Discussion
The *Huangdi Neijing* (Yellow Emperor's Inner Canon) states: If one catches a wind immediately after washing hair, they will develop Shou Feng (Head Wind). It further states: The manifestation of Shou Feng includes excessive sweating on the head and face and aversion to wind. The condition worsens one day before the wind blows; the headache is so severe that one cannot go out or stay indoors. On the day the wind actually blows, the condition improves slightly. Li Dongyuan stated: The *Jingui Zhenyan* (Golden Cabinet True Words) discusses that East Wind arises in Spring, causing disease in the Liver, manifesting in the neck and nape. Therefore, pathogenic factors in Spring often affect the head. Furthermore, all Yang meridians converge on the head and face. For instance, when the Foot-Taiyang Bladder Meridian is diseased, it causes rushing headache; when the Foot-Shaoyang Gallbladder Meridian is diseased, it causes pain at the temporal angle and jaw. Wind pathogens enter from the upper body; Wind-Cold damages the upper part. Pathogens enter from outside and lodge in the collaterals, causing chills, headache, heavy body, and aversion to cold. Treatment should focus on Fengchi (GB20) and Fengfu (GV16), regulating Yin and Yang. Deficiency is tonified, excess is drained. Sweating leads to recovery. This describes headache caused by Shanghan (Cold Damage). Headache accompanied by tinnitus and dysfunction of the nine orifices originates from the Stomach and Intestines; this is Qi-deficiency headache. Heart vexation with headache indicates pathology in the Diaphragm, involving the Hand-Taiyang Small Intestine Meridian and Hand-Shaoyin Heart Meridian; this is Damp-Heat headache. If Qi rises but does not descend, causing headache or even Dianji (Epilepsy), this is deficiency below and excess above, involving the Foot-Shaoyin Kidney Meridian and Foot-Taiyang Bladder Meridian. Severe cases involve the Kidney; this is Cold-Damp headache. If there is half-head cold pain, first select points from the Hand-Shaoyang Sanjiao Meridian and Hand-Yangming Large Intestine Meridian, then select points from the Foot-Shaoyang Gallbladder Meridian and Foot-Yangming Stomach Meridian; this is Pian Tou Tong (Migraine/Hemicrania). There is a condition called Zhen Tou Tong (True Headache); if severe, the entire brain hurts, and hands and feet are cold up to the joints; this is a death syndrome, untreatable. There is Jue Ni Tou Tong (Reversal Headache), caused by exposure to severe Cold penetrating to the bone marrow. Marrow is dominated by the Brain; Brain Qi rebellion causes headache, and teeth also hurt. Generally speaking, treating all headaches with Wind-dispelling herbs is a broad principle. The high peak of the head can only be reached by Wind herbs. Therefore, herbs with thin flavors belong to "Yang within Yin," rising from earth to heaven. However, there are differences among the Three Yin and Three Yang meridians. Thus, Taiyang headache presents with aversion to wind and floating-tight pulse; Chuanxiong, Qianghuo, Duhuo, and Mahuang are primary. Shaoyang meridian headache presents with wiry-thin pulse and alternating chills and fever; Chaihu is primary. Yangming headache presents with spontaneous sweating, fever, aversion to cold, and floating-slow-long-excess pulse; Shengma, Gegen, Shigao, and Baizhi are primary. Taiyin headache necessarily involves phlegm and heavy body, or abdominal pain due to Tan Pi (Phlegm Mass); pulse is sinking-slow; Cangzhu, Banxia, and Nanting are primary. Shaoyin meridian headache involves non-circulation of the Three Yin and Three Yang meridians, with cold feet and rebellious Qi constituting Han Jue (Cold Reversal); pulse is sinking-thin; Mahuang, Xixin, and Fuzi are primary. Jueyin top-of-head pain, or vomiting saliva/cold limbs with floating-slow pulse, is treated primarily with Wuzhuyu Tang. Blood-deficiency headache is treated primarily with Danggui and Chuanxiong. Qi-deficiency headache is treated primarily with Renshen and Huangqi. Combined Qi and Blood deficiency headache is treated with Tiaozhong Yiqi Tang with small amounts of Chuanxiong, Manjingzi, and Xixin, yielding miraculous effects. Baizhu Banxia Tianma Tang is for Tan Jue Tou Tong (Phlegm-Reversal Headache). Qingkong Gao is for Feng Shi Re Tou Tong (Wind-Damp-Heat Headache). Qianghuo Fuzi Tang is for Jue Ni Tou Tong (Reversal Headache). If dampness is in the head, use bitter herbs to induce vomiting; do not adhere rigidly to fixed formulas. My teacher once suffered from headache; upon onset, cheeks were blue-yellow, dizzy with eyes unwilling to open, reluctant to speak, heavy body, and feeling like vomiting. Ji Guo said: This is a combined disease of Jueyin and Taiyin, named Feng Tan Tou Tong (Wind-Phlegm Headache). Treat with Ju Fang Yuhu Wan, and moxa Xiashi (GB43) point for cure. Thus, we know that Formula is the Substance, Method is the Application. Those who stick to Substance without knowing Application have flaws. Balancing Substance and Application makes one a superior physician. Can learners stick to one aspect without knowing how to vary?
Pulse Diagnosis
The *Huangdi Neijing* states: A short pulse at Cun Kou indicates headache.
The *Mai Jing* (Pulse Classic) states: Yang-wiry pulse indicates headache. It also states: Floating pulse at Cun Kou indicates Wind-stroke, fever, and headache.
Tight pulse indicates headache due to Shanghan pathogen. Tight pulse appearing at Cun Kou indicates Wind-injury headache.
The *Mai Jue* (Pulse Rhymes) says: Headache with short-choppy pulse implies death; floating-smooth Wind-Phlegm is easily removed.
Methods
Zhu Danxi said: Headache is mostly due to Phlegm; severe pain is mostly due to Fire. One should clear Phlegm and descend Fire.
In people weakened by overwork and lower deficiency, resembling Shanghan with fever and sweating, severe pain in both Taiyang acupoints indicates Min Fire rushing up from below. Tonify Middle and Boost Qi Decoction (Buzhong Yiqi Tang) with Chuanxiong and Danggui is indicated. Severe cases add Zhimu, Manjingzi, and Xixin.
Qi stagnation in various meridians also causes headache; treat by regulating Qi according to the meridian.
For Hemicrania: Right side belongs to Phlegm and Heat; use Cangzhu and Banxia for Phlegm, and Wine-processed Pian Huangqin for Heat. Left side belongs to Wind and Blood deficiency; use Jingjie and Bohe for Wind (some say Jingjie and Bohe are essential herbs for headache, add based on syndrome differentiation), and Chuanxiong, Danggui, Shaoyao, and Wine Huangbai for Blood deficiency. Various schools fail to distinguish the meridian involved, hence poor efficacy. Shaoyang hemicrania often involves constipation; purgation may be appropriate.
Formula 1: Miraculous effect for Wind-Damp-Heat headache.
Pian Huangqin (1 liang, wine-processed fried), Cangzhu, Qianghuo, Fangfeng (each 5 qian), Cangerzi (3 qian), Xixin (2 qian).
