Half a month passed, and various medicines were ineffective. I recalled a family-prescribed formula: use one soft-shelled turtle from a pond, process it according to regulations, cook it into a soup with plenty of ginger and rice, add a small piece of brown sugar, do not use salt or soy sauce, boil until cooked. Eat one or two bowls; after three days without defecation, the large intestine will become firm, and the anus will retract and no longer prolapse. This condition is likely due to deficiency of Spleen Earth, causing both the Lung and Large Intestine to lose nourishment from the source of transformation (the origin of Qi and Blood generation). Therefore, the Large Intestine cannot perform its function of astringency; this is the mother organ (Spleen) being deficient leading to the deficiency of the child organ (Lung/Large Intestine). The turtle belongs to the class of shelled creatures, corresponds to Metal in Five Elements, but possesses the warm nature of Earth, and can tonify the Spleen and Lung. Moreover, the Lung dislikes cold, and previously many cold and cool herbs such as Huangqin (Scutellariae Radix) and Lianlian (Coptidis Rhizoma) had been used. Now, using the pungent nature of Shengjiang (Zingiberis Rhizoma) to tonify Lung Metal, and the sweet nature of Shatang (Saccharum) to tonify Spleen Earth, once Lung Qi is solid, the Large Intestine also becomes solid, thus able to perform its astringent function. Hence, the efficacy is so remarkable.
Discussion
The *Neijing* states: "All vomiting of acid fluids, sudden diarrhea with tenesmus, belong to Fire." Li Dongyuan said: "Vomiting (Ou), spitting (Tu), and hiccups (Yue) all belong to the Stomach. The Stomach is the manager, differing only by the quantity of Qi and Blood." Vomiting belongs to the Yangming channel; Yangming has abundant Blood and Qi, so there is sound and matter expelled, indicating disease in both Qi and Blood. Spitting belongs to the Taiyang channel; Taiyang has abundant Blood but little Qi, so there is matter expelled without sound, indicating disease in Blood. Hiccups belong to the Shaoyang channel; Shaoyang has abundant Qi but little Blood, so there is sound without matter expelled, indicating disease in Qi. Liu Hejian said: "Excessive heat in the Stomach and Diaphragm causes vomiting, which is the manifestation of Fire Qi flaming upward." There are three types of spitting: Qi, Accumulation, and Cold, all discussed from the perspective of the Triple Burner. The Upper Burner is at the stomach opening, connecting upwards to Heaven Qi, governing reception without discharge. The Middle Burner is at Zhongwan, connecting upwards to Heaven Qi and downwards to Earth Qi, governing the rotting and ripening of food and drink. The Lower Burner is below the navel, connecting downwards to Earth Qi, governing discharge without reception. Therefore, Upper Burner spitting arises from Qi; Qi is Yang of Heaven, its pulse is floating and surging, symptoms include vomiting immediately after eating, thirst for water, dry and constipated stool, Qi rushing up to the chest causing pain; treatment should focus on descending Qi and harmonizing the middle. Middle Burner spitting arises from accumulation; there is Yin and Yang, where food and Qi combine to form accumulation causing pain; its pulse is floating and long, symptoms may be pain followed by spitting, or spitting followed by pain; treatment should use potent drugs to remove accumulation, and use Binglang (Arecae Semen) and Muxiang (Aucklandiae Radix) to move Qi. Lower Burner spitting arises from Cold; it is a matter of Earth's way, its pulse is deep and slow, symptoms include morning food vomited in the evening, evening food vomited in the morning, clear and profuse urination, constipation; treatment should use potent drugs to unblock impaction, warm and disperse Cold, and once stool gradually passes, harmonize with Middle Burner herbs to prevent re-constipation, allowing spontaneous recovery. Additionally, there are cases of external Cold Damage where Yangming excess heat is too severe causing vomiting and reflux; internal injury from diet filling the Taiyin, preventing Stomach Qi from circulating freely causing vomiting; vomiting due to Stomach Heat; vomiting due to Stomach Cold; chronic illness with Qi deficiency and severely衰败 Stomach Qi, causing nausea and hiccups upon smelling grain aroma; excessive Spleen Dampness failing to transform fine essences, causing clear Phlegm and retained fluid stagnating in the Upper and Middle Burners, causing frequent nausea and vomiting of clear water. Each should be treated by analogy; one must not adhere to a single viewpoint.
Pulse Diagnosis
The *Maijing* states: "Vomiting with a weak pulse, free urination, slight body heat, and appearance of Jue (cold extremities) indicate a difficult-to-treat case."
If the Dizhong pulse is floating, it indicates deficient Stomach Qi, Cold Qi above, and belching below; the two Qi clash, exiting but not entering; the person vomits and cannot eat; if terrified, death ensues; if relaxed, improvement occurs.
If the pulse is tight on the Yang side and rapid on the Yin side, the person vomits immediately after eating; a floating and rapid Yang pulse also indicates vomiting.
If the Cun Kou pulse is tight and Kou (hollow), tight indicates Cold, hollow indicates Deficiency; Deficiency and Cold clashing result in a Yin knotting and slow pulse; the person experiences dysphagia; if the Guan position pulse is rapid, the person vomits.
A string-like (Xian) pulse indicates Deficiency; lack of residual Stomach Qi leads to morning food vomited in the evening, evening food vomited in the morning, transforming into Fanwei (reverse regurgitation). Cold is above, but the doctor mistakenly uses purgatives, causing the pulse to become Xian, hence named Deficiency.
If the Cun Kou pulse is faint and rapid, faint indicates lack of Qi; lack of Qi leads to Ying (Nutritive) Deficiency; Ying Deficiency leads to insufficient Blood; insufficient Blood leads to cold in the chest, thus vomiting.
Methods
Zhu Danxi said: "Heat in the Stomach and Phlegm in the Diaphragm: Erchen Tang (Two-Aged Decoction) added with stir-fried Zhizi (Gardeniae Fructus), Ginger-stir-fried Huanglian (Coptidis Rhizoma), and Shengjiang (Zingiberis Rhizoma), decocted and taken."
For any vomiting, purgatives must absolutely not be used, as this goes against the pathology.
For chronic vomiting due to Stomach Qi deficiency failing to receive grain, treat with Renshen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Huangqi (Astragali Radix), Baizhu, and Xiangfu (Cyperi Rhizoma). For distension, fullness, shortness of breath, and vomiting, tonify Middle Qi; only Tiao Zhong Yi Qi Tang (Regulate and Tonify Middle Qi Decoction) should be used.
Liver Fire emerging from the Stomach, reversing upward causing vomiting: Yiqing Wan (Suppress Green Pill). (Formula see Fire Section.)
Summer vomiting that does not cease: Wuling San (Five-Ingredient Powder with Poria) added with Ginger Juice, mixed into a decoction and taken.
Vomiting worms: Use Heixi (Black Tin) roasted to ash, mixed with Binglang (Arecae Semen) powder, and taken with Rice Drink.
Nausea and vomiting of clear water: Can be due to Heat, Phlegm, or Deficiency; always use Shengjiang (Zingiberis Rhizoma), adding other herbs according to the syndrome.
Heat in the Stomach: Erchen Tang added with Ginger-juice processed Huanglian (Coptidis Rhizoma) and Huangqin (Scutellariae Radix). If accompanied by Deficiency, add Renshen (Ginseng Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma). Also stated: For weak Stomach causing vomiting, Erchen Tang added with Sharen (Amomi Fructus), Huoxiang (Agastaches Herba), and Baizhu (Atractylodis Macrocephalae Rhizoma).
Pathology caused by Phlegm-Fluid, or due to excessive consumption of raw/cold foods causing Spleen-Stomach disharmony, leading to vomiting, nausea, dizziness, epigastric discomfort, or alternating chills and fever: Erchen Tang added with Dingxiang (Syzygii Flos), Wumei (Mume Fructus), and Shengjiang (Zingiberis Rhizoma), decocted and taken. If epigastric stuffiness and pain occur, add Caodoukou (Cardamomi Rotundus Fructus).
Excessive Phlegm-Heat causing nausea and vomiting: Daotan Tang (Guide Out Phlegm Decoction) added with Suosha (Amomi Fructus Rotundus), Ginger-stir-fried Huanglian (Coptidis Rhizoma), and Zhuru (Bambusae Caulis in Taenia).

Medical Case
The wife of the Huang family in the city, nearly thirty years old, suffered postpartum fatigue and food injury, leading to Stomach deficiency unable to receive grain for over forty days. Upon smelling grain aroma, she felt nauseous and vomited; upon smelling medicine aroma, she also vomited. She sought my treatment. I said: "If medicine cannot enter the mouth, what method can be used?" Insisting repeatedly, I finally formulated a prescription: Renshen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Fuling (Poria) each 5 qian, Gancao (Licorice Root) 0.02 qian, Chenpi (Citri Reticulatae Pericarpium), Huoxiang (Agastaches Herba), Sharen (Amomi Fructus) each 0.5 qian, Stir-fried Shenqu (Massa Medicata Fermentata) 1 qian, Ten-year-old Chen Cangmi (Coeli Semen Retentum) 1 he, Shunliushui (Flowing Water) two large cups boiled, soak Fulonggan (Caulis Longani Folium / Lodestone Ash? Note: Fulonggan usually refers to Furnace Soil/Caulis Longani Folium contextually often interpreted as prepared earth/mineral in ancient texts, here likely prepared soil/ash), grind fine, stir turbid, let settle clear, take one cup, add Ginger and Date, decoct previous herbs to 0.7 cups, take slightly cool. This medicine was retained without vomiting. Separately, give Chen Cangmi decoction frequently, advance previous medicine 2-3 times daily, gradually able to eat porridge and stabilize. Later, treating over ten people with this method, all were effective.
