Pediatric patients presenting with swelling and nodules measuring one to two cun in size are diagnosed with "jie" (furuncle). This condition resembles a carbuncle, characterized by heat and pain. If prolonged, it will suppurate and ulcerate; recovery occurs once the pus and blood are completely discharged. This is generally caused by wind-heat pathogenic factors.
For pediatric furuncles arising from initial onset due to heat accumulation in the five zang organs, Dahuang San (Rhubarb Powder) is indicated.
Ingredients: Chuan Dahuang (Radix et Rhizoma Rhei, 0.5 liang, crushed and lightly stir-fried), Zhizi Ren (Semen Gardeniae, 1 fen), Huangqin (Radix Scutellariae, 0.5 liang), Chuan Shengma (Rhizoma Cimicifugae, 0.5 liang), Gancao (Radix Glycyrrhizae, 1 fen, used raw).
Preparation and Administration: The above ingredients are coarsely ground and sieved to make a powder. Take 1 qian per dose, add one small cup of water, decoct until reduced to 0.5 cup, filter out the dregs, allow to cool to warm temperature, and administer in adjusted doses according to the child's age and size.
For pediatric cases where heat-toxin is congested and attacks the skin causing furuncles, manifesting as redness, swelling, and pain, accompanied by occasional irritability, fever, and sleeplessness, Xijiao Wan (Rhinoceros Horn Pill) is indicated.
Ingredients: Xijiao Xie (Horn Rhinocerotis, 3 fen), Chuan Shengma (Rhizoma Cimicifugae, 0.5 liang), Huangqin (Radix Scutellariae, 0.5 liang), Xuanshen (Radix Scrophulariae, 0.5 liang), Huangqi (Radix Astragali, 0.5 liang, crushed), Renshen (Radix Ginseng, 0.5 liang, radix removed).
Preparation and Administration: The above ingredients are finely ground and sieved into powder. Mix with refined honey and pound three hundred times to form pills the size of hemp seeds. Administer seven pills per dose with a decoction of raw Gancao (Radix Glycyrrhizae), adjusting the dosage based on the child's age and size.
For pediatric furuncles caused by deficiency-heat, Dihuang Wan (Rehmannia Pill) is indicated.
Ingredients: Shenggan Dihuang (Radix Rehmanniae Preparata, 1 liang), Guixin (Cortex Cinnamomi, 0.5 liang), Chuan Dahuang (Radix et Rhizoma Rhei, 1 liang, crushed and lightly stir-fried), Chi Shaoyao (Radix Paeoniae Rubra, 0.5 liang), Chi Fuling (Poria cum Ligno Rubrum, 0.5 liang).
Preparation and Administration: The above ingredients are finely ground and sieved into powder. Mix with refined honey to form pills the size of mung beans. Administer seven pills per dose with boiled and cooled water, adjusting the dosage based on the child's age and size.
For early-stage pediatric furuncles where heat qi begins to congeal, causing pain and distress, applying this formula can dissipate the swelling. Xiaoshi San (Niter Powder) is indicated.
Ingredients: Xiaoshi (Natrii Sulfas, 0.5 liang), Zitan Xiang (Lignum Santali Rubrum, 0.5 liang, crushed), Tingli (Semen Lepidii seu Descurainiae, 1 fen, used raw), Mangcao (Herba Siegesbeckiae, 1 fen), Chuan Dahuang (Radix et Rhizoma Rhei, 0.5 liang), Baiyao (Specific herb name omitted or unclear in source).
Preparation and Administration: The above ingredients are finely ground and sieved into powder. Mix with jiangshui (fermented rice liquid) to an appropriate consistency. Apply to the swollen area using a bamboo spatula. Replace when dry, continuing until heat subsides and swelling disappears.
For pediatric soft furuncles, Rushxiang Gao (Frankincense Ointment) is indicated.
Ingredients: Rushxiang (Resina Agallochii, 0.5 liang), Nifen (Calomelas, 1 he/fen), You (Oil, 1 liang), Huangla (Cera Flava, 0.5 liang), Songzhi (Resina Pini, 1 fen), Miduoseng (Litharge, 1 fen, finely ground).
Preparation and Administration: First, boil the oil, wax, songzhi, and rushxiang together. Then add nifen and miduoseng, mixing to form an ointment. Spread the ointment on cloth according to the size of the furuncle and apply as a plaster.
For pediatric soft furuncles with unbearable redness, swelling, and pain.
Ingredients: Tianlinggai (Os Craniis, 1 piece, coated with butter and roasted until yellow), Maxiedi (Solea Textilis, 1 pair, old ones burned).

Ingredients: Chixiaodou (Semen Phaseoli Angularis, 49 grains), Rushxiang (Resina Agallochii, 0.5 fen), Nifen (Calomelas, 0.5 liang).
