[Qian Yi] If an infant experiences vomiting and diarrhea within three days after birth, accompanied by high fever, refusal of milk and food, undigested milk/food in stool, or white stool, this is a Cold Damage (Shanghan) disease. It should be treated with purgation to harmonize the Stomach Qi. For purgation, use Baibing Zi (White Pill); for harmonizing the Stomach, use Yihuang San (Yellow-Protecting Powder) as the main formula.
If an infant is between three days and ten days old and experiences vomiting and diarrhea, with a body temperature that is warm or cool, refusal of milk and food, and greenish-white undigested stool, this indicates Excess above and Deficiency below. There may also be concurrent symptoms involving the Five Zang organs: Lung involvement presents as sleeping with eyes half-open (sleepy eyes), panting; Heart involvement presents as palpitations and thirst; Spleen involvement presents as fatigue and excessive sleep; Liver involvement presents as yawning and irritability; Kidney involvement presents as muteness and photophobia. One must first observe the concurrent organ symptoms, first draining the excess and then tonifying the deficiency. For example, if the Spleen is deficient, use Yihuang San. These two conditions often occur in autumn and summer.
If vomiting and diarrhea occur after the Summer Solstice in May, accompanied by high fever, this is a Heat syndrome, because infants have nine parts heat to one part cold in their viscera. If caused by heat injury leading to indigestion and deep yellow diarrhea, use Yulu San (Jade Dew Decoction).
Yulu San, also known as Ganlu San (Sweet Dew Decoction).
Hanshuishi (Halloysitum, half tael, soft and slightly dark green-black with fine lines) Sheng Gancao (Raw Glycyrrhizae Radix et Rhizoma, one qian) Shigao (Gypsum Fibrosum, half tael, hard, white, and fibrous, unbreakable by hand. If unavailable, replace with Fangjiexie [Calcitum], which is hard, white like Gypsum, and breaks into square fragments.)
Grind all ingredients into a fine powder. Take one zi (approx. 0.5-1g), or five fen or one qian, mixed with warm water after meals. (Zhang Yuansu Haicang said: This should not be taken unless there is severe Fire of the Ministerial Heat in the Kidney.)
If vomiting and diarrhea occur after the Great Heat in June, with the body feeling very warm but resembling heat, indicating six parts heat and four parts cold in the viscera. Symptoms include vomiting, indigestion, yellowish-white diarrhea, thirst, variable feeding. Take small amounts of Yihuang San before meals and large amounts of Yulu San after meals. The Fifth Taiwei of Guangqin Palace suffered from incessant vomiting and diarrhea with undigested rice and grain. Physicians used warming medicines, causing increased wheezing and uncontrollable vomiting after one day. Qian Yi said: Treat with cooling medicines. This is because internal heat injury exists. Use Shigao Tang (Gypsum Decoction) three times. Other physicians argued that due to heavy vomiting/diarrhea and undigested food, the Spleen should be tonified, questioning the use of cooling medicine. Wang Xin and others insisted on tonifying the Spleen with Dingxiang San (Clove Powder) three times. When Qian Yi's physician arrived later, he said: Do not take this. In three days, abdominal distension, fever, thirst, and vomiting will occur. Three days later, it happened as predicted. This is because the intense heat of June penetrates the abdomen, causing thirst; heat injures the Spleen and Stomach, leading to severe vomiting and diarrhea. The use of warming medicines further heated the Upper Jiao, causing wheezing and thirst, worsening in three days. Unable to treat, they summoned Qian Yi again. Seeing heat signs, he administered Baihu Tang (White Tiger Decoction) three times, followed by Baibing Zi to purge. Reduce dosage by two fen daily. On days two and three, give Baihu Tang twice each. On day four, give Shigao Tang once, plus freshly prepared Maidong (Ophiopogonis Radix), Huangqin (Scutellariae Radix), Bingpian (Borneolum Syntheticum), Niuhuang (Calculus Bovis), Tianzhuhuang (Spatholobi Caulis? No, Tianshuhuang/Hyacinth Bean? Context implies Tianshuhuang or similar cooling phlegm herb, likely Tianzhuhuang [Hyacinthis Semen]), and Fuling (Poria), coated with Zhusha (Cinnabaris). Take five pills, washed down with Zhuyetang (Bamboo Leaf Decoction). Fever subsided, and the patient recovered.
If vomiting and diarrhea occur after Start of Autumn in July, with a warm body, indicating three parts heat and seven parts cold in the viscera. Inability to feed, excessive sleepiness, irritability, hiccups, sighing, sleepy eyes, white lips, frequent hiccups, desire to defecate but no thirst. Take more Yihuang San before meals and less Yulu San after meals. The Seventh Taiwei of Guangqin Palace, aged seven, suffered from vomiting and diarrhea in July. Symptoms included complete anorexia, lethargy, irritability upon waking, hiccups and dry retching, irregular stools, and no thirst. Physicians treated it as Wind-Convulsion, suspecting lethargy. Qian Yi said: Tonify the Spleen first, then clear heat. Give Shijunzi Wan (Quisqualis Pill) to tonify the Spleen and Shigao Tang to clear heat. The next day, mix Mercury and Liuhuang (Sulfur) powder, taken with ginger water. Qian Yi stated: For vomiting/diarrhea in May, use nine parts purgation and one part tonification; in August, use nine parts tonification and one part purgation. This was Spleen-deficiency diarrhea; improper treatment led to deficiency. Purgation would cause death. Tonify the Spleen; if Shijunzi Wan is too slow, use warming Stomach and benefiting Spleen herbs. Physician Li asked: Why hiccups after eating? Qian Yi: Spleen deficiency and scant fluids cause vomiting/hiccups. Asked: Why green-brown watery stool? Answer: Extreme cold and deficiency of intestines/stomach. Qian Yi cured him.
If vomiting and diarrhea occur after Autumn Equinox in August, with a cold body, indicating Yang deficiency. Inability to feed, dry retching/hiccups, green-brown watery stool. Tonify the Spleen with Yihuang San; do not purge.
[Tian Shi] For infant vomiting/diarrhea during intense summer heat: if pathogenic heat is in Lower Jiao, it causes diarrhea; if in Upper Jiao, vomiting. Fluid loss causes thirst; use Yulu San. Even if vomiting, administer frequently; recovery expected within three days. If fever and large pulse with yellow urine, use Wuling San (Five-Ingredient Powder with Poria) and Yi Yuan San (Benefiting Origin Powder) equally, mixed in hot water, taken warm. If cool body and fine pulse with blue-green urine, take Yihuang San in morning and Yulu San in afternoon. If weak after four-five days, use Yigong San (Extraordinary Efficacy Powder), Hezhong Wan (Harmonize Middle Pill), or Kaiwei Wan (Open Appetite Pill).
[Zhang Yuansu Jiegu] For wind-induced diarrhea, use Fangfeng (Saposhnikoviae Radix) and Qianghuo (Notopterygii Rhizoma et Radix). (Indicates Liver involvement with wind convulsions/spasms.) For heat-induced diarrhea, use Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix), and Dahuang (Rhei Radix et Rhizoma). (Indicates Heart involvement with fever.) For cold-induced diarrhea, use Fuzi (Aconiti Lateralis Radix Praeparata). (Indicates Kidney involvement with cold limbs.) For damp-induced diarrhea, use Baizhu (Atractylodis Macrocephalae Rhizoma) and Fuling (Poria). (Indicates Spleen involvement with somnolence/heaviness.) For Lung disease causing diarrhea, use Shaoyao (Paeoniae Radix) and Guixin (Cinnamomi Ramulus); for wheezing, use Maidong (Ophiopogonis Radix) and Renshen (Ginseng Radix et Rhizoma); severe cases add Binglang (Arecae Semen); if constipation, add Dahuang. (Indicates Lung involvement with cough/wheezing.) Assess chronicity. Acute cases stop discharge; chronic cases risk Changfeng (Intestinal Wind), requiring elimination of stale matter and promotion of new growth. Use Xuanfeng San (Dispel Wind Powder), follow with organ-specific Jun-Chen drugs, preferably Yihuang San.
[Qian Yi] For Cold Damage vomiting/diarrhea with warm body, alternating chills/fever, somnolence, coarse breathing, yellow-white stool, vomiting, indigestion, occasional cough, and concurrent organ symptoms: decoct organ-specific Jun-Chen drugs, take Daqing Gao (Great Cyan Paste) first, then Yihuang San. If previously purged or no indication for purging, absolutely avoid purgation. This is Cold damaging Spleen/Lung, preventing entry into Spleen. (Jiegu notes: Warm body/vomit/diarrhea/cough indicates Wind-Wood invading Spleen; Mother deficiency leads to Son weakness. Decoct Binglang Doukou Tang, take Daqing Gao, then Yihuang San.)
For Wind-Cold vomiting/diarrhea with fever, somnolence, ability to feed, incessant drinking, phlegm vomiting, yellow watery stool: this is Stomach deficiency-heat causing thirst/vomiting/diarrhea. Generate Stomach fluids to stop thirst, then use dispersing herbs. For thirst, use Baizhu San (Atractylodes Macrocephala Powder); for dispersing, use Daqing Gao. (Jiegu notes: Thirst/scanty urine -> Wuling San. Fever/vomiting -> Baizhu San, then Binglang Muxiang Tang + Daqing Gao.)
For Wind-Cold vomiting/diarrhea with cool body, frothy vomit, green-white stool, irritability without thirst, hiccups/sighing, sleepy eyes: this is lingering mild Wind-Cold causing vomiting/diarrhea. Tonify Spleen then disperse. Tonify Spleen with Yihuang San; disperse with Daqing Gao. Common in Spring/Winter. (Jiegu notes: Cool body/no thirst -> Cold. Decoct Fuzi Guizhi Tang, take Daqing Gao.)
Infants injured by Wind-Cold with vomiting/diarrhea: if doctors treat Spleen deficiency with warming tonics without success, then switch to cooling herbs without resolving exterior, it becomes "Original Wind Injury." Improper aggressive treatment depletes Spleen Qi, unable to resolve internally or externally. After ten+ days, symptoms include sleepy eyes, warm body, wind in Spleen/Stomach causing loose stools. Remove wind from Spleen with Xuanfeng San; when wind recedes, dysentery/diarrhea stops. Follow with Shijunzi Wan to tonify Stomach. Chronic vomiting/dysentery may lead to Spleen deficiency generating wind, resulting in Slow Convulsion.
[Zhang Yuansu Haicang] For excessive vomiting/diarrhea causing Spleen/Stomach deficiency and impending wind signs: use Sizijunzi Tang (Four Gentlemen Decoction) minus half dose of Baifuzi (Typhonii Rhizoma), decoct with Shengjiang (Zingiberis Rhizoma Recens).
[Qian Yi] Yulu San (See above).

Bury water jar in open ground, place box on jar, seal mud. Fire for three days/nights with three jin fuel, add one jin top fire, cool to retrieve.)
Grind in wooden mortar with willow pestle. Take two qian with Ginger Rice Drink. Adults: one liang powder mixed with steamed dough pill, size of sesame seed. Take 50-100 pills on empty stomach with Rice Drink.
Yan says: For extreme deficiency from vomiting/diarrhea, quickly generate Stomach Qi with Lizhong Wan. Mix Jinye Dan powder with Ginger Rice Drink. Must take large doses. Once Stomach Qi returns and limbs warm, if spasms persist, reduce Jinye Dan by 1-2 fen, increase Qingzhou Baiwanzi (Qingzhou White Pills) by 1-2 fen, mix and take as above. Combine with Yigong San, Qianghuo Gao, Wenbai Wan, Gouteng Yinzi, etc., frequent congee. Even critical cases often survive (8-9 out of 10). Shen Cunzhong noted Jinye Dan saved several infants with severe vomiting/diarrhea and absent qi; large doses are effective.
[Qian Yi] Doukou San (Cardamom Powder) treats vomiting/diarrhea with irritability/thirst, abdominal distension, scant urine.

Chop ingredients. Take three qian, decoct with 1.5 cups water, ten slices Shengjiang. Reduce to 0.8 cup, take warm before meals.
[World Physicians] Baifu Wan (See Spleen-Damp Section). Qian Yi states: Undigested milk in vomiting/diarrhea is Food Injury; purgation is indicated.
Son of Feng Chengwu, age five, vomiting/diarrhea, high fever, anorexia. Qian Yi observed less black iris, more white sclera, pale complexion, saying: This child will be sickly. Pale complexion indicates Spirit timidity; small black iris indicates Kidney deficiency. Black iris belongs to Water; inherent timidity/deficiency leads to frequent illness. Even if growing up, muscles will be weak, intolerant to cold/heat, prone to deficiency/excess fluctuations, Spleen/Stomach weak. Avoid indulgence; without care, lifespan won't reach prime. Lackluster face resembles blood loss in women. Current vomiting/diarrhea/anorexia/high fever is Food Injury. Timid/deficient children should not be purged; purging deficiency harms organs: Lung->cough, Heart->palpitations/fright, Spleen->diarrhea, Kidney->worsening deficiency. Use Xiaoji Wan (Eliminate Accumulation Pill) to gently digest slight food retention. If severe injury, purgation is allowed. Without purgation, masses form. If solid food stagnation, purgation is appropriate. Purge then tonify Spleen for cure. Adjust based on deficiency/excess; always effective.
[Zhu Danxi] Pediatric vomiting/diarrhea with Jaundice.
Sanleng (Sparganii Rhizoma) Pengze (Curcumae Rhizoma) Chenpi Qingpi (Citri Reticulatae Pericarpium Viride) Shenqu (Massae Fermentatae Medicinalis) Maiya (Hordei Fructus Germinatus) Huanglian Coptidis Rhizoma) Gancao Baizhu Fuling
