[Prescription Verse] The Cucumber Carbuncle (Huanggua Yong) arises on the sides of the back, caused by Spleen Fire accumulating into toxicity. The skin is red, resembling a cucumber in shape; it swells to a height of over one cun and a length of approximately one chi, accompanied by numbness in the four limbs and heart pain radiating from the abdomen.
[Annotation] This condition manifests on the lateral aspects of the back, also known as "Rou Gui" (Flesh Turtle), resulting from the accumulation of Spleen Fire toxins. The skin and flesh are red, shaped like a cucumber, swelling about one cun high and roughly one chi long, with numbness in the four limbs and pain radiating to the heart. A red, vibrant appearance with rapid suppuration indicates a favorable prognosis (Shun); a purple, sunken appearance with a faint pulse, spontaneous sweating, delirium, hardness, and delayed suppuration indicate an unfavorable prognosis (Ni). In the initial stage, Xianfang Huoming Yin (Immortal Life Elixir) should be taken, augmented with Qianghuo (Notopterygii Rhizoma et Radix) and Chaihu (Bupleuri Radix), or Duoming Dan (Life-Rescuing Pill) may be used. All other internal and external treatment methods follow the protocols for Carbuncles and Ulcers (Zhongyang Men and Ku Yang Men).
Prescriptions: Xianfang Huoming Yin (see Zhongyang Men); Duoming Dan (see Yin Yang Er Qi Ju).
[Prescription Verse] Shensu Fa occurs at the Shensu acupoint. Single lesions result from alcohol consumption, sexual excess, and damp-heat; double lesions arise from sexual exhaustion and liver fire. Favorable and unfavorable outcomes are distinguished by whether the lesion appears red and vibrant or purple-black and sunken.
[Annotation] This condition develops at the Shensu (BL23) acupoint, located in the hollows of the muscles on either side of the lumbar vertebrae. It can occur singly or doubly. Single lesions are caused by excessive alcohol and sexual activity combined with damp-heat; double lesions are triggered by sexual exhaustion and anger/fire. If the lesion is red, vibrant, and highly swollen, with pus forming around the fourteenth day, it is a favorable sign (Shun). If the lesion is purple-black, dry, hard, and fails to produce pus when expected, it is an unfavorable sign (Ni). If the pus is thin and damages the fascia, it indicates a severe deficiency of True Yang and blood/qi. Initially, Renshen Yangrong Tang (Ginseng Nourishing Harmony Decoction) or Jiaban Bawei Wan (Modified Eight-Ingredient Pill) should be taken to restore the source. Internal and external treatments for favorable and unfavorable cases follow the protocols for Carbuncles and Ulcers.
Prescriptions: Renshen Yangrong Tang; Jiaban Bawei Wan (both see Ku Yang Men).
[Prescription Verse] Shi Ju (Stone Scrofula) results from cold coagulation and blood stasis accumulation. It occurs most persistently between the waist and hips. It is hard as stone with unchanged skin color, causing intermittent numbness and pain, and is difficult to dissipate or suppurate.
[Annotation] This condition is formed by the coagulation of cold qi and blood stasis, occurring between the waist and hip region, with a lingering course that is difficult to resolve. The scrofula feels numb and painful, resists dissipation and suppuration, is hard as stone, and the skin color remains unchanged. Initially, Moyao Wan (Myrrh Pill) should be taken internally. Externally, fresh Shanglu (Phytolaccae Radix) should be mashed and applied to the affected area, followed by direct moxibustion with moxa cones on the top of the lesion until it softens. After suppuration, treat according to the protocols for ulcerated carbuncles and scrofula.
Prescription: Moyao Wan
Composition: Tao Ren (Peach Seed, stir-fried, 1 liang), Xiangru (Frankincense), Moyao (Myrrh), Chuanxiong (Ligustici Chuanxiong Rhizoma), Chuanjiao (Sichuan Pepper, remove pits and closed ends), Danggui (Angelicae Sinensis Radix), Chishao (Red Peony Root, each 5 qian), Ziran Tong (Pyritum, calcined in vinegar seven times, 2.5 qian).
Grind all ingredients into fine powder. Melt 2 liang of Huangla (Yellow Beeswax) over fire, add the powder, and stir continuously until uniform. Form into pills the size of bullet seeds. Take one pill per dose, dissolve it in a cup of good wine, boil until reduced to 0.5 sheng, and drink while hot.
[Prescription Verse] Moyao Wan treats Zhong Shi Ju (Middle Stone Scrofula); Frankincense, Myrrh, Peach Seed, Ligusticum, Angelica, and Red Peony are suitable. Combined with Sichuan Pepper, Pyritum, and Yellow Beeswax, taken with wine to promote blood circulation and resolve stasis.
