Zi He treated a patient who, due to excessive drinking of water while thirsty on the road, gradually developed edema and distension. Someone used Sanhua Shenwan (Three-Flower Divine Pill), but feared its potency was too harsh; someone else used Wuling San (Five-Ingredient Powder for Dividing and Promoting Urination) to separate and promote the water passages, but it was considered too slow. The condition lingered for many days with no effect. This is merely the skill of unskilled physicians. Later, the patient's hands, feet, and kidneys all became swollen, and both urination and defecation were obstructed and difficult. Chang Zhongming sought treatment from Dai Ren. Dai Ren instructed Chang Zhongming to apply external medicine, but the patient died before the treatment could be administered. Dai Ren said: "For severe cases of water disease, the pathogenic force is like a overflowing river; trying to remove it with cups and ladles is extremely difficult. One must use the method of Yu the Great to dredge and drain the waters to achieve a cure."
He also treated another patient suffering from Yongshui (Surging Water), presenting with a yellowish complexion, panting, swelling in both legs that rebounded quickly after pressure, a gurgling sound in the abdomen when walking, frequent thirst, and an inability to lie down or sleep. Dai Ren instructed him to first induce vomiting to expel phlegm and then induce sweating. Subsequently, he administered Zhouche Wan (Boat Cart Pill) and Junchuan San (Dredging River Powder) to purge downward, followed by Yishen San (Kidney-Benefiting Powder) again to purge, and finally used agents that differentiate Yin and Yang and promote water metabolism to purge once more. Once the water evil was exhausted, the disease resolved. (Note: Dai Ren's cases all involve initial excess conditions where the primordial Qi has not yet been depleted; such cases should be treated this way. Modern people claim Zi He's methods are flawed; how can this be reasonable?)
The *Jicheng* records the treatment of a patient who had a large appetite but suffered diarrhea after eating meat. Suddenly, his head and eyes swelled severely, preventing him from opening them. His chest and diaphragm felt full as if beaten, his legs were numb up to the knees, he was aversive to wind, and his genitals appeared erect and elongated. His pulse was deep on the left side, unresponsive to heavy pressure, and short and choppy on the right. The physician ordered him to drink a decoction of Baizhu (Atractylodis Rhizoma) alone on an empty stomach, followed by inducing vomiting. Afterward, a decoction of Baizhu (2 qian), Mahuang (Ephedrae Herba), Chuanxiong (Chuanxiong Rhizoma, 5 fen), and Fangfeng (Saposhnikoviae Radix, 3 fen) was given, along with 50 pills of Baohe Wan (Preserve Harmony Pill). Sweating occurred during the vomiting process, with the pathogenic influence mostly located in the upper jiao. Consequently, the swelling subsided, the eyes opened, breathing smoothed, and appetite returned. In the subsequent prescription, Mahuang and Fangfeng were removed, and Baizhu (3 qian), Mutong (Akebiae Caulis), and Tongcao (Tetrapanacis Medulla, 5 fen each) were added to the decoction, again accompanied by 50 pills of Baohe Wan. The patient stabilized within five days.
He also treated a woman who was prone to anger. After consuming roasted meat, she developed facial swelling, loss of appetite, and body fatigue. Her pulse was deep and choppy, with a wide and empty quality on the left. This indicated a constitutionally weak state with phlegm and qi stagnation, blocked by phlegm, preventing qi from descending. The primary treatment strategy should be tonifying deficiency and promoting the resolution of phlegm. Every morning, a large dose of Erchen Tang (Two-Aged Decoction) supplemented with Renshen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Rhizoma) was administered. She was then induced to vomit the medicine. After the Chen hour (7–9 AM), Sane Tang (Three-Harmony Decoction) with tripled Baizhu was given to induce sleep. Afterwards, Shenyou Wan (Divine Elixir Pill, 7 pills) was used to stir and resolve the phlegm. The patient recovered after one month.
