Patient Zhang: Hidden heat pathogens accumulate in the Sanjiao (Triple Burner). Heat from the Heart channel causes fever, irritability, and thirst. Red macules appear all over the body. Restlessness at night with insomnia. Both pulses are rapid and forceful. (Hidden heat in the Sanjiao)
Prescription: Antelope Horn (Lingyang Jiao), Rhinoceros Horn (Xijiao - *Note: Use substitutes in modern practice*), Calyx of Forsythia (Lianqiao Xin), Heart of Scrophularia (Xuanshen Xin), Fresh Rehmannia Root (Xian Shengdi), Honeysuckle Flower (Jinyinhua), Trichosanthes Root (Huafen), Acorus Tatarinowii Rhizome (Shichangpu).
Second visit: Alternating chills and fever. There must be tangible pathogenic factors attacking and touching, affecting the area below the epigastrium. Thirst, preferring warm drinks. The disease has manifested but refuses to recede. This indicates the pathogen has been hidden for a long time at the boundary of the Jueyin Liver Channel.
Prescription: Cinnamon Twig (Guizhi), Coptis Rhizome (Chuanlian), Scutellaria Root (Huangqin), Trichosanthes Root (Huafen), Oyster Shell (Muli), Immature Bitter Orange (Zhishi).
Patient Yan: Damp-warmth pathogens are contracted mixedly. Macules and rashes appear on the body. Drinking water does not relieve thirst. Nighttime irritability prevents sleep. Forced eating during illness only aids the pathogenic vigor. It is proposed to use the method of Liangge Decoction (Cool the Diaphragm) modified to vent the macules. (Damp-warmth)
Prescription: Calyx of Forsythia (Lianqiao), Mint (Bohe), Apricot Seed (Xingren), Curcuma Zedoaria Tuber (Yujin), Juice of Immature Bitter Orange (Zhishi Zhi), Stir-fried Arctium Fruit (Chao Niubang), Gardenia Fruit (Shanzhi), Gypsum (Shigao).
Second visit: The edges of the tongue are red. Confusion and delirious speech. Symptoms are lighter in the morning and heavier at night. Macules and rashes are faint. This indicates that warm-dampness has entered the blood collaterals. The Heart governs blood. Pathogens interfere with the Danzhong (Chest/Pericardium region), gradually leading to blockage, creating a critical danger of coma and convulsions. Bitter, sinking, and cold medicines entering the Middle Jiao directly; acrid-warm digestive medicines merely deplete stomach fluids. These are major contraindications for warm pathogens. It is proposed to use light agents clearing and dispersing the Blood level to vent the macules, further incorporating aromatic substances to expel turbidity and open the internal orifices. Yu Jiayan of recent times clearly stated these prohibitions, which should be followed.
Prescription: Rhinoceros Horn (Xijiao), Scrophularia Root (Xuanshen), Calyx of Forsythia (Lianqiao), Honeysuckle Flower (Yinhua), Acorus Tatarinowii Rhizome (Shichangpu).
Decoct first until reduced to 6 fen. Then mix in one cup of Snow White Platinum Juice (Xuebai Jinzhi). Before administration, grind in one pill of Zhou Shaochuan's Calculus Bovis (Niuhuang Wan).
Patient Jiang: Warm pathogens cause rash eruption. Damp-heat accumulates internally. Stool is closed and unpassable. First, open the Upper Jiao.
Prescription: Apricot Seed (Xingren), Perilla Seed (Suzi), Pepo of Trichosanthes (Gualou Pi), Aster Root (Ziwan), Gardenia Fruit (Shanzhi).
Certain Patient: Wind-warmth causes Sha-rash. (Wind-warmth)
Prescription: Mint (Bohe), Calyx of Forsythia (Lianqiao), Apricot Seed (Xingren), Arctium Fruit (Niubangzi), Platycodon Root (Jiegeng), Mulberry Bark (Sangpi), Licorice Root (Gancao), Gardenia Fruit (Shanzhi).
Certain Patient: Wind-warmth causes rash eruption.
Prescription: Mint (Bohe), Red Peony Root (Chishao), Calyx of Forsythia (Lianqiao), Arctium Fruit (Niubangzi), Platycodon Root (Jiegeng), Mulberry Bark (Sangpi), Licorice Root (Gancao), Gardenia Fruit (Shanzhi).
Patient Fei: Violent cold suddenly adds to the condition, hidden heat becomes more intense. Pathogen stagnation causes Qi and Blood obstruction. Sha-rash refuses to reach the exterior. Phlegm and Qi obstruct each other. Spirit is confused, panting and short of breath. Gradually entering the Pericardium Collateral. There is worry of internal closure and external collapse. Heat pours downward, causing spontaneous diarrhea that is sticky and unsatisfactory. The method should be to open the blockage, detoxify, and resolve the congestion in the Danzhong. Only when the spirit clears can changes be prevented. (Heat pathogen entering Pericardium Collateral)
Prescription: Calyx of Forsythia (Lianqiao Xin), Calcined Talcum (Fei Huashi), Acorus Tatarinowii Rhizome (Shichangpu), Stir-fried Honeysuckle Flower (Chao Jinyinhua), Belamcanda Rhizome (Shegan), Tongcao Stem.
Decoct and dissolve one pill of Calculus Bovis (Niuhuang Wan).
Zhu (12 years old): After Sha-rash, much phlegm, coughing, and rapid breathing. (Damp-heat stagnating in Lungs)
Prescription: Reed Rhizome (Lugen, 1 liang), Apricot Seed (Xingren, 1 qian 5 fen), Platycodon Root (Jiegeng, 1 qian), Calcined Talcum (Fei Huashi, 1 qian 5 fen), Mulberry Bark (Sangpi, 8 fen), Tongcao Stem (Tongcao, 1 qian).
Certain Patient: After Sha-rash, hidden fire is not cleared. Internal heat and body pain. (Yin injury after Sha-rash)
Prescription: Polygonatum Reticulatum Rhizome (Yuzhu), Naring Radix (Baisha Shen), Moutan Cortex (Digupi), Dendrobium Stem (Chuanhu), Ophiopogon Tuber (Maidong), Raw Licorice Root (Sheng Gancao).
Certain Patient: After Sha-rash, heat persists. Yin injury.
Prescription: Raw White Peony Root (Sheng Baishao), Honey-fried Licorice Root (Zhi Gancao), Raw Coix Seed (Sheng Biandou), Stir-fried Ophiopogon Tuber (Chao Maidong), Dendrobium Stem (Chuanhu), Rice Sprout (Guya).
Patient Zhang: Cool wind attacks externally, hidden heat steams internally. Autumn Metal rules, corresponding internally to the Lung. Panting, coughing, and body heat. Initially daytime heat, subsequently evening heat. From the Qi level gradually involving the Blood level. Gingival flesh turns purple, and rashes erupt from the muscles. Acrid-cold clearing and dispersing is appropriate. (External cold, internal heat)
Prescription: Mint (Bohe), Calyx of Forsythia (Lianqiao), Gypsum (Shigao), Light Bamboo Leaf (Dan Zhuye), Apricot Seed (Xingren), Mulberry Bark (Sangpi), Coix Seed (Yiren).
Patient Wu: Disease erupted during violent cold. Skin surface and head/face did not transmit. This is external accumulation of cold and internal hidden heat. Lung diseases govern Wei-Qi. Resolving both the Wei-Qi and Qi levels is appropriate.
Prescription: Ephedra Herb (Mahuang), Gypsum (Shigao), Arctium Fruit (Niubangzi), Juice of Tangerine Peel (Zhike Zhi), Apricot Seed (Xingren), Belamcanda Rhizome (Shegan), Platycodon Root (Jiegeng), Raw Licorice Root (Sheng Gancao).
Patient Yin: Dry cracking and pain around the mouth. Urticaria-like red patterns appear on the head, face, and upper half of the body. This is Yangming Blood-Heat, accumulating into toxin over time. Thin people tend toward heat and often have this symptom. Why do physicians fail to recognize it? (Yangming Blood-Heat)
Prescription: Xijiao Dihuang Tang (Rhino-Horn and Rehmannia Decoction).
Patient Jiang: Warm pathogens cause spontaneous diarrhea, accompanied by urticaria. (Warm pathogens sinking inward)
Prescription: Scutellaria Root (Huangqin), Calyx of Forsythia (Lianqiao), Arctium Fruit (Niubangzi), Platycodon Root (Jiegeng), Fermented Soybean (Xiangchi), Mint (Bohe), Apricot Seed (Xingren), Tangerine Peel (Juhong), Tongcao Stem.
Li (27 years old): Eruptive scrofula and hot swelling, appearing only on the front of the face. Severe outbreaks occur every September-October, improving gradually in May-June. This has persisted for seven or eight years. I reason that in summer, Yang Qi spreads freely, Ying and Wei circulate without interruption. In autumn and winter, cold air presses externally, causing Qi and Blood stagnation. This is damp-heat having no outlet for release, resulting from the stagnation of Shaoyang Wood-Fire and the steaming dampness of Yangming. Thus, the Upper Jiao is particularly affected. The method is acrid-cool, assisted by bitter-cold, so that stagnant heat in the Yang level can be dispersed, hopefully slowing the outbreak. (Gallbladder Fire and Stomach Dampness Stagnation)
Prescription: Prunella Spike (Xiakucao), Fresh Chrysanthemum Leaf (Xian Juye), Kudingcha Leaf, Edge of Fresh Lotus Leaf (Xian Heyebian), Antelope Horn (Lingyang Jiao), Black Gardenia Peel (Hei Zhipi), Curcuma Zedoaria Tuber (Yujin), Coix Seed (Yiren).
Tang (45-46 years old): Numbness, sudden high swelling, and eruptive scrofula. Must be wind-dampness invading the skin membranes. This is a disease of the body shell. Ancients often used promoting and unblocking agents. (Wind-dampness)
Prescription: Turmeric Tuber (Pian Jianghuang), Antelope Horn (Lingyang Jiao), Cinnamon Twig (Chuan Guizhi), Ligusticum Chuanxiong Rhizome (Fuxiong), Pinellia Tuber (Banxia), White Mustard Seed (Baijiezi).
Certain Patient (19 years old): Wind wheals with itching, coughing, and abdominal pain. Pathogens adhere to the exterior and interior. Double harmonization should be used.
Prescription: Arctium Fruit (Niubangzi), Apricot Seed (Xingren), Calyx of Forsythia (Lianqiao), Platycodon Root (Jiegeng), Mulberry Branch (Sangzhi), Fritillaria Bulb (Xiang Beimu).
Decoct medicine and swallow with Fangfeng Tongsheng Wan.
Sha-rash presents visible colors but no texture under touch. Dense like brocade patterns, sparse like mosquito bites. Distributed on the chest and abdomen, or seen on the four limbs. Generally, bright red eruption is auspicious. Purple patches indicate severe condition. Black color indicates ominous prognosis. Cyan color indicates untreatable condition. What is inside manifests outside, allowing judgment of organ safety and victory/defeat of pathogenic vs. healthy Qi. This likely occurs when Shanghan (Cold Damage) and plague symptoms fail to be resolved via dispersion, pathogens accumulate in the Stomach Fu, and enter the Ying (Nutrient) level. Examining medical texts, treatment methods vary. Generally, those caused by failure to sweat should seek diaphoresis; those caused by failure to purge should seek purgation. If fire is excessive, clear it; if toxin is excessive, transform it. If Ying Qi is deficient, assist deficiency and harmonize, or support and lift. Regarding "Yin Rash," the signs are very subtle. If one insists on pointing out tiny spots as Yin Rash, errors may occur. I suspect predecessors established this example merely to awaken later generations not to rigidly view certain phenomena as excess heat. Therefore, I say: one must consult pulse conditions and accompanying symptoms to be safe. "Sha" is a general term for rash, having granules like millet seeds. "Yin" is a type of rash, swollen and itchy. One must know that eruption should be even, and subsidence should be slow. It does not exceed disorders of the Taiyin and Yangming channels. Thus, Miu's approach of treating specifically via Lung and Stomach is refined. If Mr.'s method is based on the Four Qi, following seasonal dominance and recovery, using acrid-cool, acrid-warm, sweet-cold, bitter-cold, salty-cold, and bland-leaching methods for treatment. All my pediatric friends should study this especially carefully. (Annotation by Shao Xinpu)
