Based on the collected miscellaneous diseases as the sequence, *Mai Jue Ju Yao* (Pulse Diagnosis Essentials) serves as the main reference, supplemented by the standards from *Zheng Chuan* and *Quanyu*, with warm diseases, summerheat, and internal injury appended.
In stroke (Zhong Feng), the pulse is floating; if slippery, it indicates phlegm-qi; if deep and slippery, it should not be treated as wind; whether floating or deep, if weak and deficient, one should support the critical condition and treat phlegm, avoiding wind-dispersing methods; a floating and slow pulse is auspicious, while a rapid and urgent pulse indicates death.
If paralysis results from stroke, a spleen pulse that is slow indicates an untreatable condition. *Kuai Jie Fang Shi* states: "Wind disease with spleen slowness is futile effort; consumption disease with a rapid heart pulse makes life hard to preserve."
In cold invasion (Zhong Han), the pulse is tight and choppy, with both yin and yang excess; the method dictates no sweating, and sweating will endanger life.
A tight yang pulse indicates cold in the upper jiao causing vomiting; a tight yin pulse indicates cold in the lower jiao causing spontaneous diarrhea; if both yin and yang pulses are tight, cold affects both upper and lower, dictating no sweating; spontaneous sweating instead indicates yang collapse and is untreatable.
In wind injury (Shang Feng), the pulse is floating at yang and weak at yin, with pathogen in the six channels, possibly wiry and rapid.
Floating at yang indicates wind invading the defensive qi; weak at yin indicates weak nutritive qi. Pathogens in the six channels all present wiry pulses.
Summerheat damaging qi results in a deficient pulse, presenting as wiry, surging, knotted, or slow, with no omission in its characteristics.
A pulse that is deficient and weak, or floating and large and scattered, or hidden and invisible, these weak and hidden states all belong to deficiency categories.
When summerheat-heat disease is severe, yin and yang are extremely excessive; superficially smooth, deeply scattered and choppy; after sweating, if the pulse becomes restless and large, death can be predicted precisely.
After sweating, a restless and large pulse indeed indicates death; if seven to eight days after entering the interior, the pulse is not restless and rapid but rather choppy and small, this also indicates death.
Warm disease pulses have no fixed name, appearing according to the affected channel; before sweating, a strong pulse indicates survival, while a deficient and slow pulse indicates harm to life.
Treatment of warm disease pulses follows the manifestations in each zang-fu organ. Before sweating, a strong and urgent pulse indicates survival, while a deficient and slow pulse indicates death; after sweating, if exterior symptoms persist and the pulse is strong and urgent, death ensues; if the pathogen enters the interior causing severe abdominal pain and diarrhea, death also ensues.
Warm pulses are soggy and slow, or accompanied by choppy and small; entering the interior, they become slow and deep; at the exterior, they are floating and slow; if slow and wiry, wind-dampness is intertwining.
Floating and slow at the exterior, deep and slow at the interior, or wiry-slow or floating-slow, all indicate wind-dampness interacting.
Tight and choppy, or floating and wiry, or knotted and deficient pulses constitute dryness syndromes.
Choppy primarily indicates dryness; wind-dryness accompanies floating and wiry, blood-dryness accompanies knotted and deficient.
Deficient fire presents as rapid and floating; excess fire presents as deep, solid, and large; following their appearances, fine and rapid causes harm.
Floating, surging, rapid, and forceless indicates deficient fire; deep, solid, large, and forceful indicates excess fire. If surging and rapid appears at the left cun, it is heart fire; right cun, lung fire; left guan, liver fire; right guan, spleen fire; both chi, kidney meridian and mingmen fire.
Internal injury from labor shows a wide and uncontrolled pulse; if stomach qi is damaged, the pulse becomes hidden and hard to find. Internal injury from diet shows slippery and rapid floating/sinking; internal injury from labor and food shows rapid, large, and slightly choppy. Right guan slow and tight indicates cold-dampness seeking each other; right guan rapid and slow indicates damp-heat approaching; rapid with intermittent pulse indicates food injury and pathogenic influence.
Heart pulse changes appear at the qi kou (cun); liver wood also brings the momentum of heart fire to oppress lung metal, hence the pulse is large and urgently rapid due to lack of control. Mild internal injury shows deep and slippery right guan; severe internal injury shows floating and slippery qi kou. Right cun qi kou pulse urgently large and rapid, occasionally intermittent and choppy; choppy is the lung's native pulse, intermittent indicates yuan qi discontinuity, this is a great deficiency pulse from irregular diet and excessive labor. Right guan spleen pulse rapid yet showing slowness, double that of other organs, indicates mild labor but heavy dietary injury with damp-heat. Rapid often indicates dry-heat, slow often indicates damp-heat. If spleen pulse is large and rapid, occasionally slightly slow and intermittent, it indicates irregular diet and improper temperature regulation.
Touching the pulse and finding it deep indicates qi disorder; extreme deepness leads to hidden pulse, choppy and weak are hard to treat; if deep and slippery, qi is combined with phlegm-fluids.
Slippery indicates much blood and little qi; choppy indicates little blood and much qi. Deep and firm chi pulse indicates blood excess and qi deficiency; fine and minute chi pulse indicates both blood and qi deficiency; fine and intermittent pulse indicates qi decline; cut-off pulse indicates qi about to cease; hidden and choppy are hard to treat, nearly ceasing.
All hemorrhagic syndromes show knotted pulses; according to their upper/lower positions, verify the source. Generally, for hemorrhage, the pulse should be deep and fine; if floating and large appears, treatment later will be difficult.
Pulses showing various choppy, soggy, and weak qualities indicate blood loss. Floating pulse with white and thin complexion indicates interior deficiency and blood loss. Pulse with light touch, exclusive floating at chi, and blurry yellow eyes indicates nasal bleeding. Or deep, wiry, and deficient, with white face, shortness of breath, closed eyes, and full lower abdomen, indicates nasal bleeding due to labor. Taiyang pulse large and floating indicates nasal or hematemesis. Like a hanging hook beating the hand, or deep and wiry, indicates nasal bleeding. Lung pulse wiry and urgent indicates coughing up blood. Floating and weak pulse, vanishing upon pressure, indicates lower bleeding. Irritable cough indicates hematemesis and intestinal pus-blood. Wiry and cut-off pulse indicates death; slippery and large indicates survival. Warm blood with hot body indicates death. Extremely deficient, knotted, and slow pulse indicates blood loss and essence loss.
Wiry on one side indicates fluid retention; or deep, wiry, and slippery, or knotting, choppy, and hidden, phlegm-fluids are in the middle joint.
Phlegm-fluid pulses are all wiry with slight deep-slippery, except lung fluid which has panting but is not wiry. Double wiry indicates cold-fluid. Or after severe purgation leading to weakness, if floating, wiry, large, and solid, thick phlegm in the diaphragm should be vomited. Long-term knot pulse, or choppy or hidden, indicates phlegm-fluid firmly stuck in the middle, blocking the pulse vessel at the joints.
Stagnation pulses are all deep; blood stagnation is knotted, qi stagnation is choppy, damp stagnation is slow and deep, heat stagnation is extremely rapid. Phlegm stagnation is slippery and wiry, slippery-tight due to food, severe stagnation leads to obstruction, or knotting, intermittent, and hurried.
Six stagnations' pulses all accompany deepness, severe cases hidden, even more severe knotting, intermittent, and alternating. Only with stomach qi can it be treated; above appears at cun, middle at guan, below at chi, same for left and right.
Flat pulse wiry and large indicates labor damage and deficiency; large and forceless indicates easy-to-support yang decline; rapid and forceless indicates difficult-to-remove yin fire; weak cun indicates upper damage, floating-large indicates interior depletion; both chi and cun minute indicates five-labor physique. Blood emaciation left soggy, qi timidity right pushed, left-right minute indicates no remaining qi and blood. Consumption disease pulse rapid, or choppy and fine like silk, tidal sweating, coughing blood, flesh wasting indicates death.
Generally speaking of deficiency damage, it is damage due to deficiency. Deficiency labor is inability to tolerate labor due to deficiency, becoming more deficient due to labor. Consumption disease is the extreme of labor, i.e., five labors and six extremes. Lao means firm, indicating the disease is entrenched and insoluble. Various deficiency pulses are mostly wiry and large at cun/guan and minute-choppy at chi; with fire, chi is also large. Large indicates righteous qi deficient and evil abundant; wiry indicates interior cold. If large and forceless, yang qi is deficient; large-rapid-forceless indicates yin blood deficient; left-right minute indicates chronic cold. Consumption symptoms include bone-steaming tidal fever, night sweats, coughing blood, diarrhea or constipation; only when flesh is severely wasted and pulse is rapid, fine, and choppy does death occur. Ancient saying: Minute and rapid does not constitute disease, not named labor.
Wind-cold-summerheat-dampness, qi stagnation generating saliva, lower deficiency upper excess, all cause dizziness. Wind pulse floating, cold pulse tight, damp pulse fine, summerheat pulse deficient, phlegm pulse wiry and slippery, stasis pulse knotted and choppy. Rapid indicates big fire evil, deficient-large indicates long-term extremity, first regulate qi-phlegm, then follow symptom-pulse. Headache yang pulse wiry, floating wind, tight cold, heat must be surging-rapid, damp fine and firm. Qi-deficient headache, though wiry with choppy, phlegm-jue is slippery, kidney-jue is firm-solid.
Six-channel pulse-symptoms are same as cold damage; seeing *Bing Ji Jue*: Headache short-choppy must die, floating-slippery wind-phlegm are easily removed.
Eye disease is originally fire disease, heart-liver rapid-surging, appearing at right cun/guan, ministerial fire rushing up.
Left cun pulse surging-rapid indicates heart fire blazing; guan wiry and surging indicates liver fire flourishing; right cun/guan both wiry and surging indicates liver wood carrying ministerial fire momentum to insult metal it cannot overcome, and control earth it overcomes.
Ear disease is kidney deficiency, pulse slow-soggy, floating-large is wind, surging-moving is fire thief. Deep-choppy qi condensation, rapid-solid heat block, this long deafness is solely blamed on kidney. Sudden disease floating-surging, both chi same, or both chi rapid, indicates yin deficiency fire rushing.
If left cun surging-rapid, heart fire blazing; both chi surging-rapid, ministerial fire blazing, person must have nocturnal emission, tinnitus, or deafness.
Right cun surging-rapid, nasal bleeding or nasal discharge; left cun floating-slow, nasal mucus wind pathogen.
Nasal flow clear mucus.
Mouth and tongue sores, pulse surging-rapid; if pulse deficient, middle qi insufficient.
Classic says: Left cun surging-rapid heart heat, right cun floating-rapid lung heat. Left guan wiry-rapid and deficient, gallbladder deficiency severe; surging and solid liver heat. Right guan deep-solid, spleen-stomach excess heat, combined with surging-rapid mouth sores, or wooden tongue, double tongue. Pulse deficient indicates middle qi insufficiency.
Toothache kidney deficiency, chi soggy and large, fire blazing chi surging, teeth loose and broken. Right cun/guan rapid, or surging and wiry, belongs to intestines-stomach, wind-heat much saliva.
Chi surging-large and deficient, kidney deficiency; toothache loose and sparse, ministerial fire flaring up; right cun/guan surging-rapid, or wiry and surging, wind-heat in intestines-stomach.
Gout pulse deep-wiry, liver-kidney afflicted by damp. Shaoyin weak-floating, wind-blood pulling urgent. Or choppy and small, post-drinking wind attack.
Cun deep and wiry, deep governs bone, wiry governs tendon, deep is kidney, wiry is liver, sweating entering water, water hurting heart, thus joint pain and yellow sweat. Shaoyin pulse floating and weak, weak blood insufficient, floating is wind, wind-blood fighting, pain like pulling. Or chi choppy-small, shortness of breath, spontaneous sweating, joint pain unable to bend/extend, all caused by drinking sweating facing wind.
Wind-cold-dampness combine to form bi. Floating-choppy-tight, three pulses complete.
Pulse floating-slow belongs to damp, numbness-paralysis; pulse tight-floating belongs to cold, painful-bi; pulse choppy-knotted belongs to dead blood, numbness unaware of pain/itch; pulse floating-soggy belongs to qi deficiency, anterior guan numbness in upper body, posterior guan numbness in lower body.
Macules/rash pulse deep-hidden, or scattered or absent; yang floating-rapid, fire seen on trunk; yin solid-large, heat steaming skin.
Hua Boren said: Pulse is the wave of blood. So macule eruption is blood scattering on skin, hence pulse hidden; fire excess on surface, hence yang pulse floating-rapid; lower jiao excess heat, hence yin pulse solid-large.
Cough causes: floating wind, tight cold, rapid heat, fine damp, sexual fatigue choppy-difficult. Right guan slight-soggy, diet injuring spleen; left guan wiry-short, liver extreme labor-fatigue. Lung pulse floating-short, cough and period, five zang coughs, each view own part. Floating-tight deficiency-cold, deep-rapid excess-heat; surging-slippery much phlegm, wiry-choppy little blood. Form large pulse fine, insufficient breath, deep-small hidden, all are perilous pulses, only floating-large with cough survives, exterior symptoms interior pulse, reference balanced.
Exterior symptoms muscle/flesh wasting, fever diarrhea, interior pulse deep-urgent must die.
Huo Luan vomiting/diarrhea, pulse slippery but uneven, or slight-choppy, intermittent-hidden shocking. Heat much surging-slippery, wiry-slippery food discussion.
Right guan slippery is Huo Luan vomiting/diarrhea, pulse choppy-knot-intermittent-hidden, although due to phlegm-food obstruction, do not hastily judge death. However, it may only appear briefly, gradually becoming slippery-large is auspicious. Thus verse says: Huo Luan condition pulse slight-slow, little qi no speech very hard to treat. Pulse wiry severe also dies. Surging-slippery is heat; wiry-slippery is diaphragm leftover food-retained fluid, should vomit.
Heart pain slight-urgent, pain severe hidden, yang slight yin wiry, or short and rapid. Tight-solid stool difficult, slippery-solid phlegm accumulation. Heart bi drawing back, pulse slight-large, cun deep-slow, guan tight-rapid-sharp.
Yang slight deficiency in upper jiao, hence chest bi pain. Heart pain, pulse yin wiry therefore. Chest bi disease, panting cough spit. Chest bi pain shortness of breath, cun kou pulse deep-slow, guan small-tight-rapid.
Abdominal pain guan pulse, tight-small-urgent, or moving and wiry, severe deep-hidden. Wiry food slippery phlegm, chi tight umbilicus-abdomen. Heart-abdominal pain pulse, deep-small is fortune, floating-large-wiry-long, life cannot return.
Pulse small-fine-urgent-quick, middle abdomen stabbing pain. Chi pulse tight-solid, umbilicus and lower abdomen pain, should purge. If chi pulse hidden, lower abdomen pain has mass-hernia.
Malaria pulse naturally wiry, wiry-rapid much heat, wiry-slow much cold, wiry-slight deficiency-fatigue, wiry-slow should warm, tight-small purge-down, wiry-floating vomit-it, wiry-tight sweat-release. Also deaths, pulse scattered and resting.
Although malaria long-term extremely deficient, pulse slight seems not wiry, yet must see wiry within deficient-rapid, just not beating hand. All sweating/vomiting/purging, pulse wiry-small-tight, matching flesh holding long time suitable for purging; wiry-slow suitable for warming; wiry-tight suitable for sweating/acupuncture; floating-large suitable for vomiting; wiry-rapid wind-fire, stop with diet consideration. This sweating/vomiting/purging method, extended to hundred diseases all so.
Dysentery pulse mostly slippery, pressing deficient-cut off, chi slight no yin, choppy little blood, deep-small survival, surging-wiry death rule.
Intestinal dysentery, although avoid body heat, also avoid cold limbs.
Fullness pulse slippery-large, phlegm-fire making trouble, wiry-hidden middle deficiency, slight-choppy decline-inferior.
Chest fullness mostly phlegm-fire, hence cun slippery and large. Right guan wiry-slow or hidden, liver attacking spleen deficiency generating saliva, qi stagnation not comfortable. Slight reversing upper, choppy reversing lower, qi-blood deficiency. Slight qi decline much vexation, choppy blood little much jue.
Damp pulse naturally deep, deep-slow cold invasion, deep-rapid fire-heat, deep-deficient slippery-depletion. Summerheat-warm slow-weak, mostly in summer months.
Slight-small survival, floating-wiry death, violating five deficiencies also death.
Acid regurgitation pulse shape, mostly wiry and slippery. Or deep-slow, chest cold-fluid; or rapid-surging, diaphragm phlegm-heat.
Occasional vomiting acid water, wanting to become reverse vomiting.
Five jaundices excess-heat, pulse must surging-rapid. Or slight-choppy, syndrome belongs to deficiency-weak.
Due to yangming meridian internal heat storage, or thirst drinking water, or self-sweat bathing water, or hunger-hurting fullness, or drunk-full sexual room yellowing, pulse mostly deep; due to sudden heat bathing cold, post-drink facing wind, pulse mostly floating. Generally wine jaundice deep-wiry or fine, long-term black jaundice. Fuyang pulse slow, food dare not full, or tight-rapid stomach heat digesting grain. Accompanying cold then food finished reverse full, named grain jaundice. Chi pulse floating kidney injury, Fuyang pulse tight spleen injury. All yellow condition cun kou pulse, near palm no pulse, mouth-nose black color untreatable.
Edema syndrome, has yin has yang, yin pulse deep-slow, color green-white, no thirst but diarrhea, urine clear-choppy. Pulse or deep-rapid, color red-yellow, dry stool red urine, combined thirst is yang, deep-small must die, floating-large no harm.
Yang pulse must see yang syndrome, yin pulse must see yin syndrome. Deep-small water more abundant uncontrollable, floating-large heart fire generates earth, water can control.
Distension-fullness pulse wiry, spleen controlled by liver, surging-rapid heat distension, slow-weak yin cold, floating empty distension, tight middle solid. Floating-large can survive, deficient-small critical danger.
Focus on guan.
Nocturnal emission white turbidity, should verify at chi, knot-knotted-moving-tight, two syndromes' target. Slight-choppy essence injury, surging-rapid fire forcing, also heart deficiency, left cun short-small, pulse slow can survive, urgent-quick early death.
Quickly know deficient-floating, frequent nocturnal emission death.
Lumbar pain pulse, must deep and wiry, deep qi stagnation, wiry damages kidney origin. Or floating-tight, wind-cold entangling, damp injury soggy-fine, solid sprain-strain. Choppy blood stasis, slippery phlegm-fire frying, or drawing back pain, deep-slippery easy recovery.
Chi pulse deep, lumbar-back pain, frequent essence loss, little food, pulse deep-slippery-slow, treatable.
Hernia pulse wiry-urgent, accumulation-mass brewing, observe which part, liver is root organ. Heart slippery lung deep, wind hernia floating-swirling, guan floating-slow, wind-deficiency illness. Yang urgent mass, yin urgent hernia shape. Deep-slow floating-choppy, hernia-mass cold pain, pain severe hidden, or fine or moving. Firm-urgent survival, weak-urgent loss.
Hernia originally liver meridian, wiry defense qi not moving and aversion to cold, urgent not wanting food, wiry-tight fighting then cold hernia. Fuyang pulse floating-slow, floating wind-deficiency, slow cold hernia. Three yang urgent mass, three yin urgent hernia. Heart-stomach pulse slippery then heart wind hernia disease, taiyang pulse floating then kidney wind hernia disease, shaoyang pulse floating then liver wind hernia disease.
Hong Kong foot pulse, floating-wiry wind, soggy-damp slow-cold, heat rapid and surging. Tight anger, scattered worry rush, fine sorrow excess, knot qi attack. Both chi not responding, doctor must useless work.
Left chi not responding hard recovery, cun kou irregular untreatable.
Xiao Ke liver disease, heart slippery and slight, or tight-surging-rapid, yang excess yin exhausted. Blood deficiency soggy-scattered, labor then floating-slow, short-floating do not treat, rapid-large hard to treat.
Floating defense qi deficient, short nourishment exhausted, hence untreatable; rapid big fire blazing, also untreatable. But Shuhe again says: Xiao Ke pulse rapid-large lives, deficient-small life perishes must strive. Why? Cover initial onset rapid-large not firm-solid, fire still can be shaped. Deficient-small is floating-short, understand its meaning, also not opposite.
Dryness knot pulse, deep-hidden do not doubt. Heat knot deep-rapid, deficiency knot deep-slow. If wind-dryness, right chi floating-fat.
Elderly deficient people stool knot, pulse sparrow-pecking untreatable.
Two hypochondriac pain, pulse must double-wiry. Tight-fine-wiry, many anger bias; deep-choppy-urgent, phlegm-stasis fault.
Double-wiry, liver qi surplus. Liver pulse urgent and hypochondrium has qi branch-full, drawing lower abdomen pain, sometimes urination difficult, suffering eye dizziness headache, waist-back heavy, feet cold, women menstrual water not coming, sometimes sometimes. Deep-soggy-choppy-scattered, its color luster, should disease overflow drink, much drinking water and saliva overflow into skin muscle intestine outside; or combined beating hand firm-urgent, face color not luster, blood stasis. Or due to falling-drop causing it.
Lin disease pulse, fine-rapid what harm? Shaoyin slight, qi closing bladder. Women seeing it, yin producing sores. Great solid easy cure, deficient-chopply its perishing.
Large and solid survival, deficient-fine-chopply death.
Urination not through, floating-wiry and choppy. Knotted then urine red, rapid then yellow-red, difficulty urination is long, solid seen left chi.
Urination not beneficial difficult coming is long-close, is bladder heat extreme, hence pulse solid.
Five accumulations belong to yin, deep-hidden attaching bone, liver wiry heart knotted, kidney deep-urgent-slippery, spleen solid and long, lung floating panting sudden. Six gathers knot deep, entrenched then floating-knot. Also mass, its pulse mostly wiry, wiry-urgent mass disease, wiry-fine firm, heavy-middle scattered, food forming癖 . Left turn heavy, qi chest front; if meat , right turn horizontal spin. Accumulation mass, tight then pain winding, deficient-weak death, solid-strong can recover.
Pulse deep-hidden and fine, at cun, accumulation in chest middle; slightly out cun kou, accumulation in throat middle; at guan upper, accumulation at navel side; above guan upper, accumulation in heart down; slightly down guan, accumulation in small intestine. Chi slight, accumulation at qi chong. Pulse out at right, accumulation at right; pulse out at left, accumulation at left; pulse two out, accumulation in center, each by its part place it.
Liver accumulation pulse wiry and fine, lung accumulation pulse floating and hairy, kidney accumulation pulse deep and urgent-slippery, heart accumulation pulse deep and knotted, up-down no constant place, spleen accumulation pulse solid and long, eating then much vomiting.
*Nei Jing* discusses red pulse arriving, panting and firm, accumulation in middle, named heart bi, obtained from thinking, white pulse panting and floating, accumulation in chest, named lung bi, obtained from drunk and making inside. Panting, means pulse arriving like sudden panting state. Green pulse long and left-right bouncing hand, accumulation qi in heart down branch , named liver bi, obtained from cold-damp, same as hernia. Yellow pulse large and deficient, accumulation in abdomen middle, named jue hernia. Women same method obtained disease, making four limbs sweating facing wind. Black pulse upper firm and large, accumulation qi in lower abdomen
