How to determine that a patient contracted an illness from sleeping outdoors? This is due to Yin pathogenic factors intermingled within Yang. How to determine that a patient contracted an illness in summer? This is because the various Yang Qi have entered into Yin.
How to determine that a patient contracted an illness in spring? Because the Liver pulse (Gan Mai) is absent. Absence of the Heart pulse (Xin Mai) indicates illness contracted in summer; absence of the Lung pulse (Fei Mai) indicates illness contracted in autumn; absence of the Kidney pulse (Shen Mai) indicates illness contracted in winter; absence of the Spleen pulse (Pi Mai) indicates illness contracted in the months of the four seasons (the last month of each season).
If a patient's five Zang organs (Wu Zang) are exhausted, Shen (Spirit) cannot be retained, and the voice is hoarse, death will ensue. If a patient picks at the seams of their clothes and speaks deliriously (Chan Yu), it is incurable. If a patient's Yin and Yang Qi are both exhausted, pulling at clothes, grasping at empty air, and speaking nonsense, death will ensue. If a patient speaks nonsense, is confused, or unable to speak, it is generally untreatable; however, if these symptoms appear in a Heat disease (Re Bing), it may be treatable. If a patient's Yin and Yang Qi are both exhausted, resulting in loss of voice and inability to speak, death will occur in three and a half days.
If yellow Qi appears around the eyes of a patient, the illness is about to heal. If a patient has a yellow complexion and green eyes, they will not die; but if the green color resembles withered grass (Cao Zi), they will die. If a patient has a yellow complexion and red eyes, they will not die; but if the red color resembles stagnant blood, they will die. If a patient has a yellow complexion and white eyes, they will not die; but if the white color resembles dry bone, they will die. If a patient has a yellow complexion and black eyes, they will not die; but if the black color resembles soot, they will die. If a patient's facial features and eye colors are harmonious and balanced, they will not die. If a patient has a black complexion and green eyes, they will not die. If a patient has a green complexion and white eyes, they will die. If a patient has a red complexion and green eyes, they will die in six days. If a patient has a yellow complexion and green eyes, they will die in nine days; this is called "Disordered Meridians" (Luan Jing). Drinking alcohol while exposed to wind allows pathogenic factors to enter the Stomach meridian, causing Gallbladder Qi to leak recklessly, turning the eyes green; even with heavenly rescue, recovery is impossible. If a patient has a red complexion and white eyes, they will die in ten days. Due to worry, anger, and overthinking, Heart Qi becomes internally depleted; if the complexion unexpectedly looks good, prepare coffins immediately, as death will occur in ten days. If a patient has a white complexion and black eyes, they will die, indicating that prosperity has departed and blood vessels are empty and depleted. If a patient has a black complexion and white eyes, they will die in eight days, indicating internal injury to Kidney Qi and accumulation of pathogenic factors. If a patient has a green complexion and yellow eyes, they will die in five days.
If a patient is bedridden, suffers from heart pain and shortness of breath, this indicates Spleen exhaustion and internal injury; after one hundred days, they may recover and heal. If they can rise, pace around, and sit on the floor, or stand leaning against the bed, those who can cure such conditions can be considered divine physicians. If a patient lacks luster in their face, resembling earth color, and cannot accept food or drink, they will die in four days. If a patient's eyes lack luster and teeth turn black, it is incurable. If black Qi appears in the ears, eyes, nose, and mouth of a patient and enters the mouth, death is certain. If the eyes, ears, and cheekbones of a patient are red, death will occur within five days. If black Qi emerges from the hairline on the forehead, extending straight down to the nasal bridge and above the cheekbones, death will also occur within five days. Whether for patients or healthy individuals, if black or white Qi appears on the face and enters the eyes, nose, and mouth, death will occur within three days. If a patient or healthy person suddenly has a face color like horse liver—looking green from afar and black up close—death will ensue. If a patient has a black complexion, fixed staring eyes, and aversion to wind, death will ensue. If a patient has a black complexion and blue lips, death will ensue. If a patient has a green complexion and black lips, death will ensue. If a patient has a black complexion, fullness under both hypochondria, and inability to turn themselves over, death will ensue. If a patient's eyes roll and stare fixedly, and they breathe with shoulder movement (Jian Xi), they will die in one day. If a patient has Yin binding and Yang exhaustion, with protruding eyes and confusion, death will ensue. If a patient's Yin and Yang are exhausted, with sunken eye sockets, death will ensue. If the connection point of the eyebrows of a patient tilts downward, they will die in seven days. If a patient's mouth opens like a fish mouth and cannot close, with Qi exiting but not returning, death will ensue. If a patient's mouth is open, they will die in three days. If a patient's lips are blue and the philtrum is inverted, they will die in three days. If a patient's lips are everted and the philtrum is full, death will ensue. If a patient's lips and mouth suddenly become dry, it is incurable. If a patient's lips are swollen and teeth are scorched, death will ensue. If a patient's teeth suddenly turn black, they will die in thirteen days. If a patient's tongue curls and testicles retract, death is certain. If a patient sweats without flow and the tongue curls and turns black, death will ensue. If a patient's hair stands straight, they will die in fifteen days. If a patient's hair is dry like hemp and prone to anger, death will ensue. If a patient's hair and eyebrows stand upright and outward, death will ensue. If a patient's nails are blue, death will ensue. If a patient's nails are white, it is incurable. If the flesh under the nails of a patient's hands and feet turns black, they will die in eight days. If a patient's Ying and Wei Qi are exhausted and the face is edematous, death will ensue. If a patient has sudden swelling and a dark-green/black face, death will ensue. If a patient's palms are swollen and lack lines, death will ensue. If a patient's umbilicus is swollen and protrudes, death will ensue. If a patient's scrotum and penis are both swollen, death will ensue. If a patient's pulse is cut off, mouth is open, and feet are swollen, they will die in five days. If a patient's dorsum pedis is swollen, accompanied by vomiting and heavy head, death will ensue. If the upper part of a patient's dorsum pedis is swollen and both knees are as large as a dou (a unit of volume), they will die in ten days. If a patient wets the bed with feces unconsciously, death will ensue. If a patient emits a corpse-like odor, it is incurable.
In Liver disease, if the skin is white, death occurs on Geng-Xin days (Metal days). In Heart disease, if the eyes are black, death occurs on Ren-Gui days (Water days). In Spleen disease, if the lips are blue, death occurs on Jia-Yi days (Wood days). In Lung disease, if the cheeks are red and eyes are swollen, death occurs on Bing-Ding days (Fire days). In Kidney disease, if the face is swollen and lips are yellow, death occurs on Wu-Ji days (Earth days).
The standard for observing complexion colors: Green should resemble the luster of cyan jade (Cang Bi), not the dullness of indigo plant. Red should resemble cinnabar wrapped in silk, not the dullness of hematite. White should resemble goose feathers, not the stark whiteness of salt. Black should resemble double-lacquered wood, not the dry blackness of charcoal. Yellow should resemble realgar wrapped in gauze, not the dullness of yellow earth.
