Heatstroke causing sudden collapse, unclear speech, and mental confusion is what the *Neijing* (Classic of Internal Medicine) refers to as "Jian Jue" (Frying Syncope). It is appropriate to use Tianwang Buxin Dan (Emperor of Heaven's Tonic for the Heart Pill) added with Longchi (Dragon Teeth) and Muli (Oyster Shell), to calm the spirit and consolidate Yin.
During periods of summer heat, if one suddenly collapses with unclear speech and mental confusion, resembling wind-stroke, this precisely corresponds to the *Neijing*'s description: excessive mental labor causes Yang Qi to hyperactive; when essence is exhausted and summer heat accumulates, it leads to Jian Jue (Frying Syncope). It is appropriate to use Buxin Dan to clear Heart fire, combined with Longchi and Muli to calm the spirit, preventing internal closure and external collapse, thereby offering hope for success.
Heatstroke presenting with a large pulse, body heat, thirst, and profuse sweating indicates that heat has damaged Yuan Qi (Original Qi). It is appropriate to use Renshen Baihu Tang (Ginseng and White Tiger Decoction), using sweet-cold herbs to nourish Zheng Qi (Upright Qi).
When the body is extremely hot but without aversion to heat, accompanied by thirst, profuse sweating, and a flooding pulse, this is what Li Dongyuan referred to as "pulse deficiency with body heat," which belongs to Shangshu (Summerheat Injury). Using Shigypsum (Gypsum Fibrosum) and Zhimu (Anemarrhenae Rhizoma) from Baihu Tang clears Stomach heat, while Renshen (Ginseng Radix et Rhizoma), Gancao (Licorice Root), and Jingmi (Glutinous Rice) nourish Stomach fluids, preventing the blazing heat from damaging Yin.
After heatstroke, although consciousness is clear, there is reluctance to speak, fatigue, lying down, coolness in the morning and heat in the evening, and delirium at night. This indicates heat remaining in the Gallbladder, burning Ying Heat (Nutritive Heat). It is appropriate to use Qinggu San (Bone-Clearing Powder) added with Xian Changpu (Fresh Acori Tatarinowii Rhizoma), Guang Yujin (Curcumae Radix), and Yiyuan San (Benefit Origin Powder), to clear Ying level heat and dispel pathogenic heat.
After heatstroke, fatigue and reluctance to speak are signs of disease retreat; however, coolness in the morning and heat in the evening, along with delirium at night, indicate lingering residual heat burning the Ying collaterals. Therefore, Qinggu San is used to clear Ying heat and dispel pathogenic heat, combined with Changpu, Yujin, and Yiyuan San to open orifices and eliminate pathogens, which is the correct method.
(1) Renshen Baihu Tang: Originally named Baibu Jia Renshen Tang (White Tiger Decoction plus Ginseng) in *Shanghan Lun · Treatise on Differentiating Pulse, Symptoms, and Treatment of Taiyang Disease*.
After heatstroke, the tongue exhibits prickles and rot-like spots resembling gan accumulation, with delirium during sleep and clarity upon waking. This indicates heat-toxin accumulation and uncleaned Ying collaterals. It is appropriate to use Daochi San (Guide Out Red Powder) added with Xijiao (Rhinoceros Horn - note: usually substituted with Water Buffalo Horn in modern practice), Lv Dou Qiao (Mung Bean Hull), Renzhongbai (Calcinatus Human Feces), Xian Changpu (Fresh Acori Tatarinowii Rhizoma), Chen Sha (Cinnabar), and Dengcao (Juncus Effusus), to clear Ying level and resolve toxins.
This section is similar to the previous one, but additionally features tongue rot, gan-like changes, and prickles, indicating that residual heat transforms into toxin and accumulates in the Pericardium. Therefore, Daochi San is used with Xijiao, Dengcao, Lv Dou Qiao, Renzhongbai, Changpu, and Chen Sha to clear Heart heat and resolve toxins, preventing the spread of heat-toxin and avoiding corrosive damage, which is the optimal approach.
