Treatment for women suffering from continuous uterine bleeding with foul blood, irregular menstruation, or sudden severe hemorrhage accompanied by the discharge of large amounts of watery fluid. These conditions are all caused by irregular diet, physical overexertion damaging the body, or pre-existing deficiency of Heart Qi (Heart Energy). Due to dietary and labor fatigue, Heart Fire (Pathogenic Heat in the Heart) attacks the Spleen. The patient will inevitably exhibit lethargy and a desire to lie down, flaccidity and weakness of the four limbs, fatigue, lack of energy to move, shortness of breath, and rebellious Qi rushing upward. The pulse is slow yet wiry and urgent; when pressed heavily, it feels surging and large, perceptible under the middle finger. This indicates that the Spleen Earth is affected by pathogenic factors. The Spleen governs the nourishment of the whole body; the Heart governs Blood, and Blood governs the Vessels. When both the Heart and Vessels are affected, the disease resides in the Vessels. The Vessel is the mansion of Blood, and the Vessel is the spirit of humans. If the Heart does not directly command, the Pericardium (Pericardium Meridian) acts on its behalf. Therefore, it is said that the Heart's vessels belong to the Heart System, which refers to the vessels of the Pericardium and Mingmen (Gate of Life), governing menstruation. Because the Spleen and Stomach are deficient and the Pericardium takes advantage of this to invade, leakage and menstrual irregularities occur. Moreover, the Spleen and Stomach are the root of Qi, Blood, Yin, and Yang. One should employ methods to remove dampness and heat, benefit Wind to support Earth, and extend the Spleen Qi to overcome dampness. It is also stated: "When Fire is constrained, it must be dispersed."
Dang Gui (Radix Angelicae Sinensis, washed with wine) and Duhuo (Radix Angelicae Pubescentis) (each 5 fen); Man Jing Zi (Fructus Viticis Expensae) (7 fen); Fang Feng (Radix Saposhnikoviae), Zhi Gan Cao (Prepared Radix Glycyrrhizae), Sheng Ma (Rhizoma Cimicifugae), Gao Ben (Rhizoma Ligustici Chuanxiong) (each 1 qian); Chai Hu (Radix Bupleuri), Qiang Huo (Rhizoma Notopterygii), Gao Ben (Rhizoma Ligustici Chuanxiong), Huang Qin (Radix Scutellariae) (each 1.5 qian).
Crush the herbs to the size of hemp seeds; do not grind them into powder. Combine all as one dose. Decoct in three large bowls of clean, fresh water until reduced to one large bowl. Strain and take warm on an empty stomach. Wait a short while, then eat breakfast to settle the medicine. Only one dose is needed. Alternatively, moxa-needling the Xuehai (Sea of Blood) acupoint on the Foot-Taiyin Splein Meridian, fourteen moxa cones, can also cure the condition.
This prescription is a temporary measure, using Wind herbs to overcome dampness. It addresses the sinking of Stomach Qi and the pressure of Qi descending, aiming to rescue severe hemorrhage. After the discharge of foul blood stops, one must administer several doses of Bu Yi (Tonify and Supplement) formulas containing Huang Qi (Radix Astragali), Ren Shen (Radix Ginseng), Zhi Gan Cao (Prepared Radix Glycyrrhizae), Dang Gui (Radix Angelicae Sinensis), etc., to tonify the body. Adding blood-nourishing herbs to Bu Qi Sheng Yang Tang (Tonifying Qi and Raising Yang Decoction) is the correct approach. If the discharge of menstrual blood and foul matter continues incessantly, one must investigate the root cause and treat the primary meridian. Tonifying the Spleen and Stomach and retreating the hyperactivity of Heart Fire treats the root. In summer, if leukorrhea and continuous leakage occur, this decoction can stop it immediately after one dose.
Treatment for women with metrorrhagia (blood flooding) is based on the principle that Kidney Water (Kidney Yin) is deficient and cannot control the Ministerial Fire (Xiang Huo) of the Pericardium, causing Blood to move recklessly and resulting in flooding.
Huang Qin (Radix Scutellariae), Jing Jie Sui (Cacumen Schizonepetae), Man Jing Zi (Fructus Viticis Expensae) (each 1 qian); Huang Bo (Cortex Phellodendri), Zhi Mu (Rhizoma Anemarrhenae), Gao Ben (Rhizoma Ligustici Chuanxiong), Xi Xin (Herba Asari), Chuan Xiong (Rhizoma Chuanxiong) (each 2 fen); Huang Lian (Rhizoma Coptidis), Qiang Huo (Rhizoma Notopterygii), Chai Hu (Radix Bupleuri), Sheng Ma (Rhizoma Cimicifugae), Fang Feng (Radix Saposhnikoviae) (each 3 fen); Sheng Di Huang (Radix Rehmanniae Preparata), Dang Gui (Radix Angelicae Sinensis) (each 5 fen); Gan Cao (Radix Glycyrrhizae) (1 qian); Hong Hua (Flos Carthami) (a small amount).
Chop the herbs finely. Combine all as one dose. Decoct in three large bowls of water until reduced to one large bowl. Strain and take slightly warm on an empty stomach.
The Xuehai (Sea of Blood) acupoint on the Foot-Taiyin Spleen Meridian is located two cun above the medial aspect of the patella, in the white flesh margin. It treats women's continuous uterine bleeding with foul blood, irregular menstruation, rebellious Qi, and abdominal distension, where the pulse is slow. Moxa-needle three cones.
The Yin Gu (Yin Valley) acupoint on the Foot-Shaoyin Kidney Meridian is located behind the medial condyle of the tibia, below the large tendon and above the small tendon. Press to feel the pulse; flex the knee to locate it. It treats knee pain like a needle刺, inability to flex or extend, tongue flaccidity with salivation, irritability, difficult urination, acute pulling pain in the lower abdomen radiating to the genitals, pain in the inner thigh, continuous uterine bleeding in women, abdominal fullness, dyspnea, yellow urine resembling gu poison, and a sensation of pregnancy in women. Moxa-needle two cones.
