When pathogenic cold invades the body, fever may occur. This is because cold qi binds externally while yang qi resists internally. The patient presents with nasal congestion and coarse breathing, with exhaled breath feeling hot like fire. In such cases, diaphoresis (sweating to release the exterior) should be employed to dispel the cold. The six pulses are floating and rapid upon light palpation, appearing scattered and filling the fingers.
Ephedrae Herba (Mahuang, 麻黄) [half tael], Atractylodis Rhizoma (Baizhu, 白术) [half tael], Ligustici Rhizoma et Radix (Gaoben, 藁本), and Chuanxiong Rhizoma (Chuanxiong, 川芎) [each half tael].
Grind the above into a coarse powder. Take on an empty stomach at any time. Decoct three qian of the powder in one cup of water with one slice of fresh ginger. Strain and take the decoction warm.
If Heart Fire alone is hyperactive, or if there is excess in the Shaoyin or Shaoyang meridians, the body will feel hot. The patient presents with red scanty urine, constipation, headache, dry mouth, rough tongue coating, and bone pain. The six pulses are surging (Hong), large, and solid, with a bony force, and are also rapid. This formula is suitable.
Poria (Fuling, 茯苓), Ophiopogonis Radix (Maidong, 麦门冬) [heart removed], Paeoniae Radix (Shaoyao, 芍药), Ephedrae Herba (Mahuang, 麻黄), and Moutan Cortex (Danpi, 丹皮) [four zhu].
Prepare as a coarse powder using the method described above.
Liver Qi transforming into heat also causes fever. The presentation is similar to that of Heart Fire excess because wind and heat share the same nature, and Wood (Liver) and Fire assist each other. However, the difference in Liver heat is that although the body feels hot, the patient often perceives coldness internally, frequently experiencing chills along the spine and back, and coldness in both feet. There is excessive salivation, and in severe cases, vomiting and regurgitation. The six pulses are wiry (Xian), urgent, and long. Other symptoms resemble Heart Fire excess. However, while Heart Fire excess always presents with a surging pulse, Liver heat presents with a pulse that is surging yet wiry; sometimes it is only wiry without being surging, due to the predominance of Wind Qi. This formula is suitable.
Schizonepeta Spike (Jingjie Sui, 荆芥穗), Notopterygii Rhizoma et Radix (Qianghuo, 羌活), Angelicae Pubescentis Radix (Duhuo, 独活), Paeoniae Radix (Shaoyao, 芍药), and Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [equal parts].
Grind into a coarse powder. Take after meals. Decoct three qian in one cup of water. Reduce to eight-tenths volume. Strain and mix the dregs with the liquid for arbitrary consumption. Observe whether the patient’s constitution is robust and whether Liver Heat is significant enough to cause excessive salivation. If so, add Pinelliae Tuber (Tiannanxing, 天南星), Pinelliae Rhizoma Preparatum (Banxia, 半夏), Bombyx Batryticatus (Baijiangcan, 白僵蚕), and Scorpio (Quanxie, 全蝎) in small amounts. The dosage of Tiannanxing and Baijiangcan can equal that of the other herbs, while Banxia and Quanxie should be adjusted freely and not used in large quantities. If phlegm-saliva congeals and constipation occurs, combine with Semen Kochiae (Qianniuzi, 牵牛子) and the aforementioned herbs, aiming for unblocking and purging.
The Classics state: "Excess Yin generates cold; Excess Yang generates heat." If Yuan Qi (Original Qi) is deficient and Kidney Water is extremely cold, it manifests as rigors with spontaneous cold sweating. The six pulses are minute, fine, and deep. If Cold Pathogen attacks the Heart, the Kidney pulse will be striking and deep, accompanied by severe palpitations and restlessness in the epigastrium, and in severe cases, collapse. One must first warm the Kidneys and then protect the Heart. Use the Formula to Warm the Kidneys.
Achyranthis Bidentatae Radix (Niuxi, 牛膝) [soaked in wine overnight], Dendrobii Caulis (Shihu, 石斛), Morusae Radix Cortex (Bajitian, 巴戟天) [heart removed], Dioscoreae Hypoglaucae Rhizoma (Bixie, 萆薢) [boiled in salt water], and Aconiti Lateralis Radix Praeparata (Fuzi, 附子) [prepared].
Grind into powder. Mix with refined honey to make pills the size of mulberry seeds. Take seventy pills on an empty stomach with rice soup.
When Dampness is predominant, the patient sweats and fears wind. The body feels cold, there is aversion to wind, reduced appetite, fatigue, and weakness. The six pulses are turbid, slow, weak, large, and scattered. The face and eyes lack luster. It is appropriate to take the previously mentioned Pill to Warm the Kidneys and this formula.
Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Citri Reticulatae Pericarpium (Chenpi, 陈橘皮), Chuanxiong Rhizoma (Chuanxiong, 川芎) [each half tael], Asari Radix et Rhizoma (Xixin, 细辛), Licorice Root (Gancao, 甘草), Cardamomi Fructus (Caoudoukou, 草豆蔻), Ginseng Radix (Renshen, 人参) [half tael], and Zingiberis Rhizoma Recens (Ganjiang, 干姜) [four zhu].
Grind into powder. Mix with refined honey to make pills the size of mulberry seeds. Take fifty pills on an empty stomach with rice soup.
