Qi Shi said: Lung abscess (Fei Yong) and lung atrophy (Fei Wei) are caused by Qi deficiency of the Spleen and Lung, with loose interstices (skin pores and muscle intervals), allowing external pathogens to invade; or due to the mother consuming spicy and rich foods, transmitting heat to the child; or due to excessive sweating and purging during the child's illness, leading to internal loss of body fluids, with deficient fire scorching the lungs; or from taking harsh drugs that damage the Spleen and Stomach, preventing them from generating Lung Metal. The symptoms include aversion to wind, cough, nasal congestion, stiffness of the neck, difficult breathing, and in severe cases, slight swelling of the four limbs, and coughing up pus and blood. If foul-smelling sputum is expectorated, accompanied by dull chest pain and a rapid, forceful pulse, it is a lung abscess; if saliva-like foam is expectorated with a rapid, empty pulse, it is lung atrophy. Aversion to cold, wheezing, and cough indicate internal retention of cold pathogen, treated with Xiao Qing Long Tang (Minor Blue Green Dragon Decoction). Expectorating pus and foul matter indicates internal ulceration of the lung abscess, treated with Jie Geng Tang (Platycodon Decoction). I believe that for the aforementioned conditions, if there is wheezing, cough, and shortness of breath, it is Spleen-Stomach Qi deficiency, treated with Wu Wei Yi Gong San (Five-Ingredient Powder for Extraordinary Merit). Expectorating pus and phlegm with a rapid but weak left Chi pulse indicates Kidney Qi deficiency, treated with Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia). Expectorating pus and phlegm with a rapid but weak right Guan pulse indicates Spleen Qi deficiency, treated with Qi Wei Bai Zhu San (Seven-Ingredient Powder of Atractylodes Macrocephala). If fever, wheezing, cough, expectoration of pus and blood, and anorexia occur, it indicates extreme deficiency of Spleen and Lung, which is difficult to treat. Generally, tonifying Spleen and Lung and nourishing Kidney Water is beneficial. One must also examine the mutual overcoming relationships among the Five Zang organs and the emotional states of the nursing mother. Subsequent conditions should be handled similarly.
A child suffered from cold and food stagnation with vomiting and diarrhea. After using draining and regulating agents, he developed cough with pus and blood. This was due to further injury of Middle Qi transforming into lung abscess, cured with Jie Geng Tang. Later, cough with hemoptysis occurred; the previous medicine was used again, assisted by Yi Gong San, resulting in recovery.
A child had food stagnation and developed lung abscess after taking purgatives. I first used Yi Gong San to rescue Spleen and Lung, then used Jie Geng Tang to treat the lung abscess, leading to recovery.
A child had food stagnation and took harsh drugs, resulting in expectoration of fishy-smelling sputum, red face, and asthma. This was due to further injury of Yuan Qi forming lung abscess. Jie Geng Tang was used, and pus-phlegm stopped immediately. The next day, asthma worsened, indicating Spleen Qi deficiency failing to generate Lung Metal. Yi Gong San added with Xing Ren (Apricot Seed) and Bai He (Lily Bulb) cured him. Later, dysuria occurred. Ba Zheng San was taken, worsening the dysuria, accompanied by cough, sputum, red face, and night sweats. I diagnosed this as Lung Qi deficiency with heat, where previous drugs damaged True Yin, and deficient fire scorched Lung Metal. Yi Gong San was used to tonify Spleen Earth, and Di Huang Wan to nourish Kidney Water, leading to recovery.
A child recovered from lung abscess but continued to cough, with pale or sallow complexion, cold hands and feet, and frequent urination. This was due to Spleen deficiency failing to generate Metal. Taking Shen Su Yin led to spontaneous sweating, night sweats, dizziness, convulsions, enuresis, and fecal incontinence. Hands and feet became ice-cold, and the complexion turned blue-white, indicating Yang Qi collapse with deficiency cold. Ren Shen (Ginseng) 1 liang, Gan Jiang (Dried Ginger) 2 qian, and Da Zao (Jujube) 5 pieces were decocted in rice wash water. One cup was administered first, followed by two cups when nearly done. Two doses restored consciousness. Bu Zhong Yi Qi Tang was taken in the morning, and Yi Gong San in the evening, leading to recovery.
A fifteen-year-old child developed cough due to labor injuring Yuan Qi. Misuse of diaphoretic agents further injured Lung Qi, causing abscess with cough and pus-blood. Jie Geng Tang was used as the main formula, assisted by Yi Gong San. Pus decreased gradually. Using only Yi Gong San stopped the pus and cured the condition. Later, overwork from studying led to spontaneous sweating and occasional cough. External pathogen medicines were taken, worsening the cough, with chest oppression and difficult breathing. I diagnosed this as extreme deficiency of Spleen and Lung Qi. Can Shen Bu Pi Tang cured him.
A child coughed, and the condition worsened after taking Shen Su Yin. The Right Cun and Guan pulses were floating and scattered. I stated: Wind injured the skin and hair, heat injured the blood vessels, causing Qi and Blood to stagnate and accumulate in the Lung, forming an abscess. Disbelieving, he took diaphoretics, resulting in expectoration of pus and blood. I said: This is because Lung deficiency fails to grasp Qi, and Spleen deficiency fails to control saliva. Tonifying Spleen Earth to generate Lung Metal is required. Disbelieving again, he died.
A child had cold-induced cough. Over-diaphoresis led to panting, anorexia, and blood in sputum. Jie Geng Tang cured him. Later, with incomplete recovery of Yuan Qi, stool resembled dysentery. Someone used Huang Qin (Scutellaria Root), Huang Lian (Coptis Rhizome), and Zhi Shi (Immature Bitter Orange), leading to Man Pi Feng (Slow Spleen Wind) and sudden death.
Xiao Qing Long Tang treats wind-cold invasion, cough with urgent wheezing, lung distension and chest fullness, nasal congestion with runny nose, dry retching with hot cough, hiccups, choking, difficulty urinating, or lower abdominal fullness. (This is Zhang Zhongjing's method. When these symptoms are present and used timely, it yields excellent results.)
