Commentary states: Eye diseases originate superficially; if neglected over a long period, they may transform into internal cataracts (Nei Zhang), and this is not a single condition. The *Longmu Lun* (Dragon Tree Treatise) records that the development of internal cataracts follows a gradual process, typically starting in one eye and subsequently affecting both. This is due to deficiency of Qi in the Liver and Kidney, with Wind pathogen and Heat toxin attacking upward to the eyes, leading to decline of Qi and Blood. Over time, this causes harm. The symptoms usually involve no pain or itching, nor discharge or tearing. Vision appears like light smoke, thin fog, hanging threads, fly wings, or floating mosquitoes. Initially, it may seem negligible, but once the cataract forms, vision becomes severely obscured. The following discusses conditions on the verge of becoming internal cataracts, embodying the principle of "attending to difficulties while they are easy" (preventing complications before they arise).
For treating high que mu (night blindness) caused by Wind, which gradually transforms into internal cataract, Huanjing Wan (Restore Vision Pill) is suitable:
Ren Shen (Ginseng Radix et Rhizoma), Xi Xin (Asari Radix et Rhizoma, remove seedlings and leaves), Shi Jue Ming (Haliotidis Concha, washed), Bai Fu Ling (Poria, remove black skin), Zhi Mu (Anemarrhenae Rhizoma, sliced and baked), Xiong (Chuanxiong Rhizome), Mu Xiang (Aucklandiae Radix) [each 1 liang].
The above eight herbs are crushed, sifted into powder, mixed with refined honey, and formed into pills the size of wutong seeds. Take 20 pills per dose, administered orally with clear tea on an empty stomach.
For treating que mu caused by Liver deficiency, fearing transformation into internal cataract, first take Zhuogan Tang (Strengthen Liver Decoction), then follow with Xiefei Yin (Drain Lung Infusion).
Zhuogan Tang Formula:
Da Huang (Rhei Radix et Rhizoma, cut and stir-fried), Che Qian Zi (Plantaginis Semen), Xi Xin (Asari Radix et Rhizoma, remove seedlings and leaves, each 1 liang), Huang Qin (Scutellariae Radix, remove black heart), Chong Wei Zi (Leonuri Fructus), Xuan Shen (Scrophulariae Radix, each 2 liang).
The above six herbs are coarsely crushed and sifted. Take 3 qian bi per dose, add 1 zhan of water and 37 black soybeans, decoct until 6 fen remains, strain, let cool to warm, and take after meals and before bed.
