Yang excess and Yin deficiency lead to insomnia throughout both day and night. Night is governed by Yin; when Yin Qi (Vital Energy) is abundant, the eyes close and sleep is peaceful. If Yin is overcome by Yang, there will be restlessness and insomnia all night long. This is what is meant by "Yin deficiency leads to nocturnal contention." With sweating, dry nose, and inability to lie down, the pathogenic factor is in the exterior. If there are dry feces in the stomach accompanied by high fever and delirious speech, along with profuse sweating causing depletion of gastric fluids leading to an inability to lie down, the pathogenic factor is in the interior. Disharmony of the Stomach results in restless sleep, so it is appropriate to use methods that clear heat and harmonize the Stomach. After sweating, vomiting, or purging, if deficiency-heat irritability and insomnia occur, one should employ Zhi Zi Chi Tang (Gardenia and Fermented Soybean Decoction) to induce vomiting and expel trapped heat. In cases of Shaoyin Disease (Interior Cold-Heat Complex Syndrome of the Shaoyin Channel) where internal heat irritability causes insomnia, it is appropriate to support Yin and clear heat. Following recovery from illness, if insomnia persists, it is because residual heat combines with various Yang energies while Yin has not yet recovered. If excessive sweating or purging causes a sudden deficiency of Yang leading to insomnia, and there are no signs of heat, Jiang Fu Tang (Ginger and Aconite Decoction) should be used to dispel Yin-cold and restore Yang. These points must not be overlooked.
In Taiyang Disease (Exterior Syndrome of the Taiyang Channel), if for two or three days the patient cannot lie down but wishes to sit up, has epigastric fullness, and a faint pulse, this indicates pre-existing cold within. If purgatives are incorrectly administered, and diarrhea stops, chest binding (Jie Xiong) will definitely form; if diarrhea does not stop and purgatives are administered again after three or four days, diarrhea with accompanying heat (Xie Re Li) will result. For chest binding, use Li Zhong Wan (Regulate the Middle Pill); for diarrhea with accompanying heat, use Bai Tou Weng Tang (Pulsatilla Decoction).
In Shaoyin Disease, if there is diarrhea, cough, vomiting, thirst, and irritability with insomnia, Zhu Ling Tang (Polyporus Decoction) is indicated, signifying water retention. If irritability and insomnia occur after two or three days, Huang Lian E Jiao Tang (Coptis and Donkey-Hide Gelatin Decoction) is indicated.
If a patient has difficulty urinating, irregular bowel movements (alternating between difficult and easy), occasional slight fever, wheezing, dizziness, and inability to lie down, there are dry feces, and Da Cheng Qi Tang (Major Order the Qi Decoction) should be used.
After purging followed by sweating, if the patient is irritable and insomniac during the day but quiet at night, without vomiting or thirst, lacking both exterior and interior syndromes, with a deep and faint pulse, and no significant body heat, Gan Jiang Fu Zi Tang (Dried Ginger and Aconite Decoction) is indicated.
After vomiting, purging, or sweating, if there is profound discomfort (Ao Nao), deficiency-heat irritability, and insomnia, Zhi Zi Chi Tang is indicated. When Yang overcomes Yin, resulting in delirious speech, chaotic dreams, irritability, and shortness of breath, Suan Zao Tang (Sour Jujube Seed Decoction) is indicated. When Yin overcomes Yang, resulting in palpitations, lethargy, severe heat, dry heaving, groaning, delirious speech, and insomnia, Xi Jiao Jie Du Tang (Rhino Horn Detoxification Decoction) is indicated.
In Wind Attack (Zhong Feng), with sweating, a soft and weak pulse, cold limbs, and restlessness with insomnia, Xiao Jian Zhong Tang (Minor Construct the Middle Decoction) is indicated; if accompanied by irritability, Zhu Ye Shi Gao Tang (Bamboo Leaf and Gypsum Decoction) is indicated.
Following recovery, if alcohol consumption leads to vexation, nausea, dry heaving, dry mouth, groaning, delirious speech, and insomnia, Xi Jiao Jie Du Tang is indicated.
