[Case of Zhou, Female] Age: 1 year and 1 month. In the sixth lunar month of Gengshen year, she presented with fever in the body but cold ears, with faintly visible pustules, indicating a prevention of smallpox (Xiaozhen). The onset was due to contracting warm-summerheat pathogens during summer. Treatment should initially employ acrid-cool herbs to release the muscle layer, facilitating eruption. It is strictly forbidden to use acrid-warm exterior-releasing methods, which would cause deficiency at the exterior leading to itching and ulceration, while also assisting the warm-heat pathogen.
Forsythiae Fructus (Lianqiao, 3 qian) Licorice Root (Gancao, 1 qian) Platycodon Root (Kujiegeng, 3 qian) Reed Rhizome (Lugen, 3 qian) Fried Honeysuckle Flower (Chao Yinhua, 3 qian) Mint (Bohe, 8 fen) Schizonepeta Spike (Jiesui, 8 fen)
Second day: Pustules have erupted but not thoroughly; releasing the muscle layer is still appropriate. Continue with the previous prescription.
Third day: Dangerous smallpox for three days; eruption is complete, but the apices are depressed and color is dark. Apply blood-invigorating and apex-lifting method. If the color remains white and skin thin, considering the patient's inherent deficiency of both Taiyin (Spleen and Lung), using Notopterygium Root (Qianghuo) or Saposhnikovia Root (Fangfeng) will inevitably lead to collapsed and itchy pustules. Administering bitter-descending herbs will inevitably cause diarrhea.
Angelica Sinensis Root (Danggui, 2 qian) Bitter Platycodon Root (Kujiegeng, 1 qian) Scorched White Peony Root (Jiao Baishao, 1.5 qian) Honeysuckle Flower (Yinhua, 3 qian) Angelica Dahurica Root (Baizhi, 2 qian) Charred Scutellaria Root (Huangqin Tan, 1.5 qian) Akebia Stem (Mutong, 2 qian) Arnebia Root/Lithospermum Root (Zicao, 8 fen) Charred Hawthorn Fruit (Nanshan Zha Tan, 1 qian) Forsythia Fruit (Lianqiao, 2 qian) Siam Rhinoceros Horn (Xiluo Xijiao, 1 qian)
Fourth day: Qi deficiency leads to loose roots and depressed apices; Blood stasis leads to pale color and soft bases; Heavy toxin leads to clustering. Now administer detoxification, blood invigoration, and apex lifting. Support through the seventh day; if tonifying and pushing-out methods can be used, success may be achieved. Otherwise, collapse and itching by the ninth day are concerning, especially given the current diarrhea.
Angelica Sinensis Root (Danggui, 2 qian, earth-fried) Licorice Root (Gancao, 1.5 qian) Angelica Dahurica Root (Baizhi, 2 qian) Safflower (Honghua, 1 qian) Siam Rhinoceros Horn (Xiluo Xijiao, 3 qian) Soap Nut (Zaoren/Zaozhen, 1 qian) Antelope Horn (Lingyangjiao, 3 qian) Forsythia Fruit (Lianqiao, 3 qian) Bitter Platycodon Root (Kujiegeng, 2 qian) Arnebia/Lithospermum Root (Zicao, 1.5 qian) Fried Honeysuckle Flower (Chao Yinhua, 3 qian) Rooster Comb Blood (Gongji Guanxue, 3 small spoons added to half a large yellow wine cup)
Fifth day: Smallpox for five and a half days. Qi deficiency prevents detoxification from exiting outward; delaying time will inevitably lead to internal collapse and itching. Today, external pathogens are still active; bold tonification is not attempted, but mild pushing-out methods are necessary.
Angelica Dahurica Root (Baizhi, 2 qian) Forsythia Fruit (Lianqiao, 1.5 qian) Moutan Bark (Danpi, 2 qian) Soap Nut (Zaozhen, 1.5 qian) Bitter Platycodon Root (Kujiegeng, 2 qian) Angelica Sinensis Root Body (Bai Guishen, 3 qian) Licorice Root (Gancao, 0.5 qian) Arnebia/Lithospermum Root (Zicao, 1 qian) Swallow Nest Root (Yanwo Gen, 5 qian) Raw Astragalus Root (Sheng Huangqi, 3 qian) Chicken Comb Blood (Ji Guanxue, 3-5 spoons)
Decoct strongly to one cup of tea. After finishing, decoct the dregs strongly again to half a cup for consumption the next morning.
Sixth day: After six days of mild tonifying and pushing-out, swelling has risen and color is fresh, but the skin is thin and the shell is bright. Today requires strong tonification; tomorrow requires severe tonification.
Raw Astragalus Root (Sheng Huangqi, 5 qian) Bitter Platycodon Root (Kujiegeng, 3 qian) Chicken Comb Blood (Ji Guanxue, 3 drops per wine cup) Honey-fried Licorice Root (Zhi Gancao, 1.5 qian) Arnebia/Lithospermum Root (Zicao, 2 qian) Codonopsis Root (Dangshen, 3 qian) Angelica Sinensis Root Body (Bai Guishen, 3 qian) Charred Tangerine Peel (Guangpi Tan, 1 qian) Angelica Dahurica Root (Baizhi, 2 qian) Ligusticum Chuanxiong Rhizome (Chuanxiong, 1 qian) Swallow Nest Root (Yanwo Gen, 1 liang)
Decoct with rooster soup.
Seventh day: Using pushing-out and tonification for two days, color is fresh and moist, depressed areas have risen, but clear serum is only 20-30%, with many bright shells. Now at seven days, the zang-fu organs are complete, and Qi and Blood are active. It is the right time to apply formulas that tonify Qi to carry the toxin.
Raw Astragalus Root (Sheng Huangqi, 5 qian) Tangerine Peel (Guangpi, 1 qian) Honey-fried Licorice Root (Zhi Gancao, 2 qian) Angelica Dahurica Root (Baizhi, 1 qian) Bitter Platycodon Root (Kujiegeng, 3 qian) Ginseng (Renshen, 1 qian) Aucklandia Root (Guangmuxiang, 0.8 fen) Baked Myrobalan (Wei Rouguo, 1.5 qian) Ligusticum Chuanxiong Rhizome (Chuanxiong, 0.4 fen) Swallow Nest Root (Yanwo Gen, 1 liang)
Decoct with rooster soup.
Eighth day: Only one or two pustules have round, bursting apices. Head and face are filling with serum, chest and back have 3-4 parts clear serum, limbs are entirely empty and bright, and root discs are pale. This indicates dual deficiency of Qi and Blood; urgent severe tonification is needed using Ginseng, Angelica, and Deer Antler, combined with Chen’s Yi Gong San method.
Ginseng (Renshen, 1 qian) Angelica Sinensis Root Body (Guishen, 6 qian) Baked Myrobalan (Wei Rouguo, 2 qian) Yellow Hair Deer Antler (Huangmaorong, 5 qian, decocted separately with water and yellow wine) Bitter Platycodon Root (Kujiegeng, 3 qian) Angelica Dahurica Root (Baizhi, 3 qian) Aucklandia Root (Guangmuxiang, 1 qian) Honey-fried Licorice Root (Zhi Gancao, 3 qian) Swallow Nest (Yanwo, 1 liang) Raw Astragalus Root (Sheng Huangqi, 1 liang) Poria Block (Fuling Kuai, 3 qian) Charred Tangerine Peel (Guangpi Tan, 3 qian) Rooster Soup (Zhong Wan)
This medicine is boiled into four cups of tea, add half a cup of antler juice, one bowl of rooster soup, and one bowl of swallow nest soup. Mix well and simmer on fire until thick. The child takes half, the wet nurse takes half.
Ninth day: Nine days old. Yesterday used severe tonification; although arms have yellow serum, limbs remain empty and bright due to diarrhea. Use Wen Zhong’s Da Yi Gong San.
Fresh Ginger (Sheng Jiang, 1 liang) Ginseng (Renshen, 1 qian) Aucklandia Root (Guangmuxiang, 2 qian) Angelica Sinensis Root (Danggui, 5 qian, earth-fried) Baked Myrobalan (Wei Rouguo, 3 qian) Charred Tangerine Peel (Guangpi Tan, 2 qian) Baked Chebula Fruit (Wei Hezi, 3 qian) Atractylodes Macrocephala Rhizome (Baizhu, 5 qian, fried) Cinnamon Bark (Rougui, 1 qian, ground fine, coarse bark removed) Poria Block (Fuling Kuai, 6 qian) Honey-fried Licorice Root (Zhi Gancao, 3 qian) Deer Antler Tip (Lurong Jian, 6 qian, decocted in wine)
Tenth day: From the previous formula, remove Cinnamon Bark, Deer Antler Tip, and Angelica Root Body; reduce Astragalus by 4 qian; add: Alisma Rhizome (Zexie, 5 qian)
Eleventh day: Follow the previous formula.
Twelfth day: Add Coix Seed (Yiyiren, 5 qian) to the previous formula.
Thirteenth day: Serum is not fully satisfied; there are breaks between limbs. Cannot guarantee absence of smallpox toxin cough etc. Now use diuresis to assist scabbing and drive out residual toxins, achieving two goals with one move.
American Ginseng (Yangshen, 3 qian) Alisma Rhizome (Zexie, 3 qian) Charred Tangerine Peel (Guangpi Tan, 1 qian) Poria Block (Fuling Kuai, 5 qian) Raw Coix Seed (Sheng Yiren, 8 qian) Honey-fried Licorice Root (Zhi Gancao, 1.5 qian) Scorched Atractylodes Macrocephala Rhizome (Jiao Baizhu, 3 qian) Aucklandia Root (Guangmuxiang, 3 qian) Baked Myrobalan (Wei Rouguo, 2 qian) Baked Chebula Fruit (Wei Hezi, 2 qian)
Fourteenth day: Foot swelling, chest fullness, short urination indicates fluid metabolism dysfunction.
Poria Block (Fuling Kuai, 5 qian) Fried Honeysuckle Flower (Chao Yinhua, 2 qian) Winter Atractylodes Rhizome (Dong Baizhu, 3 qian) Alisma Rhizome (Zexie, 2 qian) Raw Coix Seed (Sheng Yiren, 5 qian) Charred Tangerine Peel (Guangpi Tan, 1.5 qian) Calcined Talcum (Fei Huashi, 2 qian) Forsythia Fruit (Lianqiao, 2 qian) Metaphidion Worm (Wugu Chong, 1.5 qian)
[Case of He, Male] Age: 4 years. Third day of ninth lunar month: Three days of Qi deficiency and heavy toxin. Pustules adhere in patches, accompanied by dull color and depressed apices. Attacking toxin hinders deficiency; warming and pushing out hinders toxin. Both are difficult to handle. Harmonizing middle and stabilizing exterior is ineffective. Tentatively propose blood invigoration and toxin dispersal.
Bitter Platycodon Root (Kujiegeng, 3 qian) Black Rhinoceros Horn (Wuxijiao, 5 qian) Forsythia Fruit (Lianqiao, 3 qian) Raw Licorice Root (Sheng Gancao, 1 qian) Angelica Dahurica Root (Baizhi, 1 qian) Mint (Bohe, 1 qian) Honey-fried Isaria Cicadae (Zhi Tianchong, 2 qian) Arnebia/Lithospermum Root (Zicao, 3 qian) Whole Angelica Sinensis (Quanguishen, 3 qian) Southern Hawthorn (Nanzha, 3 qian) Moutan Bark (Danpi, 3 qian) Antelope Horn (Lingyangjiao, 3 qian)
Mix three small spoons of Pig Tail Paste into each wine cup.
Fifth day: Four days old. Yesterday used blood invigoration and detoxification; significant improvement, but throat sound is slightly hoarse, facial and ocular edema is severe, lip color is crimson. The fire-toxin of epidemic pestilence is too heavy; today still suitable for detoxification.
Bitter Platycodon Root (Kujiegeng, 6 qian) Siam Rhinoceros Horn (Xiluo Xijiao, 6 qian) Forsythia Fruit (Lianqiao, 3 qian) Human Excrement Cake (Renzhonghuang, 3 qian) Peach Kernel (Taoren, 1.5 qian) Yong'an Coptis Rhizome (Guyong Huanglian, 1 qian) Isaria Cicadae (Tianchong, 3 qian) Angelica Dahurica Root (Baizhi, 1 qian) Whole Angelica Sinensis (Quanguishen, 2 qian) Fried Hawthorn Meat (Chao Zharou, 2 qian) Arnebia/Lithospermum Root (Zicao, 3 qian) Centipedia Herb (Gujingcao, 3 qian) Moutan Bark (Danpi, 3 qian) Antelope Horn (Lingyangjiao, 3 qian) Honeysuckle Flower (Yinhua, 5 qian) Violet Herb (Zihuadiding, 5 qian)
Use the previous decoction of Honeysuckle and Violet as water for subsequent decoctions.
Sixth day: Five and a half days; gradually improving. However, dangerous conditions vary; caution is essential. Today still suitable for blood invigoration and apex lifting, slightly adding interior-supporting methods.
