Question: What is referred to as a disease of the Upper Jiao (Upper Burner)?
Answer: For example, diseases occurring in the nose, ears, eyes, and mouth belong to diseases of the Upper Jiao. If there is headache, fever, cough, and nasal discharge (bi qiu), with a floating and rapid pulse, it is caused by Wind-Cold pathogenic factors.
When headache occurs, if the skin and muscles appear bright and distinct, and the pulse is slippery, it indicates retained fluid (liu yin). If headache is accompanied by chest stuffiness, and if the legs are cramped, with a fine and wiry pulse, Chaihu Gaoben Tang (Bupleurum and Levisticum Decoction) is indicated.
Chaihu Gaoben Tang Formula
Yin Chai Hu (5 fen) Huang Qin (2 fen) Ban Xia (5 fen) Ren Shen (2 fen) Gao Ben (5 fen) Chuan Xiong (2 fen) Gui Zhi (3 fen) Da Zao (2 pieces) Sheng Jiang (2 fen) Gan Cao (2 fen)
The above ten ingredients are crushed. Boil in two sheng of water until one sheng remains, remove the dregs, and take warm in divided doses.
If headache is accompanied by rebellious Qi, or if there is dizziness, with a slightly slippery pulse, Guizhi Fuling Baizhu Gancao Tang (Cinnamon Twig, Poria, Atractodes, and Licorice Decoction) is indicated.
Guizhi Fuling Baizhu Gancao Tang Formula
Fuling (5 fen) Gui Zhi (3 fen) Baizhu (5 fen) Gan Cao (3 fen)
The above four ingredients are boiled in one sheng of water until seven ge remain, remove the dregs, and take warm in divided doses.
If headache recurs frequently, and the pulse is rapid and excess, this is an omen of head sores (tou chuang). Xionghuang Jiawei Wan (Ligusticum and Rhubarb Modified Pill) is indicated.
Xionghuang Jiawei Wan Formula
Tian Ling Gai (Heavenly Spirit Cover) Da Huang (each 2 liang) Chuan Xiong (1 liang)
The above three ingredients are ground and sifted. Make pills using wine paste.
If the headache pulse is fine, the chest feels uncomfortable and warm, and white saliva is expectorated, Wuzhuyu Tang (Evodia Decoction) is indicated.
If headache is accompanied by chest stuffiness, palpitations, and the pulse at the Guan position is floating, Dahuang Huanglian Xiexin Tang (Rhubarb, Coptis, and Pinellia Decoction for Draining the Heart) is indicated.
If headache is accompanied by yellowing of the eyes and hard stool, Dachengqi Tang (Major Order the Qi Decoction) is indicated.
If there is stabbing pain in the brain, cold limbs, and sweating on the forehead, this is pain penetrating the Niwang Palace (referring to severe brain pain), also known as True Headache (Zhen Toutong). Those who develop it in the morning die in the evening; those who develop it in the evening die in the morning. Infant malnutrition heat scorching and lodging in the brain, causing the fontanelle tip to bulge, is named Fontanelle Filling (Xintian). Infant diarrhea that cannot be controlled, with sinking of true Yuan Qi, causing the fontanelle to sink, is named Fontanelle Depression (Xian). Juepi Jia Jiangcan Tang (Splee-Extracting and Silkworm Added Decoction) is indicated.
Question: How are eye diseases caused?
Answer: Malnutrition wind-heat toxic pathogens, or fetal toxins, are all causes of eye diseases. Sudden swelling of the eyes, with heat and pain, is caused by Wind-Heat. If there is a hazy membrane in the eyes, and the chest and abdomen feel distended like pregnancy, with a deep and fine pulse, it is caused by Malnutrition Heat stirring. If the eyes are red and hot, and the skin has macules, it is caused by fetal toxin. Huoji Tang (Fire-Ji Decoction) is indicated. If the eyelids are corroded and ulcerated, the body is cool and harmonious, and the pulse is deep and fine, it is caused by residual toxin. Xionghuang Jiawei Wan is indicated. If eye disease heat and pain do not resolve, Tieluo Jiang (Iron Filings Slurry) is indicated for instillation.
Tieluo Jiang Formula
Tie Luo (4 qian, soaked in 5 he of snow water for seven days until the color resembles soap pod juice) Yan Zhi (Cold-processed 2 fen)
The above two ingredients are mixed and used to instill into the eyes, four to five times daily.
If the eye disease pulse is rapid and tight, and the ocular vessels are swollen and painful, this is Wind Eye. If there is no pain, it is named Epidemic Eye (Yi Yan). Juhua Tang (Chrysanthemum Decoction) is indicated.
Juhua Tang Formula
Ju Hua (5 fen) Fang Feng (1 qian) Chuan Xiong (3 fen) Zhi Zi (Black-fried 3 fen) Gan Cao (2 fen)
The above five ingredients are boiled in two sheng of water until one sheng remains, and taken warm in divided doses.
Sores on the eyelids, caused by Spleen Stagnation, are named Eye Warts (Yan You). They heal spontaneously with reduced diet. Clear vision in the morning and dim vision in the evening is named Sparrow Eye (Que Mu, night blindness). This is caused by Malnutrition Stagnation. Yeming Sha Wan (Night-Shining Sand Pill) is indicated.
Yeming Sha Wan Formula
Ye Ming Sha (3 qian, fried and ground) Xiong Dan (5 fen)
The above two ingredients are ground and mixed, made into pills the size of hemp seeds. Take thirty pills per dose.
For Blood Eye with heat and pain, or Epidemic Eye with redness and swelling, use Huan Yuan Shui (Return-to-Origin Water) for instillation. Simultaneously administer Sanhuang Jia Xixin Maxi Tang (Three-Yellow Decoction with Added Asarum and Euphorbia) for internal treatment. Malnutrition Fire stagnation steaming and damaging essence, causing vision blurred as if looking through fog, is Malnutrition Eye (Gan Yan). For infants squinting due to Malnutrition Eye, eating Mianli Dan (Mudfish Gallbladder) or Haiji Dan (Sea Bream Gallbladder) will lead to spontaneous recovery. Infant eyelid edema is caused by overeating. Star-shaped opacities or membranes caused by smallpox in the eyes require urgent treatment. Instill Chicken Comb Blood; Egg White is also good. Opacities caused by smallpox toxin or wind toxin are treated with Chen Sha Gao (Cinnabar Ointment).
Chen Sha Gao Formula
Bi Ma Zi (1 liang) Bi Ma You (3 he) Chen Sha (4 qian)
The above three ingredients are pounded into an ointment. Apply to Baihui (GV20). It can also be applied to various sores to cure them. For postpartum retention of the placenta, apply to Yongquan (KI1). For eye injury with heat, irritability, and pain, He Gen (Lotus Root) juice is indicated for instillation.
Instillation Formula
He Gen (1 piece, remove coarse bark)
The above single ingredient is ground and squeezed for juice. Instill repeatedly; pain will resolve spontaneously. If dust or sand invades the eye, causing gritty pain and excessive tearing, grind good ink in clear water and instill. Close the eyes for a moment, then wipe clean two or three times from tail to head with a paper twist; the dust will come out. Eyes with heat, irritability, and pain are Yang Eye (Yang Yan). Eyes without heat, appearing blue-white, unable to distinguish objects, are Yin Eye (Yin Yan). Yin Eye is difficult to treat. Yang Eye with pain and ruptured pupil is also difficult to treat. Wind Eye with heat, swelling, pain, and protruding pupil is incurable.
Question: How about nasal diseases?
Answer: Nose with heat and dryness, frequently desiring moisture, but moistening leads to corrosion, ulceration, and pain, is Nasal Malnutrition (Bi Gan). Nose with heat, redness, and swelling, with a surging and rapid pulse, is Lung Wind (Fei Feng). Turbid nasal discharge flowing like a spring is named Nasal Spring (Bi Yuan), also known as Brain Leak (Nao Lou). This is caused by Wind-Heat in the head lodging in the Lung. It inevitably leads to closed eyes; severe cases lead to epistaxis. Sanhuang Xiexin Jia Xixin Bohe Tang (Three-Yellow Decoction for Draining the Heart with Added Asarum and Mint) is indicated. Flesh growing inside the nasal cavity is Nasal Hemorrhoid (Bi Zhi). If red, swollen, and hot, blocking the nasal aperture, it is named Nasal Obstruction (Bi Shi). Incision to drain stasis and turbidity is indicated. Lung Wind swelling and pain, various nasal sores, with a surging and rapid pulse, are treated internally with Huoji Tang. Nasal sores with obstruction, caused by alcohol toxin entering the Lung, are treated with Ge Hua Tang.
Ge Hua Tang Formula
Ge Hua (5 fen) Zhi Ke (5 fen) Bo He (3 fen) Shi Gao (1 qian) Gan Cao (2 fen)
The above five ingredients are boiled in one sheng of water until seven ge remain, remove dregs, and take warm in divided doses.
Infant nose with heat and dryness, leading to blockage, heavy snoring breath, and restless sleep, is named Nasal Obstruction. Honey instillation is indicated.
If the patient's lung orifice is closed by Wind-Heat, leading to blockage and difficulty breathing, Genti San (Seed of Melon Powder) is indicated, blown in using a tube; resolution comes with drying and irritation. Nasal sore swelling, with a surging and slippery pulse, is Nasal Abscess (Bi Yong). Tongfei Yin (Unblocking Lung Drink) is indicated.
Tongfei Yin Formula
Cang Er Zi (5 fen) Xin Yi Ren (5 fen) Bai Zhi (3 fen) Xi Xin (2 fen) Bo He (2 fen) Long Nao (1 fen) Sheng Jiang (2 fen)
The above seven ingredients: boil six ingredients in one sheng of water, remove dregs, add Long Nao, boil for two or three boils, and take warm in divided doses, three times daily.
Inability to smell fragrances or odors is caused by stagnation and obstruction of the lung orifice. Tongfei Yin is indicated.
Question: There is epistaxis and hematemesis. How do they differ?
Answer: The nose communicates with the Lung. Blood overflowing from the brain causes epistaxis. Blood moving in the chest causes hematemesis. If patient's blood and Qi move violently causing epistaxis, with a rapid and excess pulse, Dahuang Huanglian Xiexin Tang is indicated. Epistaxis as a condition, with aversion to cold and fever, and a floating and tight pulse, indicates Wind-Heat attempting to resolve. Epistaxis or vomiting blood, with wheezing, phlegm, and night sweats, with a deficient and minute pulse, is treated with Xuanshen Tang.
Xuanshen Tang Formula
Xuan Shen (3 fen) Di Huang (Raw, 3 fen) Zhi Zi (Fried, 3 fen) Xi Jiao (3 fen)
The above four ingredients are boiled in one sheng of water until seven ge remain, remove dregs, and take warm in divided doses.
For sudden epistaxis caused by fright and rebellious movement, first apply cold water to the face, then administer Dahuang Huanglian Xiexin Tang. For violent epistaxis or hematemesis with impending collapse of Yuan Yang, moxibustion on Yongquan (KI1) and Dadun (LR1) is indicated; warming Baihui (GV20) is also good. For patients who are emaciated, with a fine and rapid pulse, coughing blood or epistaxis, this is an Excess Above and Deficiency Below pattern. Drinking roasted liquor with radish juice is indicated. Also, consume Zi Tang (Purple Sugar) mixed with roasted liquor. Apply garlic paste to Yongquan (KI1). For patients with epistaxis and red face, indicating lone Yang surging, grind good ink into hot soup, add human urine, and drink. Huarui Shi San (Flower Rhyolite Powder) is also indicated.
Huarui Shi San Formula
Hua Rui Shi (4 qian) Shi Yan (1 qian)
Grind the above two ingredients evenly. Take with child's urine. For hemoptysis or hematochezia caused by various deficiency movements, She Xue Tang (Blood-Holding Decoction) is indicated.
She Xue Tang Formula
Dang Gui (5 fen) Shao Yao (5 fen) Di Huang (Raw, 3 fen) Chuan Xiong (2 fen) Fu She (Fried and ground, 3 fen) Sheng Jiang (2 fen)
The above six ingredients: boil the first five in two sheng of water until one sheng remains, remove dregs, add Fu She, mix well, and take warm in divided doses.
Blood disturbance disease: Yang excess causes body heat; Yin excess causes body cold. Body cold is easy to treat; body heat is difficult to treat. If nine orifices bleed and body heat does not resolve, with a slippery and rapid pulse, it is incurable. If Cold Damage heat lodges in the Bladder causing bloody discharge, this indicates the disease is resolving. For vomiting blood that is foul-smelling and unbearable, with a surging and large pulse, or dizziness, it is difficult to treat. For blood disturbance patterns, patients with dark, thin skin are easy to treat; those with white, fat skin are difficult to treat.
Question: How about ear diseases?
Answer: Ear disease: Floating pulse indicates Excess; Deep pulse indicates Deficiency. Excess leads to sores; Deficiency leads to deafness. Infant ear pus with foul odor is caused by Spleen Stagnation due to overeating. It is named Ting Er (Aural Discharge). Dahuang Huanglian Xiexin Tang is indicated. Wind-Heat attacking upward and drumming, causing tinnitus like ocean waves, is treated with Fangfeng Gongdu Yin.
Fangfeng Gongdu Yin Formula
Fang Feng (5 fen) Xi Xin (2 fen) Niu Bang Zi (5 fen) Huang Bai (Fried, 3 fen) Han Shui Shi (1 qian) Mang Xiao (3 fen) Gan Cao (2 fen)
The above seven ingredients are boiled in two sheng of water until one sheng remains, remove dregs, and take warm in divided doses.
Patient tinnitus like cicada singing, worse in the evening, better in the morning, is caused by rootless false fire surging. Wuya Hei San (Crow Black Powder) is indicated.
Wuya Hei San Formula
Wuya (Black-fried, 4 qian) Huang Bai (Black-fried, 2 qian) Zhi Zi (Black-fried, 3 qian)
The above three ingredients are ground and mixed. Take with plain water and child's urine.
Ear pain attacks, pus dripping, or cicada-like sounds are treated with Bai Fan You (Alum Oil).
