Both eyelids (Pi) are swollen and puffy. Their appearance is like a sphere. Mild cases involve damp-heat, while severe cases result in tearing. It is not dryness, redness, or swelling; the treatment should seek to clear heat.
This condition refers to the eyelids being swollen like a sphere and bulging vacuously. There are no other diseases in the upper part of the eye. Over time, there may be the hazard of chaotic red vessels. When fire is severe, the skin may turn red, but the eyes do not hurt. If damp-phlegm and fire clash and struggle with each other, there will be tearing and canthal ulceration. This is a deficiency syndrome where fire resides in the Qi level, and it must not be mistaken for an excess disease of the Blood level characterized by swelling like a cup. Rubbing the area with palms warmed by friction provides slight relief, but it returns to normal shortly after, which indicates insufficient Blood and stagnation of deficient fire in the Qi level. It is suitable to take the following wash prescription:
Tiaopi Qingdu Yin (Spleen-Regulating and Toxin-Clearing Decoction)
Trichosanthis Radix (Tianguahua), Forsythiae Fructus (Lianqiao), Schizonepetae Spica (Jingjie Sui), Glycyrrhizae Radix et Rhizoma (Gancao), Arctii Fructus (Shuniuzi), Platycodonis Radix (Jiegeng), Poria (Baifuling), Atractylodis Macrocephalae Rhizoma (Baizhu), Menthae Haplocalycis Herba (Subohhe), Saposhnikoviae Radix (Fangfeng), Citri Reticulatae Pericarpium (Guang Chenpi) (each in equal parts).
The above herbs are cut into pieces. Add two zhong of water, decoct until eight-tenths remain, remove the dregs, and take warm before meals.
Guangda Chongming Tang (See Volume 3)
Note: Before and after childbirth, disorders often arise from disharmony of Qi and Blood, leading to upward attack of dry-fire, stagnation of Yin and Yang, or wind pathogens taking advantage of deficiency to invade, with evil fire infiltrating. It may also result from depression of the Seven Emotions or the Six Qi drawing pathogens inward. One should not be rigidly confined to whether there are nebula membranes, redness, or pain. For conditions before childbirth, only An Tai (calming the fetus) and clearing fire methods should be used; for conditions after childbirth, only Yang Rong (nourishing blood/vitality) and dispersing depression methods should be used. These two types of conditions must be distinguished as excess or deficiency. Those in the Qi level should be regulated and dispersed; those in the Blood level should be tonified and moved. Thus, complications will naturally be avoided.
When a woman is pregnant, she is said to have both fetus and pregnancy, caused by obstruction in the three Yang meridians. It manifests as excess rather than deficiency, but one must distinguish whether the disaster lies in the Blood or the Qi.
This section specifically discusses eye diseases in pregnant women. The pathology is mostly excess, requiring differentiation between the Qi level and the Blood level. In the Qi level, symptoms such as rising spiral-like nebulae and dilation of the pupil may occur. In the Blood level, symptoms such as coagulated fat resembling blood stasis may appear. Generally, due to obstruction and stagnation of Yin and Yang, this differs from eye diseases in ordinary people, often posing critical danger. People fail to consider thoroughly, frequently encountering sudden worsening without preparedness. Internal purgation fears damaging the fetus and leaking Qi, while non-purgation fails to clear the source, placing the situation in a dilemma. Skillfully employing internal protection and external interception treatments ensures success. For treating eye diseases before childbirth, one need not dislike draining and unblocking methods, provided harsh drugs like Xiaoshi (Mirabilite) and Dahuang (Rhubarb) are avoided, and formulas that break Blood or drain the Small Intestine are not used. The Classic states: "If there is a cause, there is no destruction, nor destruction to the mother." Alternatively, using Fetal-Securing herbs like Baizhu (Atractylodis Macrocephalae Rhizoma) and Huangqin (Scutellariae Radix) to control it, or assisting with adjuvant herbs, causes no harm. It is suitable to take:
Baotai Qinghuo Tang (Fetus-Calming and Fire-Clearing Decoction)
Huangqin (1 qian 2 fen), Amomi Fructus Rotundus (Sharen), Schizonepetae Spica (Jingjie Sui), Angelicae Sinensis Radix (Danggui Shen), Paeoniae Radix Alba (Baishao), Forsythiae Fructus (Lianqiao), Rehmanniae Radix Recens (Sheng Dihuang), Citri Reticulatae Pericarpium (Guang Chenpi) (each 1 qian), Ligusticum Chuanxiong Rhizoma (Chuanxiong) (8 fen), Glycyrrhizae Radix et Rhizoma (Gancao) (3 fen).
The above herbs are cut into pieces. Add two zhong of water, decoct until eight-tenths remain, remove the dregs, and take warm after meals.
Jianyi Zhimu Yin (Simple Anemarrhena Decoction - Treats Heart and Spleen Heat Stagnation during Pregnancy, Red Eyes, Throat/Thorax Thirst and Bitterness, Restlessness and Frequent Startles.)
Zhu Fu Ling (Red Poria), Huangqin (Scutellariae Radix), Ophiopogonis Tuber (Maidong Rou), Anemarrhenae Rhizoma (Zhimu), Morus Alba Cortex (Sangbaipi), Astragali Membranaceus (Huangqi), Gancao (Fine Licorice) (each in equal parts).
The above herbs are cut into pieces. Add two zhong of water, decoct hot, remove dregs. Then add one small zhong of Bamboo Juice (Zhuli), mix in a bowl, and take.
Tianmendong Yinzi (Asparagus Decoction - Treats Accumulated Heat, Sudden Blindness in Both Eyes, Internal Heat and Irritability, All Heat Syndromes.)
Notopterygii Rhizoma et Radix (Qianghuo), Poria (Baifuling), Ginseng Radix (Renshen) (each 8 fen), Asparagi Tuber (Tianmendong, dehearted), Anemarrhenae Rhizoma (Zhimu, processed with salt water), Leonuri Fructus (Chongweizi) (each 1 qian 2 fen), Saposhnikoviae Radix (Fangfeng), Schisandrae Fructus (Wuweizi) (each 5 fen).
The above herbs are cut into pieces. Add two zhong of water, decoct until eight-tenths remain, remove dregs, and take hot.
Xiongsu San (Ligusticum and Perilla Powder - Treats Pregnant Women with External Wind-Cold Invasion, High Fever, Dizziness and Blurred Vision as if spinning. This is likely due to Cold overcoming the Spleen and Stomach, damaging Ying and Wei, or exposing the back to wind for cooling, causing eye and head pain, aversion to cold and fever, even chest irritability. Generally, before childbirth, two lives are at stake; one must not casually administer decoctions. At the initial stage of cold, only Xiongsu San should be used to release the pathogen externally, and the illness will heal itself.)
Perillae Folium (Zisu), Ligusticum Chuanxiong Rhizoma (Chuanxiong), Ophiopogonis Tuber (Maidong Rou, dehearted), Atractylodis Macrocephalae Rhizoma (Baizhu), Citri Reticulatae Pericarpium (Chenpi), Zingiberis Rhizoma Ustus (Ganjiang, stir-fried black), Paeoniae Radix Alba (Baishaoyao) (each 1 liang), Glycyrrhizae Radix et Rhizoma (Gancao) (5 qian).
Grind into powder. Take 5 qian per dose, with 3 slices of ginger and 3 stalks of green onion head, decoct in water and take warm.
Xiaofeng San (Wind-Dispersing Powder - Treats Pregnant Women with Vertigo and Blurred Vision, Inability to See, Swollen Glands in Cheeks and Neck. Likely due to injured fetal Qi, Heat-Toxin attacking upward, Sun-Yang meridian deep pain, vomiting, stiffness of back and neck, causing blurred vision and visual spots. If accompanied by Phlegm obstruction, it is critical and imminent; take urgently.)
Gypsum Fibrosum (Shigao), Saposhnikoviae Radix (Fangfeng), Chrysanthemi Flos (Gan Juhua), Notopterygii Rhizoma et Radix (Qianghuo), Ligusticum Chuanxiong Rhizoma (Chuanxiong), Schizonepetae Spica (Jingjie), Saiga Tataricae Cornu (Lingyangjiao), Angelicae Sinensis Radix (Danggui), Angelicae Dahuricae Radix (Baizhi), Glycyrrhizae Radix et Rhizoma (Gancao), Sojae Semen Praeparatum (Dadou Huangjuan, stir-fried) (each in equal parts).
Grind into fine powder. Take 3 qian per dose, mixed with fine tea, take after meals.
Tiandong Yinzi (Asparagus Decoction - Treats Pregnant Women near Term, Sudden Loss of Vision in Both Eyes, Cannot See Light, Headache and Blurred Vision, Swollen Cheeks and Neck, Inability to Turn Neck. This condition arises because pregnant women often stay in warm rooms, or overheat themselves, trapping heat in bedding/clothing, or take too many tonifying/hot medicines, causing extreme Liver stagnation and resulting in Fetal Heat.)
Asparagi Tuber (Tiandong), Anemarrhenae Rhizoma (Zhimu), Leonuri Fructus (Chongweizi), Saposhnikoviae Radix (Fangfeng), Schisandrae Chinensis Fructus (Liaowumei), Poria (Fuling), Rehmanniae Radix Preparata (Shudihuang), Notopterygii Rhizoma et Radix (Qianghuo), Schizonepetae Spica (Jingjie Sui), Ligusticum Chuanxiong Rhizoma (Chuanxiong), Paeoniae Radix Alba (Baishaoyao), Angelicae Sinensis Radix (Danggui).
