Discussion states: When a cold damage disease originates from a yang syndrome, premature purgation causes the pathogenic toxic qi to accumulate in the chest and diaphragm, hence it is named "Jie Xiong" (Chest Binding). Its manifestations include hardness below the heart. Pressing upon it causes pain, stiffness of the neck resembling Rou Ji (a condition characterized by rigidity of the neck and back), or pain extending from below the heart down to the lower abdomen, which is hard, full, and painful, so severe that the patient cannot bear pressure on the area. The pulse at the cun position is floating, while the pulse at the guan position is sunken; this is its characteristic sign. If the pathology is precisely at the epigastrium, pressing induces pain, and the pulse is floating and slippery, it is also called Jie Xiong. Generally, these conditions originate from Tai Yang disease (diseases of the Tai Yang channel/meridian). The pulse is floating, moving, and rapid. If the physician erroneously applies purgation, the stomach becomes empty and deficient. External pathogenic factors disturb the diaphragm, causing shortness of breath, irritability, and mental restlessness (Ao Nan). Internal sinking of yang qi leads to hardness and fullness below the heart, forming Jie Xiong.
Furthermore, if two or three days after contracting the disease, the patient cannot lie flat but desires to sit up, there must be binding in the epigastrium. If the pulse is weak and faint, this indicates pre-existing accumulated cold in the body; applying purgation erroneously will result in Jie Xiong once diarrhea stops. If purgation is applied and the pulse remains floating, Jie Xiong will definitely form. In such cases, purgation should be used. However, if the pulse is rapid/crisp (Cu), it is not Jie Xiong. If the pulse is floating, purgation is contraindicated. Symptoms resembling Jie Xiong may occur, with normal diet but frequent diarrhea; if the yang pulse is floating and the guan pulse is sunken, fine, and tight, it is classified as Zang Jie (Organ Binding). If the tongue has a white, slippery coating, it is difficult to treat. If the epigastrium is痞 (Pi - stuffiness/blockage) and hard but painless upon pressure, it is not Jie Xiong but Pi Syndrome. Careful examination is required to apply the appropriate treatment methods.
