Mr. Jin Dawen, a literary scholar, suffered from internal injury due to diet after consuming liver and white intestines, followed by exposure to wind pathogens, resulting in the concurrent occurrence of malaria (Malaria) and dysentery (Dysentery). He defecated more than forty times a day, with severe lower abdominal pain. Each time he went to the toilet, he sweated profusely like rain, experiencing tenesmus (urgency and heaviness in the anus) and painful, difficult urination. He had headache and thirst, fever in the afternoon that subsided only at dawn. The left pulse was floating, wiry, and rapid; the right pulse was soft and weak; the middle pulse was slightly slippery. This indicates internal injury from diet and external contraction of wind pathogens, causing both malaria and dysentery. The treatment principle should be prompt intervention; otherwise, when yang qi hides in late autumn, the pathogen will sink downward, making it difficult to cure. Therefore, one dose of Chai Ling Tang (Bupleurum and Poria Decoction) was administered first, after which urination became clear and painless. During malaria attacks, chills were more prominent than heat. In the evening, Ren Shen Bai Du San (Ginseng and Detoxification Powder) was given with modifications: Ren Shen (Panax Ginseng Radix), Gan Ge (Puerariae Radix), Fang Feng (Saposhnikoviae Radix), Gui Zhi (Cinnamomi Ramulus), Fen Cao (Glycyrrhizae Radix et Rhizoma), Fu Ling (Poria), Zhi Qiao (Aurantii Fructus) each 5 fen; Chai Hu (Bupleuri Radix), Bai Shao Yao (Paeoniae Radix Alba) each 1 qian. Decocted in water and taken orally. The next day, headache and dysentery both decreased, and he rose only three times at night. The prescription was changed to Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with Jiu Huang Qin (Scutellariae Radix processed with wine), Gui Zhi, and Bai Shao Yao. That night, malaria ceased, leaving only slight heat. It was then changed to Wei Feng Tang (Stomach Wind Decoction): Ren Shen, Bai Zhu (Atractylodis Macrocephalae Rhizoma), Gui Pi (Cinnamomi Cortex) each 2 qian; Bai Shao Yao 4 qian; Dang Gui (Angelicae Sinensis Radix) 1.5 qian; Fu Ling, Chuan Xiong (Chuanxiong Rhizoma) each 8 fen; Jiu Huang Qin, Jiu Lian Lian (Coptidis Rhizoma processed with wine) each 1 qian; Pao Jiang (Zingiberis Rhizoma Preparata) 2 fen; Di Yu (Sanguisorbae Radix) 5 fen. After taking this, chills and heat disappeared completely. He rose only once at night, and the stool was normal feces. Gui Zhi was removed from the previous formula, and after several more doses, he could get up, comb his hair, and go out.
Mr. Jin Jixian, on the sixteenth day of the seventh lunar month, suffered from headache and thirst, profuse sweating that did not stop, vomiting, nausea, alternating chills and heat, occurring every other day, specifically on yang days. I examined his pulse: both cun positions were surging, large, and forceful; the left pulse was floating, wiry, and rapid; the right pulse was slightly soft but also wiry and long. This is malaria caused by the combination of Shaoyang (Lesser Yang) and Yangming (Greater Yang) syndromes. Chai Ge Jie Ji Tang (Bupleurum and Pueraria Decoction to Resolve the Muscle Layer) was prescribed as the main formula: Chai Hu, Ge Gen (Puerariae Radix) each 2 qian; Bai Shao Yao, Shi Gao (Gypsum Fibrosum) each 3 qian; Fen Cao, Gui Zhi each 5 fen; Zhi Mu (Anemarrhenae Rhizoma) 1 qian; Ginger 3 slices. Decocted in water and taken orally. The next morning, the left pulse had slightly receded. Then, Bu Yin Bie Xue He Shou Wu Wan (Yin-Nourishing Tortoise-Blood Polygonatum Pill) was given to arrest the malaria, followed by one dose of Huang Qin Shao Yao Tang (Scutellaria and Peony Decoction) to prevent the onset of dysentery: Bai Shao Yao, Shi Gao, Huang Qin (Scutellariae Radix) each 3 qian; Gui Zhi 5 fen; Zhi Mu, Fen Cao each 1 qian; Chai Hu 7 fen; Ginger 3 slices. Decocted and taken. Later, Ren Shen was added (7 fen), and the malaria completely stopped, leading to recovery.
