Chapter: White Urine XII / Dribbling Urine XIII
Category: Comprehensive
[English Translation]
**Record from the Zhu Family Tradition:**
Pediatric patients presenting with urine resembling rice-washing water do not necessarily suffer from Gan syndrome (a chronic malnutrition disease in children); rather, this condition is caused by heat in the diaphragm (chest and diaphragm region). The prescription is as follows:
Yue Tiao (Gardenia Fruit, *Gardeniae Fructus*) - one piece.
Boil the above herb together with twenty stalks of Jixin (Rush Pith, *Junci Medulla*) to make a decoction. Sip it slowly, and the urine will clear.
**Record from the Ji Family Tradition:**
Treatment for Heart Heat causing mouth ulcers, red eyes, and urine like rice-washing water. Jinlu San (Golden Dew Powder):
Yujin (Turmeric Tuber, *Curcumae Radix*) - half liang;
Gancao (Licorice Root, *Glycyrrhizae Radix et Rhizoma*) (cut into pieces) - two liang;
Huashi (Talcum, *Talcum*) - half qian.
Grind the above herbs into a fine powder. Administer "yi zi" (approx. 0.5–1g) per dose, taken cold with prepared Ophiopogon (*Ophiopogonis Radix*) water.
**From *Chao Shi Bing Yuan* (Source of Diseases by Chao):**
Regarding various Lin syndromes in children: Various Lin syndromes in children are caused by heat in the Kidney and Bladder. The Bladder and Kidney are exterior-interior pairs, both governing water metabolism. Water enters the Small Intestine, descends to the Bao (uterus/bladder), travels through the Yin organs, and is excreted as urine. Kidney Qi connects downward to the Yin organs, which are the pathways for fluid. The Bladder is the fu organ of fluids. If the Bladder has heat, fluids overflow internally and flow into the marshes (lower jiao damp-heat accumulation), causing obstruction of the water passages. Water neither ascends nor descends, stagnating in the Bao, and Kidney Qi cannot connect downward to the Yin. Kidney heat causes qi to become stagnant, leading to poor water passage function. Urine dribbles continuously, hence the term "Lin." The presentation includes: small amounts of urine passed frequently, acute pain in the lower abdomen radiating to the umbilicus. There are also Stone Lin (urinary calculi), Qi Lin (qi stagnation), Re Lin (heat), Xue Lin (blood), and Han Lin (cold). Their manifestations vary by name and are detailed in subsequent chapters. However, since they can be treated with a single formula, they are collectively referred to as "Zhu Lin" (Various Lin Syndromes).
Mr. Mao states: Infants may present with various types of Lin dribbling from birth. In Stone Lin, sand and stones are excreted due to guest heat (external pathogenic heat) in the Kidney. In Cold Lin, white urine drips intermittently due to extreme cold in the Lower Jiao. In Heat Lin, painful retention occurs due to guest wind (external pathogenic wind) in the Kidney. In Blood Lin, urine resembles blood, indicating extreme heat in the Five Zang organs, representing the peak of heat pathology. For the aforementioned Lin syndromes, administering Huashi San (Talcum Powder) combined with Yang Hui San (Sheep Ash Powder) leads to recovery (both formulas are found in this section). For managing Cold Lin, warm-natured herbs must be administered concurrently. If symptoms include black complexion, abdominal distension, anorexia, aversion to crying, and retracted lips, these are signs of death and are incurable.
Zhang Huan notes: Pediatric urinary dribbling and astringency are no different from adults; they are also caused by Bladder heat. Severe cases of various Lin syndromes cause astringency, subumbilical oppression, and mental restlessness with heat.
*Ying Tong Bao Jian* (Precious Mirror for Infants and Children) states: Pediatric Lin syndrome results from accumulated heat in the Bladder obstructing the urethra, causing dribbling, astringency, and pain.
*Ying Tong Bao Jian* further states: Crying during urination with scanty output indicates Green Qi (Liver Qi/Wind Qi) stagnating in the Bladder, causing astringent pain in the penis, which constitutes Lin syndrome.
*Yu Jue* contains a song regarding pediatric Lin dribbling:
"Dribbling urine stems from Bladder heat;
Qi stagnation arises from wind binding the intestines.
Accumulations in the Bao cause Stone Lin;
Extreme heat attacking qi transforms into Blood Lin.
Clearing heat and draining wind-toxin from the Small Intestine is excellent.
Another view holds that benefiting urine and removing wind-toxin ensures correctness."
Another *Yu Jue* song repeats the above, adding: "First administer Jiang Huang San (Curcuma Powder) [see Blood Lin section, same as *Feng Sui Jing*], then Shi Wei San [see this section] to benefit urine and unblock Heart Qi."
*Shi Bi Jing* contains a song on five types of Lin among thirty-six conditions:
"The five Lin diseases transmit heat;
Distinguish each disease's origin.
Some cry and produce water accumulation;
Suddenly food stagnates in the Spleen.
Later returning to the Kidneys,
Causing persistent urinary dribbling.
Observe red veins at the eye corners;
(Alternatively: Red clouds at the eye corners.)
Blood mixed with sweat does not dry.
(Alternatively: Blue veins hanging to the ear side.)
Blue veins on the belly hang to the abdomen;
(*Feng Sui Jing*: Hangs to the 'Fu' [scrotum].)
Know that Stone Lin pain is unbearable.
Lips dry like peeling skin;
(Forty-eight Conditions: Tongue like millet grains.)
This is Wind Lin, definitely difficult to cure.
If the genitals are not swollen and qi is not urgent;
Once swollen, qi feels tormented.
First administer Kuan Chang Hai Jin San (Intestine-Widening Gold Sand Powder);
(*Feng Sui Jing*: First administer Kuan Chang San.)
It will surely unblock and ensure safety.
This is caused by Bladder heat, or heat forcing water to condense into stones. If Lung Qi stagnates, preventing the Small Intestine from separating clear and turbid, it also becomes Lin disease. Spleen accumulation and insufficient diet also cause this illness. Treatment should remove Bladder toxic vapors, then widen the Bao distension, and it will heal. (Hai Jin San formula is in this section.)"
*Feng Sui Jing* song is identical, with notes: "Blood Lin suits Jiang Huang San [see Blood Lin section]; Heat Lin suits Shi Wei Shi [see this section, same as *Yu Jue*]."
*Qian Jin Fang* (Essential Prescriptions Worth a Thousand Gold) Difuzi Tang: Treats pediatric heat-toxin entering the Bladder, sudden urinary retention, painful difficulty voiding, scanty bloody urine, recurring shortly after.
