Tiandiao (Heavenly Hook) manifests with the head and eyes looking upward during attacks, palpitations, high fever, eyes rolling back, tears flowing but not stopping, convulsions of the hands and feet, intermittent crying and laughing as if possessed by evil spirits, and in severe cases, cyanosis of the fingernails and toenails. This is caused by accumulated toxins from rich foods in the stomach of the wet nurse, leading to heat-phlegm stagnation in the infant's heart and lungs, or external wind pathogenic factors inducing the disease. The treatment method should be to resolve and dispel the pathogen, using Gouteng Yin (Uncaria Decoction). If there is wheezing and coarse breathing, use Wuxie Sijunzi Tang (Black Scorpion and Four Gentlemen Decoction).
Neidiao (Internal Hook) presents with abdominal pain, frequent panting, black lips, scrotal swelling, stooping posture, backward arching of the body, and redness at the corners of the eyes. This is caused by wind contraction in the womb and external fright. If there are internal spasms of the five zang organs accompanied by painful manic screaming, or diarrhea with legs drawn up, when internal symptoms occur, external symptoms also appear, making it extremely difficult to regulate. For internal symptoms, administer Jubao Dan (Treasure Gathering Pill); for external symptoms, administer Gouteng Yin. Cases where the infant can take milk and food are treatable. For symptoms such as abdominal pain, black lips, and scrotal swelling, use Jubao Dan. For symptoms such as internal spasms due to external fright, use Gouteng Yin. If the cause is accumulated toxins in the stomach from the wet nurse consuming strong wine and rich foods, use Jiawei Qingwei San (Modified Stomach-Clearing Powder). If the cause is accumulated heat in the liver due to the wet nurse's depression and anger, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Jiawei Guipi Tang (Modified Heart-Nourishing Decoction), both added with Loulu (Radix Euphorbiae Humifusae), taken by both mother and child. Whenever the mother consumes rich and greasy foods, before feeding the infant, she should first squeeze out the retained milk, then allow the infant to suckle.
Clinical Records:
An infant suffered from convulsions because the wet nurse was frightened. The infant frequently had red eyes, high fever, abdominal pain, cried while bending the waist. Siwu Tang (Four Substances Decoction) was used with Chaihu (Radix Bupleuri) and Fangfeng (Radix Saposhnikoviae). Additionally, Jiawei Xiaoyao San was used with Shudihuang (Prepared Radix Rehmanniae) to clear liver heat and generate liver blood; Dihuang Wan (Rehmannia Pill) was used to nourish kidney water to generate liver wood, resulting in peace for both mother and child.
An infant bent his waist while crying, with a blue complexion and black lips. This was Neidiao abdominal pain caused by cold qi invasion. Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) was used with Muxiang (Radix Aucklandiae) and Ganjiang (Rhizoma Zingiberis). One dose given to the mother resulted in immediate recovery. Later, when the mother felt cold and had abdominal pain causing crying, Renshen Lizhong Tang (Ginseng and Middle-Warming Decoction) was administered to the mother, and her child also became peaceful.
An infant cried without tears, with cold hands and feet. Liujunzi Tang (Six Gentlemen Decoction) was used with Ganjiang and Muxiang, but there was no response. Rougui (Cortex Cinnamomi) was added, and both mother and child took it, resulting in peace.
An infant suddenly cried without tears and had diarrhea, with convulsions during sleep and cold hands and feet. This was spleen earth deficiency-cold, with liver wood overacting, causing convulsions, classified as Neidiao. Yihuang San (Yellow Protecting Powder) was administered for one dose, bringing peace. Then, Sijunzi Tang was used with Chaihu and Shengma (Rhizoma Cimicifugae), allowing milk and food intake to gradually recover, resulting in peace.
An infant cried without tears, with a blue or red complexion and hot hands and feet. Clearing-heat agents were used by others, but the condition persisted for a long time. Upon examining the wet nurse, liver fire and qi stagnation were found. Jiawei Xiaoyao San and Yueju Wan (Pill Resolving Six Stagnations) were used to treat the mother, while several drops of the medicine were instilled into the infant. Within ten days, both mother and child recovered.
An infant suffered from the aforementioned symptoms and recovered after taking Weixiang San (Asafoetida Powder). The condition recurred later. After taking wind-dispelling and spirit-calming medicines, the infant developed wheezing and coarse breathing, indicating deficiency-cold of yuan qi (original qi). I first used Wuxie Sijunzi Tang, which slightly improved the condition; however, fatigue was severe. Buzhong Yiqi Tang (Middle-Boosting and Qi-Enriching Decoction) and Wuwei Yigong San were used, leading to complete recovery.
An infant suffered because the mother repeatedly contracted cold and had abdominal pain, drank liquor, had high fever with abundant phlegm, a red face, and hot hands and feet. This belonged to Tiandiao of excess heat in the stomach meridian. Qingwei San was used, taken by both mother and child, leading to recovery. Later, due to milk injury causing vomiting and diarrhea, with a complexion that was either blue or white and cold hands and feet, this belonged to spleen qi deficiency-cold. Liujunzi Tang, Muxiang, and Ganjiang were used, leading to recovery. After age three, abdominal pain due to food injury, black lips, and diarrhea with frequent urges but little stool, or scanty green stool, indicated sinking of deficiency-cold yuan qi. Buzhong Yiqi Tang was used for gradual recovery.
An infant cried with scrotal swelling, eyes rolling upward, red vessels, and tearing. This was liver heat Neidiao. Chaihu Qinggan San (Bupleurum Liver-Clearing Decoction) was used with Goutenggou (Uncaria Rhynchophylla), leading to gradual improvement of all symptoms. Gouteng Yin was then used for complete recovery. Later, it recurred. Someone used drugs to remove the root of the disease, causing reduced milk and food intake, abdominal distension and pain, and edema of the hands and feet. I first used Liujunzi Tang, Shengma, and Chaihu for several doses, slightly improving the symptoms. Then, milk and food injury caused vomiting and diarrhea. Pingwei San was used, leading to recovery with one dose.
An infant suffered from abdominal pain and convulsions due to the wet nurse's emotional constraint, persisting for a long time without recovery. Jiawei Guipi Tang was used with Loulu, taken by both mother and child, leading to gradual improvement. Later, the mother became furious, fainted, and regained consciousness. The infant then suckled, resulting in abdominal pain, diarrhea, blue complexion, and vomiting. Xiaochaihu Tang (Minor Bupleurum Decoction) was administered for two doses to both mother and child, resulting in slight improvement. The mother then experienced acid regurgitation and abdominal distension. Yueju Wan, Jiawei Guipi Tang, assisted by Jiawei Xiaoyao San, led to recovery.
