Intermittent malaria (Jianri Nue) is caused by pathogenic factors (Xieqi) and defensive qi (Weiqi) traveling together through the six fu organs, but occasionally losing synchronization. Consequently, the pathogenic factors press inward upon the five zang organs. Since the disease site is distant and the qi is deep-seated, the qi movement is sluggish and cannot exit simultaneously with the defensive qi. This is why the symptoms manifest every two or three days.
For treating intermittent malaria characterized by high fever, alternating chills and fever, and constipation, use Biejia San (Turtle Shell Powder).
Ingredients: Bie Jia (Testudinis Carapax et Plastrum, 1 liang, prepared with vinegar until yellow, remove skirt edges); Chi Shao Yao (Paeoniae Radix Rubra, 1 liang); Dang Gui (Angelicae Sinensis Radix, 1 liang, sliced and lightly stir-fried); Da Qing (Isatidis Folium, 1 liang); Zhi Mu (Anemarrhenae Rhizoma, 1 liang); Tian Ling Gai (Cranii Humanis Os, 0.5 liang).
Process the above ingredients into a coarse powder. Take 5 qian per dose, decoct with one large cup of water until reduced to 0.5 cup, strain, and mix in the Tian Ling Gai powder. Administer before meals.
For treating intermittent malaria where attacks occur every other day or every three to five days at irregular intervals, use Zhi Mu San (Anemarrhena Powder).
Ingredients: Zhi Mu (Anemarrhenae Rhizoma, 1 fen); Heng Shan (Dichroae Radix, 0.5 fen); Bie Jia (Testudinis Carapax et Plastrum, 1 fen, prepared with vinegar until yellow, remove skirt edges); Tao Ren (Persicae Semen, 1 fen, soaked in water, skin and pointed ends removed, use double-kernel seeds stir-fried with bran until slightly yellow); Fu Zi (Aconiti Lateralis Radix Praeparata, 1 fen, processed until cracked, skin and navel removed); Nuomi (Oryza Sativa Var. Glutinosa, 50 grains); Wu Mei Rou (Mume Fructus Carnosus, 1 piece); Ye Lang Ya (Sarcandrae Herba, 0.5 fen).
Finely chop all ingredients. Soak in 1.5 large cups of wine overnight. At dawn (Wugeng), decoct to obtain 0.8 cup, strain, and divide into two doses. Take warm on an empty stomach, and administer again just before the attack begins.
For treating intermittent malaria with irregular onset and persistent chills and fever, use the following prescription.
Ingredients: Heng Shan (Dichroae Radix, 1 liang); Bie Jia (Testudinis Carapax et Plastrum, 1 liang, prepared with vinegar until yellow, remove skirt edges); Chuan Sheng Ma (Bupleuri Radix, 1 liang); Zhi Zi Ren (Gardeniae Fructus, 0.5 liang).
Grind into a fine powder. On an empty stomach, take 2 qian mixed with warm water. The goal is mild vomiting; if vomiting does not occur, take another dose.
For treating intermittent malaria with persistent chills and fever, use Wumei Wan (Dark Plum Pill).
Ingredients: Wu Mei Rou (Mume Fructus Carnosus, 1 liang, lightly stir-fried); Tao Ren (Persicae Semen, 30 kernels, soaked in water, skin and pointed ends removed, use double-kernel seeds stir-fried with bran until slightly yellow); Di Gu Pi (Lycii Cortex, 1 liang); Chi (Zizyphi Fructus, specifically the heart/core, 1 liang, dried by stir-frying); Sheng Ma (Bupleuri Radix, 1 liang); Ren Shen (Ginseng Radix et Rhizoma, 1 liang).
Grind into powder. Mix with refined honey and pound for 200-300 strokes to form pills the size of paulownia seeds. Take 20 pills on an empty stomach with warm grain water, and repeat in the evening.
For treating intermittent malaria that occurs at night, use the following formula.
Ingredients: Heng Shan (Dichroae Radix, 2 liang); Shumi (Milii Glucini Semen, 100 grains); Zhu Ye (Bambusae Folium, 1 liang); Shi Gao (Gypsum Fibrosum, 2 liang).
Finely chop all ingredients. Soak in 2 large cups of water and leave exposed to the air overnight. Decoct the next morning to obtain 1.2 cups, strain, and divide into two doses. Take one dose on an empty stomach and the other dose before the anticipated attack. On the day of treatment, do not wash hands/face or rinse the mouth, and do not eat. Apply the medicinal juice to the "five centers" (palms, soles, and chest) as well as the chest, head, and face. Place the medicinal dregs near the pillow for optimal effect.
Another formula.
Ingredients: Chuan Da Huang (Rhei Radix et Rhizoma, 0.5 liang, chopped and lightly stir-fried); Gan Cao (Glycyrrhizae Radix et Rhizoma, 1 fen, stir-fried until slightly red); Heng Shan (Dichroae Radix, 3 fen); Gui Xin (Cinnamomi Ramulus Cortex, 1 fen).
Finely chop all ingredients. Decoct in 2 large cups of water until reduced to 1 cup, strain, and divide into two doses. Take warm on an empty stomach, and administer again just before the attack.
For treating intermittent malaria that may occur daily, use Xionghuang Wan (Realgar Pill).
Ingredients: Xiong Huang (Realgar, 1 liang, finely ground); Yehuogan (Vulpis Liver, collected in the twelfth lunar month, 1 liang); Awei (Ferulae Resina, 1 fen); Zhusha (Cinnabar, 0.5 liang, finely ground); Hunsun Tougu (Simiae Cranii Os, 1 liang); Tian Ling Gai (Cranii Humanis Os, 0.5 liang); She Xiang (Moschus, 1 fen, finely ground).
Grind raw ingredients into powder, mix thoroughly with the ground minerals. On the noon hour of the fifth day of the fifth lunar month, mix with refined honey to form pills the size of cowpeas. Before each attack, males tie one pill to the left middle finger and females to the right middle finger using red silk. Sniff the pill intermittently. Subsequently, swallow one pill with vinegar-infused water.
Another formula.
Ingredients: Xiaomai (Tritici Fructus, 1 he); Pi Huang (Arsenicum, 1 liang, finely ground).
In the twelfth lunar month, soak these ingredients in vinegar for 49 days. Remove and dry by baking. When a patient presents, take 7 grains of wheat and administer with cold vinegar-infused water just before the attack.
