In winter-warmth disease, if vexatious heat persists and is not resolved, accompanied by thirst, yellow tongue coating with a red tip, and a surging or rapid pulse, this indicates that the pathogenic factor has transmitted to the Qi level of the Yangming channel. It is appropriate to use Baihu Tang (White Tiger Decoction) added with Xingren (Almond), Shashen (Glehniae Radix), Sangye (Mulberry Leaf), Lianqiao (Forsythiae Fructus), etc., to clear the Stomach heat and vent the pathogen.
If the pathogen in the Lung fails to resolve, leading to thirst, yellow tongue coating with a red tip, and a surging or rapid pulse, this also signifies that the pathogen has reached the Qi level of the Yangming channel. The term "Yangming" refers to the Stomach. Whenever the pathogen in the Lung does not resolve, it must transmit to the Stomach; however, the Yangming channel is also divided into Qi and Blood levels. Currently, the presence of thirst, surging pulse, and yellowish tongue coating indicates heat in the Qi level. Therefore, Shigao (Gypsum Fibrosum) and Zhimu (Anemarrhenae Rhizoma) are used in combination with Shashen and Lianqiao to directly clear Yangming heat; Gancao (Licorice Root), Jingmi (Glutinous Rice), Tianxingren (Sweet Almond), and Sangye employ sweet and cool properties to nourish body fluids. If a crimson tongue with cracks or blackening is observed, along with vexatious heat, thirst, coma, and delirious laughter, this indicates the pathogen has entered the Blood level. It is appropriate to use Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) to cool the blood. One should carefully distinguish these conditions from the discussions on Yangming meridian disease, fu-organ disease, Qi level, and Blood level in Zhang Zhongjing's *Treatise on Cold Damage* (*Shanghan Lun*), before initiating treatment; otherwise, it would be like navigating the sea without knowing the ferry crossing.
In winter-warmth disease, if there is vexatious heat, coma, crimson tongue with yellow coating, thirst, cough, macules and papules, and a rapid pulse, this indicates the pathogen is located in the Lung and Stomach. It is appropriate to use Shashen, Lianqiao, Xuanshen (Scrophulariae Radix), Shigao, Tianxingren, Chuanbeimu (Fritillariae Cirrhosae Bulbus), Sangye, Niubangzi (Arctii Fructus), Renzhonghuang (Human Manure), and Angong Niuhuang Wan (Calculus Bovis Pill for Opening the Orifices), etc., to clear Qi-level heat and vent the macules and papules.
As heat gradually transmits to the Ying (Nutrient) level, symptoms such as coma, thirst, cough, and macules/papules appear, indicating the pathogen is between the Lung and Stomach. If one does not clear and resolve from the Qi level, the macules and papules will be difficult to vent. Macules are caused by heat-toxin in the Yangming channel, while papules are caused by wind-heat in the Taiyin channel. When both macules and papules appear simultaneously, it indicates that both channels are affected. Therefore, Shashen, Lianqiao, Xuanshen, Shigao, Tianxingren, Chuanbeimu, Niubangzi, and Renzhonghuang are used to clear Qi-level heat, combined with Angong Niuhuang Wan to open the orifices with aromatic properties.
In winter-warmth disease, if the heat is severe, accompanied by restlessness, thirst, crimson tongue with yellow coating, coma, delirium, and faintly visible macules/papules, this indicates the pathogenic heat has transmitted to the Ying level. It is appropriate to use Lingyangjiao (Saiga Tataricae Cornu), Lianqiao, Xuanshen, Shashen, Xian Shengdi (Fresh Rehmanniae Radix), Xian Shihu (Fresh Dendrobii Caulis), Xian Changpu (Fresh Acori Tatarinowii Rhizoma), Guang Yujin (Curcumae Radix), Shigao, Angong Niuhuang Wan, and Qingzhuyez (Bamboo Leaves), etc., to clear the Ying level and transform the pathogen back to the Qi level.
If the heat does not subside and is accompanied by restlessness and thirst, with a crimson tongue and yellow coating indicating severe internal heat; further compounded by coma and delirium, and faintly visible macules/papules, this shows the pathogen has entered the Ying level. If not resolved promptly, it will inevitably extend into the Blood level. Therefore, Lingyangjiao, Lianqiao, Xuanshen, Shashen, Xian Shengdi, Shihu, and Xian Changpu are used to simultaneously clear the Qi and Blood levels, assisted by Angong Niuhuang Wan to aromatically ventilate the orifices, allowing the pathogen to transform from the Ying level to the Qi level, and from the Qi level to reach the exterior, where it can potentially be resolved through sweating. Generally, if the tongue coating is yellow and white mixed with floating greasiness, and although the tip is fresh crimson and the patient is comatose and delirious, it is still fundamentally due to uncleared Qi-level heat. One must not exclusively use blood-cooling herbs, lest the Qi-level pathogen lose its exit route, merely increasing the severity of the condition. In unavoidable cases, only simultaneous clearing of Qi and Blood is suitable.
