The condition of Ying (Goiter) and Liu (Tumor) in human beings is not a swelling formed by the knotting of Yin and Yang healthy Qi, but rather results from stasis of blood, turbid Qi, and phlegm stagnation within the Five Zang Organs. Ying belongs to Yang; its color is red and it protrudes high, sometimes with a small pedicle hanging down. Liu belongs to Yin; its color is white and it presents as diffuse swelling, without itching or pain, often unnoticed by people. Xue Lizhai distinguished these very clearly. The Liver governs the sinews; anger stirs up Liver Fire, causing dryness of blood and contraction of sinews, termed Jin Liu (Sinew Tumor). The Heart governs the blood vessels; extreme sudden urgency causes Fire to flourish, forcing blood to boil, and when combined with external pathogens striking, it swells, termed Xue Liu (Blood Tumor). The Spleen governs the muscles; depression and knotting injure the Spleen, causing muscles to thin and Spleen Qi to fail to circulate, reversing into the flesh to cause swelling, termed Rou Liu (Flesh Tumor). The Lung governs Qi; labor and injury deplete Yuan Qi, making the Cou Li (pores/skin texture) insecure, allowing external cold to strike and cause swelling, termed Qi Liu (Qi Tumor). The Kidney governs the bones; indulgence injures the Kidney, suppressing Kidney Fire, leaving bones without nourishment to cause swelling, termed Gu Liu (Bone Tumor). I say: Jin Liu is characterized by hardness and purple color, with raised blue veins; if coiled severely, the knots resemble earthworms. Treatment should clear the Liver and resolve depression, nourish blood and relax sinews, using Qinggan Lulu Wan (Liver-Clearing Aloe Pill). Xue Liu is characterized by slight purple-red color, mixed softness and hardness, with faint red threads visible under the skin; if scratched, bleeding cannot be stopped. Treatment should nourish and cool blood, suppress fire and nourish Yin, calm and secure the Spirit, and harmonize blood vessels, using Qinlian Ermu Wan (Scutellaria and Anemarrhena Two-Mother Pill). Rou Liu is characterized by softness like cotton and hardness like steamed bun, unchanged skin color, neither tight nor loose, resembling piled liver throughout the year. Treatment should regulate the Spleen and widen the middle, unblock Stomach Earth, open depression and move phlegm, and regulate diet, using Jiawei Guipi Wan (Modified Restore the Spleen Pill). Qi Liu is characterized by softness without firmness, normal skin color, fluctuating in size without heat or chills. Treatment should clear Lung Qi, regulate meridians, address labor injuries, and harmonize Ying and Wei, using Tongqi Sanjian Wan (Qi-Ventilating Hardness-Resolving Pill). Gu Liu is characterized by purple-black color and form, hardness like stone, nodular elevation, immovable upon pushing, firmly attached to the bone. Treatment should tonify Kidney Qi, nourish blood and move stasis, dissipate swelling and break firmness, benefit orifices and adjust Yuan, using Shenqi Wan (Kidney Qi Pill). These five names of tumors and five methods of treating tumors lie herein. Furthermore, observing Lizhai's words: sinews and bones exposed is Jin Ying (Sinew Goiter); red vessels interwoven is Xue Ying (Blood Goiter); unchanged skin color is Rou Ying (Flesh Goiter); waxing and waning with sorrow and joy is Qi Ying (Qi Goiter); hard and immovable is Shi Ying (Stone Goiter). These are the five names of Ying. For early-stage Ying and Liu, if Yuan Qi is solid, select Haizao Yuhu Tang (Seaweed Jade Pot Decoction) or Liu Jun Wan (Six-Army Pill); if prolonged and Yuan Qi is deficient, select Hupo Heilong Dan (Amber Black Dragon Pellet) or Shiquan Liuqi Yin (Perfect Ten Qi-Flowing Decoction), selecting and taking these medicines will naturally reduce and dissolve them. One must never lightly use needles or knives; piercing leads to uncontrollable bleeding, often causing immediate danger; prolonged leakage of pus and blood ultimately harms life. Another type is Fen Liu (Powder Tumor), pinkish-red, mostly occurring behind ears and neck, also sometimes on the lower body, entirely formed by knotting of phlegm and Qi. It is suitable to pierce with a lancet to remove powder, insert Sanpin Yitiao Qiang (Three Grades Single-Spear), and after several times, the clean inner membrane heals itself. Another type is Heisha Liu (Black Sand Tumor), mostly on buttocks and thighs, varying sizes of protrusion, lifting with hand reveals black matter inside. Also use needle puncture, black sand comes out with sound, varying hardness. Another type is Fa Liu (Hair Tumor), mostly one cun below hair behind ears, small and soft protrusion, painless on pressure, also punctured with needle, hair and powder come out together. Another type is Huichong Liu (Ascaris Tumor), occurring under the hypochondrium; another type is Ju Liu (Carbuncle Tumor), connected to shoulders and upper arms, detailed in later treatment cases. I observe that ancient texts also mention Lice Tumors, differing in shape, all formed by damp-heat, pathogenic fire, turbid Qi, and blood stasis of the Five Zang Organs; this is not a proper disease. The above several tumors are all verified by my own hands, not falsehoods.
