysmenorrhea for a long time. The Qi and Blood are not flowing smoothly. Sudden severe anger injured the Liver. The rushing Qi in the lower abdomen rebels upward and invades the Lung, causing cough. Accompanied by alternating chills and fever, hoarseness of voice, and tension in the hypochondrium. No appetite and inability to eat. This indicates that the ascending on the left and descending on the right cannot rotate properly, leading to a violation of the principle that Stomach Qi should descend smoothly. Therefore, the function of the Liver should be vented/discharged, and the Stomach organ should be unblocked and descending; this is a fixed rule.

A certain patient: Labor and anger damaged Yang Qi. Rebellious Qi and stagnant Blood cause pain. Fullness, distension, loose stools. Wind-Wood insulting Earth. Since the previous formula was effective, provide a method to unblock and tonify Yangming and Jueyin. Dabansha Tang, remove honey, add Tao Ren, Bai Zi Ren, Dang Gui, Sheng Jiang, Da Zao, decoct as soup or make into pills.
A certain patient: Pulse is minute, small, and weak. This indicates Yang Qi is already declining. This year, Taiyin governs the sky. In midsummer, heat leaks into the Qi level. Cannot eat, transportation fails. Taste changes to sour and bitter. Spleen and Stomach are first constrained. Slight anger causes Wood to overcome Earth, adding to unease. Again: Anger caused pain below the epigastrium, inability to eat. This is Liver Jue invading Spleen and Stomach. Disease generating branches externally. This is certainly not good management. The Liver is the organ of Wind-Wood, and also the General Officer. Its nature is urgent and active. Therefore, diseases of the Liver are more numerous than those of other organs, especially in women.
Liver disease must invade Earth, which is insulting what it overcomes (reverse restriction). The symptoms of the primary organ, Zhang Zhongjing said: The disease of Jueyin involves thirst, Qi rushing up to strike the heart, pain and heat in the heart, hunger but no desire to eat, vomiting roundworms after eating, and if purged, diarrhea will not stop. Also, the liver diseases recorded in Neijing cannot be fully described. Generally, the pulse must be wiry. Hypochondrium may be distended or painful, biased towards cold or heat, first Jue (cold extremities) then heat. If it once invades the Stomach, then nausea, dry heaving, epigastric fullness, no desire to eat, vomiting sour water and saliva. Insulting the Spleen causes abdominal distension, stool may be loose, or不爽 (not smooth), cold limbs, numb skin. The treatment methods in the cases differ in Yin/Yang and Deficiency/Excess. Briefly listed and narrated as follows. If Liver Yin and Stomach Yin are not deficient, but Liver Yang is hyperactive and invading the Stomach, Mr. Ye's legislation uses medicine that avoids softness and uses rigidity. To vent the Liver, use Wu Zhu Yu, Jiao (Sichuan Pepper), Gui (Cinnamon); to unblock the Stomach, use Ban Xia, Ginger Juice, Jiang Fu (Ginger and Aconite). Adding Yi Zhi, Zhi (Immature Bitter Orange), Po (Magnolia Bark) simultaneously transports Spleen Yang. For Middle Jiao deficiency, Ren Shen must be used. Thus Dabansha Tang, Fuzi Jingmi Tang, Tuijin Huanglian Tang, Xiexin Fa, Zhizhong Fa, Wendan Tang, etc., are these types. If Liver Yin and Stomach fluid are already deficient, Wood-Fire is blazing, Wind-Yang disturbs the Stomach, medication should avoid rigidity and use softness. To nourish the Liver, use E Jiao, Sheng Di, Bai Shao, Ma Ren, Mu Gua; to nourish the Stomach, use Ren Shen, Mai Dong, Zhi Mu, Jing Mi, Shu Mi, etc., are these types. As for the method of peaceful treatment, rigidity and softness, cold and heat are used together. Wumei Wan, Anwei Wan, Xiaoyao San. If Sijunzi, Liu Junzi, Yigong, Jiufang Tang, etc., must add products that vent the Liver. Those using Sang Ye and Dan Pi, Mr. Ye said: Sang Ye is light and clear, clearing and venting Shaoyang Qi-heat; Dan Pi is bitter and pungent, clearing and venting Liver-Gallbladder Blood-heat. Using Jinlingzi San, Chuan Lian Zi is bitter and cold, directly venting Liver Yang; Yan Hu Suo specializes in regulating Qi stagnation and Blood sluggishness pain. These are all the outlines in the cases. I separated this category separately because symptoms such as vomiting, inability to eat, hypochondriac distension, and epigastric fullness, doctors might only consider them Spleen-Stomach diseases, not knowing they are actually caused by Liver pathogenic factors. So I specially reveal them to awaken the eyes of later generations. Moreover, people only know Feng, Lao, Gu, Ge as the four major serious illnesses, not knowing that when Earth fails and Wood attacks, Liver Qi becomes rampant day by day, Spleen and Stomach decline day by day, delaying until incurable cases are many. Can one not study this? (Hua Xiuyun)
Xu Ping: Most cases are basically Chest Bi (obstruction) and fullness, as well as symptoms of internal accumulation of Phlegm-Fluid. Formulas for treating Bi and fullness strictly forbid Ren Shen. But here, the intention is to strengthen Earth to control Wood, so Ren Shen is used very often. There must be residual diseases. Regarding Phlegm accumulation, it is not mentioned at all, which particularly belongs to unclear syndrome differentiation. Among them, there are many places of profound thought and clever intent that hit the target, but it is also half gain and half loss.
