Two Discussions, Thirty-Eight Prescriptions
Discussion: Regarding the disease of Gan Shi (Gan Dampness), symptoms may include fever or chills resembling tiger bites, sudden onset of dysentery, intermittent dysentery, or chronic non-stop dysentery without fixed timing. Patients may experience periods of sleepiness alternating with appetite, yet gradually weakening in strength, becoming emaciated daily. Symptoms include spasms and tightness in the abdomen and back, weakness in the head and neck, lethargy, and poor appetite. The diagnostic test involves pressing along both sides of the spine with a finger; if mounds rise like millet grains under the finger, it is Gan Disease (Pediatric Malnutrition/Parasitic Disease). If no mound rises, it is not Gan Disease. If mounds rise, press gradually upward; if the reaction extends to both sides of the cervical vertebrae, it indicates that parasites have entered the brain, making the disease difficult to cure. Only two out of ten cases may recover. Extensive moxibustion is required. If parasites have not entered the brain, the physician first ties a rope around the neck, pulling towards the heart to compress the head, aligning the rope with the tip of the bone below the umbilicus. The rope is then turned to the back, aligned with the spinal column. Measure horizontally across the patient's mouth corners to fix the rope length. Mark two points on the spine ensuring they are level, then perform moxibustion. On the first morning, apply two moxa cones (Zhuang). By the end of the first seven days, up to the second seven-day period, apply fourteen moxa cones. In the third seven-day period, apply seven moxa cones morning and evening. In the fourth seven-day period, apply only three moxa cones. In the fifth seven-day period, apply two moxa cones daily. Observe the moxa sores for two to three days after initial application; if pus develops easily, recovery is likely. If pus develops after five to six days, recovery is difficult. Diet should be restricted to white rice, alfalfa, bitter lettuce, Vitex fruit, Xiangjiang (a type of herb/soup), and a small amount of roasted salt. Normal diet can resume one hundred days after recovery. Medication enemas should be administered three times, with a five-day interval between each.
Scallion White (one handful) Fermented Soybean (Er Chi) Shu Jiao (Sichuan Pepper, San He) Salt (Er He)
These four ingredients are boiled in one Dou of water until seven Sheng remain. Strain and administer warm as an enema, using one Sheng per administration.
Prescription for Gan Shi causing ulcers in the mouth, teeth, and lower body:
Fei Lian Hao (Artemisia scoparia, local name in Shu)
Burn this single ingredient to ash, grind and sieve. Apply two Qian Bi (measurements) of powder to the affected area. It causes severe pain; endure it. If there is no pain, it is not Gan Disease. Strictly avoid greasy food, honey, and fish. A case report mentions a patient with Gan disease experiencing sharp pain in the mouth when eating; applying this medicine cured them. Parasites resembling horse tails were expelled from the lower body in large numbers. Recovery occurred after ten days, full health after twenty days.

Discussion: Wet Worm diseases often occur after febrile diseases or chronic dysentery, or due to pathogenic heat accumulating in the abdomen, or exposure to summer-dampness and cool air. There are dry and wet types. Dry types involve less diarrhea but lower body ulcer itching. Regardless of type, prolonged illness is fatal. Diagnosis: Teeth lack color, tongue is entirely white. Severe cases have mouth ulcers, heavy limbs, and excessive sleepiness. This indicates worms eroding the anus; if the anus rots away exposing internal organs, death ensues.
