Commentary states: In cases where the heat-toxin of Cold Damage (Shanghan, a syndrome caused by exogenous cold pathogens) is excessively abundant internally, leading to the frequent occurrence of sores, it is due to the patient having interior excess and exterior deficiency. Interior excess prevents the pathogenic factors from being resolved internally; exterior deficiency allows the pathogens to invade the muscles through the void, forming sores. These sores are of varying sizes and shaped like peas. Their color may be white or red. If the head of the sore forms a pustule with white pus on top, the toxicity is mild. If the sore is purple-black with deep roots, faintly hidden within the muscles, the toxicity is severe. In extreme cases, they cover the entire body, affecting the five Zang organs (Heart, Liver, Spleen, Lung, Kidney) and seven orifices.
