It is stated: When Yin is insufficient, Yang becomes excessive; when Yang is excessive, fever occurs. If the heat sensation starts from the back and gradually spreads to the hands, feet, and whole body, accompanied by dryness of the mouth and tongue, a desire to drink water but an inability to swallow (or thirst not relieved by drinking), this is caused by a deficiency of Yin Qi, akin to a small amount of water failing to control a raging fire. The Yang meridians originate at the extremities of the four limbs and circulate along the back. When Yin cannot restrain Yang, Yang Qi moves independently, resulting in fever. This may manifest as daytime onset with nighttime relief, or nighttime onset with resolution by dawn. The pulse is deficient, rapid, and thin. Shaoyao Huangqi Tang (Peony and Astragalus Decoction) is the primary treatment.
Shaoyao Huangqi Tang (Peony and Astragalus Decoction)
Shaoyao (Paeoniae Radix Alba, 3 qian)
Huangqi (Astragali Radix)
Gancao (Glycyrrhizae Radix et Rhizoma, prepared)
Qinghao (Artemisiae Annuae Herba, dried in shade, each 1 liang)
The above ingredients are processed into a powder. Take 5 qian per dose, add two large bowls of water, decoct until one bowl remains, strain, and take warm after meals.
If the heat sensation originates from the abdomen, ascends along the chest and axillae, and circles the neck and forehead, starting mildly and progressing to high fever without fixed timing, worsening upon hunger, with discomfort in the Zhongwan region, hyperphagia yet emaciation, sallow-yellow complexion, lusterless muscles, and dry lips, this is due to inherent Spleen Qi weakness. Previously, due to other illnesses, hot-natured medicines were mistakenly taken, allowing heat to enter the Spleen. Spleen heat accelerates digestion and induces thirst, consuming muscle tissue. The pulse is soggy, weak, and rapid. Shanju Wan (Ginseng and Tangerine Peel Pill) is the primary treatment. For cases with cough, apply modification methods and moxibustion on Pishu (BL20) for 100 cones.
Shanju Wan (Ginseng and Tangerine Peel Pill)
Chenpi (Citri Reticulatae Pericarpium, 3 liang, washed)
Maimendong (Ophiopogonis Radix, heart removed)
Renshen (Ginseng Radix et Rhizoma, radix removed, each 1 liang)
The above ingredients are ground into powder, mixed with refined honey to form pills the size of paulownia seeds. Take 30 pills before meals with rice water. If cough is present, add Wubeizi (Schisandrae Fructus, 1 liang).
If heat originates from the abdomen or back, gradually rising as steaming heat, severe during the day and subsiding at night, or occurring with sweating during sleep, with urine ranging from red to white and turbid, progressing to frequent urination or spermatorrhea, nocturnal emissions involving dreams of sexual intercourse, and progressive emaciation, this is caused by excessive mental exertion depleting Heart Qi. Yang Qi scatters, Spirit fails to reside within, and Yin has no function. This falls under the category of Deficiency Labor (Xu Lao). Dajianzhong Tang (Great Construct the Middle Decoction) is the primary treatment.
Dajianzhong Tang (Great Construct the Middle Decoction)
Shaoyao (Paeoniae Radix Alba, 6 liang)
Huangqi (Astragali Radix)
Yuanzhi (Polygalae Radix, heart removed)
Danggui (Angelicae Sinensis Radix, washed)
Zexie (Alismatis Rhizoma, each 3 liang)
Longgu (Fossilia Ossis Mastodi et Ossis Parietalis)
Renshen (Ginseng Radix et Rhizoma)
Gancao (Glycyrrhizae Radix et Rhizoma, prepared, each 2 liang)
The above ingredients are processed into a powder. Take 5 qian per dose, add two bowls of water, two Jujubae Fructus (split open), five slices of Shengjiang (Zingiberis Rhizoma Recens), decoct together until one bowl remains, strain, and take warm after meals. If abdominal urgency and pain occur, add Yi Tang (Saccharum Japonicum) the size of a date pit.
If fever occurs above the waist, leading to sweating when heat peaks, followed by cooling, then returning to normal after some time, accompanied by dizziness, abdominal distension, Qi rushing upward, frequent coughing that pulls on the hypochondrium, and palpitations during sleep, the pulse is wiry, urgent, and rapid. This is caused by external Cold pathogens attacking and internal Cold combining. Cold causes Qi contraction, and fluid accumulation transforms into Phlegm-Fluid (Yin). Physicians using hot medicines to attack Cold cause Cold to be sealed internally and Heat to increase externally. Yang Qi fails to descend and scatters upward, causing fever when active and sweating when scattered. Excessive sweating leads to loss of Yang, leaving Heart Qi internally deficient, causing fright and palpitations. This belongs to the category of Fluid Retention (Yin Jia). Xuanfuhua Wan (Inula Flower Pill) is the primary treatment.
Xuanfuhua Wan (Inula Flower Pill)
Xuanfuhua (Inulae Flos)
Guixin (Cinnamomi Cortex)
Zhishi (Aurantii Fructus Immaturus, bran-fried)
Renshen (Ginseng Radix et Rhizoma, each 5 fen)
Ganjiang (Zingiberis Rhizoma)
Shaoyao (Paeoniae Radix Alba)
Baizhu (Atractylodis Macrocephalae Rhizoma, each 6 fen)
Fuling (Poria)
Yelangdu (Radix Stellerae Chamaejasmes)
Wutou (Aconiti Lateralis Radix Praeparata)
Huangqi (Astragali Radix, wrapped in wet paper, roasted with juice, each 4 liang)
The above ingredients are ground into fine powder, mixed with refined honey to form pills the size of paulownia seeds. Take 3 pills with rice water. If ineffective, increase to 7 pills. For children, use 2 pills the size of millet grains.
If heat originates from between the bones causing irritability and pain, with cold hands and feet, cool body in the morning and fever in the evening, stiffness and pulling in the back and spine, or protruding bone joints, and soreness and weakness in the calves, this is due to Yin insufficiency with Yang sinking into Yin. Heat remains in the marrow; heat thins the marrow, making bones hollow. Yin deficiency leads to scant fluids and withered fat, causing steaming heat. The pulse is deep, thin, and rapid. This belongs to the category of Bone-steaming Consumption (Gu Zheng). Busui Wan (Marrow-Nourishing Pill) is the primary treatment.
Busui Wan (Marrow-Nourishing Pill)
Sheng Gan Dihuang (Rehmanniae Radix Preparata, sun-dried, 3 liang)
Ganqi (Lacini Cinnamomi, half liang, crushed, stir-fried until smoke ceases)
The above ingredients are ground into powder, mixed with refined honey to form pills the size of paulownia seeds. Take 30 pills with water, on an empty stomach and before bed.
If fever extends from the chest to the hairline, with the complexion of the mouth and nose resembling soot, dry eyes, dry throat, saliva like congealed fat, occasional coughing, piloerection, constipation, and red urine, this is due to Lung dysfunction where pathogenic heat steams the Upper Jiao. The pulse is rapid and large. First administer Jiegeng Tang (Platycodon Decoction). If fever persists, Wuweizi Tang (Schisandra Decoction) is used.
Jiegeng Tang (Platycodon Decoction)
Jiegeng (Platycodonis Radix, 1 liang)
Renshen (Ginseng Radix et Rhizoma)
Maimendong (Ophiopogonis Radix, heart removed)
Gancao (Glycyrrhizae Radix et Rhizoma, prepared, each half liang)
Xiaomai (Tritici Fructus Immaturus, 1 he)
The above ingredients are cut. Add three sheng of water, decoct to one sheng, strain, and divide into three doses.
Wuweizi Tang (Schisandra Decoction) from *Zhinan Fang* treats persistent severe fever.
Chaihu (Bupleuri Radix, stems removed, washed, 4 liang)
Banxia (Pinelliae Rhizoma, washed seven times in boiling water, 1 liang 1 fen)
Huangqin (Scutellariae Radix)
Wuweizi (Schisandrae Fructus)
Chifuling (Poria cum Rubro, original dosage missing)
The above ingredients are processed into a powder. Take 5 qian per dose, add two bowls of water, five slices of Shengjiang (Zingiberis Rhizoma Recens), two split Jujubae Fructus, decoct together until one bowl remains, strain, and take warm.
If fever is continuous without respite, worse in the late afternoon (Shi hour), with breath blowing out vigorously, deafness, daytime somnolence with delirious speech during sleep, preference for cold drinks, red and difficult urination, and constipation, this is due to excess Qi in the Three Yang meridians accumulating in the collaterals and connecting internally to the Fu organs, possibly arising from other diseases. The pulse is short, rapid, and numerous. Chaihu Mangxiao Tang (Bupleurum and Mirabilite Decoction) is the primary treatment.
Chaihu Mangxiao Tang (Bupleurum and Mirabilite Decoction)
Chaihu (Bupleuri Radix, 4 liang, washed, stems removed)
Huangqin (Scutellariae Radix)
Gancao (Glycyrrhizae Radix et Rhizoma, prepared)
Chifuling (Poria cum Rubro, each 1.5 liang)
Banxia (Pinelliae Rhizoma, washed seven times in boiling water, 1 liang 1 fen)
The above ingredients are processed into a powder. Take 5 qian per dose, add two bowls of water, five slices of Shengjiang (Zingiberis Rhizoma Recens), two split Jujubae Fructus, decoct together until one bowl remains, strain, add 1 qian of Mangxiao (Natrii Sulfas), mix well, and take warm. Stop when bowel movement is unobstructed.
If there is high fever, slight aversion to cold on the back, mental restlessness, frequent nausea, thirst but inability to drink, heavy and painful dizziness, photophobia, slight clarity upon exposure to cool air, and normal daily activities, this is due to irregular diet causing lack of Grain Qi in the Stomach. Heat accumulates in the Stomach; Zhongwan heat prevents the descent of Three Yang Qi, which gathers in the head. The Stomach governs the Yangming Meridian, which dislikes heat. The pulse is deficient, large, and rapid, becoming thin and small over time. This is termed Summerheat (Zhongshu). Initially, drinking good tea cures it immediately. If untreated, heat remains in the Stomach, causing other diseases. Shengjiang Zhuru Tang (Fresh Ginger and Bamboo Shavings Decoction) is the primary treatment.
Shengjiang Zhuru Tang (Fresh Ginger and Bamboo Shavings Decoction)
Zhuru (Bambusae Caulis in Taenia, size of a chicken egg)
Renshen (Ginseng Radix et Rhizoma, half liang)
Gegen (Puerariae Lobatae Radix, half liang)
Shengjiang (Zingiberis Rhizoma Recens, 1 qian, sliced)
The above ingredients are processed into a powder. Add three bowls of water, decoct to two bowls, strain, and divide into two doses. Take at any time.
If fever occurs every other day, accompanied by frequent yawning, headache, stiffness, and fatigue, this is due to pre-existing Lung heat with Qi abundance in the body. Rebellious Qi rushes upward, middle Qi is full but does not vent outward. This Qi connects internally to the Heart and externally to the muscles/flesh, causing emaciation. The pulse is wiry, large, and rapid. This is called Dan Malaria (Dan Nue). Chaihu Gualou Tang (Bupleurum and Trichosanthes Decoction) is the primary treatment.
Chaihu Gualou Tang (Bupleurum and Trichosanthes Decoction)
Chaihu (Bupleuri Radix, stems removed, washed, 8 qian)
Shaoyao (Paeoniae Radix Alba)
Renshen (Ginseng Radix et Rhizoma, each 2 qian)
Banxia (Pinelliae Rhizoma, washed seven times in boiling water, 2.5 qian)
Gancao (Glycyrrhizae Radix et Rhizoma, prepared, 2 qian)
Gualou (Trichosanthis Fructus, 2 qian)
The above ingredients are cut. Add two sheng of water, ten slices of Shengjiang (Zingiberis Rhizoma Recens), two split Jujubae Fructus, decoct together until one sheng remains, divide into three doses, strain, and take. In cases of high fever, remove Shaoyao and Gualou, and use Huangqin (Scutellariae Radix, 3 qian); this is named Xiao Chaihu Tang (Minor Bupleurum Decoction).
If body heat and sweating occur, with fullness and irritability not relieved by sweating, this is because the Taiyang Meridian first received Wind pathogen, which combats with Heat. Kidney Qi rebellion causes irritability and fullness. This is termed Wind-Jue (Feng Jue). Zexie Tang (Alisma Decoction) is the primary treatment.
Zexie Tang (Alisma Decoction)
Zexie (Alismatis Rhizoma, half liang)
Baizhu (Atractylodis Macrocephalae Rhizoma)
Fangfeng (Saposhnikoviae Radix, each 2 liang)
Shigao (Gypsum Fibrosum, ground)
Chifuling (Poria cum Rubro, each 1 liang)
The above ingredients are processed into a powder. Take 5 qian per dose, add two bowls of water, decoct until one bowl remains, strain, and take warm.
If the four limbs are hot, feeling like roasting or burning upon exposure to wind, this is due to Yin failing to overcome Yang. Yang excess causes heat originating in the limbs. Like small water failing to extinguish a great fire, Yang dominates externally. Tusizi Wan (Cuscuta Seed Pill) is the primary treatment.
Tusizi Wan (Cuscuta Seed Pill)
Tusizi (Cuscutae Semen, pounded 100 times in a mortar, sifted to remove impurities)
Wuweizi (Schisandrae Fructus, each 1 liang)
Sheng Gan Dihuang (Rehmanniae Radix Preparata, 1 liang)
The above ingredients are ground into powder, mixed with refined honey to form pills the size of paulownia seeds. Take 30 pills with water before meals.
If fever occurs with sudden deafness, swollen cheeks, hypochondriac pain, and immobility of the joints, this is due to Shaoyang Qi rebellion with heat remaining in the meridian. Xiao Chaihu Tang (Minor Bupleurum Decoction) is appropriate.
If there is dry mouth, red urine, abdominal fullness, and heart pain, this is due to heat remaining in the Hand-Shaoyin Meridian with Qi rebellion. Chifuling Tang (Red Poria Decoction) is the primary treatment.
Chifuling Tang (Red Poria Decoction)
Chifuling (Poria cum Rubro, 4 liang)
Gancao (Glycyrrhizae Radix et Rhizoma, raw, 1 liang)
Muxiang (Aucklandiae Radix, half liang)
The above ingredients are processed into a powder. Take 5 qian per dose, add two bowls of water, decoct until one bowl remains, strain, and take warm.
