(The treatment principles for miscellaneous diseases in men and women are the same. The formulas and theories in these two volumes, although originating from gynecological texts, contain content omitted by many miscellaneous disease texts; therefore, they are preserved.)
Injuries caused by fatigue and overexertion are treated with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction); this constitutes an acute onset of disease. If mismanaged, symptoms such as fever, tidal fever, night sweats, and cough may arise, which is termed Xu Lao (Deficiency Consumption). If further mismanaged, symptoms such as dry skin and falling hair, inability of food to transform into muscle tissue, heat in the bone marrow, and amenorrhea may appear, which is termed Zhai (Tuberculous Disease). Bone-steaming fever progressing to Chuan Shi Lao (Transmissible Consumption) represents a distinct category; its origin does not lie in fatigue, but its progression can lead to familial extinction. I have already detailed and listed these in the first volume of miscellaneous diseases. However, since men primarily govern Jing (Essence) and women primarily govern Xue (Blood), the causes of disease differ, and thus treatments must also differ; therefore, specific methods should be established separately. Chen's *Liangfang* (Excellent Prescriptions) categorizes Lao Zhai, Gu Zheng Lao, Xue Feng Lao, Qi Xu Feng Lao, Leng Lao, Re Lao, and Ke Re into separate sections, which inevitably confuses later practitioners, leaving them without a clear direction. These are now corrected as follows. Physicians should refer to the Xu Lao and Chuan Shi Lao sections in miscellaneous diseases when applying these, thereby avoiding omissions.
[Principles for Early-Stage Disease]
[Bao Ming Ji] In treating female Xu Lao, Ju Fang states there are six formulas used throughout the course of treatment. For example, Da Sheng San and Shu Di Huang Wan are used to treat Xu Lao without heat; midway through treatment, Mu Dan Jian Wan is administered on an empty stomach before meals, and Ren Shen Jing Jie San is taken after meals before sleep to treat Xu Lao with heat.
[Dai Yuanli] There are cases of blood deficiency following illness and cases of constitutional blood deficiency. Patients present with alternating chills and fever, or heat in the five centers (palms, soles, and chest), weakness in speech, sallow complexion, dizziness, and various other ailments. Use Xiong Gui Tang (Szechwan Lovage and Angelica Decoction) with a small amount of mutton; or administer Shiquan Dabu Tang (Ten Significant Tonifying Decoction), Siwu Tang (Four Substances Decoction), or Yangrong Tang (Nourish the Ying Decoction). For those with blood deficiency but vigorous Qi, Yi Qi Tang (Suppress Qi Decoction), consisting of Cyperi Rhizoma powder, is appropriate.
Zengsun Siwu Tang (Modified Four Substances Decoction) [Yijian Fang]: Treats female Qi and Blood insufficiency, leading to lethargy in the four limbs, fatigue, and shortness of breath. [Xu Lao Without Heat]
[Li Gao] Female Leng Lao (Cold Consumption) belongs to insufficiency of Qi and Blood, deficiency-cold of Zang-Fu, leading to cold pain below the umbilicus, cold hands and feet, irregular menstruation, indigestion, occasional vomiting, aversion to cold with fever, joint soreness, emaciated skin, and sallow complexion.
[Xue Ji] The aforementioned symptoms include true cold internally and externally, but there are also true heat internally and externally, true heat internally with false cold externally, and true cold internally with false heat externally. If digestion is difficult, stool is loose, borborygmus and abdominal pain occur, food is feared as cold, hands and feet are reversed cold, face is sallow, vomiting occurs, and wind-cold is feared, this is true cold internally and externally. Use Fuzi Lizhong Tang (Aconite Regulate the Middle Decoction) to restore Yang, and Bawei Dihuang Wan (Eight-Ingredient Rehmannia Pill) to strengthen Fire. If diet is normal, stool is firm, chest/abdomen are distended, food is preferred cold, hands/feet are vexed-hot, face is red, vomiting occurs, and wind-cold is not feared, this is true heat internally and externally. Use Huanglian Jiedu Tang (Coptis Detoxification Decoction) to eliminate Yang, and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) to strengthen Water. If diet is normal, stool is firm, chest/abdomen are distended, food is preferred cold, hands/feet are reversed cold, face is sallow, vomiting occurs, and wind-cold is feared, this is true heat internally with false cold externally. Also use Jiedu Tang and Liuwei Wan. If appetite is low, stool is loose, acid regurgitation and belching occur, chest/abdomen are full, hands/feet are reversed cold, face is red, vomiting occurs, and wind-cold is feared, this is true cold internally with false heat externally. Also use Fuzi Lizhong Tang and Bawei Wan. Seek the root cause to treat. The Classic says: "Benefit the source of Fire to dissipate Yin shadows; strengthen the master of Water to control Solar flare." If one does not understand the insufficiency of True Water and Fire, and indiscriminately uses cold/hot medicines, old diseases will not leave, and new ones will arise. "Shu" refers to the Master, i.e., Heart and Kidney. Seeking Shu means seeking Heart and Kidney for Water/Fire insufficiency. Source of Fire is the root of Yang Qi, which is the Heart. Master of Water is the root of Yin Qi, which is the Kidney. It does not mean Fire is Heart, Origin is Liver, Water is Kidney, Master is Lung. One woman ate little, vomited, spat phlegm, had sallow face, abdominal pain, irregular menstruation, and reversed cold hands/feet. This was true cold internally and externally; treated with Liujun Zi Tang (Six Gentlemen Decoction) plus Fuzi and Muxiang, cured. One woman suddenly vomited sour water, felt internal heat and thirst, refused food, desired only cold water, had green-red face, and rejected medicine immediately upon entry. This was true heat internally and externally. After ten days, she drank a decoction of single Huanglian. Gradually added Baizhu, Fuling, Chenpi, Danggui, and Gancao. After a month, she could consume rice water/congee, and recovered with regulation. One woman had internal heat/thirst, constipation, fear of wind-cold, and reversed cold hands/feet. This was true heat internally with false cold externally. First treated with Huanglian Jiedu Tang, then Liuwei Dihuang Wan, cured. One woman initially suffered from phlegm-panting, heat, and thirst. Doctors used Fire-descending and Qi-scattering methods, causing daily emaciation and Qi depletion. By Jiachen year, Wood was vigorous, Phlegm abundant, body hot, mouth rotting, abdominal distension, coma, near death from starvation. This was deficiency-heat without Fire; treated with formulas strengthening Water to generate Earth, effective immediately. Years later, early summer, head drooped when sitting, back felt cold/windy when lying, vexed heat/dizziness, cough/vomit/phlegm, numb/cold hands/feet. This was true cold internally with false heat externally. Administered large tonifying Jiang/Fu formulas. Within 3-4 doses, major symptoms subsided; continued with modified previous prescription, cured.
Han Mao treated his sister-in-law, 30+ years old, who had 18 pregnancies, 9 losses, 8 stillbirths/infant deaths. During a family crisis, excessive shock and worry led to unconsciousness, ulcers on lips/tongue/throat, lower body collapse, profuse leukorrhea. For 40+ days, occasionally conscious, suicidal despair. Doctors cooling the upper body worsened lower symptoms; heating the lower body caused fainting. This was Wang Yang (Collapse of Yang). Urgently used Yan Zhu Da Fuzi (Salt-boiled Large Aconite) 9 qian as sovereign, paired with Bohe and Fangfeng, assisted by Jiang, Gui, Xiong, Dang. Decocted, cooled in well water, and given. Before finishing the dose, she slept soundly all night and recognized people upon waking. Asked why? Answered: Formulas state "False against False, True against True." Upper is false heat, so use false cold medicine to follow it; Lower is true cold, so use true heat medicine to oppose it. Thus upper/lower harmonized and disease resolved. Followed with Nv Jin Dan combining 2-3 formulas, removing disease and regulating Yuan Qi. Soon, gave birth to two sons. (The above discussions and verified treatments may or may not directly correspond to Xu Lao, but they represent the great principles of treating cold/heat variations, which must be examined.)
