Jueyin (Jueyin Meridian) governs with Wind-Wood Qi. In individuals with a Yin-dominant constitution, if the pathogen is in the Taiyang (Greater Yang) meridian and Liver Wood is depressed about to erupt, any erroneous treatment such as sweating, purging, or warm needle therapy will cause Yang Qi to fail and Cold Water to flourish. Yi Wood (Liver) loses warmth, its vital generating Qi is obstructed, and thus Jueyin diseases arise successively.
If a patient has chronic cold and sweating is induced again, the Stomach becomes cold, inevitably leading to vomiting of roundworms. If the internal organs have accumulated cold for a long time, inducing sweating destroys Stomach Yang; the cold Stomach cannot settle the roundworms, resulting in their expulsion. Roundworms are generated by Wood transformation. The *Suwen* states: Jueyin is the organ where Yin ends and Yang begins. As the most Yin organ, extreme cold causing vomiting of roundworms indicates that Water Qi surges upward while Fire Qi fails to return to the Middle Jiao, revealing a latent fatal mechanism. Therefore, among various external and internal injuries, the appearance of vomiting roundworms constitutes a critical condition. Death occurs when Yang Qi perishes; life returns when Yang Qi recovers. Only in warm diseases does vomiting roundworms indicate Heat rather than Cold, differing from other syndromes.
When Taiyang disease does not resolve, it may enter the Fu-organs of Yangming or the Zang-organs of Taiyin. Bias toward Cold or Heat hides crisis; these are all "Huai Bing" (ruined/complicated diseases) originating from Taiyang. However, if neglected, the pathogen moves from Exterior to Interior. Waiting until it enters Yangming to use Chengqi Tang (Miraculous Root Decoction), or entering Taiyin to use Sini Tang (Frigid Extremities Decoction), still allows for remedies for complicated diseases. But if purging is used too early before Yangming disease forms, resulting in Jie Xiong (Chest Binding); or if purging is mistakenly used when Taiyin disease is about to form, resulting in Pi Zheng (Stagnation Syndrome), then neither Yangming nor Taiyin disease properly forms. Neither Chengqi Tang nor Sini Tang can be used. This is a complication within complications, irretrievable. Zhang Zhongjing changed Chengqi Tang and Sini Tang to Xianxiong Tang (Trap Chest Decoction) and Xiexin Tang (Drain Epigastrium Decoction) methods here, turning adversity into顺 (smoothness). His virtue and divine merit are unsurpassed.
Diseases originating from Yang often enter Yangming and become Heat diseases; diseases originating from Yin often enter Taiyin and become Cold diseases. For diseases originating from Yang, one must wait for the Exterior syndrome to resolve and Internal Heat to solidify before using purgatives; otherwise, it is premature. Premature purging causes Exterior Yang to sink inward, forming Jie Xiong (Chest Binding), which is a Yangming Huai Bing. Diseases originating from Yin should never be purged; mistaken purging causes Interior Yin to rise, forming Pi Zheng (Stagnation Syndrome), which is a Taiyin Huai Bing.
If a disease originating from Yang is wrongly purged, Heat enters and forms Jie Xiong. If a disease originating from Yin is wrongly purged, it forms Pi Zheng. The reason for forming Jie Xiong is that purging was done too early.
Those with fever and aversion to cold originate from Yang; those without fever but with aversion to cold originate from Yin. (From the first chapter of Taiyang).
Disease originating from Yang indicates Wind injuring Wei Qi (Defensive Qi). Wind injuring Wei Qi obstructs it, forcing Ying Blood (Nutritive Qi) to generate Internal Heat. Those with deficient Zang Yin often transmit to Yangming. When the meridian heat is vigorous, the method should be to release the Exterior. Wait until Exterior Heat transmits to the Stomach before attacking with purgatives. When pathogens transmit internally, Fu-Heat has not yet formed, but Chest-Heat arises first because Yang is abundant above. If purging is done prematurely before Heat enters the Fu, Middle Qi is injured, reversing ascending and descending functions. Stomach Earth rebels upward, Gallbladder Wood cannot descend downward. Ministerial and Sovereign Fire combine to cause pathology. Lung Metal suffers more heat, skin pores do not open, meridian heat enters the chest and diaphragm. Meridian and Fu Qi reject each other, causing hardness, fullness, and pain—this is Jie Xiong.
Disease originating from Yin indicates Cold injuring Ying Blood. Cold injuring Ying Blood binds Wei Qi, generating External Cold. Those with weak Fu Yang often transmit to Taiyin. Mistaken purging causes Spleen Yang to sink downward, Yin Evil fills upward blocking the epigastrium, called Pi Zheng. Before purging, meridian heat was not vigorous, so post-purging there is no initial Heat entry; only stagnation and fullness remain unresolved. Over time, depression intensifies and generates Heat. Internal injury due to Spleen deficiency often presents with epigastric fullness; mistakenly administering Cold-Cooling herbs makes the stagnation worse, which is this very disease.
Jie Xiong syndrome involves Upper Heat and Lower Cold, but Lower Cold is not severe, so Xianxiong Tang is used to drain Damp-Heat from the Upper Jiao. Pi Zheng also involves Upper Heat and Lower Cold, but Lower Cold is heavier, so Xiexin Tang is used to clear Upper Heat and warm Lower Cold. Jie Xiong occurs only in Yangming and Shaoyang (Lesser Yang) meridians because Yang prosperity generates Upper Heat (Yangming rebellion causes Shaoyang failure to descend; two Qi depress and rise, Diaphragm Heat blocks, skin pores do not open, so meridian heat enters internally). Pi Zheng occurs only in Taiyin because Yin prosperity generates Lower Cold. Jie Xiong results from premature purging; Pi Zheng results from mistaken purging; they are vastly different. (Both Jie Xiong and Pi Zheng result from Stomach Qi failing to flow freely and Jia Wood [Gallbladder] rebelling upward, but they differ in Yin and Yang).
