Cheng Shaohu, due to drinking raw wine and eating hard dried tofu, developed facial edema and chest distension the following day. This condition has persisted for half a year, accompanied by borborygmus in the abdomen, edema of the four limbs, swelling of both legs and the scrotum, dry mouth, and difficulty in urination and defecation. He is forty-six years old. At night, he suffers from asthma when lying down, and coldness below the knees. Initially, he was prescribed a decoction of **Renshen** (Ginseng Radix), **Cangzhu** (Atractylodis Rhizoma), **Chenpi** (Citri Reticulatae Pericarpium), **Luobozhi** (Radish Seed), **Banxiaqu** (Pinelliae Fermentatum), **Gegen** (Puerariae Lobatae Radix), **Houpo** (Magnoliae Officinalis Cortex), **Zhishi** (Aurantii Fructus Immaturus), **Poguzhi** (Psoraleae Fructus), **Fuzi** (Aconiti Lateralis Radix Praeparata), and **Fuling** (Poria) for oral administration. After two doses, the asthma slightly stabilized, but borborygmus persisted in the abdomen. **Baidouren** (Cardamomi Rotundus Fructus), **Baijiezi** (Sinapis Semen), and **Sangbaipi** (Mori Cortex) were added. Urination became quite profuse, and the edema gradually subsided.
Wu Douyi was a native of Nanxi, Shexian. In early August, while traveling from Xindu to Huzhou, he accidentally contracted wind-pathogens, resulting in fever, excessive phlegm, abdominal pain, and dysentery with tenesmus. He had a pre-existing condition of intestinal wind with blood in the stool. Additionally, his father had died of dysentery and fever in Huzhou the previous year, causing him great fear. Upon pulse diagnosis, the pulses at both Guan positions were slippery, large, and forceful, while the Chi and Cun positions were deficient. Therefore, I prescribed **Baishao** (Paeoniae Radix Alba), half raw and half wine-fried, five qian as the sovereign drug to relieve pain; **Danggui** (Angelicae Sinensis Radix) to invigorate blood; **Jiu Lian** (Coptidis Rhizoma processed with wine), **Jiu Qin** (Scutellariae Radix processed with wine), **Chaihu** (Bupleuri Radix), and **Jiegeng** (Platycodonis Radix) to clear heat and raise yang qi as the minister drugs; **Zhiqiao** (Aurantii Fructus) and **Huaihua** (Sophorae Flos) to treat intestinal wind as the assistant drugs; **Yiyuan San** (Beneficial Essence Powder) and **Muxiang** (Aucklandiae Radix) to strengthen the large intestine; and **Shanzha** (Crataegi Fructus) to resolve blood stasis, harmonize the liver, and stabilize the spleen as the adjunct drugs. After one dose, the fever decreased by half, and the dysentery lightened slightly. The next day, the same prescription was continued, along with external regulation using **Danxi Baohe Wan** (Danxi Harmonizing the Middle Pill). After five days, he was able to go outdoors.
My nephew Sun Ershui, who suffered from warm malaria, had a very obese physique.
My nephew Sun Ershui was thirty years old and very obese. During summer, he often immersed himself in streams and slept under the shade of pine trees. By August, he experienced severe chills and fever between the Si and Wu hours (9 AM - 1 PM), which subsided only at dawn. He could not get out of bed and refused food. He vomited yellow, bitter bile, felt distension and oppression in the chest and diaphragm, and his tongue was dry with prickly,沉香-colored coating. His tongue was stiff and unable to speak; he had to rinse his mouth with cold water to utter a single sentence. If he spoke three sentences, he had to rinse his mouth with water three times in between. He only consumed watermelon and fresh lotus root. On the first day of chills and fever, he vomited a mouthful of blood; now he has blood in his stool. The symptoms were extremely critical and malignant, accompanied by coughing. This is a case of warm malaria. It arose because the physician failed to release the exterior pathogens promptly and hastily used **Huangbo** (Phellodendri Cortex) to trap the pathogenic qi, leading to serious consequences. The current treatment should focus on clearing heat, resolving the exterior, and stopping vomiting, so that food intake can resume, thereby preserving life. **Chaihu** (Bupleuri Radix) and **Zhimu** (Anemarrhenae Rhizoma), each three qian; **Shigao** (Gypsum Fibrosum), seven qian; **Gegen** (Puerariae Lobatae Radix), two qian; **Juhong** (Citri Reticulatae Pericarpium Rubrum) and **Zhuru** (Bambusae Caulis in Taeniam), each one qian and five fen; **Jiu Qin** (Scutellariae Radix processed with wine) and **Zhishi** (Aurantii Fructus Immaturus), each two qian; **Gancao** (Glycyrrhizae Radix et Rhizoma) and **Beimu** (Fritillariae Thunbergii Bulbus), each five fen. Decocted in water and taken orally. After three doses, vomiting ceased. The formula was then changed to **Erchen Tang** (Two-Cured Decoction) plus **Chaihu** (Bupleuri Radix), **Zhishi** (Aurantii Fructus Immaturus), **Huangqin** (Scutellariae Radix), **Huanglian** (Coptidis Rhizoma), **Tianhuafen** (Trichosanthis Radix), **Biejia** (Testudinis Carapax et Plastrum), **Baizhu** (Atractylodis Macrocephalae Rhizoma), and **Heshouwu** (Polygoni Multiflori Radix) for regulation until recovery. He was instructed to avoid meat; consuming meat too early might cause relapse.
