Shenghui Fang (Supreme Benefit Form): Regarding various types of dysentery in children, it refers to stool with no fixed color. It may be watery mixed with undigested food, pus and blood, green or yellow, or red and white intermixed. The changes are chaotic and irregular, discharging in a mixed manner. This is all due to improper breastfeeding and feeding, where cold and heat pathogens interact within the body. Over time, this leads to spleen and stomach deficiency, causing the condition to linger and not heal, resulting in emaciation of the muscles and body.
Yingtong Baojian (Precious Mirror for Infants and Children): Pediatric dysentery occurs because cold and heat qi clash within the intestines and stomach, causing stagnation and obstruction in the meridians, leading to dysentery. If heat is predominant, the stool is red; if cold is predominant, the stool is white.
Yingtong Baojian records the fatal signs of dysentery: Frequent dysentery accompanied by an inability to eat, abdominal distension, panting, black stool, fever, and thirst.
Huiyan Guanzheng (Observing Signs with the Hui Eye): For red-and-white stagnant dysentery, one may first administer Pingwei Wan (Pingwei Pill), see formula in the Hiccup section; Shenling San (Ginseng and Atractylodes Powder), see formula in the Stomach Qi Disharmony section; and Xingpi San (Spleen-Awakening Powder), see formula in the Vomiting and Diarrhea section. If within three days the dysentery turns yellow and the stool becomes formed like normal feces, then according to the child's size and leanness, administer decoctions or pills to promote bowel movement (see formulas in the Acute and Chronic Convulsions section). Subsequently, regulate the qi, administer Shenling San, then use dysentery-specific medicines, followed by Ejiao San (Donkey-Hide Gelatin Powder, see formula in the Red and White Dysentery section) and Xianglian Wan (Cinnamon and Coptis Pill, see formula in the Section on Unceasing Thirst and Diarrhea), combined with mild medicines from Pingwei San and Xingpi San for regulation. If extreme thirst is observed, refusal to eat, unchanged stool color, and discharge of green bloody fluid, this is called "transmitting red-white stagnation." If this state persists for two periods of seven days (fourteen days), it will develop into Suokou Gan Li (Lock-Mouth Canker Dysentery) leading to death. Generally, for dysentery, treatment with Ejiao San and Xianglian Wan is also effective. If the stool contains three or four parts of fecal matter, purgatives must be used. If at such times only one or two drops are passed, and treatment is sought again while the child refuses to eat, this is also difficult to treat.
Yingtong Baojian Song on Various Pediatric Dysenteries:
Pediatric dysentery presents in many ways; do not view them all as one type. Cold dysentery sometimes appears green or white, or interferes with a cinnabar-red color. Heat invading the meridians causes fresh blood discharge; insect poison damaging the bowels causes purple liver-like matter to pass. Intestinal deficiency over many days results in膏血 (paste-like blood); extreme thirst indicates dryness of the zang-fu organs. Spleen weakness leads to full-body swelling; heat rushing to the liver and gallbladder causes cloudy vision. There is also prolapse of the rectum, which is hard to bear looking at; urgently seek famous medicines for peace and safety.
Yu Jue Song on Heat Accumulation in Pediatric Diarrhea and Dysentery:
Pediatric diarrhea and dysentery injure the stomach; cold and heat transmit to the spleen, damaging the large intestine. If stomach qi is disharmonious, grain cannot be transformed; if evil attacks and transforms into insects, it is extraordinary.
For pediatric diarrhea and dysentery, one must first regulate the qi, then administer dysentery medicine. If caused by insect poison, one must judge deficiency and excess to medicate and expel, further regulating stomach qi.
