According to "Han Dong Wang Xiansheng Jiabao", there are nine types of pediatric accumulation (Ji, a condition characterized by the stagnation and accumulation of pathogenic factors or food) that can be cured. Facial edema indicates accumulation. Accumulation is related to the Spleen; since the Spleen governs the muscles of the body, it manifests on the face, thus indicating Spleen accumulation. The Spleen corresponds to Earth, which has no fixed shape, so morning and evening edema varies; typically, swelling is more pronounced in the morning. During sleep, the Spleen fails to transform and transport, leading to facial swelling. If this syndrome occurs after an illness, it is deficiency with accumulation, and medicines for regulating the Spleen, resolving accumulation, and moving Qi should be used. Lying flat with the face pressed against the ground indicates accumulation. Why pressed against the ground? Because the accumulation is in the Spleen and is cold accumulation. How do we know? The Spleen favors Earth nature, so the disease is in the Spleen; cold belongs to Yin, so it is known to be caused by injury from cold, hard foods. Medicines that purge cold accumulation and move Qi should be used. Abdominal distension indicates accumulation. The accumulation is in the Lung. How do we know? The Lung governs Qi; once affected by accumulation, the Qi becomes cold, causing abdominal fullness and shortness of breath, thus indicating the disease is in the Lung. For abdominal distension, first regulate Qi, and after Qi moves, continue to regulate Qi. Urine resembling oil indicates accumulation. The accumulation is in the Small Intestine. How do we know? Accumulation originates in the Spleen, transmits to the Heart, and if the Heart does not accept it, it enters the Small Intestine. The Small Intestine is the Fu organ of the Heart, so the disease is in the Small Intestine; water pathways should be regulated, and urine will resemble rice wash mixed with oil. Yellowing of the body indicates accumulation. This is accumulation Qi injuring the Heart. The Heart governs blood vessels and nourishes hair and skin throughout the body. When disturbed by accumulation Qi, yellowing occurs, indicating Heart injury by accumulation Qi. Hollow Powder (Kongxin San) and medicines to remove accumulation should be used; such patients likely experience intermittent fever. Red and white dysentery indicates accumulation. The accumulation is in the Lung, transmitted to the Large Intestine, and acquired through external cold injury. How do we know? The Lung governs Metal (Geng-Xin) in the West, whose color is white; red stool indicates external pathogens, so Lung transmission to the Large Intestine causes red and white dysentery. Purging and regulating Qi afterwards should be used. Yellow-red eyes with blue pupils indicate accumulation. The accumulation is in the Liver. How do we know? The Liver governs Wood (Jia-Yi) in the East, whose color is green; when disturbed by accumulation Qi, it appears yellow-red. Blue pupils occur because eyes belong to the Five Zang organs, governed by the Liver; if the Liver suffers accumulation, the eyes turn blue, indicating Liver accumulation. If transmitted to the Gallbladder, the patient experiences bitter taste and loss of appetite, and cooling medicines should be used to resolve it. Generalized edema indicates accumulation. The accumulation is not in the Zang but in the Fu organs. How do we know? Because the accumulation was previously purged but later activated by medicine, it is no longer in the Zang but manifests as swelling between the skin. Only medicines to remove deficiency-type accumulation should be used, followed by tonification. Frequent passage of white feces indicates accumulation. This is cold accumulation in the Spleen. How do we know? The Spleen governs transformation; cold accumulation in the Spleen causes cold slipperiness and passage of white feces, indicating the disease is in the Spleen. Regulating Qi first, then using warming medicines for tonification is appropriate.
There are six types of accumulation that cannot be cured. Panting and urgency indicate Lung accumulation. The Lung governs Qi; panting urgency means Lung Qi exhaustion. The patient's face is pale without any blood color, so it is incurable. Black complexion indicates Kidney accumulation. A black face signifies Kidney Qi exhaustion. The patient cannot distinguish good from bad, stares straight with lifeless eyes, and will die within one day. Vomiting hot air indicates Ying (Nutritive) accumulation. It is incurable because Blood exhaustion is untreatable. Blood is governed by the Heart; if the Heart cannot control it, hot air flows out incessantly. Sores on the palms and soles indicate Wei (Defensive) accumulation. Wei is Qi; if Stomach Qi does not generate, sores appear on hands and feet. If Wei Qi exhausts, Qi cannot return, and death occurs within half a day. Nausea and dry retching indicate Stomach accumulation. Why is it incurable? The Stomach digests food; Stomach heat causes aversion to vomiting, so it is untreated. The patient definitely cannot digest milk, vomits dryly even without eating, and has a cyan-yellow complexion without blood color. Chronic diarrhea that stops and resumes indicates accumulation rotting the Spleen. How do we know the Spleen is rotten? The patient passes white feces due to undigested food; after stopping, they pass dark red-black feces and die, indicating rotten Spleen and incurability.
"Baojian" discusses five types of pediatric accumulation: due to stagnant Zang Qi accumulating in one place and not moving, it is called accumulation. Heart forms Fuliang (Hidden Beam), above the umbilicus, attacking upwards to the Heart and downwards to the stomach opening. Spleen forms Piqi (Stagnant Qi), horizontally obstructing above the stomach opening. Liver forms Feiqi (Fat Qi), to the left of the umbilicus. Lung forms Xiben (Panting Accumulation), to the right side of the umbilicus. Kidney forms Bentun (Rushing Pig), below the umbilicus. Each has variations; these are not formed by food but by Qi accumulation. Zang organs belong to Yin, so they remain immobile in one place. Ju refers to the retention and aggregation of Qi in the Six Fu organs. Fu organs belong to Yang; Yang Qi rotates continuously, so its aggregation is not fixed in one place, manifesting as abdominal pain upon onset. Signs of accumulation and ju include sallow emaciation, lack of muscle growth, erect hair or swollen body, urgent abdomen with fatigue, often due to Water Qi.
"Zhenzhu Nang" describes signs of deficiency-with-accumulation. Panic-induced deficiency accumulation arises from initial fright causing accumulation, reactivated by further fright, resulting in green and foul stools. Deficiency-with-accumulation arises from food injury causing diarrhea and vomiting, gradually weakening until the disease persists, hence called deficiency accumulation. Another type of deficiency-with-accumulation involves frequent purging and turning that fails to dislodge the accumulation, leaving it latent, also called deficiency-with-accumulation. If panic accumulation is purged, stool turns green with fright. If food accumulation, stool forms clumps. Gan-induced deficiency accumulation arises from Gan disease turning into diarrhea; weakness persists while Gan remains, hence deficiency-with-accumulation, with white matter in the discharged stool.
[Zeng] All pediatric accumulation syndromes arise from irregular feeding, overconsumption of cold, raw, hard foods, inability of Spleen and Stomach to transform, accumulation stagnating in Zhongwan, and external attack by Wind-Cold. Or sleeping uncovered at night, causing headache, sallow face, fever, slightly swollen eyelids, abdominal pain and distension, cold feet with warm belly, drowsiness and confusion, loss of appetite, vomiting or hiccups, sour belching, and foul-smelling stool, all caused by old accumulation. If detected within one or two days, first use Baishang Yin (for external sensations) to release the exterior, then use Danggui San (for tidal fever) decocted with ginger to warm and move stagnation, then administer Wuxi Wan and Liusheng Wan with heavy laxative action, followed by Yunqi San (for hernia) for regulation and tonification. [Tang] For all stagnations, differentiate deficiency and excess. Children vary in constitution, size, color, and geography across time, so generalizations are invalid. My current method: for excess, administer Jinshi Wan. For deficiency with slight Baiji Gan emaciation, use Fei'er Wan (see Fei'er Wan of Sanyin for abdominal distension). [Xue] At the onset before Yuan Qi is damaged, or with abdominal distension/pain and difficulty urinating/defecating, first use Baibing Zi or Muxiang Binglang Wan to purge. After purging, harmonize with Baizhu San or Wuwei Yigong San; add Gegen for thirst, Banxia for vomiting. If fever persists after purging, or vomiting/diarrhea occurs with loss of appetite, this indicates dual Spleen-Stomach injury; use Liujunzi Tang. Cold fingers/toes and preference for hot drinks indicate Spleen-Stomach deficiency-cold; add Paojiang and Muxiang to the previous formula. Sallow-white complexion and dull eyes indicate dual Spleen-Lung deficiency; use Sijunzi plus Chaihu and Shengma. Abdominal pain, diarrhea with prolapse, or urinary difficulty use Sini San. Fever, afternoon tidal fever, or ongoing diarrhea indicate Spleen Qi sinking; tidal fever with thirst and irregular stool suggesting transformation into Gan syndrome both use Buzhong Yiqi Tang assisted by Fei'er Wan. The Classic says: Where evil gathers, Qi must be deficient; remaining and not departing makes the disease excess. Regulation of the Spleen must be primary, assisted by resolution and guidance. As ancients said, nourishing righteousness removes accumulation naturally. This is the meaning.
"Qianjin" Zishuang Wan treats pediatric fever, headache, indigestion, abdominal fullness, or lower abdominal colic with urinary/fecal difficulty. Or frequent defecation attempts without other illnesses, only due to overeating, unawareness of stopping, irregular nursing, or fright palpitations and chills-fever. Only this pill treats it; if ineffective, repeat dosage. Pediatric downward movement indicates steaming syndrome; reduced feeding, uncomfortable breathing, night crying and insomnia indicate internal disharmony; all suit this pill, no other medicines needed. Frequent use is miraculous. Qianjin secret formula. (Chen Yi et al. note in preface that any mention of taking Zishuang Wan refers to the four ingredients in the Bianzheng chapter; those who cannot take Zishuang Wan take Chi Wan. Chi Wan acts faster for severe diseases. There is no Chi Wan formula here, but this uses Cinnabar, stronger than Zishuang Wan, suspected to be Chi Wan.)
Croton Seed (Ba Dou, peeled) Prunus Kernel (Rui He Ren, peeled. 18 zhu each) Licorice Root (Gan Cao, roasted, 5 zhu) Oyster Shell (Mu Li, fired red) Beeswax (Huang La, 8 zhu each) Kansui (Gan Sui) Cinnabar (Zhu Sha, 2 zhu each) Ophiopogon Tuber (Mai Men Dong, heart removed, 10 zhu)
Eight ingredients. Wash Croton seeds in boiling water, grind, squeeze oil out with new cloth. Grind Licorice, Kansui, Oyster Shell, Ophiopogon separately, sieve. Grind Prunus kernels thoroughly, mix with powder, pound 2000 times. If too dry to form pills, add honey. Half-year-old infants take amount equal to two Sesame seeds; 1-2 year olds take amount equal to half two Sesame seeds; 3-4 year olds take two Sesame seed-sized pills; 5-6 year olds take two larger Sesame seed-sized pills; 7-8 year olds take two Adzuki bean-sized pills; 9-10 year olds take two slightly larger Adzuki bean-sized pills. Take at cockcrow. If not passed by sunrise, drink porridge water to induce passage. Pills pass in pairs. If purging is severe, drink cold porridge to stop.
[Mao Xiansheng] Qingjin Dan treats all pediatric accumulations.
Talcum Powder (Hua Shi, powdered) White Starling Droppings (Bai Dingxiang, sieved) Aconite Tuber (Tian Nan Xing, 2 qian bi each) Indigo Naturalis (Qing Dai, sieved, level coin scoop 2 qian) Calomel (Qing Fen, 2 qian) Mercury (Shui Yin, weighed 2 qian, melt Tin 2 qian in copper pot, add Mercury, mix well, pour on ground, cool) Sichuan Croton Seed (Chuan Ba Dou, peeled/cleaned, 72 whole seeds, soak overnight in well water, hang in wind to dry, grind paste)
Mix all ingredients, form pills with soft rice, size of small mung beans. Ensure Croton oil does not leak. Administer with decoction based on syndrome. Post-typhoid accumulation: swallow with light Scallion Decoction. Remove Gan parasites: swallow with beef roast juice. Fright-wind with tight/hard abdomen, cyan-black face: swallow with Gold/Silver Mint Scallion Decoction. Injury with slight warmth/distension on abdomen/skin, night fever, resembling Gan but not quite, cyan-yellow face, slightly yellow eyes: abdominal accumulation. Roast 14 Soap Bean seeds in ash fire, decoct in 1 cup water to 1/2 cup, administer. Accumulation with diarrhea: swallow with Fish Decoction. Qi accumulation: swallow with Stir-fried Fragrant Decoction. Dosage: 1 year old 14 pills, 3 years 18 pills, 7 years 24 pills, adjust by size. Administer at 4th watch, pass completely by dawn. When accumulation clears, tonify based on syndrome. May vomit saliva initially, which is acceptable.
