The pulse is mostly deep. If injured by wind, the pulse floats; if injured by cold, it is deep and fine; if injured by summerheat, it is deep and minute; if injured by dampness, it is deep and slow. Diarrhea accompanied by abdominal distension with a wiry pulse indicates a fatal prognosis. It is also said that a slow pulse, occasionally minute, indicates survival; a floating, large, and rapid pulse indicates death.
Diarrhea belongs to dampness and qi deficiency, but may also be caused by fire, phlegm, food stagnation, or cold. There are also spleen diarrhea and kidney diarrhea. Diarrhea of watery stool without abdominal pain belongs to dampness. Food entering the stomach cannot be retained and digested, resulting in undigested food in the stool, which belongs to qi deficiency. Abdominal pain with diarrhea like hot soup, where pain comes in waves followed by diarrhea, belongs to fire. Diarrhea that occurs intermittently, varying in amount, belongs to phlegm. Severe abdominal pain followed by diarrhea, with pain relief after defecation, belongs to food stagnation. Abdominal pain with cold limbs belongs to cold. Frequent diarrhea is called spleen diarrhea. Diarrhea occurring at dawn (between 3-5 AM) is called kidney diarrhea. Differentiation and treatment should be applied accordingly. Generally, diarrhea is mostly caused by dampness injuring the spleen, primarily treated with Weiling Tang (Cinnamon Twig and Atractylodes Decoction) modified.
One discussion on summerheat injuring dampness, stagnant fluids mixed with food, leading to spleen-stomach disharmony, presenting with abdominal pain, diarrhea, thirst, difficult urination, indigestion of water and grain, and failure to distinguish yin and yang, this is due to dampness.
Weiling Tang (Cinnamon Twig and Atractylodes Decoction) is the main formula:
Cangzhu (Atractylodis Rhizoma), Nongpo (Moulaudii Cortex), Chenpi (Citri Reticulatae Pericarpium), Zhuling (Polypori Umbellati Sclerotium), Zexie (Alismatis Rhizoma) (each 1 qian); Baizhu (Atractylodis Macrocephalae Rhizoma) (2 qian, de-rhizome and stir-fried); Bai Fu Ling (Poria) (1.5 qian); Baishao (Paeoniae Radix Alba) (stir-fried, 1.5 qian); Rougui (Cinnamomi Cortex), Gancao (Glycyrrhizae Radix et Rhizoma) (roasted, each 0.3 qian).
Decoct one dose with three slices of Shengjiang (Zingiberis Rhizoma Recens) and two dates, take warm. For prolonged diarrhea, add Shengma (Cimicifugae Rhizoma) and Fangfeng (Saposhnikoviae Radix). For heat, add Huanglian (Coptidis Rhizoma) wine-fried. For cold, add Ganjiang (Zingiberis Rhizoma) stir-fried. For acute watery diarrhea, add Huashi (Talcum). For food stagnation, add Shanzha (Crataegi Fructus) and Shenqu (Massa Medicata Fermentata). For phlegm, add Banxia (Pinelliae Rhizoma) and Wumei (Mume Fructus). For qi deficiency, add Renshen (Ginseng Radix et Rhizoma) and Baizhu. For anger-induced cases, add Muxiang (Aucklandiae Radix). For chronic diarrhea, add Ganjiang and Roudoukou (Myristicae Semen). For acute dysentery with red and white mixture, abdominal pain, and tenesmus, remove Rougui, add Binglang (Arecae Semen), Muxiang, and Huanglian, decoct and take.
Another discussion on diarrhea where food enters the stomach but cannot be retained, resulting in undigested food, belongs to qi deficiency.
Yiqi Jianpi Tang (Qi-Supplementing and Spleen-Strengthening Decoction):
Renshen (2 qian); Baizhu (1.5 qian, de-rhizome, earth-fried); Bai Fu Ling (3 qian, peeled); Chenpi (2 qian); Baishao (stir-fried, 3 qian); Cangzhu (1.5 qian, soaked in rice wash water); Ganjiang (stir-fried black, 0.8 fen); Hezi (Myrobalani Fructus) (roasted, 2 qian); Roudoukou (Myristicae Semen) (wrapped in flour and roasted, 0.6 fen); Shengma (Alcohol-washed, 0.4 fen); Gancao (roasted, 0.8 fen).
Decoct with dates and ginger. Add Rougui for abdominal pain. Avoid greasy food.
Another discussion on diarrhea with abdominal pain, watery stool like hot soup, pain preceding diarrhea, belongs to fire. Use Jiawei Siling San (Modified Four Poria Powder):
Baizhu (1.5 qian); Bai Fu Ling (2 qian, peeled); Zhuling (2 qian); Zexie (2 qian); Mutong (Lini Stem and Fruit) (2 qian); Zhizi (Gardeniae Fructus) (3 qian); Huangqin (Scutellariae Radix) (2 qian); Baishao (3 qian); Gancao (0.8 fen).
Decoct with ten stems of Dengxincao (Junci Medulla), take on an empty stomach.
Another discussion on diarrhea varying in amount or intermittent, belongs to phlegm. Use Jiawei Erchen Tang (Modified Two-Aged Decoction):
Chenpi (2 qian); Banxia (Pinelliae Rhizoma) (ginger-fried, 2 qian); Bai Fu Ling (3 qian, peeled); Cangzhu (1.5 qian); Nongpo (ginger juice-fried, 0.8 fen); Sharen (Amomi Fructus) (0.8 fen); Shanyao (Dioscoreae Rhizoma) (stir-fried, 1.5 qian); Cheqianzi (Plantaginis Semen) (2 qian); Mutong (2 qian); Gancao (0.8 fen).
Decoct one dose with three slices of ginger, one Wumei, and ten stems of Dengxincao, take warm.
Another discussion on severe abdominal pain preceding diarrhea, with pain relief after defecation, belongs to food stagnation. Use Xiangsha Pingwei San (Costus and Amomi Powder for Calming the Stomach):
Cangzhu (1.5 qian); Chenpi (2 qian); Nongpo (ginger-fried, 0.8 fen); Baizhu (1.5 qian); Bai Fu Ling (3 qian, peeled); Banxia (ginger-fried, 2 qian); Sharen (1 qian); Xiangfu (Cyperi Rhizoma) (stir-fried, 2 qian); Shenqu (Massa Medicata Fermentata) (stir-fried, 3 qian); Baishao (2 qian); Gancao (roasted, 0.8 fen).
Decoct with ginger.
Another discussion on Liu Caochuang's Tongxie Yaofang (Essential Formula for Painful Diarrhea), used for food-injury abdominal pain relieved by defecation. Now, diarrhea with persistent pain is attributed to spleen earth failing and liver wood invading.
Baizhu (3 qian, stir-fried); Baishao (1 qian, stir-fried); Chenpi (stir-fried, 1.5 qian); Fangfeng (1 qian).
Decoct and take warm.
Another discussion on diarrhea with abdominal pain and cold limbs, belongs to cold. Use Fuzi Lizhong Tang (Aconite Decoction for Regulating the Middle):
Baizhu (1.5 qian); Ganjiang (0.8 fen); Renshen (2 qian); Bai Fu Ling (3 qian, peeled); Sharen (1 qian); Nongpo (ginger juice-fried, 0.8 fen); Cangzhu (1.5 qian, soaked in rice wash and fried); Shu Fuzi (Prepared Aconiti Lateralis Radix) (0.8 fen); Gancao (roasted, 0.8 fen).
Decoct with ginger and water.
Another discussion on diarrhea, weakness, easy satiety, and frequent loose stools, this is spleen diarrhea. Use Fupi San (Spleen-Supporting Powder):
Lianrou (Lotus Seed) (de-heart, skin on, 1.5 liang); Chenpi (1 liang); Bai Fu Ling (1 liang); Baizhu (east-wall earth-fried, 2 liang); Maiya (Hordei Fructus Germinatus) (stir-fried, 5 qian).
Grind into fine powder. Take 2 qian per dose with 2 qian white rock sugar, washed down with plain boiled water. Tonifies spleen and assists yuan qi, promoting appetite and relieving thirst.
Another discussion on slippery diarrhea, day and night without control, due to deficiency-cold of intestines and stomach unable to retain, use Bazhu San (Eight Pillars Powder):
Renshen (2 qian); Baizhu (1.5 qian); Roudoukou (roasted, 2 qian); Ganjiang (stir-fried, 0.8 fen); Hezi (roasted, 2 qian); Da Fuzi (Large Aconite Root) (0.8 fen, wrapped in flour, roasted, peel and navel removed); Suké (Papaveris Pericarpium) (honey-water fried, 2 qian); Gancao (roasted, 0.8 fen).
Decoct one dose with one slice of ginger, one Wumei, and a bundle of Dengxincao, take warm.
Bupi Wan (Spleen-Supplementing Pills), indications same as above:
Baizhu (de-rhizome 10 liang, divided into four parts: one part mixed with Roudoukou, one with Wuweizi, one with Pogostemonis Patchouliae Folium [Note: Text says 'Po Gu Zhi' which is Bu Gu Zhi/Acaciae Semen, likely typo in source or specific variant, standard is Acaciae Semen], one with Evodiae Fructus, each 2 liang, stir-fry together then remove these four herbs, use only Baizhu); Lianrou (de-heart, stir-fried); Renshen (each 1 liang); Gancao; Baishao (stir-fried, each 5 qian); Muxiang (roasted, 4 qian); Shanyao (stir-fried); Chenpi (each 7 qian); Ganjiang (3 qian, stir-fried).
Grind into fine powder, mix with porridge and stir-fried Shenqu powder, make pills like paulownia seeds. Take 100 pills per dose on an empty stomach with light ginger soup. Suitable not only for the elderly but also for weak individuals with spleen diarrhea or undigested food diarrhea.
Another discussion on diarrhea due to spleen-kidney deficiency, morning diarrhea at dawn, loss of appetite or indigestion, unformed stools, this is kidney diarrhea. Defecation immediately after eating indicates failure of spleen-kidney interaction to separate clear from turbid. Although spleen qi is strong, kidney qi is insufficient, causing undigested food diarrhea. Treatment usesershenshen Wan (Two Spirits Pill) as the main formula.
Ersen Wan (Two Spirits Pill):
Pogostemonis Patchouliae Folium (Bu Gu Zhi) (4 liang, stir-fried); Roudoukou (Myristicae Semen) (2 liang, raw).
Grind into powder. Cook 49 large red dates and 4 liang chopped ginger in water until soft. Remove ginger, mix date flesh with powder to make pills like paulownia seeds. Take 50 pills on an empty stomach with salt water. Adding Wu Zhu Yu (Evodiae Fructus) (1 liang, soaked and stir-fried) and Wuweizi (Schisandrae Fructus) (1 liang) makes Six Spirits Pill (Sishen Wan), highly effective for chronic long-term diarrhea.
One person who enjoys drinking has slippery bowels, difficult urination, reduced appetite, chest fullness, and swelling in legs/feet, syndrome of spleen-kidney deficiency-cold, treated with Jinkui Shenqi Wan (Golden Book Kidney Qi Pill), appetite improved, swelling subsided.
Then used Bawei Wan (Eight Flavor Pill), stomach strong, spleen healthy, recovered.
Another person had diarrhea every dawn, kidney diarrhea, cured with Wuweizi San (Schisandra Powder) after several doses. Relapsed due to careless lifestyle, lasting over a year, due to Mingmen Fire deficiency failing to generate spleen earth. Treatment should tonify the mother (Fire generates Earth). Used Bawei Wan to tonify the mother, diarrhea stopped, appetite returned. Li Dongyuan said: "Strong spleen-stomach qi leads to eating and gaining weight; deficiency leads to inability to eat and emaciation." All depends on Mingmen Fire as the source of transformation and nourishment. Thus Bawei Wan was effective; Liuwei Wan (Six Flavor Pill) could also be used.
Another discussion on slippery bowels, obstructed urination, limb swelling, wheezing/cough with sputum, due to spleen-kidney qi and blood deficiency, use Shiquan Dabu Tang (Ten Significant Tonic Decoction) taken with Sishen Wan. (Formula see Tonification section).
Another discussion on chronic diarrhea due to kidney deficiency, use Liuwei Dihuang Wan (Six Flavor Rehmannia Pill) added with Wuweizi, Pogostemonis Patchouliae Folium (Bu Gu Zhi), Roudoukou, and Wu Zhu Yu. (Formula see Tonification section).
Generally, chronic diarrhea often results from indiscriminate use of digestive and diuretic agents damaging true yin, causing yuan qi to fail, leading to chronic diarrhea. Without using Buzhong Yiqi Tang (Middle-Boosting Qi-Supplementing Decoction) and Sishen Wan to nourish the root, later complications include chest oppression, abdominal distension, and difficult urination, often leading to death.
One person had chronic diarrhea, causing sinking of yuan qi, food retention, undigested stools, muscle wasting, heavy limbs, facial and foot edema, shortness of breath, indicating severe spleen-stomach deficiency. Use Buzhong Yiqi Tang. (Formula see Internal Injury section). Modify by removing Danggui (Angelicae Sinensis Radix), adding wine-fried Baishao, Fuling, Zexie, Shanyao, Lianrou, Muxiang, and charred Ganjiang. This is an excellent formula for stopping diarrhea.
One case of diarrhea due to internal injury and fatigue, slow digestion, and latent spleen-stomach damp-heat, triggered by diet/fatigue, with sore/heavy limbs, use Yiqi Tang (Qi-Supplementing Decoction) minus Danggui, added with stir-fried Shaoyao, Fuling, Cangzhu, Zhuling, Zexie, decoct with ginger and dates.
In summary, the above formulas treat various types of diarrhea: damp, qi-deficiency, fire, phlegm, food stagnation, spleen-earth failure/liver-invasion, cold, spleen, spleen-stomach, yuan qi sinking, kidney, deficiency-cold slippery, and chronic diarrhea. Apply according to the specific condition.
One person had bowel sounds immediately after eating, followed by diarrhea, afraid to eat, all medicines failed. Used raw persimmon seeds, wrapped in paper, baked in wet ashes, eaten. Cured in 3-4 doses.
Secret recipe for diarrhea: One egg, break small end, insert 7 grains of Hu jiao (Piperis Longi Fructus), seal with paper, roast/bake, take with good liquor (baijiu) or better yet, burn liquor. Swallow pepper whole.
Diarrhea for 2-3 days with abdominal pain: Decoct Shengjiang, Dou Chi (Sojae Semens Praeparatum), and Hu jiao, take hot to stop.
For acute uncontrollable diarrhea with no urine: Stir-fried Cheqianzi powder, 2 qian per dose, mixed with rice drink. Roots/leaves can be crushed for juice. This herb promotes urination without damaging yuan qi.
For diarrhea: One pig stomach, clean fat membrane, put garlic inside, boil until soft, mash into paste. Add equal parts of Cangzhu (rice wash prepared), Chenpi, Nongpo (ginger-fried), Gancao (roasted), grind to powder. Mix with paste to make pills like paulownia seeds. Take 30-50 pills on empty stomach with rice soup or salt water.
Case of Prefect Huang from Xuzhou with chronic diarrhea for 2-3 years, relieved temporarily by 3 cups of baijiu, feared medicine. Pulse was wiry and rapid. First took this formula to resolve alcohol toxicity, then Lizhi Jianpi Wan (Qi-Calming Spleen-Strengthening Pills) with added Zexie, cured. (Formula see Cold Damage section).
Xuan Huanglian (Coptidis Rhizoma) (1 liang), Shengjiang (4 liang).
Stir-fry slowly until ginger is crisp and dry, remove ginger, keep Huanglian, grind to powder. Take 2 qian on empty stomach with winter tea infusion. Severe cases need no more than two doses. Specifically treats chronic spleen diarrhea.
Loose stools with undigested grain: Huanglian (wine-fried), Baishao (roasted), Wu Zhu Yu (stir-fried), equal parts. Grind to powder, make pills with millet rice like paulownia seeds. Take 50-60 pills on empty stomach with rice soup.
For diarrhea with cold hands/feet, no thirst, abdominal pain: Renshen, Baizhu, Ganjiang, Gancao, decoct, take hot. For severe cold, add Fuzi.
For chronic diarrhea with large intestine slipping: Wu Bei Zi (Galla Chinensis) stir-fried 5 liang, powder, paste pills like paulownia seeds. Take 5 pills with rice drink, 3 times daily.
Liuwei Wan, Jinkui Shenqi Wan, Bawei Wan, Shiquan Dabu Tang (see Tonification); Buzhong Yiqi Tang (see Internal Injury).
