Shao Zhuyu, a censorial official, presented the case of his grandson, Shao Lv. He had long suffered from excessive phlegm and fullness/stuffiness in the chest and diaphragm, with epilepsy attacks occurring annually for years; medicinal treatments had been ineffective. Mengying palpated the pulse and said: "There is a deficiency in the Qi level (the functional level of Qi circulation). Phlegm-fluids are obstructing the rotation and transportation of Clear Yang (light, ascending Yang Qi). The prescription should follow the principle of 'Heaven's Strength' (referring to the Qian hexagram in the I Ching, symbolizing vigorous movement), so that the Grand Qi (Zong Qi) will naturally strengthen and flow without cessation, making the chest feel as open and vast as the sky." He prescribed Liu Jun Zi Tang (Six Gentlemen Decoction) minus Gancao (Licorice Root), plus Huangqi (Astragalus Root), Guizhi (Cinnamon Twig), Xiebai (Allium Macrostemon Bulb), Gualou Ren (Trichosanthes Seed), Shichangpu (Acorus Rhizome), Jili (Tribulus Fruit), and Xuanfu (Inula Flower). After taking it, the stuffiness gradually relieved, and the epilepsy did not recur.
Zheng Zhitang’s mother-in-law, over sixty years old, developed right-sided hemiplegia, speech impediment (tongue stiffness and inability to speak), flushed face, and constipation in the second month of spring. Physicians treated her with wind-dispelling herbs without effect. On the fourth day, she was consulted by Mengying. The right pulse was surging and slippery, and the left pulse was wiry and rapid, indicating a pattern of Yangming Fu-organ Excess Syndrome (excess heat combined with dry stool in the intestines). He prescribed: Shichangpu (Acorus Rhizome), Danningxing (Arisaema with Bile), Zhimu (Anemarrhena Rhizome), Huafen (Trichosanthes Root), Zhishi (Immature Bitter Orange), Gualou Ren (Trichosanthes Seed), Qinjiao (Gentiana Macrophylla Root), Xuanfu (Inula Flower), Maren (Hemp Seed), and Zhuli (Bamboo Sap). Some worried that purgation might lead to collapse due to diarrhea and dared not use it, not knowing that the character "Zang" (Viscera) in ancient texts referring to the suitability of purgation for stroke affecting the Zang-Fu actually refers to "Fu" (Hollow Organs). Ke Qin said, "Reading without insight costs patients their lives," which applies here. The condition worsened over twenty days. Zheng Zhitang asked Tong Qiumen to urgently request Mengying’s re-evaluation. The tongue coating was cracked, the tongue body crimson, unable to even swallow rice water, with abdominal distension and coarse breathing, indicating imminent exhaustion of Yin fluids. Urgent purgation was necessary. He added Dashuang (Rhubarb Root) four qian to the previous formula, administered as squeezed juice. Five episodes of black, hard stools were passed consecutively. The speech impediment immediately decreased, and she gradually began drinking thin gruel. Dashuang was then removed, and Xiyangshen (American Ginseng), Shengdi (Rehmannia Root), Maidong (Ophiopogon Tuber), Danpi (Moutan Bark), and Bohe (Mint) were added. After five doses, bowel movements resumed, and speech became clear. Sweet-cool herbs were used exclusively to replenish fluids and wash away heat. Ten days later, the tongue color lightened, and appetite returned to normal. The treatment shifted to nourishing Yin, achieving gradual recovery. Over three years later, she reportedly died of another illness.
Zhao Qiu, a Jinshi degree holder, suffered from left-sided hemiplegia last autumn. His brother Zhao Dilou and high disciple Xu Zhiqing, a Xiucai degree holder, advocated clearing heat and eliminating phlegm, but treatment was not rapidly effective. Mengying was invited to diagnose. The pulse was very slow and sluggish, the coating extremely yellow and greasy, with constipation and excessive talking. A bowl of Zhuli (Bamboo Sap) was mixed into the herbal decoction. Additionally, Longhui Wan (Longdan Xiegan Wan or similar bile-clearing pill) and Guntan Wan (Rolling Phlegm Pill) were administered alternately. Approximately one jin of each pill was consumed before the condition began to improve. In spring, he went out of his room, sleep and diet having recovered. However, he habitually favored rich flavors and did not abstain from fatty and sweet foods. In early summer, the disease suddenly relapsed. Mengying diagnosed the pulse as slippery and swift like a snake, prognosticating death. Indeed, he died in early autumn.
Lai Bingchi’s mother, nearing seventy, suffered from left-sided hemiplegia. Physicians prescribed Zaizao Wan (Revitalization Pill) and tonics. After twenty days, the condition remained unchanged. Mengying palpated the pulse: wiry, slow, and slippery. The malar region was red, the coating yellow, voice faint, tongue stiff, with difficult urination/defecation and no expectoration. He stated: "This is 'Phlegm Stroke' (Tan Zhong), where phlegm is latent and untransformed." He prescribed: Xijiao (Rhino Horn - *Note: modern substitutes like Buffalo Horn are used*), Lingyangjiao (Antelope Horn), Zhuru (Bamboo Sap/Dried Juice), Beimu (Fritillaria Bulb), Shichangpu (Acorus Rhizome), Fa Banxia (Prepared Pinellia Tuber), Huafen (Trichosanthes Root), Zhimu (Anemarrhena Rhizome), Baiwei (Cynanchum Root), Doujuan (Soybean Sprouts), Sangzhi (Mulberry Twig), and Sigualuo (Luffa Reticulate Fiber). After three doses, the coating transformed, and the voice became clearer. After six to seven doses, sensation returned to the legs, phlegm was expectorated, and bowels opened. Subsequently, leg pain became unbearable, feeling hot as if branded. Mengying ordered application of scallion and honey to draw out the heat, and the pain gradually ceased. Half a month later, sleep and diet stabilized. Beyond twenty days, hand grip returned. Over a month later, she could walk with support.
Xu Mengxiang, nearing sixty, suffered from hand tremors preventing him from holding a brush. Mengying treated him with "tonifying and unblocking" and "extinguishing wind" herbs, along with swallowing Zhimi Fuling Wan (Zhimi Poria Pill), resulting in stabilization.
In mid-autumn, a patient presented with symptoms resembling stroke: urinary incontinence, rising phlegm, drowsiness/confusion, delirious speech, profuse sweating, and flushed face. Mengying was urgently consulted. The pulse was floating, wiry, surging, and slippery. This was attributed to inhaling heat-pathogens, compounded by recent continuous consumption of Ginseng Decoction. He prescribed: Lingyangjiao (Antelope Horn), Shichangpu (Acorus Rhizome), Lianqiao (Forsythia Fruit), Zhizi (Gardenia Fruit), Sangye (Mulberry Leaf), Juhua (Chrysanthemum Flower), Lianshi (Melia Fruit), Shihu (Dendrobium Stem), Zhimu (Anemarrhena Rhizome), Huafen (Trichosanthes Root), Zhuli (Bamboo Sap), Yinhua (Honeysuckle Flower), Qinghao (Artemisia Annua Herb), and Baiwei (Cynanchum Root). Symptoms improved after one dose, and consciousness cleared after two. Lingyangjiao and Shichangpu were removed, and Zhuru (Bamboo Sap/Dried Juice), Beimu (Fritillaria Bulb), and Huashi (Talcum) were added. The patient passed white, pus-like stools for several days, then felt hungry and ate, gradually recovering through regulation. Within a month, he could paint; in late autumn, he was still traveling and studying in Jiangyou.
Han Zulin, though seventy years old, ate and drank heavily and liked taking medicine. Physicians, considering his age, frequently used Guizhi (Cinnamon Twig), Fuzi (Aconite Root), Renshen (Ginseng Root), and Rongsu (Deer Antler Velvet) to prolong his life. Unexpectedly, on the evening of the 8th, he ate heartily at dinner. Upon waking at midnight, he suddenly shivered and trembled, followed by convulsions of the four limbs, and died the next day. Was this not what people call the "Ziwu Syndrome"? Mengying said: "This elder has a constitution of vigorous Yang. Excessive intake of fatty and sweet foods increases phlegm-fire daily. At seventy, True Yin (Kidney Yin) is daily depleted. Long-term use of such fire-assisting and Yin-burning drugs causes Wind to emerge from Fire, uprooting the foundation instantly. This is almost identical to the 'Acute Convulsive Wind' (Ji Jing Feng) syndrome described in pediatrics."
